Latissimus Dorsi Mini-Flap as a Volume Replacement Technique after Partial Mastectomy for Breast Cancer in the Upper and Central Breast Quadrants: A Single Center Experience

Background: The latissimus dorsi (LD) muscle flap plays an essential role in breast reconstruction after partial mastectomy for cancer because of its stability and versatility. We evaluated both oncologic and aesthetic outcomes in addition to the related complications of this flap as an adjunct to breast conserving surgery in the management of breast cancer patients. Methods: All patients underwent a one-stage procedure with immediate reconstruction through two-steps operation;wider local excision utilizing oncoplastic principles and mini flap harvest & volume replacement. Results: The study included 34 cases with early breast cancer;30 patients had partial breast resection and defect refilling by LD mini-flap, three patients underwent mastectomy and one patient underwent extended LDF. The mean defect volume was (212.63 cm3 ± 59.57) cm3, while the mean flap volume was (218.27 cm3 ± 53.64 cm3). Patient self-evaluation of the cosmetic outcome was excellent in 20%, good in 60% and satisfactory in 20% of patients. Panel evaluation according to Harvard scale showed excellent in 36.7%, good in 36.7%, fair in 26.7% of patients. The median hospital stay was 4 days. The postoperative complications included wound gap in 4 patients (13.3%), postoperative donor site seroma in 16 patients (53.3%). No flap loss or necrosis, no affection on arm or shoulder mobility occurred. Lastly, no tumor recurrence till now. Conclusion: Latissimus dorsi mini-flap can achieve adequate cosmetic and oncologic outcomes with a low incidence of complications in patients with early stage (I/II) breast cancer and small to medium sized breasts.

Vitamin D Level in Multiple Sclerosis Patients. Could Vitamin D Level Be Routine Investigation for Multiple Sclerosis Patients?

Objectives: To study the prevalence of vitamin D deficiency among newly diagnosed multiple sclerosis patients. Patients and Methods: It is a case control cross matching age related study done on totally 40 subjects (20 patients are newly diagnosed as MS;patients don’t start any medication for MS (naive patients) and 20 subjects are controls with the same age and sex). Base line vitamin D level was measured (i.e. vitamin D, 25-OH (total)) and MRI brain with contrast was done for all patients. Results: Low total vitamin D level was seen among 65% of patients with MS (13/20);however, this was only 20% of normal controls (4/20). Conclusion: Hypovitaminosis D is common in MS patients.

Nitrate(NO_(3)^(-))removal by photochemical-reduction has received extensive ***,the low selectivity of NO_(3)^(-)reduction to N_(2)hinders the application of this *** this study,a novel Cu@Fe-Cu-CuFe_(2)O_(4)-x photocatalyst was prepared by modifying CuFe_(2)O_(4)with KBH4 and Cu(II),and used to selectively reduce NO_(3)^(-)to N_(2)with a two-step reduction *** step(1),with Cu@Fe-Cu-CuFe_(2)O_(4)-x/ultraviolet(UV)system,91.0%NO_(3)^(-)was reduced to 52.3%NO_(2)-and 39.4%N_(2)within 60 *** rapid removal of NO_(3)^(-)was due to the synergistic effect of oxygen vacancies,Fe-Cu corrosion cell,and CuFe_(2)O_(4)*** step(2),H_(2)C_(2)O_(4)and H_(2)O_(2)were introduced into the effluent of step(1)to promote CO_(2)·-formation via Fe(II)and Fe(III)catalysis and UV radiation,which boosted the selective reduction of NO_(2)-to N_(2).When H_(2)C_(2)O_(4)and H_(2)O_(2)dosages were both 4.0 mmol·L^(-1)and the reaction time was 30 min,the removal efficiency of NO_(2)-achieved 100%and the selectivity of N_(2)was 83.0%.Overall,the two-step reduction process achieved 95.0%NO_(3)^(-)removal efficiency and 90.1%N_(2)selectivity with initial NO_(3)^(-)concentration of 30 mg·N·L^(-1).In addition,the denitrification mechanism of the two-step reduction process was tentatively proposed.

