Sepsis is one of the leading worldwide causes of morbidity and mortality in critically-ill patients. Prediction of outcome in patients with sepsis requires repeated clinical interpretation of the patients' conditions, clinical assessment of tissue hypoxia and the use of severity scoring systems, because the prognostic categorization accuracy of severity scoring indices alone, is relatively poor. Generally, such categorization depends on the severity of the septic state, ranging from systemic inflammatory response to septic shock. Now, there is no gold standard for the clinical assessment of tissue hypoxia which can be achieved by both global and regional oxygen extractabilities, added to prognostic pro-inflammatory mediators. Because the technology used to identify the genetic make-up of the human being is rapidly advancing, the structure of 30 000 genes which make-up the human DNA bank is now known. This would allow easy prognostic categorization of critically-ill patients including those suffering from sepsis. The present review spots lights on the main severity scoring systems used for outcome prediction in septic patients. For morbidity prediction, it discusses the Multiple Organ Dysfunction score, the sequential organ failure assessment score, and the logistic organdysfunction score. For mortality/survival prediction, it discusses the Acute Physiology and Chronic Health Evaluation scores, the Therapeutic Intervention Scoring System, the Simplified acute physiology score and the Mortality Probability Models. An ideal severity scoring system for prognostic categorization of patients with systemic sepsis is far from being reached. Scoring systems should be used with repeated clinical interpretation of the patients' conditions, and the assessment of tissue hypoxia in order to attain satisfactory discriminative performance and calibration power.

The objective of the current work is to investigate the possible beneficial effects of forced feeding of fenugreek to rabbit does on their milk composition, milk yield, anemia screening parameters, immunity and the growth performance of bunnies. Twenty white New Zealand female rabbits were randomly assigned into 5 groups: normal control and 4 treated groups (vehicle (molasses), fenugreek powdered seeds + molasses, sprouted fenugreek + molasses, and fenugreek oil + molasses). All treated groups took the given doses orally for two days before parturition and 13 days after parturition for 2 successive pregnancies. Blood samples were collected from each doe on the 13th day of lactation for determination of hematological, biochemical and immunological parameters. Milk samples were taken for evaluation of milk composition. All rabbit does and their bunnies were weighed for calculation of the milk yield. The fenugreek seeds and oil administered groups (G3 & G5) recorded the highest percent values of fat, protein, lactose, total solids and solid not fat contents of the rabbits’ milk in both 1st and 2nd lactation periods relative to other tested groups. At the beginning and mid of lactation periods, the milk yields recorded non-significant differences either between tested groups or between 1st and 2nd pregnancy in the same group. Whereas, at the end of lactation, germinated seeds (G4) and oil (G5) treated groups recorded significantly higher rates of milk yield relative to control and fenugreek seeds groups. Bunnies’ body weights suckling from dams of group III (G3) recorded the highest weights relative to other tested groups in both 1st and 2nd pregnancy. Objectives: The impairment of right ventricular (RV) myocardial mechanics is evident in hypertrophic cardiomyopathy (HCM). It is independently influenced by LV mechanics and correlated to the severity of LV phenotype. We investigated the changes in RV global and regional deformation following surgical septal myectomy using vector velocity imaging (VVI). Methods: 25 HCM patients, 68% males with mean age (34.5 ± 12 years) were examined before and within two months after surgical myectomy using VVI. In addition to conventional echocardiographic parameters, peak systolic strain (εsys), strain rate (SR) and time to peak εsys (TTP) of regional RV free wall (RVFW) & septal walls were analyzed in longitudinal (long) directions from apical four-chamber view and their (Δ) changes were calculated. Similar parameters were quantified in LV from apical 2 & 4 CH views. Intra-V-delay was defined as SD of TTP and inter-V dyssynchrony was estimated from TTP difference between the most delayed LV segment & RVFW. Results: All study patients showed improvement of their functional class from NYHA class III to class I and reduction of LVOT gradient to below 20 mmHg except one patient who had 30 mmHg gradients at rest. There was significant reduction of septal thickness, left atrial diameter & volume, LVOT gradient, LVMI, severity of mitral regurgitation, tricuspid annular velocities (P < 0.0001), RV diameter (P < 0.02) and increase in LV internal dimensions(P < 0.001) post myectomy. However, there was significant reduction of RV and LV systolic mechanics;RV global εsys % (from -16.1% ± 4.4% to -12.9% ± 2.9%, P < 0.0001) and LV global εsys %: from -11.6% ± 2.8% to -9.4% ± 2.2%, P < 0.0001) respectively. Background and Objective: Epidemiological and clinical studies have shown that vascular calcification and arterial vascular stiffness are associated with all-cause and cardiovascular (CV) mortality in the general populations and end-stage renal disease (ESRD) patients. The objective of this study was the assessment of arterial stiffness and vascular calcification in ESRD patients in Sohag University Hospital. Patients and Methods: In this cross-sectional descriptive study, 100 patients with ESRD (63 males and 37 females) were included. The patients were on regular hemodialysis (HD) for 65.18 ± 50.25 months. Arterial stiffness was measured by brachial pulse pressure (PP). A simple vascular calcification score (SVCS) based on plain radiographic films of pelvis and hands was used for assessment of vascular calcification. The patients undergoing clinical evaluation and serum calcium, phosphorus and parathyroid hormone (PTH) were obtained. Results: PP > 70 mmHg was observed in 24 patients (24%) and vascular calcifications were detected in 50 patients (50%). SVCS ≥ 3 was observed in 36 patients (36%) and 10 patients (10%) had both SVCS ≥ 3 and PP > 70 mmHg. PP > 70 was significantly associated with presence of vascular disease (odds ratio;5, 95% CI = 1.56 - 16.01;P = 0.007). In multivariate analysis, PP > 70 was independently associated with elevated systolic blood pressure (odds ratio;1.34, 95% CI = 1.11 - 1.6;P = 0.002) and serum PTH (odds ratio;1.004, 95% CI = 1 - 1.008;P = 0.03). SVCS ≥ 3 was association with age (odds ratio;1.05, 95% CI = 1.02 - 1.09;P = 0.003) and presence of vascular disease (odds ratio;3.19, 95% CI = 1.3 - 7.85;P = 0.01). Elevated systolic blood pressure was independently associated with SVCS ≥ 3 (odds ratio;1.08, 95% CI = 1.03 - 1.14;P = 0.002). Conclusion: Arterial stiffness and vascular calcifications are common in patients with ESRD on regular hemodialysis. PP and SVCS are simple and inexpensive methods in assessment of vascular calcifications

