Cancer frequently develops resistance to the majority of chemotherapy treatments.This study aimed to examine the synergistic cytotoxic and antitumor effects of SGLT2 inhibitors,specifically Canagliflozin(CAN),Dapagliflozin(DAP),Empagliflozin(EMP),and Doxorubicin(DOX),using in vitro experimentation.The precise combination of CAN+DOX has been found to greatly enhance the cytotoxic effects of doxorubicin(DOX)in MCF-7 cells.Interestingly,it was shown that cancer cells exhibit an increased demand for glucose and ATP in order to support their growth.Notably,when these medications were combined with DOX,there was a considerable inhibition of glucose consumption,as well as reductions in intracellular ATP and lactate levels.Moreover,this effect was found to be dependent on the dosages of the drugs.In addition to effectively inhibiting the cell cycle,the combination of CAN+DOX induces substantial modifications in both cell cycle and apoptotic gene expression.This work represents the initial report on the beneficial impact of SGLT2 inhibitor medications,namely CAN,DAP,and EMP,on the responsiveness to the anticancer properties of DOX.The underlying molecular mechanisms potentially involve the suppression of the function of SGLT2.

Incidence, Clinical Presentation and Angiographic Findings among Patients with Coronary Artery Ectasia in Gaza

Background: Coronary artery ectasia (CAE) is a rare form of aneurysmal coronary heart disease. This condition increases risk of ischemia that leaded to stable angina pectoris and also acute coronary syndrome. Objective: To evaluate the incidence of CAE and to analyze clinical and angiographic characteristics among patients with symptomatic coronary artery disease. Methods: Retrospective trial of diagnostic coronary angiography which was performed at alshifa hospital in Gaza, included all patients with coronary heart disease and underwent diagnostic coronary angiography from March 2014 to March 2023. We evaluate the incidence, clinical presentation and angiographic findings of patients with symptomatic coronary ectasia. Results: Of the 12,534 angiograms analyzed, CAE was found in 539 patients, an incidence of 4.3%. The mean age was 59.2 ± 11.7 years, and male gender predominate 81.8%. A 285 (52.9%) of patients found to have single ectatic vessel at their angiographic results. In two vessels 120 patients (22.3%), in three vessels 95 patients (17.6%) and in three vessel plus left main in 39 patients (7.2%). 70.9% angiograms showed pure ectasia without associated significant stenotic lesions in the same vessel. The right coronary artery (RCA) was the commonest affected vessel by ectasia. The most common presentation of patients is non ST segment elevation acute coronary syndrome. Conclusion: The incidence of coronary ectasia was 4.3%. Non ST segment elevation acute coronary syndrome was the most common clinical presenting. Only 30% of patients have additionally significant coronary artery stenosis. The majority of patients had single vessel involvement and right coronary artery was the most common involved vessel. Despite a dapper understanding of CAE in last years, there are still come critical issues about optimal treatment. Large multicenter randomized control trials are recommended to guide the clinician in the management of this complex sitting of patients.

Background: Early excision and grafting has been the preferred method of managing major burns around the world since 1970. Considering the advances in health care and the development of new antibiotics over the past 50 years, delayed grafting as a technique for the management of burns over 15%-20% of total body surface area (TBSA) could have comparable results to that of early excision. This study aims to highlight the outcomes of practicing delayed grafting in burn patients. Methods: A case series analysis was performed of 51 patients who were admitted to the burns unit in Sultan Qaboos Hospital Salalah with over 20% TBSA between January 2014 and December 2019. The patients received prophylactic antibiotics and silver sulphadiazine dressing until the burn eschar had completely separated, followed by grafting. Results: Two patients were lost during the entire duration of the study. The mortality rate was comparable to that of early excision, while the rate of hypertrophic scarring was lower than the range reported by other studies. Conclusion: In the management of patients with over 20% TBSA, delayed grafting after complete separation of eschar is still a valid technique.

BACKGROUND Neonatal sepsis,a formidable threat to newborns,is a leading cause of neonatal mortality,with late-onset sepsis manifesting after 72 hours post-birth being particularly concerning.Pneumonia,a prevalent sepsis presentation,poses a significant risk,especially during the neonatal phase when lung defenses are compromised.Accurate diagnosis of pneumonia is imperative for timely and effective interventions.Saliva,a minimally invasive diagnostic medium,holds great promise for evaluating infections,especially in infants.AIM To investigate the potential of serum C-reactive protein(CRP),salivary CRP(sCRP),and mean platelet volume(MPV)as diagnostic markers for late-onset neonatal pneumonia(LONP).METHODS Eighty full-term neonates were systematically examined,considering anthropometric measurements,clinical manifestations,radiology findings,and essential biomarkers,including serum CRP,sCRP,and MPV.RESULTS The study reveals noteworthy distinctions in serum CRP levels,MPV,and the serum CRP/MPV ratio between neonates with LONP and healthy controls.MPV exhibited a robust discriminatory ability[area under the curve(AUC)=0.87]with high sensitivity and specificity at a cutoff value of>8.8.Correlations between serum CRP,sCRP,and MPV were also identified.Notably,sCRP demonstrated excellent predictive value for serum CRP levels(AUC=0.89),underscoring its potential as a diagnostic tool.CONCLUSION This study underscores the diagnostic promise of salivary and serum biomarkers,specifically MPV and CRP,in identifying and predicting LONP among neonates.These findings advocate for further research to validate their clinical utility in larger neonatal cohorts.