Efficacy of Physiotherapy and Conductive Education in Improving Motor Skills and Mental Function in Children with Cerebral Palsy

Background: Children with Cerebral Palsy (CP) often receive physiotherapy to achieve maximum motor potential and prevent secondary conditions. Conductive Education (CE) is an education programme that combines special education and rehabilitation. Purpose: Study the effect of physiotherapy and conductive education in development of fine, gross motor skills and mental function in the children with CP. Subjects and Methods: This is a prospective case study that was carried out on 105 children with CP less than 4 years old selected from outpatient clinic in National Institute for Neuro Motor System. Cases were taken consecutively from February 2019 to September 2019. A written informed consent was taken from all participants after proper explanation of the study. They were classified into three groups (group I, group II and group III) according to their age. All children are subjected to complete history taking, clinical examination, use of developmental quotient (DQ) sheet before and after 6 months of regular physiotherapy and conductive education program. Results: There was statistically significant improvement in fine motor, cognation and gross motor in 3 groups after 6 months of regular physiotherapy and conductive education program. 小细胞肺癌(SCLC)在肺癌患者约占15%,通常与吸烟有密切关系,小细胞肺癌被归类为侵略性程度非常高的肺癌。SCLC的治疗方法首先从化学疗法和放射疗法开始。由于SCLC通常分为局限性(LD)和广泛性(ED)两个阶段,SCLC的患者中有70%被诊断为广泛阶段。DLL3被认为是SCLC免疫疗法的理想靶标,因为它在SCLC患者中高表达,而在正常组织中几乎不表达。免疫检查点抑制剂和DLL3靶向抗体偶联药物是目前SCLC免疫疗法中研究比较多的免疫治疗方法。双特异性抗体是通过将T细胞和肿瘤细胞直接桥连起来从而发挥T细胞的肿瘤杀伤活性。CAR-T细胞是将抗体片段表达在T细胞的表面,提高了T细胞对肿瘤细胞的识别与杀伤活性。本论文构建了靶向DLL3的双特异性抗体和CAR-T,并研究了它们对小细胞肺癌的杀伤活性。双特异性抗体的构建采用经典的knob-into-hole结构,其中靶向DLL3的抗体片段SC16.15与h Fc(knob)融合,靶向CD3的抗体片段OKT3与h Fc(hole)融合。CART细胞构建采用第三代结构,由抗体片段SC16.15、CD8跨膜区、共激活域CD28、4-1BB和CD3z构成。DLL3双特异性抗体和CAR-T的肿瘤杀伤活性均通过细胞水平和和体内移植瘤小鼠模型测试。双特异性抗体的细胞结合活性分别用DLL3阴性的A431细胞和DLL3阳性的小细胞肺癌细胞系、T淋巴瘤细胞系Jurkat和原代PBMC细胞进行测试。双特异性抗体和CAR-T的肿瘤杀伤活性通过荧光素酶法进行测试,首先用慢病毒载体将荧光素酶报告基因导入上述细胞系中,通过测试荧光素酶活性的下降确定双特异性抗体和CAR-T的体外杀伤活性。双特异性抗体和CAR-T的体内抑瘤活性在NSG小鼠中测试,通过向NSG小鼠皮下接种SCLC细胞,待成瘤后,通过腹腔注射PBMC细胞和尾静脉注射双特异性抗体,或腹腔注射CAR-T细胞,观察对肿瘤生长的抑制作用。体外杀伤结果显示,靶向DLL3的双特异性抗体和CAR-T均可迅速杀死DLL3阳性的小细胞肺癌细胞系H446,H196,H82,和超表达DLL3的A431细胞系系。体内抑瘤活性试验显示,双特异性抗体和CAR-T对肿瘤的生长均具有明显的抑制作用。联合PD-1检查点抑制抗体可以提高DLL3双特异性抗体的抑瘤活性,但未能加强CAR-T的抗肿瘤活性。总体来说,本研究的结果表明,以DLL3为靶点的双特异性抗体和CAR-T对小细胞肺癌均有很强的抑瘤活性,其中,DLL3双特异抗体联合PD-1抗体对SCLC的抑瘤效果更理想。

