Follow-up strategy for early detection of delayed pseudoaneurysms in patients with blunt traumatic spleen injury: A single-center retrospective study

BACkGROUnD The spleen is the most commonly injured solid organ in blunt abdominal trauma,and splenic pseudoaneurysm rupture is associated with a high risk of *** management has become the standard treatment for hemodynam-ically stable patients with splenic *** the other hand,delayed splenic pseudoaneurysms can develop in any patient,and at present,there are no known risk factors that may reliably predict their ***,there is a lack of consensus regarding the most appropriate strategies for monitoring and mana-ging splenic injuries,especially lower-grade(I-III).AIM To determine the predictors of pseudo-aneurysm formation following splenic injury and develop follow-up strategies for early detection of *** We retrospectively analyzed patients who visited the Level I Trauma Center bet-ween January 2013 and December 2022 and were diagnosed with spleen injuries after blunt abdominal *** Using the American Association for the Surgery of Trauma spleen injury scale,the splenic injuries were categorized into the following order based on severity:Grade I(n=57,17.6%),grade II(n=114,35.3%),grade III(n=89,27.6%),grade IV(n=50,15.5%),and grade V(n=13,4.0%).Of a total of 323 patients,35 underwent splenectomy and 126 underwent angioembolization.19 underwent delayed angioembolization,and 5 under-went both initial and delayed *** 14 patients who had undergone delayed angioembolization,no extravasation or pseudoaneurysm was observed on the initial computed tomography *** are no particular patient-related risk factors for the formation of a delayed splenic pseudoaneurysm,which can occur even in a grade I spleen injury or even 21 days after the *** mean detection time for a delayed pseudoaneurysm was 6.26±5.4(1-21,median:6,inter-quartile range:2-9)*** We recommend regular follow-up computed tomography scans,including an arterial and portal venous phase,at least 1 week an

目的通过对4~6岁电声双模式助听儿童的高兴、中性及难过3种情感韵律进行研究,探讨不同听觉年龄电声双模式助听儿童的韵律识别能力。方法选取2022年9月至2023年12月某幼儿园就读的生理年龄4~6岁,听觉年龄1~4岁的电声双模式助听儿童32例,按照听觉年龄分为1.5岁组4例、2岁组6例、2.5岁组6例、3岁组6例、3.5岁组4例及4岁组6例。采用华东师范大学康复科学系自制的15个日常用句,分别用高兴、中性和难过3种不同的情感语气读出,让电声双模式助听儿童识别,共45句。分析不同听觉年龄对高兴、难过和中性3种情感韵律的识别能力。结果 1.5岁组、2岁组、2.5岁组、3岁组、3.5岁组及4岁组的情感韵律识别能力得分比较,差异无统计学意义(F=1.246,P>0.05)。6组3种不同类型情感韵律识别能力比较,差异无统计学意义(F=1.174,P>0.05),情感韵律类型和听觉年龄组之间无交互作用,差异无统计学意义(F=0.980,P>0.05)。6组不同听觉年龄情感韵律识别的正确率比较,差异有统计学意义(F=4.106,P<0.01)。结论听觉年龄1~4岁的大龄电声双模式助听儿童,情感韵律识别能力随听觉年龄的增长而升高;但不同听觉年龄对高兴、难过和中性3种情感韵律的识别不存在差异。随着听觉年龄的增长,要重视大龄电声双模式助听儿童情感韵律识别能力的训练。

CO_2 laser-micro plasma arc hybrid welding for galvanized steel sheets

A laser lap welding process for zinc-coated steel has a well-known unsolved problem-porosity formation. The boiling temperature of coated zinc is lower than the melting temperature of the base metal, which is steel. In the autogenous laser welding, the zinc vapor generates from the lapped surfaces expels the molten pool and the expulsion causes numerous weld defects, such as spatters and blow holes on the weld surface and porosity inside the welds. The laser-arc hybrid welding was suggested as an alternative method for the laser lap welding because the arc can preheat or post-beat the weldment according to the arrangement of the laser beam and the arc. CO2 laser-micro plasma hybrid welding was applied to the lap welding of zinc-coated steel with zero-gap. The relationships among the weld quality and process parameters of the laser-arc arrangement, and the laser-arc interspacing distance and arc current were investigated using a full-factorial experimental design. The effect of laser-arc arrangement is dominant because the leading plasma arc partially melts the upper steel sheets and vaporizes or oxidizes the coated zinc on the lapped surfaces. Compared with the result from the laser-TIG hybrid welding, the heat input from arc can be reduced by 40%.

