BACKGROUnD The prevalence of heart failure(HF)increases with age,and it is one of the leading causes of hospitalization and death in older ***,there are little data on in-hospital mortality in patients with HF≥75 years in *** A retrospective analysis of the Spanish Minimum Basic Data Set was performed,including all HF episodes discharged from public hospitals in Spain between 2016 and *** was performed using the International Classification of Diseases,10th ***≥75 years with HF as the principal diagnosis were *** calculated:(1)the crude in-hospital mortality rate and its distribution according to age and sex;(2)the risk-standardized in-hospital mortality ratio;and(3)the association between in-hospital mortality and the availability of an intensive cardiac care unit(ICCU)in the *** We included 354,792 HF episodes of patients over 75 *** mean age was 85.2±5.5 years,and 59.2%of patients were *** most frequent comorbidities were renal failure(46.1%),diabetes mellitus(35.5%),valvular disease(33.9%),cardiorespiratory failure(29.8%),and hypertension(26.9%).In-hospital mortality was 12.7%,and increased with age[odds ratio(OR)=1.07,95%CI:1.07–1.07,Pnd was lower in women(OR=0.96,95%CI:0.92–0.97,Pn predictors of mortality were the presence of cardiogenic shock(OR=19.5,95%CI:16.8–22.7,Pnd advanced cancer(OR=2.6,95%CI:2.5–2.8,Pn hospitals with ICCU,the in-hospital risk-adjusted mortality tended to be lower(OR=0.85,95%CI:0.72–1.00,P=0.053).COnCLUSIOnS In-hospital mortality in patients with HF≥75 years between 2016 and 2019 was 12.7%,higher in males and elderly *** main predictors of mortality were cardiogenic shock,stroke,and advanced *** was a trend toward lower mortality in centers with an ICCU.

Role of upper endoscopy in diagnosing opportunistic infections in human immunodeficiency virus-infected patients

Highly active antiretroviral therapy (HAART) has dramatically decreased opportunistic infections (OIs) in human immunodef iciency virus (HIV)-infected patients. However,gastrointestinal disease continues to account for a high proportion of presenting symptoms in these patients. Gastrointestinal symptoms in treated patients who respond to therapy are more likely to the result of drug-induced complications than OI. Endoscopic evaluation of the gastrointestinal tract remains a cornerstone of diagnosis,especially in patients with advanced immunodeficiency,who are at risk for OI. The peripheral blood CD4 lymphocyte count helps to predict the risk of an OI,with the highest risk seen in HIV-infected patients with low CD4 count (n update of the role of endoscopy in diagnosing OI in the upper gastrointestinal tract in HIV-infected patients in the era of HAART.

Global evolution of Equidae and Gomphotheriidae from South America

The contemporary South American mammalian communities were determined by the emergence of the Isthmus of Panama and by the profound climatic oscillations during the *** and gomphotheres were 2 very conspicuous groups of immigrant mammals from north America that arrived in South America during the *** present study compiles updated data on the phylogeny,systematics and ecology of both groups in South *** horses in South America are represented by 2 genera,Hippidion and Equus,as are the gomphotheres,represented by Cuvieronius and *** genera of horses include small(Hippidion devillei,***,*** and ***)and large forms(Equus neogeus and ***),which dispersed into South America using 2 different *** possible model for this dispersion indicates that the small forms used the Andes corridor,while larger horses dispersed through the eastern route and through some coastal *** the case of gomphotheres,Cuvieronius and Stegomastodon reached South America in 2 independent dispersal events,and Cuvieronius dispersed across the Andean corridor,while large Stegomastodon spread along the eastern *** and gomphotheres present values ofδ^(13)C from woodlands to C4 grass*** present lower values ofδ^(13)C than Equus in the late Pleistocene,whereas the gomphotheres diverge from value ofδ^(18)O,reflecting that Cuvieronius inhabited the Andean corridor and Stegomastodon dispersed through eastern *** gomphothere and horse species recorded in South America became extinct around the time that humans arrived.

Resistance of full-scale beams against close-in *** modeling and field tests

This paper explores the performances of a finite element simulation including four concrete models applied to a full-scale reinforced concrete beam subjected to blast loading. Field test data has been used to compare model results for each case. The numerical modelling has been, carried out using the suitable code LS-DYnA. This code integrates blast load routine(COnWEP) for the explosive description and four different material models for the concrete including: Karagozian & Case Concrete, Winfrith, Continuous Surface Cap Model and Riedel-Hiermaier-Thoma models, with concrete meshing based on 10, 15, and 20 mm. Six full-scale beams were tested: four of them used for the initial calibration of the numerical model and two more tests at lower scaled distances. For calibration, field data obtained employing pressure and accelerometers transducers were compared with the results derived from the numerical simulation. Damage surfaces and the shape of rupture in the beams have been used as references for comparison. Influence of the meshing on accelerations has been put in evidence and for some models the shape and size of the damage in the beams produced maximum differences around 15%. In all cases, the variations between material and mesh models are shown and discussed.

Effect of a cardiac telerehabilitation program during COVID-19 associated social isolation

This study aimed to compare the impact of a cardiac telerehabilitation(CTR)protocol aimed at patients with cardiovascular diseases(CVDs)during the period of coronavirus disease 2019(COVID-19)associated with social *** retrospective cohort study included 58 participants diagnosed with stable cardiovascular diseases(CVDs),which were divided into three groups:conventional cardiac rehabilitation(CCR)group(n=20),composed of patients undergoing conventional cardiac rehabilitation;cardiac telerehabilitation(CTR)group(n=18),composed of patients undergoing cardiac telerehabilitation and control group(n=20),composed of patients admitted for cardiac rehabilitation who had not started training *** results showed that body mass index was reduced(p=0.019)and quality of life was improved(e.g.,limitations due to physical aspects[p=0.021),vitality[p=0.045]and limitations due to emotional aspects[p=0.024])by CCR compared to *** outcomes were not improved by CTR(p>0.05).However,this strategy prevented clinical deterioration in the investigated *** CCR achieved a superior effect on clinical improvement and quality of life,CTR was relevant to stabilize the blood pressure and quality of life of patients with cardiovascular diseases during the period of COVID-19-associated social isolation.