Combinations of metal and lanthanide oxides have been done through casted films for potential medical applications. In this regard, samarium(Ⅲ) oxide/chromium(Ⅲ) oxide/graphene oxide(GO)/polycaprolactone(PCL) based films nano-composites(NCs) were fabricated, pointing their utilization as a biological scaffold for wound dressing purposes. also, samarium(Ⅲ) oxide and chromium(Ⅲ) oxide have been merged as promising optical constituents due to their unique optical behavior. The structural and compositional examination of the studied NCs was executed by X-ray diffraction(XRD), Raman, and field emission scanning electron microscopy(FESEM). The Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL NC exhibits a surface with a lower roughness degree owing to the presence of GO. Cr_(2)O_(3)shows size reduction upon GO insertion to reach 1.2 μm as the average grain size, whilst Sm_(2)O_(3)records an average grain size of less than 1 μm. As well, the polymeric nano-compositions exhibit variation in contact angle values that hit 29.76°± 3.52°for Sm_(2)O_(3)/PCL, and 48.62°± 1.37°for Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL as the second lowest contact angle. The optical behavior contributes to absorption edge relocation along the x-axis from 1.7 eV for pure PCL, to 2.65 eV for Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL. Regarding biological responses, the cell exposed to 2.5 μg/m L of Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL shows cell viability of 119.31%, while 5 μg/m L hits 99.6%. Additionally, the resulting cell attachment micrographs show layers of fibroblast tissue, besides the proliferation and growth of cultivated cells. Thus, the Sm_(2)O_(3)/Cr_(2)O_(3)/GO/PCL scaffold provides 3D proliferation of fibroblast cells endorsing the wound healing process.

The goal of this research is to develop a new,simplified analytical method known as the ARA-residue power series method for obtaining exact-approximate solutions employing Caputo type fractional partial differential equations(PDEs)with variable coefficient.ARA-transform is a robust and highly flexible generalization that unifies several existing transforms.The key concept behind this method is to create approximate series outcomes by implementing the ARA-transform and Taylor’s expansion.The process of finding approximations for dynamical fractional-order PDEs is challenging,but the ARA-residual power series technique magnifies this challenge by articulating the solution in a series pattern and then determining the series coefficients by employing the residual component and the limit at infinity concepts.This approach is effective and useful for solving a massive class of fractional-order PDEs.Five appealing implementations are taken into consideration to demonstrate the effectiveness of the projected technique in creating solitary series findings for the governing equations with variable coefficients.Additionally,several visualizations are drawn for different fractional-order values.Besides that,the estimated findings by the proposed technique are in close agreement with the exact outcomes.Finally,statistical analyses further validate the efficacy,dependability and steady interconnectivity of the suggested ARA-residue power series approach.

Mechanical performance of cold mix asphalt containing cup lump rubber as a sustainable bio-modifier

The road construction industry aims to contribute to the protection of already compromised environment. Cold mix asphalt(CMA) is a measure initiated by the road industry to protect the environment and preserve energy. Despite having additional benefits, CMA has attracted little attention due to its inferior performance. CMA's performance is enhanced using a sustainable binder bio-modifier, natural cup lump rubber(CLR) is one of them. This study evaluated the tensile properties, rutting, moisture susceptibility, and adhesion properties of CLR-modified CMA(CMA-CR). The tensile property was enhanced by 26% due to CLR modification. CMA-CR had excellent rutting resistance of less than 2 mm rut depth at 10,000 load cycles, showing 70% improvement compared with conventional CMA.Moisture susceptibility evaluation indicated that CMA-CR had tensile strength ratio(TSR)value of 104%, satisfying the minimum 80% requirement of AASHTO T283. It also retained more than 96% bitumen coating. The moisture damage resistance was improved by 12% and 10% in terms of TSR and stripping, respectively. The durability results revealed that the CMA-CR mixture prevented higher mass loss, representing 14% improvement compared with conventional CMA.

Common bile duct(CBD)stones are a common biliary tract disease.For asymp-tomatic CBD stones,stone removal by endoscopic retrograde cholangiopancreato-graphy(ERCP)is recommended in available guidelines.Because asymptomatic CBD stones is a benign disease with no noticeable symptoms,the risk vs benefit strategy should be thoroughly considered before performing ERCP in these patients.Clinical care review,technical aspects of the procedure,and patient preferences should also be considered.

Comparative analysis on the performance of different types of input-and command-shaping controllers in minimizing payload residual vibration of an overhead crane with an inclined supporting track

Reducing the effects of external disturbance on overhead crane systems is crucial,as they can impair the controller performance and cause excessive vibrations or oscillations of the payloads.One such external disturbance is the inclination of the supporting track of the crane trolley,which causes the system dynamics model to change.An open-loop control strategy is widely utilized to control the payload sway motion and generally does not require any alterations in the physical structure of a system or the installation of sensors and/or actuators.Input and command shaping are two common open-loop control techniques applied to control overhead cranes.In this paper,the effect of moving an overhead crane system along an inclined supporting track is investigated.In addition,the ability of different types of input-and command-shaping control schemes to suppress the residual vibrations due to trolley track inclination is demonstrated.Two types of input-shaping controllers,which are double-step,zero vibration,and one command waveform(WF)shaper based on a trigonometric function,are used and tested.A linear equation of motion of the overhead crane resting on an inclined surface is developed to simulate the overhead crane and payload motion.The effectiveness of the different types of open-loop controllers to suppress residual vibrations is verified by both simulation and experimental results.In addition,a new WF command shaper is proposed and designed to overcome track inclination while eliminating payload residual vibration.A comprehensive comparative analysis,both numerically and experimentally,is performed on the new proposed shaper to measure its effectiveness in handling inclination when compared to other types of open-loop controllers.The new shaper outperforms other controllers in eliminating payload residual vibration for a wider range of inclination angles.
