Continuous Intra-Articular Ropivacaine/Tramadol Combination Infusion Therapy Improves Postoperative Outcome of Total Knee Arthroplasty

Objectives: To evaluate the postoperative analgesic yield of continuous intra-articular (IA) ropivacaine/tramadol infusion as against infusion of ropivacaine alone after total knee arthroplasty. Patients and Methods: Sixty patients accompanied by arthrosis were randomly divided into three equal groups: The ropivacaine group, which obtained a continuous intraarticular (IA) infusion of ropivacaine alone;the combination group, which obtained a continuous (IA) infusion of a combination of ropivacaine and tramadol at a rate of 6 ml/h for 72 h postoperatively;and the control group, which did not receive IA medications. After release of the ischemic tourniquet and assurance of haemostasis, a vacuum drainage tube was applied. The (IA) infusion was delivered through a multipored catheter for 72 h. Postoperative pain was assessed using visual analogue pain scale (VAS), and intravenous meperidine was administered as rescue analgesia if the (VAS) pain score was greater than or equal to (4) or on patient’s request. The total rescue analgesia consumption, angle of flexion of the knee and hospital stay were recorded. Results: The ability to achieve better angle of flexion was significantly higher in the combination group compared with the other groups, with a significant difference in favour of the ropivacaine group compared with the control group. At 2 h postoperatively, the mean pain (VAS) scores were significantly lower in patients who received (IA) analgesia compared with the control group and in the ropivacaine/tramadol group versus the ropivacaine group. The number of requests and total dose of rescue analgesia consumed were significantly lower with (IA) analgesia, with a significant difference in favour of the combination group. There was significant correlation between pain (VAS) scores and angle of flexion of the knee joint. Influence of preceding crop and tillage system on forage yield and quality of selected summer grass and legume forage crops under arid conditions

Among the crop production factors,preceding crop and tillage management affect the sustainable use of soil resources and ultimately crop growth and *** study aimed at investigating the impact of preceding winter crops(grass or legume)and different tillage systems on forage yield,quality and nutritive values of three summer grass(Sudan grass,pearl millet and teosinte)and two legume forage crops(cowpea and guar)under arid *** results exhibited that growing forage crops after legumes(as berseem clover)produced the highest fresh and dry forage yields and quality attributes compared with grasses(as wheat)with the exception of crude fiber content,which was ***,tillage practices showed positive impact on forage yields and quality *** maximum forage yields and quality parameters were recorded under conventional tillage(CT)practice compared with reduced tillage(RT)and no-tillage(NT)*** the evaluated crops,the highest yields of fresh forage,dry forage,crude fiber,crude protein and total digestible nutrient were exhibited by grass forage crops(Sudan grass,pearl millet and teosinte),whereas the highest crude protein content and the digestible energy values were produced by legume forage crops(cowpea and guar).The maximum fresh forage,dry forage,crude fiber,crude protein,total digestible nutrient and digestible crude protein yields were produced by pearl millet followed by Sudan grass under CT and RT after berseem *** highest net return was recorded by sowing pearl millet after berseem clover and applying CT followed by RT practices,which could be recommended for the commercial ***,it could be assumed that the combination of growing grass forage crops after legume crops under CT or RT systems could enhance forage crop yield and quality with an improvement in soil properties for sustainable agriculture with low cost and the highest net income.

Elevated Temperatures Tensile Characteristics of Cast A356/Al2O3Nanocomposites Fabricated Using a Combination of Rheocasting and Squeeze Casting Techniques

In the present investigation, the tensile properties of A356/Al2O3 nanocomposites at both ambient and elevated temperatures were studied. The A356/Al2O3 nanocomposites were fabricated using a combination between the rheocasting and squeeze casting techniques. The A356 matrix alloy was reinforced with Al2O3 nanoparticulates having average sizes of 60 nm and 200 nm with different volume fractions up to 5 vol%. The results revealed that the A356/Al2O3 nanocomposites exhibited better mechanical properties than the A356 monolithic alloy. Such improvement in the mechanical properties was observed at both room and elevated temperatures up to 300°C. Increasing the volume fraction and/or reducing the size of Al2O3 nanoparticulates increase both the tensile and yield strengths of the nanocomposites.

Objective:To use two diagnostic antigens belonging to the frequently associated in Theileria domain,Theileria equi(***)protein 82(Te 82)and *** 104 k Da microneme-rhoptry antigen precursor(Te 43),to diagnose *** infection in horses as compared with equi merozoite antigen-2(EMA-2).Methods:In the current study,we applied a cocktail-ELISA containing two antigens(EMA-2+Te 82)to diagnose *** infection either in experimentally infected horses or in field ***:Our findings have revealed that a cocktail formula of EMA-2+Te 82 provided a more practical and sensitive diagnostic candidate for diagnosing *** infection in horses as compared with Te 82 or Te 43 ***:The ELISA technique using a cocktail formula of EMA-2+Te 82 offers a practical and sensitive diagnostic tool for diagnosing *** infection in horses and using of this promising cocktail formula will be applicable for epidemiological surveys and will help control the infection in horses.

