Objective:To investigate the role of zinc in reducing the deleterious effects of cadmium on male ***:Rats were injected subcutaneously with CdCl_2 and ZnCl_2 at dose level of2.2 mg/kg(1/40 of LD50 of cadmium per day).Results:The rats treated with cadmium exhibited a significant increase in levels of testicular malondialdehyde,tumor necrosis factor-alpha,nitrogen oxide and inducible nitrogen oxide synthase immunostaining reaction,as well as an elevation of blood hydroperoxide and follicle stimulating *** addition,a significant decrease in testicular ascorbic acid,zinc,reduced glutathione,catalase,superoxide dismutase,sex organ weight,plasma testosterone and luteinizing hormone were observed in the cadmium *** motility and count were decreased with cadmium treatment,while sperm abnormalities elevated *** treatment was found to mitigate the toxic effects of cadmium on oxidative stress,spermatogenesis,sex hormones,and inflammatory *** injected with cadmium showed intense histopathological *** manifested protective role and markedly reduced tissues damage induced by ***:The protective effect of zinc can be attributed to its antioxidant and antiinflammatory properties.

Miocene siliciclastic-carbonate deposits are widely exposed in Cairo-Suez District, Egypt. These deposits are underlain and overlain by continental sediments of Oligocene and post Miocene, respectively. Three stratigraphic sections were investigated at Gabals Geneife, Homeira and Gharra. Lithostrtigraphically, the Miocene sequence could be differentiated into two main rock units representing shallow deposits with relatively intermittent deep marine incursions. These are from base to top, Gharra Formation and Genefe Formation. Detailed macrofossils investigations led to the recognition of four macrofossil zones, namely Alectryonella plicatula-Crassostrea frondosa Range Zone, Echinolampas amplus-Scutella ammonis Range Zone, Chlamys（Macrochlamys） sardoa-Chlamys（Argopecten） submalvinae Range Zone, and Chlamys gentoni-Pecten（Oppenheimopecten） benedictus-Pecten（P.） ziziniae Assemblage Zone. Microfacies analysis and identified taxa indicated that the Miocene sequence was deposited in transgressive-regressive cycles ranged from near shore, warm shallow inner to middle shelf marine environments with storm influence during the accumulation of the oyster banks.

Objectives:Pressure ulcer(PU)is one of the most common problem among the bedridden elderly and has significantly more burden on elderly and *** study is aimed to evaluate the effects of the training program for caregivers to prevent PUs among elderly residents at geriatric ***:A quasi-experimental design was used to carry out the current study.A purposive sample comprised of all formal(39 nurses)or informal caregivers(39)and all immobilized elderly residents(35)who are found in all geriatric centers in Helwan district.A structured questionnaire was used to assess caregivers’socio-demographic characteristics,knowledge,attitude,and observational checklists for their practice for prevention of PU at pre-and posttest and during ***:After the training program,there were improvements in the level of knowledge,practice,and positive attitude of caregivers about PU prevention with a statistically significant difference between pre-,post-,and follow-up training programs(Pelderly caregivers’attitudes as well as practice for prevention could be important in reducing the burden of PU among the ***,conducting a training program for caregivers at different geriatric homes about caring skills for elders can prevent PU,and using spontaneous reposition is very effective to prevent PUs.

