Objective: To explore the analgesic effects and uterine hemodynamics of perpendicular needling(PN)and transverse needling(TN) at SP 6 in patients with primary dysmenorrhea(PD).Methods: In this randomized controlled trial, patients with PD diagnosed with cold-dampness congealing pattern were randomly assigned in a ratio of 1:1 to receive PN or TN at bilateral SP 6 for 10 *** was performed when the menstrual pain score was over 40 mm on the first day of menstruation, as measured using the visual analog scale for pain(VAS-P). The primary outcome was average menstrual pain(VAS-P). Secondary outcomes included the pulsatility index(PI), resistance index(RI), and systolic-diastolic peaks ratio(S/D) in uterine arteries as measured using color Doppler ultrasonography;anxiety as assessed using the Hamilton Anxiety Rating Scale(HAMA), blood pressure(BP),and heart rate(HR).Results: Forty-eight patients completed the study. The TN group exhibited a significant reduction in VAS-P scores(-5.71 mm, 95% confidence interval(CI): -8.78, -2.63, P =.001), RI values(-0.05, 95% CI:-0.09, -0.01, P=.015), and HAMA values(-2.50, 95% CI: -4.78, -0.22, P=.032) when compared with the PN group. No significant differences in PI, S/D, BP, or HR values were observed between the two groups(P >.05).Conclusion: TN at SP 6 was superior to PN in alleviating menstrual pain and anxiety in patients with *** analgesic effect of TN may be due to its better ability to improve uterine arterial blood flow via decreases in RI values.

背景:黄褐斑是面部色素沉着性疾病,是一种常见的获得性、对称性棕色皮肤色素沉着性疾病。该病常见于基因易感人群患者的面颊部和颈项部。与颈项部相比,面颊部更容易出现黄褐斑,色素沉着最常见于前额、颧骨、上唇和下颌部。根据色素沉着区域,黄褐斑可以分为面中部黄褐斑,颧骨处黄褐斑以及下颌处黄褐斑三种类型。女性和男性均易发病,但女性发病机率较男性更高,新病人中有90%均为女性。黄褐斑的发病率在妊娠期增高,女性第二次或第三次妊娠晚期更为常见,口服避孕药女性也易发病。据报道,黄褐斑发病率一般在1.5%33.3%之间,妊娠期发病率可高达50%70%。遗传因素和日晒是导致黄褐斑最重要的因素。此外,妊娠、口服避孕药、服用雌激素、孕酮类药物、甲状腺机能亢进、甲状腺机能减退、过度使用化妆品、压力、吸烟、光毒性和抗惊厥药物等是造成黄褐斑发病的其他危险因素。中医学认为,冲任失和,气血失调,脏腑功能失调均可导致黄褐斑。气血升降失调及气血不足均可使面部和颈项部失于濡养,进而出现色素沉着,产生黄褐斑。黄褐斑的常见中医证型主要有四种:肝气郁滞、脾虚、肾虚和气滞血瘀。目前,黄褐斑有多种治疗方案,包括对苯二酚、壬二酸、维生素A、曲酸、类固醇、化学剥脱剂等药物治疗和近年来常用的激光治疗。然而,这些疗法治疗黄褐斑的疗效并不乐观,黄褐斑的治疗仍然是皮肤科医生的一大挑战。针刺是中医治疗黄褐斑的一种安全面普遍的方法。目前,越来越多的针刺治疗黄褐斑的临床随机对照试验得以开展以探寻更有效的方法。但多数试验在某些环节都存在或多或少的方法学缺陷或不足,如是否真正随机分组,随机分配序列在实施分配期间是否进行了隐藏,是否做到盲法,是否考虑了结果的意向性统计分析等。因此,开展更多高质量的临床研究才能为针刺治疗黄褐斑提供更可靠的证据,从而影响西医皮肤科医生考虑选择将针刺作为黄褐斑治疗的一种方法。本研究的主要目的是评估针刺治疗黄褐斑的疗效。目的:评价针刺治疗黄褐斑的疗效。方法:试验设计:该试验性随机对照临床试验是由中国北京中医药大学针灸推拿学院设计和指导,于2017年3月至2018年3月在伊朗医科大学附属德黑兰拉齐皮肤病医院(Razi Dermatologic Hospital)实施完成。受试者:该试验共纳入53名受试者,均为于德黑兰拉齐皮肤病医院寻求治疗的女性黄褐斑患者。患者的色素沉着主要发生在面颊、前额和下颌处。经初筛评估后,符合全部纳入标准者则被纳入本试验研究;如果符合任何一个排除标准,患者将被排除在外。纳入标准包括:女性年龄应在18至60岁之间,面部有黄褐斑并渴望参与本试验研究,并签署知情同意书。排除标准包括:在过去三个月以内,使用过药物、激光、或其他任何黄褐斑治疗手段的女性患者,自发性或家族遗传性白癜风患者,其他色素沉着性皮肤病患者,妊娠期、哺乳期、口服避孕药患者,对苯二酚过敏史患者,治疗前3个月有过日光浴,以及不愿参与该试验者。该试验遵循伊朗德黑兰医科大学人类研究伦理委员会声明和批准的伦理原则。所有的受试者在试验开始之前应当签署知情同意书并且有权力在试验的任何阶段退出试验。随机:由未参与该试验研究的统计学家使用R软件3.3.3版本软件包确定随机序列号,53例纳入患者被随机分为两组:针刺组26例;4%对苯二酚组(对照组)27例。干预措施:针刺组接受针刺疗法,针刺频率为一周三次,连续针刺4周,共治疗12次。所选腧穴包括局部穴位(黄褐斑所在部位)和远端穴位。远端穴位包括足三里、三阴交、血海和曲池穴,运用平补平泻针法。此外,根据中医辨证分型对每位受试者选穴酌情加减,主要包括:肝气郁滞型,运用泻法针刺太冲穴;肾虚型,运用补法针刺太溪、肾俞和关元穴;脾虚型,运用补法针刺阴陵泉、脾俞穴;气滞血瘀型运用泻法针刺膈俞、太冲穴。局部皮肤酒精消毒后,采用一次性消毒毫针(0.25×40 mm),直刺进针,并施以适宜的针刺手法以激发针感(得气),以患者耐受为度,留针30分钟。局部穴位,采用一次性消毒毫针(0.18 × 25 mm),平刺进针,针尖朝向病变皮肤中心。局部穴位进针后不做任何针刺手法,留针30分钟。对照组受试者于每晚睡觉前局部外用4%对苯二酚,连续使用4周。所有受试者均应避免长时间日晒,并要使用防晒系数50以上的防晒霜,防止紫外光辐射对皮肤的损害。结局指标:主要结局指标是黄褐斑面积和严重程度指数(MASI)。两组患者均在治疗前及治疗结束时由两名未参与该试验的皮肤科医生进行MASI评分。次要结局指标是治疗评分(treatment score),是由相同的皮肤科医生在治疗结束后根据皮科专用面部相机Cologne,VisioFace Quick Courage and Khazaka(德国产)拍摄的面部照片进行评估。患者对治疗的满意程度作为另一个次要结局指标,并在治疗结束时以及治疗结束后一个月进行评估。统计学

