A series of site-directed mutants of the ferredoxin-dependent spinach nitrite reductase has been characterized and several amino acids have been identified that appear to be involved in the interaction of the enzyme with ferredoxin. In a complementary study, binding constants to nitrite reductase and steady-state kinetic parameters of site-directed mutants of ferredoxin were determined in an attempt to identify ferredoxin amino acids involved in the interaction with nitrite reductase. The results have been interpreted in terms of an in-silico docking model for the 1：1 complex of ferredoxin with nitrite reductase.

Background:A colonoscopy can detect colorectal diseases,including cancers,polyps,and inflammatory bowel diseases.A computer-aided diagnosis(CAD)system using deep convolutional neural networks(CNNs)that can recognize anatomical locations during a colonoscopy could efficiently assist *** aimed to construct a CAD system using a CNN to distinguish colorectal images from parts of the cecum,ascending colon,transverse colon,descending colon,sigmoid colon,and ***:We constructed a CNN by training of 9,995 colonoscopy images and tested its performance by 5,121 independent colonoscopy images that were categorized according to seven anatomical locations:the terminal ileum,the cecum,ascending colon to transverse colon,descending colon to sigmoid colon,the rectum,the anus,and indistinguishable *** examined images taken during total colonoscopy performed between January 2017 and November 2017 at a single *** evaluated the concordance between the diagnosis by endoscopists and those by the *** main outcomes of the study were the sensitivity and specificity of the CNN for the anatomical categorization of colonoscopy ***:The constructed CNN recognized anatomical locations of colonoscopy images with the following areas under the curves:0.979 for the terminal ileum;0.940 for the cecum;0.875 for ascending colon to transverse colon;0.846 for descending colon to sigmoid colon;0.835 for the rectum;and 0.992 for the *** the test process,the CNN system correctly recognized 66.6%of ***:We constructed the new CNN system with clinically relevant performance for recognizing anatomical locations of colonoscopy images,which is the first step in constructing a CAD system that will support us during colonoscopy and provide an assurance of the quality of the colonoscopy procedure.

To investigate the factors influenced the efficacy of tolvaptan (TLV) in liver cirrhosis. METHODSWe retrospectively enrolled 61 consecutive patients with refractory hepatic ascites. All of them had been treated with furosemide and spironolactone before admission, and treated with TLV for 7 d in our hospital. The effect of TLV was defined by the rate of body weight loss, and the factors that influenced TLV efficacy were analyzed using multiple regression. RESULTSCoexistent hepatocellular carcinoma (HCC) was the only significant predictive variable that attenuated the efficacy of TLV. In stratified analysis, high doses of furosemide decreased the efficacy of TLV in patients with HCC, and increased efficacy in those without HCC. In the latter, a high Child-Pugh-Turcotte score had a positive influence and a high concentration of lactate dehydrogenase had a negative influence on the effectiveness of TLV. CONCLUSIONDevelopment of ascites may differ between patients with liver failure and those with HCC progression. A sufficient preceding dose of furosemide decreases diuretic effect of TLV.

General Trust (GT), defined as a default expec-tation of other people’s trustworthiness, is assumed to be a predictor for promotion of health and welfare in individuals as well as for strengthening of social capital in the community. An improvement of health and quality of life of the elderly is recently a crucial agenda. Thus, the purpose of the study was to explore which factors regarding health and life associated with GT among frail elderly people living at homes in Japanese rural area. The study selected the subjects who were designated within three mildest degrees in seven stages of long-term care levels and who met at an item and more in the national basic check-list. 209 participants were interviewed by trained health personnel using a structured questionnaire. GT was measured by a dichotomous outcome of inquiring “do you trust people in general?”. In the univariable analysis, educational status (p = 0.004), activity competence index including instrumental activity of daily living (IADL) (p = 0.020), Mini-Mental State Examination (MMSE) (p = 0.029) and Self-rating Depression Scale (SDS) (p = 0.010) were significantly related to GT. By logistic regression analysis using a stepwise method with a like-lihood ratio, educational status alone was sig-nificantly associate with GT (p = 0.010, odds ratio = 1.195 [95% confidence interval;1.043 - 1.371]). Health related factors had nothing to do with GT. Our finding suggested that the higher educated elderly might have had more opportunities to encounter the others and more indispensability to deliberately discern their trustworthiness than the lower, and consequently had higher GT through social intelligence being strengthened.

