BACKGROUND： Nerve root avulsion is a frequent finding in patients with brachial plexus injury following road traffic accidents or as a result of severe arm traction during complicated deliveries. This injury constitutes a challenging clinical and surgical problem. The morphological characteristics of motoneurons after nerve root avulsion deserve further analysis. OBJECTIVE： To study the different morphological changes of α-motoneurons under light and electron microscopy after C8 spinal ventral rootlets avulsion and transection at various stages. DESIGN： Controlled animal study. SETTING： Department of Anatomy, King Faisal University. MATERIALS： The experiment was carried out at the Department of Anatomy, College of Medicine, King Faisal University between January 2005 and March 2006. Six adult Sprague Dawley rats weighing 200-350 g, irrespective of gender, were used for this study. The animals were bred at the animal house, College of Medicine, King Faisal University, and fed on rat maintenance diet. Water and standard diet were supplied ad libitum. Animal interventions were carried out according to animal ethical standards. METHODS： Three animals were randomly chosen for avulsion of the right ventral rootlets of C8 spinal nerves. The other three received transection of the right ventral rootlets of C8 spinal nerves. ①Avulsion experiment： After rats were anesthetized, the right ventral rootlets of C8 spinal nerves were identified. The ventral rootlets were avulsed from the spinal cord by traction with a fine hook （Fine Science Tools Inc., No. 10031-13, Germany）. Traction was exerted in a direction parallel to the course of the spinal root. Under the operating microscope, the C8 segment was exactly located. After checking the successfulness of the surgical procedure, the C8 segment was separated from the spinal cord. AIM:To investigate the impact of niosome nanoparticles carrying umbelliprenin(UMB),an anti-angiogenic and anti-inflammatory plant compound,on the expression of vascular endothelial growth factor(VEGF-A)and connective tissue growth factor(CTGF)genes in a human retinal pigment epithelium(RPE)-like retina-derived cell ***:UMB-containing niosomes were created,optimized,and ***-like cells were treated with free UMB and UMB-containing *** IC_(50)values of the treatments were determined using an MTT *** expression of VEGF-A and CTGF was evaluated using real-time polymerase chain reaction after RNA extraction and cDNA ***’characteristics,including drug entrapment efficiency,size,dispersion index,and zeta potential were *** UMB had an IC_(50)of 96.2μg/mL,while UMB-containing niosomes had an IC_(50)of 25μg/***:Treatment with UMB-containing niosomes and free UMB resulted in a significant reduction in VEGF-A expression compared to control cells(P=0.001).Additionally,UMB-containing niosomes demonstrated a significant reduction in CTGF expression compared to control cells(P=0.05).However,there was no significant reduction in the expression of both genes in cells treated with free ***:Both free UMB and niosome-encapsulated UMB inhibits VEGF-A and CTGF genes ***,the latter demonstrates significantly greater efficacy,potentially due to the lower UMB dosage and gradual *** findings have implications for anti-angiogenesis therapeutic approaches targeting age-related macular degeneration.

AIM:To determine whether the levator palpebrae superioris(LPS)/superior rectus(SR)muscle complex,can influence the position of the upper lid and fornix in acquired anophthalmic ***:This comparative non-randomized and non-interventional study included retrospective data of 21 patients with unilateral acquired anophthalmic sockets repaired with spheric ***-resolution computed tomography(CT)measurements of the LPM/SR muscle complex and clinical topographic position of the upper lid,superior and inferior fornix depth in primary gaze position were *** data were presented as frequency and percentage proportions and quantitative variables comparing the socket measurements with the normal contralateral orbit was statistically analyzed using non-parametric tests considering Py shorter LPS length(P=0.01)and significantly thicker SR(P=0.02)than the normal *** was present in anophthalmic sockets but not in normal orbits(P=0.002),while levator function was normal in both(P>0.05,all comparisons).The superior fornix depth was similar in the anophthalmic socket and the contralateral normal orbit(P=0.192)as well the inferior fornix depth(P=0.351).CONCLUSION:Acquired anophthalmic sockets repaired with spheric implants have shorter LPS,thicker SR,and more lagophthalmos than normal *** relationship of the LPS and SR with other orbital structures,associated with passive or active forces acting in the final position of the lids and external ocular prosthesis should be further investigated.

