Rectal prolapse is a circumferential,full-thickness protrusion of the rectum through the *** is a rare condition,and only affects 0.5%of the general *** treatment modalities have been described,which have changed significantly over *** in the last decade,laparoscopic and robotic surgical approaches with different mobilization techniques,combined with medical therapies,have been widely *** patients have presented with a wide range of complaints(ranging from abdominal discomfort to incomplete bowel evacuation,mucus discharge,constipation,diarrhea,and fecal incontinence),understanding the extent of complaints and ruling out differential diagnoses are essential for choosing a tailored surgical *** is crucial to assess these additional symptoms and their severities using preoperative scoring ***,radiological and physiological evaluations may explain some vague symptoms and reveal concomitant pelvic ***,there is no consensus on or standardization of the optimal extent of dissection,type of procedure,and materials used for rectal fixation;this makes providing maximum benefits to patients with minimal complications *** recent publications and systematic reviews have not recommended the most appropriate treatment *** review explains the appropriate diagnostic tools for different conditions and summarizes the current treatment approaches based on existing literature and expert opinions.

Mediastinal neoplasms are rare in the elderly,and clinical suspicion is the first and most important step of differential *** tumors can be misdiagnosed because their symptoms or signs can overlap with cardiovascular diseases,which have a higher prevalence among the older *** diagnostic process should be managed with multimodality imaging and clinical ***,the case of a 74-year-old male patient,who presented with shortness of breath,is examined.A chest X-ray revealed an increased cardiothoracic ratio,and he was diagnosed with hemopericardium following an emergent chest computed *** the echocardiography,it was suspected that a hyperechogenic area adjacent to the heart might be due to a mass,and further examinations confirmed a mediastinal neoplasm.A surgical biopsy was performed,and it was determined to be a mesenchymal *** conclude,clinicians should keep in mind the possibility of paracardiac neoplasm in the elderly,as well as in other age groups,when encountering mediastinal widening so that the patient can be protected from unnecessary interventions such as pericardiocentesis.

An Investigation of Postmortem Urotensin II Receptor Levels in Brain and Kidney Tissues in a Rat Model of Cardiac Ischemia

This study aimed to investigate changes in postmortem urotensin Ⅱ receptor(UTR)levels in brain and kidney tissues in a rat model of cardiac *** rats were divided into two groups:a control group and a cardiac ischemia-induced *** ischemia was created by an intraperitoneal injection of a single lethal dose of isoproterenol(ISO;850 mg/kg).Plasma UT,blood urea nitrogen,and creatinine levels were determined 0 h *** and kidney UTR mRNA expression levels were determined 0,1,3,6,12,24,4&and 72 h *** histopathological appearance of brain and kidney tissues was also *** UT and plasma creatinine levels were increased in the cardiac ischemia-induced group as compared with those in the control group(P<0.001).Ischemia resulted in histopathological changes in brain and cerebellum *** morphological evaluation revealed Purkinje cell degeneration(P=0.037)and dark neurons(P=0.004).The UTR expression level decreased after 1 h postmortem in the brain and after 3 h postmortem in the kidneys in the cardiac ischemia-induced group as compared with that in the control group(P<0.001).The observed changes in UTR expression levels may be valuable in clinical practice in the field of forensic *** changes may be used as a marker in postmortem evaluations of sudden death caused by ischemia-induced cardiac shock.

Gallbladder function and dynamics of bile flow in asymptomatic gallstone disease

AIM： To investigate the effects of gallbladder stones on motor functions of the gallbladder and the dynamics of bile flow in asymptomatic gallstone disease. METHODS： Quantitative hepatobiliary scintigraphy was performed to detect the parameters of gallbladder motor function [gallbladder ejection fraction （GBEF）, gallbladder visualization time （GBVT）, gallbladder time to peak activity （GBTmax）, gallbladder half emptying time （GBT1/2）, and transit time of bile to duodenum （TTBD）] in 24 patients with asymptomatic cholelithiasis who were diagnosed incidentally during routine abdominal ultrasonographic examination and 20 healthy subjects with normal gallbladder. RESULTS： Even though there was no significant difference in the clinical and laboratory parameters between the patient and control groups, all parameters of gallbladder function except TTBD were found to differ significantly between the two groups. GBEF in the patient group was decreased （P = 0.000） and GBVT, GBT GBT1/2 in the patient group were longer （P = 0.000, P = 0.015, P = 0.001, respectively）.there were not any clinical and laboratory findings, gallbladder filling and emptying could be impaired in patients with gallstone disease.

1 Introduction Nigella sativa, known as black seed, has analgesic, anti-inflammatory, antioxidant and anticancer effects. It has been shown to reduce the development of kidney failure when given prior to the use of nephrotoxic drugs particularly due to its antioxidant action. However, as far as the authors could ascertain, there is no human study in literature showing these effects. Here we present a case of acute renal failure after the use of N. sativa, rather than exhibiting antioxidant or antidiabetic effects.

Background：Obstructive sleep apnea syndrome （OSAS） is a disease with increasing prevalence,which is mainly characterized by increased cardiopulmonary mortality and *** is well-known that OSAS patients have increased prevalence of cardiovascular diseases including coronary heart disease,heart failure,and *** aim of this study was to evaluate the presence of prolonged and fragmented QRS complexes,which have previously been associated with cardiovascular mortality,in OSAS ***：Our study included 51 patients （mean age 41.6 ± 10.1 years） who were recently diagnosed with OSAS （apnea-hypopnea index [AHI] ≥5 events/h） and never received *** control group consisted of 34 volunteers （mean age 43.1 ± 11.6 years） in whom OSAS was excluded （AHI 〈5 events/h）.The longest QRS complexes was measured in the 12-lead electrocardiogram （ECG） and the presence of fragmentation in QRS complexes was ***：Fragmented QRS frequency was significantly higher in patients with OSAS （n =31 [61%] vs.n =12 [35%],P =0.021）.QRS and QTc durations were also significantly longer in OSAS patients than controls （99.8 ± 13.9 ms vs.84.7 ± 14.3 ms,P 〈 0.001;411.4 ± 26.9 ms vs.390.1 ± 32.2 ms,P =0.001,respectively）.Analysis of the patient and controls groups combined revealed a weak-moderate correlation between AHI and QRS duration （r =0.292,P =0.070）.OSAS group had no correlation between AHI and QRS duration （r =-0.231,P =0.203）.Conclusions：In our study fragmented QRS frequency and QRS duration were found to increase in OSAS *** parameters are related with increased cardiovascular *** the prognostic importance of ECG parameters,it may be reasonable to recommend more detailed evaluation of OSAS patients with fragmented or prolonged QRS complexes with respect to presence of cardiovascular diseases.