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors

Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. CA 19-9 < 37 U/mL, smaller tumor size, and R0 resection were found to be independent factors for long-term survival ≥ 5 years.

Background:Bile duct injury(BDI)after cholecystectomy remains a significant surgical *** guideline exists to guide the timing of repair,while few studies compare early versus late repair *** study aimed to analyze the outcomes in patients undergoing immediate,intermediate,and delayed repair of ***:We retrospectively analyzed 412 patients with BDI from March 2015 to January *** patients were divided into three groups based on the time of BDI *** 1 underwent an immediate reconstruction(within the first 72 hours post-cholecystectomy,n=156);group 2 underwent an intermediate reconstruction(from 4 days to 6 weeks post-cholecystectomy,n=75),and group 3 underwent delayed reconstruction(after 6 weeks post-cholecystectomy,n=181).Results:Patients in group 2 had significantly more early complications including anastomotic leakage and intra-abdominal collection and late complications including anastomotic stricture and secondary liver cirrhosis compared with groups 1 and *** outcome was observed in 111(71.2%)patients in group 1,31(41.3%)patients in group 2,and 157(86.7%)patients in group 3(P=0.0001).Multivariate analysis identified that complete ligation of the bile duct,level E1 BDI and the use of external stent were independent factors of favorable outcome in group 1,the use of external stent was an independent factor of favorable outcome in group 2,and level E4 BDI was an independent factor of unfavorable outcome in group *** BDI and level E4 BDI were independent factors of unfavorable ***:Favorable outcomes were more frequently observed in the immediate and delayed reconstruction of post-cholecystectomy *** ligation of the bile duct,level E1 BDI and the use of external stent were independent factors of a favorable outcome.
