Mechanical lithotripsy and/or stenting in management of difficult common bile duct stones

BACKGROUND:Mechanical lithotripsy and/or stent insertion is the alternative therapeutic approach in difficult endoscopic retrograde cholangiopancreatography (ERCP)*** study was designed to investigate the appropriate treatment for extraction of bile duct stones in difficult cases of ERCP. METHODS:Between 2000 and 2008,744 ERCP procedures were performed in 592 patients with choledocholithiasis in our endoscopy *** demographic features,and clinical and laboratory findings were collected from a prospectively held *** duct calculi were extracted by basket and/or balloon catheter following ERCP and *** with retained stones were regarded as difficult *** patients were treated with mechanical lithotripsy and those with incomplete clearance of stones underwent stent placement. RESULTS:Two hundred and forty-five patients(41%) were male and 347(59%)were female with a mean age of 58 years(range 19-95 years).Stones were impacted in 27 patients(5%).Stone extraction was performed by basket and/or balloon catheter in 610 ERCP procedures,and lithotripsy was performed in 70 ERCP ***- four patients underwent stent insertion,and 20 underwent stent *** occurred in 39 patients (5%),with no mortality associated with the *** occurred in 9 patients and basket impaction in *** pancreatitis and cholangitis developed in 12 and 11 patients,respectively. CONCLUSION:Difficult cases of bile duct stones can be treated successfully with lithotripsy,and a stent should be applied when the common bile duct cannot be cleared completely.

A Comparison between Premortem and Postmortem Diagnosis in Trauma Cases

This study evaluated the comparison of premortem and postmortem diagnoses of trauma cases that died during treatment and then autopsied. We had analyzed the autopsy reports of forensic deaths retrospectively which occurred between 2013 and 2014. The cases that died because of trauma and which had complete medical reports were included the study. Totally 626 cases were autopsied and 307 cases records were examined from them. The mean age was found to be 28.11. According the treatment period, it was detected that 116 (37.8%) cases died within the first 24 hours. Discrepancies between premortem and postmortem diagnoses were determined in 20.6% of cases. 5.9% had a main diagnostic discrepancy and 14.7% were of second lethal diagnoses. The discrepancies were observed mostly in the multiple injury cases. In our study, diagnostic discrepancy rate was higher in multiple injury cases especially who died by explosion. When clinicians focus on the treatment according to their main diagnosis, they overlook the fatal injuries in other parts of the body. This study shows that autopsy is the most reliable method for the confirmation of the clinical diagnosis in trauma patients. Therefore, particularly surgery physicians should compare the results of the autopsy diagnoses to their own diagnosis.

Changes of total antioxidant capacity and total oxidant status of aqueous humor in diabetes patients and correlations with diabetic retinopathy

AIM:To measure changes of total oxidant status(TOS)and total antioxidant capacity(TAC) of aqueous humor(AH) in diabetic retinopathy(DR) patients,and to determine if there were any differences in TOS and TAC of AH in diabetic patients without retinopathy compared with non-diabetic patients.·METHODS:One hundred and three eyes of 103patients who were enrolled for cataract surgery were included in this *** were grouped according to presence of diabetes and stage of *** to cataract surgery,0.1mL to 0.2mL of AH was aspirated and analyzed for TAC and TOS level using a colorimetric method.·RESULTS:TOS levels were highest among proliferative diabetic retinopathy(PDR) patients and lowest in patients with only *** were statistically significant between all groups(P <0.05).Whereas result between diabetic without retinopathy patients and non-proliferative diabetic retinopathy(NPDR) patients was not statistically significant(P =0.757).TAC levels were highest in patients with only cataract and lowest among PDR patients and results were statistically significant between all groups(P <0.05).·CONCLUSION:Aqueous humor TAC levels are low in diabetic patients and reduced further in DR patients,TOS levels are increased in diabetic patients and this is exacerbated in DR patients.

Comparison of Plegisol and Modified ST Thomas Hospital Cardioplegic Solution in the Development of Ventricular Fibrillation after Declamping of the Aorta

Ventricular fibrillation seen just after declamping of the aorta is an undesirable condition causing myocardial injury. To return to normal rhythm, often internal shocks are applied. But defibrillation itself can also contribute to myocardial injury. So prevention of fibrillation is more important than treatment. 236 patients undergoing coronary artery by-pass surgery were included in this retrospective clinical study. 144 of those patients were operated using modified St. Thomas’ Hospital cardioplegic solution, for stopping the heart. In the other 92 patients, plegisol cardioplegic solution was used. We compared the two groups for the development of ventricular fibrilation after declamping of the aorta. In the modified St. Thomas’ Hospital group, ventricular fibrillation after declamping of the aorta was seen less frequently, this being statistically significant (22.2% vs. 52.2%, p = 0.026). This study shows that the modified ***’ Hospital cardioplegic solution is preferred for avoiding ventricular fibrillation occuring just after declamping of the aorta.