Relation between Carotid Artery Plaques Characteristics as a Predictor of Haemodynamics Changes after Carotid Artery Stenting

Objectives: To study relationship between carotid artery plaques characteristics and haemodynamic changes after carotid stenting. Patients and Methods: This observational prospective (pilot) study included 20 patients—who underwent CAS. The study was carried out in Ain shams university hospitals—Railway hospital (ERMED) and Suez insurance hospital from December 2018 to February 2020. Inclusion criteria, Symptomatic (defined as amaurosis fugax, TIA, Minor stroke or Major stroke) stenosis > 70%. Asymptomatic stenosis > 80% (accidentally discovered during pre-operative assessment for CABG and during full assessment for irrelevant stroke). Results: Regarding to plaque character for degree of stenosis, calcification and ulceration revealed significantly higher degree of stenosis in hemodynamic events group (P = 0.024). On the other hand, that there was no statistically significant difference between the two groups as regard calcification, ulceration and lesion location. Conclusion: The study revealed that HI is a common occurrence following CAS procedures and significantly higher degree of stenosis in hemodynamic events group. Plaque shape, ulcerations hardly affect haeodynamic instability after carotid stenting.

Serum Level of Vitamin C in a Sample of Egyptian Patients with Multiple Sclerosis

Background: Antioxidants such as vitamin C can eliminate free radicles responsible for oxidative stress. Objective: To assess the serum level of vitamin C in a sample of multiple sclerosis (MS) patients to investigate a possible role of vitamin C in the pathogenesis of MS. Subjects and Methods: Forty patients with MS and twenty healthy controls were included in this study. Vitamin C level was estimated for each participant. Results: Serum vitamin C levels were significantly lower among the patients (ranging from 0.28 mg/dL to 0.75 mg/dL with mean 0.47 ± 0.14 mg/dL and median of 0.46 mg/dL) compared to controls (ranging from 0.60 mg/dL to 2.90 mg/dL with mean 1.87 ± 0.61 mg/dL and median of 1.90 mg/dL) (P Conclusion: Patients with MS tended to have low serum vitamin C levels, and younger patients had lower serum levels.

The case study is aimed at providing a more thorough analysis of a case of Epilepsy presented with an unusual aura of cough. The study hopes to generate an interest for further studies into the topic and focuses on abnormal unusual presentations of auras. Cough as an aura is not a common presentation. Any stereotyped repeated clinical phenomenon should consider being epileptic in its origin.

Comorbidity of Attention-Deficit/Hyperactivity Disorder (ADHD) in Children with Epilepsy, Egyptian Study

Background: Epilepsy is one of the most common neurological conditions with about 65 million people affected worldwide. It is also a common condition in children, where its prevalence is approximately 3.2 - 5.5/1000 in the developed world. Children with epilepsy (CWE) experience not only seizures but also multiple cognitive, behavioral, and emotional problems. ADHD is one of the more common comorbidities of childhood epilepsy. Objectives: To discuss the relationship between childhood epilepsy and comorbidities especially ADHD. Study Design: This is a prospective observational analytical cross-sectional study carried out on one hundred and fifteen patients at Ain Shams University and Nasr-city Health Insurance out-patient clinics, Cairo, Egypt fulfilling the inclusion criteria. Patients were selected by simple random sample. Results: ADHD is very common epilepsy comorbidity in Egyptian epileptic children with under-diagnosis and treatment. Conclusion: Frequent assessment for ADHD in epileptic children is mandatory for better quality of life.
