Deafness: Epidemiological, Clinical and Therapeutic Aspects in 15 Year Olds and over at the University Hospital Center Gabriel Toure of Bamako

Objective: To study the epidemiological, clinical and therapeutic aspects of hearing loss in 15-year-olds and over at the CHU GT of Bamako. Materials and Method: This is a longitudinal prospective study carried out in the EnT department of the University hospital Gabriel Toure in Bamako over 13 months (from September 2018 to September 2019). We carried out an exhaustive sampling of all the patients who consulted in the department for hearing loss and whose deafness was confirmed at pure tone audiometry with age greater than or equal to 15 years. The exclusion criteria were all patients under the age of 15 as well as a hearing loss related to earwax or foreign bodies and refusals to participate in the study. Results: Two hundred and thirty (230) patients were collected during the study. Dominated by the male sex with a sex ratio of 1.5 or 3 men for 2 women, with an average age of 38.16 ± 19 years with extremes ranging from 15 to 86 years. Among our patients, 37.40% had unilateral deafness, progressive installation evolving more than 12 weeks with a rate of cases had headaches 41% and had a history of chronic otitis media (CMO) 27%. There were a few cases of mixed type deafness, 30% had mild deafness, 22% moderate deafness and 40% severe deafness, according to the BIAP classification. Among our patients, 22.16% had conductive hearing loss, and those with sensorineural hearing loss accounted for 15.24%. Conclusion: Pure tone audiometry remains essential in the diagnosis of deafness. It not only makes it possible to confirm or invalidate the deafness, but also to define the type of deafness and especially to classify the deafness according to the degree of the average hearing loss established by the BIAP.

Chronic Purulent Otorrhea in the South of the Sahara: Epidemiological, Clinical and Therapeutic Profiles about 135 Cases

Objective: To study the clinical and therapeutic epidemiological profile of chronic purulent otorrhea in the EnT and CCF departments of the Gabriel Touré university hospital. Materials and Method: This was a retrospective study from May 2019 to April 2021. We made an exhaustive sampling of all patients who consulted for chronic purulent otorrhea with a complete medical file. All ages were included (except from 0 to - 5 years), and all sexes combined. The data medium was the patient records and the department’s operating report register. The non-inclusion criteria were patients with incomplete files and patients who were less than 5 years old. Results: In total, we collected 135 cases. The average age of our patients was 29.27 years. The most represented age group was 16 to 25 years old. We noted a male predominance in 60.7%. The associated signs found were hearing loss, earache, tinnitus, ear pruritus, vertigo, headache and facial paralysis. The main pathology involved is chronic otitis media (CMO) in 55.5%, the most serious of which is dangerous chronic otitis (8.1%). All of our patients have received medical treatment. The treatment was surgical in 64% of patients after medical treatment. With the mean follow-up of 6 months, tinnitus and reperforation were the most common complications found in 6.2% and 3.1% respectively. Conclusion: Chronic purulent otorrhea is the most common ear discharge in EnT consultation. The main pathology involved is CMO, the most formidable of which is cholesteatoma. Anatomical Variations of Recurrent Laryngeal nerve: The Danger in Thyroid Surgery

Objectives: We report two types of anatomical variations of the recurrent laryngeal nerve in two patients. Through these two patients we wanted to highlight our surgical approach of the recurrent nerve in an unusual position and to describe the surgical implication of these almost rare variations. Case report: patient aged 28 and 58 admitted for goiter. They underwent a right lobisthmectomy. Both recurrences were approached retrograde. The anatomical variations of the nerve concerned the non-recurrent laryngeal nerve in the first patient. In the second patient there were three anatomical variations, namely an extra laryngeal bifurcation of the nerve, a pre-vascular position of the nerve and a branch connecting the vagus nerve and the recurrent nerve. no recurrence nerve injury was noted. Conclusion: The anatomical variations of the nerve are numerous. A careful dissection is a guarantee of a good prognosis.

Audiometric Profile of Deafness at the University Hospital Center Gabriel Toure of Bamako