Efficient Cooling System Using Electrocaloric Effect

The electrocaloric effect in thin films of an electrocaloric material has the potential to be used for efficient cooling systems for high power electronic devices. We numerically calculated the effect of parameters in electrocaloric refrigeration with a thermal switch of fluid motion on the thermal performance. The system of changing air and water flow with the pulse generation of cold energy increased the heat transfer efficiency to 67% at a frequency of 5 Hz. The optimum time delay of water flow to increase the heat transfer efficiency was zero at low frequency and became half of the time period to change heat for a high frequency of 100 Hz. When the heat transfer efficiency was high, the final temperature change in water flow was not the maximum temperature change.

Heat Transfer and Flow Analysis in Loop Heat Pipe with Multiple Evaporators Using Network Model

Thermal performance of a loop heat pipe with two evaporators and two condensers was examined using a lumped network model analysis. Thermosyphon-type vertical loop heat pipe and capillary-pump-type horizontal loop heat pipe were calculated by examining the change of heating rate of two evaporators. Calculation results showed that the vapor and liquid flow rates in the loop heat pipe and the thermal conductance of the heat pipe changed significantly depending on the distribution ratio of the heating rate of the multiple evaporators. The thermal performance of the vertical loop heat pipe with two evaporators was also examined and experimental results of flow direction and thermal conductance of the heat pipe agreed with the analytical results. The lumped network model analysis is therefore considered accurate and preferable for the practical design of a loop heat pipe with multiple evaporators.

BACKGROUND Postoperative delayed bleeding(PDB) after gastric endoscopic submucosal dissection(ESD) is the most common adverse event in patients receiving antithrombotics even with second-look endoscopy. Moreover, with the increasing prevalence of cardiovascular and cerebrovascular diseases in an aging population with associated lifestyle-related diseases, an increasing number of patients receive antithrombotics. Several attempts have been made to prevent PDB in aging population;however, a consensus has yet to be *** To examine the efficacy of third-look endoscopy(TLE) for PDB *** One hundred patients with early gastric neoplasms receiving antithrombotics were prospectively enrolled and subjected to ESD with TLE between February 2017 and July 2019. The primary endpoint was PDB rate, which was compared with our preset threshold. Furthermore, we divided the bleeding period into early-and late-onset PDB(E-PDB and L-PDB, respectively) and analyzed its rate. As a secondary analysis, we compared PDB rates with those of a historical control group, using propensity score matching, and calculated the PDB rates per antithrombotic agent use in each *** In total, 96 patients and 114 specimens were finally evaluated. The overall PDB rate was 7.9%(9/114) [90%CI: 4.7-13.1, P = 0.005], while the late-and early-onset PDB rates(L-PDB and E-PDB) were 5.3% [90%CI: 2.7-9.9, P < 0.0001] and 2.6% [90%CI: 1.1-6.4, P = 0.51], respectively. Propensity score matching generated 58 matched pairs for TLE and control groups. No differences were found in overall PDB incidence(10.3% vs 20.7%, P = 0.12), whereas L-PDB occurrence significantly differed(5.2% vs 17.2%, P = 0.04) between groups. Considering antithrombotics' use, the overall PDB rate was higher for direct oral anticoagulants and multiple antithrombotics in the control group, while L-PDB incidence was lower in the TLE group for these agents(8.7% vs 23.1% and 5.0% vs 29.4%, respectively).CONCLUSION TLE for g