Performance of passive and reactive profiled median barriers in traffic noise reduction

Median noise barriers, like parallel noise barriers, can be employed to reduce the impact of traffic on roadside communities via the direct propagation path. The performance of different shapes of median barriers was compared using reactive and passive surfaces and a 2D boundary element method (BEM). In the case of reactive surfaces, quadratic residue diffusers (QRDs) and primitive root diffusers (PRDs) were used on the top and stem surfaces of median barriers. To introduce passive barriers, two different absorbent materials including fibrous material and a grass surface with flow resistivity of 20 000 and 2500 kg/(s·m2), respectively, were similarly applied. The effect of thin absorptive barriers was similar at lower frequencies and better at mid and high frequencies to that of their equivalent rigid barriers. More improvement was achieved by covering the top surface of thick barriers with grass rather than with fibrous material. The performance of QRD and PRD barriers where the diffuser was located on the top surface was more frequency dependent than that of barriers coated with fibrous material. A comparison of the average A-weighted insertion loss in the thick barriers showed that the greatest improvement (2.59 dB (A)) was achieved using a barrier of 30-cm thickness covered with grass.

Optimal distribution feeder reconfiguration （DFR） is a valuable and costless approach to increase the load balance, reduce the amount of power losses, and improve the voltage of the buses. In this way, this paper aims to investigate the optimal DFR strategy as a proper tool to improve the reliability of the radial distribution networks. The idea of failure rate reduction is employed to see the effect of feeder current reduction on the reliability of the system more accurately. The objects to be investigated are system average interruption frequency index （SAIFI）, system average interruption duration index （SAIDI）, average energy not supplied （AENS） and total active power losses. The problem is then formulated in a stochastic framework based on the point estimate method （PEM） to handle the uncertainty effects. The feasibility and satisfying performance of the proposed method is examined on a standard IEEE test system.

