Allylalumination of alkynes （i.e. forming Negishi＇s （Z）-alkenyl dialkylalane） followed by alkenylation of epoxides provides 1,4-disubstituted （Z）-3,6-alkadienols. The alkenylation can be facilitated by the presence of a neighbouring coordinating group in the epoxides. This ＂one-pot＂ approach has been successfully applied in the large-scale production of C 10--C 15 fragment of 9,10-didehydroepothilone D （a synthetic epothilone analog as an anti-cancer agent in phase 2 clinical trials）.

Impact on dietary intake of a self-directed, gender-tailored diabetes prevention program in men

AIM To investigate changes in dietary intake following a 6-mo randomised controlled trial of the self-directed, gendertailored type 2 diabetes mellitus(T2DM) Prevention Using LifeS tyle Education(PULSE) program in men. METHODS Men aged 18-65 years, with a body mass index(BMI)25-40 kg/m^2, and at high risk for developing T2DM were recruited from the Hunter Region of New South Wales,Australia. Eligible participants were randomised into one of two groups:(1) waitlist control; or(2) PULSE intervention. Dietary intake was assessed at baseline and immediately post-program using the Australian Eating Survey food frequency questionnaire and diet quality measured using the Australian Recommended Food Score(ARFS). RESULTS One hundred and one participants(n = 48, control; n= 53, intervention, mean age 52.3 ± 9.7 years, BMI of 32.6 ± 3.3 kg/m^2) commenced the study. Following the active phase, differences between groups were observed for proportion of total energy consumed from healthful(core) foods(+7.6%EI, P < 0.001), energydense, nutrient-poor foods(-7.6%EI, P < 0.001),sodium(-369 mg, P = 0.047), and diet quality(ARFS)(+4.3, P = 0.004), including sub-scales for fruit(+1.1, P= 0.03), meat(+0.9, P = 0.004) and non-meat protein(+0.5, P = 0.03). CONCLUSION The PULSE prevention program's nutrition messages led to significant improvements in dietary intake in men at risk of T2DM.

Priming of the Cells： Hypoxic Preconditioning for Stem Cell Therapy

Objective： Stem cell-based therapies are promising in regenerative medicine for protecting and repairing damaged brain tissues after injury or in the context of chronic diseases. Hypoxia can induce physiological and pathological responses. A hypoxic insult might act as a double-edged sword, it induces cell death and brain damage, but on the other hand, sublethal hypoxia can trigger an adaptation response called hypoxic preconditioning or hypoxic tolerance that is of immense importance for the survival of cells and tissues. Data Sources： This review was based on articles published in PubMed databases up to August 16, 2017, with the following keywords：＂stem cells,＂＂hypoxic preconditioning,＂＂ischemic preconditioning,＂ and ＂cell transplantation.＂Study Selection： Original articles and critical reviews on the topics were selected. Results： Hypoxic preconditioning has been investigated as a primary endogenous protective mechanism and possible treatment against ischemic injuries. Many cellular and molecular mechanisms underlying the protective effects of hypoxic preconditioning have been identified. Conclusions： In cell transplantation therapy, hypoxic pretreatment of stem cells and neural progenitors markedly increases the survival and regenerative capabilities of these cells in the host environment, leading to enhanced therapeutic effects in various disease models. Regenerative treatments can mobilize endogenous stem cells for neurogenesis and angiogenesis in the adult brain. Furthermore, transplantation of stem cells/neural progenitors achieves therapeutic benefits via cell replacement and/or increased trophic support. Combinatorial approaches of cell-based therapy with additional strategies such as neuroprotective protocols, anti-inflammatory treatment, and rehabilitation therapy can significantly improve therapeutic benefits. At Mont Terri Underground Research Laboratory （URL） Switzerland, a specific experiment has been per- formed in a tunnel, in order to investigate the hydro-mechano-chemical （HMC） perturbations induced in the argillaceous formation by forced ventilation. This experiment has been selected in the international project DECOVALEX to be used for process model development and validation. The numerical simula- tion of the geochemical response to the ventilation experiment （VE） is the object of the present paper, focusing on the transport of chloride as a conservative species and sulphate as a reactive species. Uti- lising the validated hydro-mechanical （HM） results from earlier steps of the DECOVALEX task, reactive and non-reactive transport models, incorporating the current understanding of the geochemistry at the site, were successfully constructed for the whole experimental period. The associated parametric and process uncertainty analyses clearly demonstrate that the basic HM understanding must be sound. How- ever, to demonstrate this degree of robustness, the explicit inclusion of process representations of water desaturation, liquid vaporisation, species exclusion porosity, and redox processes, is required.

Bone-produced fibroblast growth factor 23(FGF23)increases in response to inflammation and iron deficiency and contributes to cardiovascular mortality in chronic kidney disease(CKD).Neutrophil gelatinase-associated lipocalin(NGAL or lipocalin 2;LCN2 the murine homolog)is a pro-inflammatory and iron-shuttling molecule that is secreted in response to kidney injury and may promote CKD *** investigated bone FGF23 regulation by circulating *** 23 weeks,Col4a3KO mice showed impaired kidney function,increased levels of kidney and serum LCN2,increased bone and serum FGF23,anemia,and left ventricular hypertrophy(LVH).Deletion of Lcn2 in CKD mice did not improve kidney function or anemia but prevented the development of LVH and improved survival in association with marked reductions in serum ***2 deletion specifically prevented FGF23 elevations in response to inflammation,but not iron deficiency or phosphate,and administration of LCN2 increased serum FGF23 in healthy and CKD mice by stimulating Fgf23 transcription via activation of cAMP-mediated signaling in bone *** results show that kidney-produced LCN2 is an important mediator of increased FGF23 production by bone in response to inflammation and in ***2 inhibition might represent a potential therapeutic approach to lower FGF23 and improve outcomes in CKD.

Fracture Rate and Serious Complications of Vena Cava Filters

Purpose: To retrospectively evaluate the prevalence of fracture and fragment embolization of inferior vena cava (IVC) filters. Methods: Electronic medical records and imaging studies of all Kaiser Permanente patients who received IVC filters from August 2000 until August 2010 were retrospectively reviewed for filter complications. Results: 283 patients received an IVC filter during the study period. 143 patients were deceased, while 140 are living. Among deceased patients, the average age at the time of death was 69.8 ± 15.3 [range: 24.7 - 99.2] years;55.9% were men;the mean implantation-to-image time was 13.6 ± 20.6 [range: 0 - 92.4] months, and there were no reported major complications attributable to filter migration or fracture at a mean of 16.8 ± 24.8 [range: 0 - 119.6] months following implantation. One of 14 (7.1%) G2 filters perforated the aorta, which already had a stent graft in place. Among those patients still living, the average age was 67.3 ± 15.2 [range: 15.2 - 97.3] years, 47.1% were men, the mean implantation-to-image time was 33.3 ± 36.5 [range: 0.1-141.7] months, and there were no reported major complications at a mean of 35.3 ± 36.5 [range: 0 - 141.7] months following implantation. Three of 60 (5.0%) Trapease filters were found to have at least 1 strut fracture. There were no cases of filter migration or fragment embolization. The overall fracture rate of all filters with an implantation-to-image-time greater than two years (mean implantation-to-image time 4.7 ± 2.7 [range: 2.0 - 11.8] years) was 3 of 67 (4.5%). Bard G2 and G2X filters had a 0% fracture and embolization rate at a mean of 19.0 ± 16.6 [range: 0.07 - 49.5] months after implantation. Conclusions: IVC filters, regardless of type, have a low prevalence of fracture and we found no cases of fragment embolization.