Mixing features in an electromagnetic rectangular micromixer for electrolyte solutions

In this study, a new magnetohydrodynamic（MHD） mixer for electrolyte solutions with pumping function is reported, and the mixing performance of the device for two different electrolyte solutions is numerically examined in a uniform magnetic field. Application of different potentials to different electrodes allows the current to be induced. The combination of the induced current and magnetic field yields Lorentz force, resulting in the fluid motion. The numerical simulation for the flows in the device is carried out with commercial software CFX. The validity of CFX code for the present numerical model is presented. The mixing performance of the fluids is investigated in many different cases with different combinations of input voltage of the electrode. This study shows that the mixing performance can be enhanced by applying spatially alternating positive and negative voltages to the electrodes. The present simulation results show that with a magnetic field intensity lower than 0.5 T, a voltage difference smaller than 2.0 V, and an electric conductivity of electrolyte solution of 1.5 S/m the pumping capabilities ranging 1.6×10^（-7）-3.6×10^（-6） kg/s and the mixing indexes higher than 0.90 can be obtained with sophisticated designs of the micromixer.

Flow imbalance in MHD flows in splitting ducts with locally different electric conductivities of the walls

In this study, three-dimensional liquid-metal magnetohydrodynamic splitting flows in a duct with one inflow channel and two outflow channels with locally different wall electric conductivities（different conductance ratios） are computationally examined. The numerical calculation is achieved with the use of CFX code, with a structured grid system chosen after a series of mesh independence tests. The detailed features of the MHD flows are depicted in a case where the electric conductivities of the two outflow ducts are quite different. Also, the interdependency among the current, fluid velocity, pressure and electric potential is elucidated in order to describe the electromagnetic characteristics of the liquid-metal flows. The result shows that the imbalance of the mass flow rate in the two outflow channels strongly depends on the wall conductivity（i.e., conductance ratio） of each outflow channel.

As there is substantial progress in the kSTAR tokamak engineering, all themajor structures and sub-systems are under fabrication and in procurement phase. The vacuum vessel,port, cryostat cylinder, lid, and bellows are being rigorously fabricated in the factory. The lowerpart of the kSTAR such as cryostat base and gravity support has been almost finished in itsfabrication. There are also great progresses and significant results in manufacturing of thesuperconducting magnet, including four Toroidal Field (TF) coils, lower and upper PF7 coils whichare the largest Poloidal Field (PF) coils. The TFOO coil, which has been made for test and back-upof the TF magnet system, was successfully tested in the cool-down and current charging. As thefabrications and procurements of major structures have been actively proceeded, assembly works werealso launched from Aug. 2003. More detailed description on these status, results, and plans will bedescribed in this paper.

Quality incentive based congestion control for multimedia communication over IP networks

In this paper we introduce a framework for using quality as an incentive to promote proper application level congestion control. Through integrating a joint-source channel coder and feedback-based congestion control scheme, we are able to construct accurate and efficient quality incentives. The framework is applicable in all network architectures where end-to-end congestion control may be used, and is as such not specific to either best-effort or traffic class-based architectures. The concept is presented along with preliminary simulations that highlight the resulting rate control accuracy. We also discuss how to implement some well-known congestion control schemes within our framework.

Temperature segregation and its impact on the quality and performance of asphalt pavements

Temperature segregation is non-uniform temperature distributionacross the uncompacted asphalt mat during paving operations and mayhave detrimental effects on the quality and performance of asphaltpavements. However, many research studies conducted across the UShave reported mixed observations regarding its effects on the initialquality and long-term performance of asphalt pavements.?The objectiveof this study was to determine the effects of the temperature segregationon the density and mechanical properties of Louisiana asphalt *** asphalt rehabilitation projects across Louisiana were selected.A multi-sensor infrared bar (Pave-IR) system and a hand-held portablethermal camera were used to measure the temperature of asphalt *** core samples were collected from various areas with varyingseverity levels of temperature segregation and tested for the density,fracture resistance (J ) by semi-circular bending(SCB), rut depth by wheel tracking, and dynamic modulus (|E*|) byindirect tension (IDT) devices.?Two distinctive patterns of non-uniformtemperature distribution were observed: a cyclic and irregular temperaturesegregations. Laboratory test results showed that highly temperaturesegregated asphalt pavements (i.e., temperature differentials ≥41.7°C) can have significantly lower densities and the mechanicalproperties than the non-segregated area, especially when the temperaturedifferentials are measured at compaction.