This research was conducted to evaluate the sustainability of rice production using fuzzy logic in Mazandaran province,*** untapped use of modern agricultural technology,such as agricultural machinery,has become a global dilemma,which has forced governments and environmentalists to take a serious stand against the dangers of massive non-standard agricultural practices in the *** types of mechanization,socioeconomic,and economic modeling were used to determine the sustainability and their values were collected through interview method using a 320-item questionnaire from 2016 to *** evaluation was carried out using soil and climate characteristics,so that about 27.4%of the area was in the perfectly suitable range(S1),35.2%in the appropriate range(S2),19.5%in the critical range(S3),and 17.9%in the inappropriate range(N)for rice *** were selected from two regions(R1 and R2)by the GIS on two lower levels and more than one hectare(A1,A2)according to agricultural methods(mechanical and traditional).The results showed that the highest and lowest means of the product were 4314.44 and 3501.95 kg ha1,*** sustainability of production was within the range of“sustainability”and“almost sustainable”in terms of mechanistic and economic factors in the fuzzy range of“moderate sustainability”and also with respect to social factors that are in a better position than mechanistic and economic *** of final sustainability model showed that the average of the lasting sustainability index was in the range of“almost unsustainable”and“moderate sustainability”.

Accumulation of heavy metal in Platanus orientalis, Robinia pseudoacacia and Fraxinus rotundifolia

Plants are capable of reducing environmental pollutions through uptaking contaminants in their tissues. In the study site, twenty one-year-old leaves and shoots as well as twenty soil samples were sampled and analyzed by Inductively Coupled Plasma （ICP）. We investigated the uptaking ability of lead （Pb） and cadmium （Cd） by the leaves and shoots of Platanus orientalis, Robinia pseudoacacia and Fraxinus rotundifolia in Karaj city, the western Tehran, Iran. We also evaluated the total metal accumulation capacity by using metal accumulation index （MAI）. Results indicated that there was no significant difference in uptaking contents of Cd and Pb in the leaves among the trees. However accumulation of Cd in shoots of F. rotundifolia and R. pseudoacacia was significantly higher than that of P. orientalis. The accumulation of Pb in shoots of R. pseudoacacia was significantly higher than the other species. The amount of Pb in the soil of the study area was significantly higher than Cd. Concentrations of Cd and Pb in leaves of the three species are in the ranges of 2.4 2.7 mg Kg-1 and 7.1 14.4 mg Kg-1 , respectively. R. pseudoacacia had the highest MAI value for leaves （2.21） and F. rotundifolia had the highest MAI value for shoots （2.4）.

Objective:To investigate the relationship between ambient sulfur dioxide(SO2)exposure and semen quality ***:A systematic literature search was conducted to identify relevant studies investigating the association between SO2 exposure and semen quality *** search encompassed the timeframe from January 2000 to May 2023 and included electronic databases such as Web of Science,Google Scholar,PubMed,Cochrane,and *** effect estimates with 95%confidence intervals(CI)were calculated using percent changes(PC).The meta-analysis included seven studies with 6711 participants and 15087 semen ***:The results revealed a significant negative association between ambient SO2 exposure and certain semen quality *** particular,SO2 exposure was associated with a significant decrease in progressive motility(PC=0.032;95%CI:-0.063 to-0.001;P=0.044)and sperm concentration(PC=-0.020;95%CI:-0.036 to-0.005;P=0.012).However,no statistically significant associations were observed for total sperm count(PC=-0.038;95%CI:-0.079 to 0.003;P=0.070),seminal fluid volume(PC=-0.009;95%CI:-0.048 to-0.030;P=0.662)and sperm motility(PC=-0.17;95%CI:-0.363 to 0.022;P=0.830).In addition,the results of the subgroup analysis revealed specific variables that were associated with the decrease in relevant sperm ***:This systematic review and meta-analysis provides compelling evidence supporting a consistent negative association between exposure to ambient SO2 and semen quality parameters.

Vanadium doped nanoporous Ti0.9Sn0.1O2 thin film has been prepared on an alumina substrate by sol-gel method with Pluronic P123 as the organic template, and humidity sensing properties of it has been investigated. It is found that V-doped nanoporous Ti0.9Sn0.1O2 thin film shows good humidity sensing properties, and impendence of it de- creases more than four orders of magnitude in the relative humidity （RH） range of 11%--95% at 25 ℃. The response and recovery time of this sensor are about 13 and 17 s, respectively. High sensitivity, narrow hysteresis loop, rapid response and recovery, prominent stability and good repeatability are obtained. A possible mechanism is sug- gested to explain the humidity sensitive properties.