Objective: To determine the audiometric profile of deafness in our practice. Materials and Methods: Longitudinal prospective study was conducted out in the EnT department and cervicofacial surgery of Gabriel Toure University Hospital in Bamako, we made an exhaustive sampling of all the patients who consulted for hearing loss, tinnitus, vertigo, hiring report, medical expertise and whose deafness was confirmed to tonal audiometry with an age greater than or equal to 15 years. It was spread over 10 months (June 2016 to March 2017). A total of 200 patients were collected. Exclusion criteria were all patients under 15 years of age as well as deafness related to earwax or foreign bodies, and refusal to participate in the study Results: During our study period, 6055 outpatients were admitted out of which 734 patients underwent audiometric testing and 200 patients (3.30%) met our criteria. The male sex was the most represented with a rate of 60%. The sex ratio was 1.5 or 3 men for 2 women. The 15 to 25 age group was the most represented at 37.5%. The average age was 37.18 years old with extremes ranging from 15 to 83 years old. Pupils/students were the most represented with a rate of 29.5%, followed by housewives in 23%, farmers in 10.5% and military in 10%. As antecedent 26% of our patients had a chronic otitis media (OMC), against 23% who had no antecedent otological and 17% had a traumatic antecedent. Bilateral deafness was the most common with a rate of 64.5%. The mode of progressive appearance was the most frequent in 74.5%. As functional signs 46.22% of our patients had a hearing loss associated with tinnitus. Otoscopic examination was pathological in 34.5% of our patients. Mixed deafness was the most common in 43.35%, followed by perception deafness in 32.19% and transmission deafness 24.46%. Mean deafness was the most common with a rate of 48.91%, was severe in 20.22%, mild in 18.31%, deep in 11.47% and cophotic in 01.09%. Aims: To describe the diagnostic aspects, surgical indications and post-operative complications of thyroidectomies performed in our department. Materials and method: A descriptive retrospective study that took place in the EnT Department and Cervicofacial Surgery of Gabriel TOURE University Hospital of Bamako. We did a comprehensive sampling of all goiter cases from January 2013 to December 2018. Were included in the study, the records of patients of all ages and genders, admitted into the ward and scheduled for thyroidectomy (partial or total). The exclusion criteria were incomplete hospitalization records. There were a total of 139 files were retained. Results: In 60 months, 139 cases were collected out of 1720 patients hospitalized for surgery, representing a hospital prevalence of 8.08%. The average age was 46.89 years. (123 women and 16 men). The socio-professional categories were dominated by housewives (68.34%). The reported functional signs were tachycardia, asthenia and other signs of dysthyroidism in 59% as well as signs of compression in 24.46%. In 72 cases or 51.80%, the patients consulted between 2 and 10 years of disease progression. Twenty patients or 14.39% had a history of familial goiter and 2 patients had a history of thyroid surgery. On physical examination the swelling was antero-cervical in 56.83% of cases. In 96 cases or 69.06% the glandular diameter was between 5 and 9 cm. In 2 cases or 1.43% we noted cervical adenopathy in the jugulo-carotid chain. Ultrasound, TSHus and fT4 were performed first-line and systematically in all our patients. Ultrasound objectified an appearance of multinodular goiter in 106 cases or 76.26%. In 60.43% of cases the patients were TI classes RADS 3, they were TI RADS 4A in 16 cases or 11.51%. CT scans were performed in 3 patients or 2.15% to specify the loco-regional extension, to look for possible lymph node invasion, and to compress or dipping the goiter. Aim: Our goal was to determine the histological profile of EnT lesions. Method: We carried out a retrospective and descriptive study in the EnT department and Cervico-Facial Surgery at the university hospital center Gabriel Touré (MALI) from 1995 to 2014. It made it possible to collect 450 cases. Result: We found a male predominance;51.11% or a sex ratio of 1.04. The mean age of patients was 38.04 years with a standard deviation of 19.49 years and extremes of 3 months and 91 years. Benign tumors were the most frequent (154 cases or 34.22%) including 75 cases of adenomas and 43 cases of papillomas. Cancers accounted for 48.11% (or 153 cases) of all tumors and 34% of lesions. Carcinomas accounted for 81.04%, malignant lymphomas 13.72% and sarcomas 5.22% of cases. Among the 140 cases (31.11%) of inflammatory pseudotumors, we found 12 cases of tuberculosis and 15 cases of rhinoscleroma. Three (3) cases of dysplasia were observed. Conclusion: Interest should be focused on histology in order to initiate appropriate therapy.

近几年来，由于全球互通微波存取（WiMAX）技术的数据速率高、部署成本低以及覆盖范围广，使其被广泛地应用于提供无线最后一英里的连接服务。同时，WiMAX被认为是能在大城市区域实现宽带无线接入（BWA）的最有效的方法之一。但是，随着一些诸如VoIP以及视频流等对时延敏感的应用的出现，就需要网络能够提供连续的通信服务，此时就需要有一种高效的切换机制来满足服务质量（QoS）需求。目前，一些研究小组也正致力于研究如何提高切换机制的效率。在WiMAX中，切换被定义为过程和决策的集合，这个集合使得一个移动站（MS）能从一个基站（BS）的空中接口移动到另一个基站。但是，当MS在BS之间移动时，WiMAX系统的移动性仍会造成几个缺陷，尤其是与信道资源浪费、切换延时以及数据丢失有关的缺陷，这些缺陷可能会影响到应用的实时性。本文评估并实现了一种基于BS信号强度（RSSI）和MS方向信息的切换机制。在该机制中，假设通过使用全球定位系统（GPS）可以获得MS在任意时刻的位置信息。假设基站的位置是固定的，MS到网络中不同BS的距离已知。则MS可以计算出它邻近的BS接收到的信号强度。根据这些收集到的信息以及切换要求，MS就可以选择启动不同的进程来触发切换机制。这种触发切换机制的方法，同时利用到了RSS和移动节点与基站之间的距离信息，且由于RSSI是与距离有关的，这样MS就可以减少一些不必要的切换。仿真中分析了VoIP上影响WiMAX网络性能的主要因素的几个参数，如切换时延、吞吐量以及丢包率等。仿真结果表明，最终得到的参数值对于一些如VoIP、视频流等实时应用是容忍的。