目的:在真实世界的前瞻性队列研究中观察心内科门诊“极高危ASCVD”患者在治疗起始阶段采用联合降脂策略的疗效与安全性。方法:连续入组2021年1月至6月在湘雅医院心内科高脂血症专病门诊就诊的“极高危ASCVD”患者,根据其实际降脂策略分为三组:阿托伐他汀单药组;阿托伐他汀联合依折麦布组;阿托伐他汀联合依洛尤单抗组。主要观察终点为治疗一个月后低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]、非-HDL-C变化,次要终点为总胆固醇(TC)、甘油三酯(TG)、LDL-C、超敏C反应蛋白(hs-CRP)变化及安全性指标。结果:无论哪种联合方式,在“极高危ASCVD”患者降脂起始阶段联合降脂治疗的疗效均显著优于阿托伐他汀单药组:LDL-C、Log[Lp(a)]、非-HDL-C、TC的降幅更大,差异均有统计学意义(均P0.05)。以“LDL-C<1.4 mmol/L或<1.8 mmol/L”作为降脂达标的标准时,两组联合降脂治疗的LDL-C达标率均高于阿托伐他汀单药组,差异均有统计学意义(均P<0.05);阿托伐他汀联合依洛尤单抗组的LDL-C达标率高于阿托伐他汀联合依折麦布组,差异均有统计学意义(均P<0.05)。单独或联合“LDL下降超过50%”的降幅为达标标准,三组均无人能达标。治疗后三组间的肝脏转氨酶水平变化差异均无统计学意义(均P<0.05);三组间心肌酶同工酶(CK-MB)均下降,但差异无统计学意义(P<0.05)。与阿托伐他汀单药组相比,两联合降脂组治疗组的血糖均下降,差异具有统计学意义(P<0.05);阿托伐他汀联合依折麦布组的血糖比阿托伐他汀联合依洛尤单抗组的降幅更大,差异有统计学意义(P<0.05)。结论:起始联合降脂治疗1个月对心内科门诊“极高危ASCVD”患者的降脂效果及LDL-C的达标率优于阿托伐他汀单药组。以LDL-C<1.4 mmol/L或<1.8 mmol/L为降脂达标目标时,阿托伐他汀联合依洛尤单抗组治疗1个月后LDL-C达标率高于联合依折麦布组;以“LDL下降超过50%”的降幅作为降脂达标标准时,1个月内很难达标。门诊“极高危ASCVD”患者采用起始联合的降脂治疗1个月无不良反应。起始联合降脂策略可用于心内科门诊“极高危ASCVD”患者中LDL-C数值需早期达标的人群,阿托伐他汀联合依洛尤单抗可用于需在1个月内LDL-C<1.4 mmol/L或<1.8 mmol/L的人群。

Numerical Study of Hydrogen Peroxide Thermal Decomposition in a Shock Tube

Hydrogen peroxide(H_2O_2) has its significance during the combustion of heavy hydrocarbons in the internal combustion(IC) engines. Owing to its importance the measurements of H_2O_2 dissociation rate have been reported mostly using the shock tube apparatus. These types of experimental measurements are although quite reliable but require high cost. On the other hand, numerical simulations provide low cost and reliable solutions especially using computation fluid dynamics(CFD) software. In the current study an experimental shock tube flow is modeled using open access platform OpenFOAM to investigate the thermal decomposition of H_2O_2. Using two different convective schemes, limited Linear and upwind, the propagation of shock wave and resultant dissociation reaction are simulated. The results of the simulations are compared with the experimental data. It is observed that the rate constant measured using the simulation data deviates from the experimental results in the low temperature range and approaches the experimental values as the temperature is raised.

Background: Juvenile idiopathic arthritis (JIA) is the most common rheumatic disease in children. Both the humoral and cell mediated immunities are involved in the pathogenesis of JIA. It is reported that overall immunoglobulin levels in JIA patients are significantly higher than their control during the active state of disease. Methodology: This prospective observational study was conducted over a period of 18 months All the newly diagnosed oligo-articular and poly-articular JIA patients having active disease were included by purposive sampling. Data were collected by a semi-structured predesigned questionnaire. Result: Most of the study subjects (57.6%) belonged to age group > 3 - 9 years. Oligo JIA was diagnosed in 66.7% and poly JIA in 33.3% of JIA children. The difference in mean (±SD) ESR (33.52 ± 21.29 and 15.09 ± 7.71 mm in 1st hour) at active and inactive states was highly significant. Mean (±SD) difference of IgG, IgM and IgA in active and inactive states of disease were highly significant. Conclusion: Higher and abnormal levels of immunoglobulin (IgG, IgM, and IgA) were present among JIA patients in active disease state which became normal during inactive disease state after treatment.