Simple, accurate, sensitive and validated UV spectrophotometric and chemometric methods were developed for the determination of imidapril hydrochloride （IMD） in the presence of both its alkaline （AKN） and oxidative （OXI） degradation products and in its pharmaceutical formulation. Method A is the fourth derivative spectra （D4） which allows the determination of IMD in the presence of both AKN and OXD, in pure form and in tablets by measuring the peak amplitude at 243.0 nm. Methods B, C and D, manipulating ratio spectra, were also developed. Method B is the double divisor-ratio difference spectrophotometric one （DD-RD） by computing the difference between the amplitudes of IMD ratio spectra at 232 and 256.3 nm. Method C is the double divisor-first derivative of ratio spectra method （DD-DR1） at 243.2 nm, while method D is the mean centering of ratio spectra （MCR） at 288.0 nm. Methods A, B, C and D could successfully determine IMD in a concentration range of 4.0-32.0 mg/mL. Methods E and F are principal component regression （PCR） and partial least-squares （PLS）, respectively, for the simultaneous determination of IMD in the presence of both AKN and OXI, in pure form and in its tablets. The developed methods have the advantage of simultaneous determination of the cited components without any pre-treatment. The accuracy, precision and linearity ranges of the developed methods were determined. The results obtained were statistically compared with those of a reported HPLC method, and there was no significant difference between the proposed methods and the reported method regarding both accuracy and precision.

Objective: This study investigated cytotoxicity and induction of apoptosis in human cervical cancer cells(HelA) and prostate cancer cells(PC-3) using the most active fraction of Moringa peregrina seed ***: Dried and powdered seeds were extracted using 95% ethanol. The total ethanolic extract was further dissolved in distilled water and separated into petroleum ether, chloroform, ethyl acetate and aqueous extracts. Based on the results of in vitro anticancer studies of all extracts, the most highly active extract was selected for evaluation of apoptosis induction and cell cycle analysis on HelA and PC-3 cells at its half maximal inhibitory concentration using flow cytometry;DNA fragmentation by agarose gel electrophoresis and the expression of protein were measured by Western ***: The chloroform fraction from the ethanolic extract of M. peregrina(CFEE) was the most active antitumor fraction. The selectivity index, determined using the normal Vero cell line, indicated that CFEE had a high degree of selectivity against HelA and PC-3 cells. CFEE induced apoptosis, confirmed by cell cycle arrest at sub-G0 phase and DNA fragmentation. CFEE induced an increase in mRNA expression of caspase-3, a decrease in Bcl-2 mRNA expression, and decreased ATP levels. CFEE increased protein expression of caspase-3 and decreased protein expression of poly-ADP-ribose polymerase-1(PARP-1).Flow cytometric analysis showed an appreciable increase in the number of cells in the early apoptotic stage in CFEE-treated HelA and PC-3 cells. CFEE treatment significantly increased lipid peroxidation(malondialdehyde level) in HelA and PC-3 ***: Seed extract of M. peregrina displayed a significant antitumor effect through apoptosis induction in HelA and PC-3 cells.

Background: Serum ferritin(SF) and consequently hepatic iron have long been considered important in liver fibrosis progression. They have been studied in different liver diseases with no previous reports in neonatal cholestasis(NC). This study aimed to measure SF in different etiologies of NC and investigate its relation to hepatic iron and fibrosis. Methods: SF was measured in 75 infants, including 50 with NC and 25 with sepsis. SF was compared between these two groups. Biochemical parameters, hepatic iron grades, and liver fibrosis and other histopathological characteristics and correlated with SF were assessed in NC group. Finally, a comparison between intrahepatic cholestasis and obstructive etiology was performed. Results: SF was elevated in NC(1598 ± 2405 ng/mL) with no significant difference from those with sepsis( P = 0.445). NC and sepsis constituted augmenting factors leading to more elevation of SF(2589 ± 3511 ng/mL). SF was significantly correlated with hepatic iron grades( r = 0.536, P ell transformation( P = 0.009) and hepatocyte swelling( P = 0.023) than those with obstructive etiology(672 ± 566 ng/mL) and their prevalent pathological findings of ductular proliferation( P = 0.003) and bile plugs( P = 0.002). SF was unrelated to the grade of liver fibrosis( P = 0.058). Conclusions: SF is non-specifically elevated in NC, with positive correlation to hepatic iron grades. SF ≥ 803.5 ng/mL can predict higher grades( ≥ grade 3) of hepatic iron. However, an active role of increased SF and hepatic iron in disease progression remains questionable.