Bioclinical Study of Coronavirus Disease 2019 in the Laboratory of Virological Hemorrhagic Fever in Guinea

Introduction: This study aimed to describe the clinical and microbiological characteristics of individuals confirmed to have coronavirus disease 2019 (COVID-19) in the laboratory of virological hemorrhagic fever in Guinea. Method: This was a cross-sectional and descriptive study conducted from March 19 to June 19, 2020, in the virological hemorrhagic fever laboratory in Guinea. Samples collected from individuals living in the Conakry region, which were positive for the reverse-transcriptase polymerase chain reaction test for COVID-19, were included in this study. Results: Among the confirmed cases (n = 743), the males (n = 440, 59%) were more than the females (n = 303, 41%). The mean age of the confirmed cases was 33.31 ± 16.08 years. Almost all the confirmed cases (n = 515, 69.3%) had contact with a confirmed case of COVID-19. At the time of collection, 78.7% (481) of the cases were asymptomatic, while 35.3% (262) were symptomatic. At the time of the sample collection, the main symptoms were fever (51.53%), cough (49.24%), physical asthenia (46.18%), sore throat (33.2%), headache (28.62%), anosmia (9.92%), nausea, vomiting, and abdominal pain (5.34%), and diarrhea (1.53%). Only 2% of the cases had comorbidities, including diabetes (n = 7) and hypertension (n = 5). The viral load of the asymptomatic and symptomatic patients was similar with the cycle threshold value, all of which varied from 10 - 19, 20 - 29, and 30 - 36, respectively. In the first test, 27 samples were declared undetermined;two days later, samples were re-taken and re-tested and 18 of them were positive for COVID-19. The average length of hospitalization was 14 days for patients admitted to the hospital. On the other hand, the non- hospitalized patients isolated themselves at home for 22 days. ng>Introduction:ng> Tongue papillomas are benign epithelial tumors of the tongue lining. It is a precancerous lesion of viral origin caused by the Human papilloma virus (HPV). ng>Objective:ng> To describe the clinical and histological manifestations of a particular type of oral papilloma. ng>Observation:ng> A young woman aged 28 years resident in Siguiri (Guinea), received in consultation for a painless mass on the tongue slow evolving for 6 months duration. There was no notion of mouth bleeding. She had no particular medical and surgical history and negative for retroviral test/HIV test. Examination of the oral cavity found a fungating mass in at Right or Left posterior third of the tongue, painful on contact, sessile and pedunculated and not bleeding on contact and of soft consistency. The excision of the tongue mass was performed under general anesthesia. Biopsy was taken and sent for HPE. The consequences were mild, allowing her to be released 3 days after surgery. Histological examination showed an acanthotic stratified para-keratinized squamous epithelium containing small parts of connective tissue and koilocytes containing connective tissue nuclei reminiscent of papilloma. There was no malignancy cell. ng>Conclusion:ng> The papilloma of the tongue is a benign proliferative lesion characterized by slow and painless growth. I would say early diagnosis and surgical excision should be done to avoid recurrence, contact bleeding and morbidity secondary to mass effect.

Introduction: Cervical cystic lymphangiomas are rare benign dysembryoplastic tumors of lymphatic origin. Its severity in the child is due on one hand to their fast evolution and the compression of the way aerodigestive and on the other hand, by the classical difficulty of their excision. The purpose of this work was to report a case of giant cervical cystic lymphangioma in a child to discuss the difficulties associated with its management in our context. Observation: This was a 3-year-old female child who was admitted to the EnT department of CHU “Luxembourg” for right lateral cervical swelling. The clinical examination had noted a large anterior-latero cervical swelling of soft, renitent, painless palpation, movable in relation to the superficial and deep plane, measuring about 20 cm × 17 cm, the skin was healthy. It wasn’t particularity to the rest of the physical examination. The diagnosis of giant cervical cystic lymphangioma was discussed. Thorough excision of the swelling by right lateral cervicotomy was performed. Anatomopathological examination of the operative specimen confirmed the diagnosis. The postoperative course was simple and the evolution was favorable. Conclusion: Cervical cystic lymphangiomas are rare. Their management involves surgery, with short and long-term post-operative outcomes are often excellent.
