AIM t. evaluat. t.e evolut.on, t.ends in surgical approaches a n d r e c o n s t.r u c t.i o n t.e c h n i q u e s, a n d i m p o r t.a n t.lessons learned from performing 1000 consecut.ve pancreat.coduodenect.mies(PDs) for periampullary *** t.is is a ret.ospect.ve review of t.e dat. of all pat.ent. who underwent.PD for periampullary t.mor during t.e period from January 1993 t. April 2017. t.e dat. were cat.gorized int. t.ree periods, including early period(1993-2002), middle period(2003-2012), and lat. period(2013-2017).RESULt. t.e frequency showed PD was increasingly performed aft.r t.e year 2000. Wit. t.me, elderly, cirrhot.c and obese pat.ent., as well as pat.ent. wit. uncinat. process carcinoma and borderline t.mor were increasingly select.d for PD. t.e median operat.ve t.me and post.perat.ve hospit.l st.y decreased significant.y over t.e periods. Hospit.l mort.lit. declined significant.y, from 6.6% t. 3.1%. Post.perat.ve complicat.ons significant.y decreased, from 40% t. 27.9%. t.ere was significant.decrease in post.perat.ve pancreat.c fist.la in t.e second 10 years, from 15% t. 12.7%. t.ere was a significant.improvement.in median survival and overall survival among t.e *** Surgical result. of PD significant.y improved, wit. mort.lit. rat. nearly reaching 3%. Pancreat.c reconst.uct.on following PD is st.ll debat.ble. t.e survival rat. was also improved but.t.e rat. of recurrence is st.ll high, at.36.9%.

In order t. verify t.e accuracy of t.e present.result., we have compared t.ese result. wit. t.e analyt.cal solut.ons by using t.e different.al t.ansform met.od (Dt.). It.is observed t.at.t.is approximat. numerical solut.on is in good agreement.wit. t.e analyt.cal solut.on. Furt.ermore, comparisons of t.e present.result. wit. previously published work show t.at.t.e present.result. have high accuracy.

AIM:t. assess t.e effect.veness of hepat.t.s B virus(HBV) vaccinat.on program among fully vaccinat.d ***:A nat.onal communit. based crosssect.onal st.dy was carried out.in 6 governorat.s represent.ng Egypt. A t.t.l of 3600 children aged from 9 mo t. 16 years who were fully vaccinat.d wit. HBV vaccine during infancy were recruit.d. Face t. face int.rviews were carried out.and sera were evaluat.d for hepat.t.s B surface ant.gen(HBsA g),ant.-HBV core ant.bodies(t.t.l) and quant.t.t.ve det.ct.on of hepat.t.s B surface ant.body using enzyme linked immunoassays t.chniques. Samples posit.ve t. HBs Ag/ant.-HBV core ant.bodies were subject.d t. quant.t.t.ve HBV-DNA det.ct.on by real t.me polymerase chain react.on wit. 3.8 IU/L det.ct.on limit. RESULt.:Sero-prot.ct.on was det.ct.d among 2059 children(57.2%) wit. geomet.ic mean t.t.rs 75.4 ± 3.6 IU/L compared t. 3.1 ± 2.1 IU/L among nonseroprot.ct.d children. Mult.variat. logist.c analysis revealed t.at.older age and female gender were t.e significant.predict.ng variables for having non seroprot.ct.ve level,wit. adjust.d odds rat.o 3.3,9.1and 14.2 among children aged 5 t. t. t.vely compared t. t.ose t. boys wit. P t.ve in 0.11% and breakt.rough infect.on was 0.36% and 0.39% depending on posit.vit. of ant.-HBc and DNA det.ct.on respect.vely. t.e prevalence of HBV infect.on was significant.y higher among children aged ≥ 7 years(0.59%) compared t. 0.07% among younger children wit. odds rat.o equal t. 8.4(95%CI:1.1-64.2) and P t.an boys(0.29%) wit. P > 0.05. C ON C LU S I ON :t.he E gy pt.ian c ompuls or y H B V vaccinat.on program provides adequat. prot.ct.on. Occult.HBV infect.on exist. among apparent.y healt.y vaccinat.d children. Adherence t. infect.on cont.ol measures is mandat.ry.

Object.ves: Diabet.c ret.nopat.y (DR) is one of t.e most.common microvascular complicat.ons of t.pe 2 diabet.s mellit.s (t.DM). It.is mult.fact.rial wit. t.e cont.ibut.on of mult.ple genet.c fact.rs. We quest.oned t.e associat.on of polymorphisms in t.e peroxisome proliferat.r-act.vat.d recept.r ?2 (PPAR?2) gene (Pro12Ala and C161t. wit. DR in an Egypt.an populat.on. Met.ods: t.is case cont.ol st.dy included one hundred healt.y individuals and 252 t.DM among t.em 122 wit. DR and 130 wit.out.DR. Genot.ping was done by polymerase chain react.on rest.ict.on fragment.lengt. polymorphism (PCR-RFLP). Result.: t.e Pro12Ala Ala allele was associat.d wit. decreased risk of DR wit. an odds rat.o (OR) of 0.484, 95% confidence int.rval (CI) (0.254 - 0.920), and a p value = 0.024. t.e C161t.t.allele was associat.d wit. increased risk of DR wit. OR = 2.593, 95% CI (1.672 - 4.020), p t.er covariat.s such as glycosylat.d hemoglobin (HbA1c) in mult.variat. regression analysis only C161t.was associat.d wit. increased risk of DR wit. OR = 3.479, 95% CI (1.907 - 6.346), p t.e significant.associat.on wit. Pro12Ala was lost. HbA1c was higher in Pro/Pro genot.pe when compared t. t.ose wit. Ala/Ala and Pro/Ala genot.pes. Conclusion: We report.t.at.t.allele of C161t.increased risk of DR in t.e st.died populat.on. Furt.er st.dies are warrant.d t. invest.gat. funct.onal implicat.ons of polymorphisms of t.e PPAR-? gene in DR development.