AIM: To evaluate transient elastography (TE) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver ***: A total of 90 children [50 with chronic hepatitis C virus (HCV), 20 with autoimmune hepatitis (AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement (LSM) using TE. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction (FAF) using digital image analysis (morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was also performed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiver-operating characteristic curve. Cut-off values with optimal clinical performance (optimal sensitivity and specificity simultaneously) were ***: The majority of HCV group had minimal activity (80%) and no/mild fibrosis (72%). On the other hand, the majority of AIH group had mild to moderate activity (70%) and moderate to severe fibrosis (95%) and all Wilson disease group had mild to moderate activity (100%) and moderate to severe fibrosis (100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter (r = 0.839 vs 0.879, P < 0.0001 for both). LSM discriminated individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH (16.15 ± 7.23 kPa) compared to Wilson disease (8.30 ± 0.84 kPa) and HCV groups (7.43 ± 1.73 kPa). Background: The efficacy of octreotide to prevent postoperative pancreatic fistula(POPF) of pancreaticoduodenectomy(PD) is still controversial. This study aimed to evaluate the effect of postoperative use of octreotide on the outcomes after ***: This is a prospective randomized controlled trial for postoperative use of octreotide in patients undergoing PD. Patients with soft pancreas and pancreatic duct elayed gastric emptying(P = 0.472), and early postoperative mortality(P = 0.727).Conclusions: Octreotide did not reduce postoperative morbidities, reoperation and mortality rate. Also, it did not affect the incidence of POPF and its clinically relevant variants.

AIM： To evaluate the vitreomacular interface in cases with wet age-related macular degeneration（AMD） and to compare them to eyes with dry AMD and normal ***： This was a cross-sectional comparative study that included 87 eyes with wet AMD,42 eyes with dry AMD and 40 eyes without AMD as a control *** coherence tomography（OCT） examination was performed for all patients to assess the vitreomacular ***： In the wet AMD group,34.5% of cases had vitreomacular adhesion（VMA）.Only 14.3% of dry AMD cases and 10% of control cases had *** was a significant difference between the control group and the wet AMD group（P=0.004） as well as the dry and wet AMD group（P=0.017）.There was also a significant difference between the incidence of VMA in patients with subretinal choroidal neovascularization（CNV,type 1） and intraretinal CNV（type 2 or type 3）（P=0.020）.CONCLUSION： There is an association between posterior vitreous attachment and *** is also an increased incidence of VMA with intra-retinal CNV.

Objective： Preservation of the pelvic autonomic nerves in order to lower bladder and sexual dysfunction after radical rectal cancer surgery ＆ to evaluate functional outcome, local recurrence. Methods： A prospective study was under- taken on Egyptian patients. Forty one patients participated in the study in the period from December 2002 till June 2004 where they underwent radical surgery but with preservation of the pelvic autonomic nerves this was followed by adjuvant pelvic radiotherapy. Results： Six months, 1-year and 2-year follow-up of urinary function was complete in 32 out of 41 （78%）, 30 out of 41 （73%） and 27 out of 41 patients （65%） respectively There was no statistically significant correlation between the extent of nerve preservation and the reported minor voiding dysfunction. None of the patients reported major incontinence. Six months, 1-year and 2-year follow-up of sexual function revealed that 22 out of 41 patients （53%） were sexually active. Three out of 41 patients （7.3%） developed local recurrence. 38 （92.7%） patients were free of local recurrence, regarding pa- tients who received adjuvant radiotherapy 3 out of the 34 （8.8%） patients developed local pelvic recurrence while 9 patients （26.5%） developed distant metastases （3 of them did not receive adjuvant chemotherapy）, while patients who received adju- vant chemotherapy, 2 out of 20 patients （10%） developed local recurrence while distant metastases developed in 6 patients （30%）. Conclusion： Preservation of the pelvic autonomic nerves minimizes bladder and sexual dysfunction especially in male patients after rectal cancer surgery.
