Natural deep eutectic solvents(NADESs) as functional liquid media have received increasing attention in recent years, which are directly composed of natural products. [1-3] A novel method based on natural deep eutectic solvent coupled with microwave-assisted hydrodistillation was used for the extraction of Cuminum cyminum L. essential oil. Several experimental parameters, such as NADESs type, NADESs/water ratio, NADESs/power ratio, sample pretreatment stage of microwave power, set-up temperature, duration, and hydrodistillation duration were all studied. Under the optimal conditions, the NADESs composed by choline chloride and L-lactic acid(molar ratio with 1:3) obtained the highest EO yield of 2.2%. Also, the EOs were analyzed by using gas chromatography-mass spectrometry(GC-MS), with a total of 108 compounds being identified. In this case, microwave helps to reduce the time for extraction and NADESs can effectively improve the EO yield. Especially, compared with binary DES system, NADESs obviously brought about more identified compositions. Hence, the NADESs-based approach has the potential of being extended to the analysis of low content of volatile compounds from other plant resources.

Challenges faced by community health nurses to achieve universal health coverage in Myanmar:A mixed methods study

Hepatocellular carcinoma in patients with chronic kidney disease

AIM: To investigate outcomes of hepatocellular carcinomas (HCCs) in patients with chronic kidney disease (CKD). METHODS: Four hundred and forty patients referred between 2000 and 2002 for management of HCCs were categorized according to their CKD stage, i.e. , estimated glomerular filtration rate (eGFR) > 90 (stage 1), 60-90 (stage 2), 30-60 (stage 3), 15-30 (stage 4), and < 15 (stage 5) mL/min per 1.73 m 2 , respectively. Demographic, clinical and laboratory data were collected and mortality rates and cause of mortality were analyzed. The mortality data were examined with Kaplan-meier method and the significance was tested using a log-rank test. An initial univariate Cox regression analysis was performed to compare the frequency of possible risk factors associated with mortality. To control for possible confounding factors, a multivariate Cox regression analysis (stepwise backward approach) was performed to analyze those factors that were significant in univariate models (P < 0.05) and met the assumptions of a proportional hazard model. RESULTS: Most HCC patients with CKD were elderly, with mean age of diagnosis of 60.6 ± 11.9 years, and mostly male (74.8%). Hepatitis B, C and B and C coinfection virus were positive in 61.6%, 45.7% and 14.1% of the patients, respectively. It was found that patients with stages 4 and 5 CKD were not only older (P = 0.001), but also had higher hepatitis C virus carrier rate (P = 0.001), lower serum albumin level (P = 0.001), lower platelet count (P = 0.037), longer prothrombin time (P = 0.001) as well as higher proportions of advanced cirrhosis (P = 0.002) and HCCs (P = 0.001) than patients with stages 1 and 2 CKD. At the end of analysis, 162 (36.9%) patients had died. Kaplan-Meier analysis revealed that patients with stages 4 and 5 CKD suffered lower cumulative survival than stages 1 and 2 CKD (log-rank test, χ 2 = 11.764, P = 0.003). 缅甸是中国重要的邻国,随着中国的快速发展,缅甸与中国的贸易、以及文化的交流也越来越多,汉语也慢慢地在缅甸地区被更多的人重视,学习汉语的人数也在持续增加。本文主要调查了缅北景栋地区华文教学的师资培训的现状调查,目的是想从中找出景栋地区师资培训存在的问题,并根据问题提出相应的对策。笔者分别从师资培训的内容、培训的形式、培训的方法、培训的时长等方面通过问卷调查法以及访谈法了解到了缅北景栋地区的师资培训。目前采用的培训形式,虽然不是说取得了多好的培训效果,但是对于教师而言,还是有一定的收获。在培训的内容中,让教师懂得应用汉语本体知识来进行教学,也提高了教师的备课以及教学方法。但还是存在着一些突出的问题。缅北景栋地区师资培训所存在的问题：(1)培训内容上,内容较多,缺乏针对性(2)培训形式上,多以讲座方式进行,缺乏互动(3)培训时间短促,学而不足(4)理论与实践相脱节。为了提高景栋地区的汉语教学,我们针对这些问题提出了自己的对策和建议：(1)根据景栋地区华文教学的情况,应制定一套适合景栋地区师资培训的内容,一定要因地制宜。(2)因受训者大部分是属于比较腼腆的类型,所以培训师应了解受训者的心理,找出一套适合受训者的培训形式。(3)应根据培训内容来安排培训时长,以便培训内容和培训时间相一致,让受训者更好的吸收到更多的知识量。(4)培训师应把实践与理论相结合在一起,这样受训者在培训中学到的理论教学,也能应用到实践教学中。

Modern use of Chinese herbal formulae from Shang-Han Lun

Background The Chinese medical archive, Shang-Han Lun, is said to be written by ZHANG Zhong-jing （150-219 A.D.） This great influential work introduced the specific symptoms of six-channel disorders （Tai-Yang, Yang-Ming, Shao-Yang, Tai-Yin, Shao-Yin, and Jue-Yin） and their corresponding treatments, the combined syndromes, deterioration due to malpractice, and the concept of six-channel transitions. The concept of Shang-Han Lun is widely accepted by Chinese herbal doctors. However, no clinical data about Shang-Han symptoms are described in oriental or western medical reports. Methods The clinical prescription data of traditional Chinese medicine visits were extracted under the National Health Insurance in Taiwan. The application rate of 42 Shang-Han formulae in clinical practice was analyzed in detail with the software SPSS. Results Between 1999 and 2002, the prescription rate of Shang-Han formula was only 5.22% among a total of 528 889 576 Chinese herbal formula prescriptions. The most frequently used formula was Tai-Yang formulae （71.31%）, followed by Shao-Yang formulae （17.49%） and the most commonly prescribed individual Shang-Han formulae were Ge-Gen Tang （16.11%）, Shao-Yao-Gan-Cao Tang （12.97%）, Xiao-Qing-Long Tang （11.79%）, Ban-Xia Xie-Xin Tang （10.24%）, and Xiao-Chai-Hu Tang （9.11%）, which comprised 60.22% of the utilization rate of total Shang-Han formulae. Conclusions From the prescription patterns of Shang-Han formulae, there was no evidence of transitions among the six channels. Despite the fundamental role of Shang-Han Lun in traditional Chinese medicine, prescription of Shang-Han formulae was limited in clinical practice.

＜正＞ Diclofenac is reported to have cytotoxic effects and induces apoptosis in various cell lines. This study is conducted to investigate the mechanism of apoptosis in liver of rat treated with diclofenac. Rats were dosed with 3. 5 and 10mg/kg diclofenac in saline for 15 days. Livers were then removed, weighed and processed for histopathological analysis. 10 mg/kg diclofenac after 15 days treatment found to trigger the accumulation of inflammatory cells such as lymphocytes and nuetrophils. This was observed mainly in the centrilobular region. Apoptotic cells were observed following TUNEL assay and were also found to be the hepatocytes at the centrilobular area. This evidence was supported by the changes seen in the mitochondria membrane observed using TEM.