Higher Energy Does Not Mean Better Outcome Following Endovenous Laser Ablation of Great Saphenous Vein

Background: Endovenous laser ablation is a relatively newer alternative to treat great saphenous vein insufficiency. We evaluated the efficiency and safety of treatment endovenous laser procedures on the different saphenous vein diameters with different energy levels. Methods: Data regarding endovenous laser ablation of symptomatic chronic great saphenous venous insufficiency in 209 patients were prospectively recorded. Patients were grouped into two main groups based on their diameters as 5 to 7 millimeters (Group A) or more than 7 millimeters (Group B). Patients in each group was randomized into two groups as >90 J/cm (A1 and B1) or 80-90 J/cm (A2 and B2). Postoperative outcome and complications were recorded during follow-ups at 1st week;1st, 3rd and 6th months to examine the venous reflux and recanalization. Results: Perioperative complaints as pain, cramps and ankle swelling were more commonly observed in A1 group. Fatigue was more common in A2 and B2 groups. No major complications as deep vein thrombosis or skin burns were observed. Conclusions: Endovenous laser ablation is a safe and effective procedure with a high satisfaction rate shortening hospitalization durations and early ambulant activity. Pain, ankle swelling and fatigue are the most common minor complaints in the early postoperative period.

Background： We examined the impact of adjuvant modalities on resected pancreatic and periampullary adenocarcinoma（PAC）.Methods： A total of 563 patients who were curatively resected for PAC were retrospectively analyzed between 2003 and ***： Of 563 patients, 472 received adjuvant chemotherapy（CT） alone, chemoradiotherapy（CRT） alone, and chemoradiotherapy plus chemotherapy（CRT-CT） were analyzed. Of the 472 patients, 231 were given CRT-CT, 26 were given CRT, and 215 were given CT. The median recurrence-free survival（RFS） and overall survival（OS） were 12 and 19 months, respectively. When CT and CRT-CT groups were compared, there was no significant difference with respect to both RFS and OS, and also there was no difference in RFS and OS among CRT-CT, CT and CRT groups. To further investigate the impact of radiation on subgroups, patients were stratified according to lymph node status and resection margins. In node-positive patients, both RFS and OS were significantly longer in CRT-CT than CT. In contrast, there was no significant differencebetween groups when patients with node-negative disease or patients with or without positive surgical margins were ***： Addition of radiation to CT has a survival benefit in patients with node-positive disease following pancreatic resection.

Non-Traumatic Intracranial Heterotopic Ossification: A Case from Turkey

Heterotopic ossification (HO) is the abnormal, non-neoplastic presence of lamellar bone in soft tissue. The ectopic formation of lamellar bone in non-osseus tissues secondary to traumatic injuries of the spinal cord or the brain is defined as Neurogenic HO. The pathophysiology of HO is not clear. But several theories like overactive humoral mechanisms after fracture healing, imbalance of pro-osteoinductive and anti-osteoinductive mediators located on the soft tissues and gene mutations in such as bone morphogenetic proteins-4 (BMP-4) are proposed. Casualty factors leading to increased risk of HO include older age, blast mechanism of injury, location of injury and traumatic brain injury. The aim of this paper is to demonstrate a case of HO located in the brain without history of trauma or any other risk factors.

The Relationships Between Milk Constituents and Various Milk Properties in Anatolian Buffaloes

The objectives of this study were to investigate the relationships among milk composition, renneting time, urea concentration, acidity, density and pH of Anatolian Buffaloes" milk. As a total of 115 individual milk samples from 53 Anatolian buffalo cows that calved in 2004 and 2005 on days of their lactations 30 ± 15, 60 ± 15, 90 ± 15, 120 ± 15, 150 ± 15, 180 ± 15, 210 ± 15, 240 ± 15 and 270 ± 15 in 8 units of Ilikpinar village were collected in morning milkings in June, September, December and March. Samples were analysed for total dry matter (TDM), fat, protein, ash, density, pH, acidity, renneting time and urea content. Data were classified according to the following environmental factors: lactation stages: 1 (30 ± 15, 60 ± 15, 90 ± 15 days): 2 (120 ± 15, 150 ± 15, 180 ± 15 days): 3 (210 ± 15, 240 ± 15, 270 ± 15 days); calving year: 1 (2004), 2 (2005); calving season: 1 (January-May), 2 (September and October); month of samples collection: 1 (June), 2 (September), 3 (December), 4 (March); lactation order: 1 and 2:1, 3 and 4:2, 5 and 6:3. Means and correlation coefficients for the characteristics investigated were calculated. There were negative significant correlations between daily milk yield with TDM, fat and protein percentages, and between pH and all of the milk constituents. Density reduces as TDM, fat and protein contents increase. Relationships between density and coagulation time with milk yield and pH were not significant. Relationships between milk urea concentration with none of milk constituents, milk yield, density, pH and titratable acidity were not significant statistically. It was concluded that genetic selection has to be directed towards increasing fat, protein and total not fat dry matter yields. Under selection programs in which milk yield is taken into consideration, fat and protein yields also increase, but fat and protein concentrations decrease.

Hypertension is a major modifiable cardiovascular risk factor. Hypertension is also recognized as the mostimportant risk factor for global disease burden. It is well established that a sustained reduction in blood pressure by drugs reduces the incidence of cardiovascular morbidity and mortality. In recent years, studies and new guidelines published for the management of hypertension. Awareness, treatment and control of hypertension are very poor, despite the new guidelines. We highlighted the management of hypertension in the light of current literature.