Variations of Cup-to-Disc Ratio in Children

Purpose: The aim of this study is early detection of increased cup to disc ratio and a survey study of cup-disc ratio in Egyptian children age group less than 15 years. Methods: Three hundred and eighty-two (382) children were enrolled in this study. They were classified into three age groups;infants’ Group (1) from 0-2 years, preschool Group (2) from 2-6 years and school children Group (3) from 6-15 years old. C/D ratio was measured for each child. Results: In Group (1), normal C/D ratio ranged from 0 to 0.1. The C/D ratio > 0.3 or a binocular difference of >0.2 could be regarded as one of the criteria of newborn glaucoma. Group (2): the normal ratio ranged from 0.1-0.2. Group (3): the normal ratio ranged from 0.2-0.3. The highest ratio 0.7 was found in two eyes. An asymmetry of 0.2 or more in the C/D ratios of the eyes was found in 4.8% of the children. Conclusion: C/D ratio > 0.3 and asymmetry of 0.2 is suggestive of glaucoma especially in infants of lower age group.

Dielectric and Magnetic Properties of Nano-Structure BiFeO3Doped with Different Concentrations of Co Ions Prepared by Sol-Gel Method

BiFe1-xCoxO3 (x = 0, 0.03, 0.05 and 0.1) symbolic as (BFO, BF3CO, BF5CO and BF10CO) in powder form has been prepared by sol-gel technique using ethylenediamine tetraacetic acid (EDTA) as a chelating agent. X-ray diffraction (XRD) and FTIR analysis showed rhombohedra distorted BiFeO3 structure with compressive lattice distortion induced by the Co substitution at Fe sites. The transmission electron microscope (TEM) shows irregular particles. The additive of cobalt oxide has led to grains refining giving the following crystallite sizes of 18 nm for BF5Co. The scanning electron microscope (SEM) study reveals that the samples morphology shows relatively uniform grain size distribution. The dielectric properties of BiFeO3 nano-particles in the frequency range of 1 up to 5 MHz at RT revealed that the A.C. conductivity of the prepared samples reaches its maximum value in BF5CO. By decreasing BiFeO3 particle size as a result of doping with different Co ion concentrations, an enhancement in magnetization and a simultaneous suppression in current leakage occurred. The remnant magnetization Mr of BiFe1-xCoxO3 (x = 0, 0.03, 0.05, 0.1) ceramics significantly enhanced, which provides potential applications in information storage.

Evaluation of the Environmental, Social Effects for the Egyptian National Railways Restructuring

This paper presents the main findings of the environmental and social impact assessment (ESIA) for the Egyptian national railways restructuring. This paper consists of the two parts: 1) modernizing the signaling system on the Arab El Raml-Alexandria Line and creating a Central Traffic Management Center in Tanta;and 2) renewing 200 km of track in Lower and Upper Egypt. The purpose of this evaluation is to identify and examine the potential socio-cultural, economic, and physical impacts of the railway restructuring, and propose measures to mitigate its adverse impacts, and develop plans for managing and monitoring the environmental aspects of the railway restructuring. This paper describes outlines of the environmental and social sensitivities of the railway restructuring area, potentially significant impacts of the proposed project and plans for their mitigation. Detailed information on the environmental and socio economic settings of the railway restructuring, components and activities of the proposed project and the results of the environmental and social impact assessment studies are presented in the body of the paper.

Effect of Catharanthus roseus (Vince rosea) and Turmeric (Curcuma longa) Extracts as Green Corrosion Inhibitors for Mild Steel in 1 M HCl

The corrosion inhibition of mild steel by Catharanthus roseus (Vince rosea) and Turmeric (Curcuma longa) extracts as green inhibitors in 1 M HCl solutions has been investigated by using potentiodynamic polarization, electrochemical impedance spectroscopy (EIS), hydrogen evolution measurements and scanning electron microscope (SEM) techniques. Results obtained show that these investigated compounds are good inhibitors and act as mixed type inhibitors. The inhibition efficiencies increase with increasing inhibitor concentration. The SEM results show the formation of protective film on the mild steel surface in the presence of Catharanthus roseus and Turmeric extracts. The results obtained from different tested techniques are in good agreement.

Predictors of graft function and survival in second kidney transplantation: A single center experience

BACKGROUND The increasing kidney retransplantation rate has created a parallel field of research,including the risk factors and outcomes of this advanced form of renal replacement *** presentation of experiences from different kidney transplantation centers may help enrich the literature on kidney retransplantation,as a specific topic in the field of kidney *** To identify the risk factors affecting primary graft function and graft survival rates after second kidney transplantation(SKT).METHODS The records of SKT cases performed between January 1977 and December 2014 at a European tertiary-level kidney transplantation center were retrospectively reviewed and *** the descriptive characteristics,the survivals of patients and both the first and second grafts were described using Kaplan-Meier *** addition,Kaplan-Meier analyses were also used to estimate the survival probabilities at 1,3,5,and 10 post-operative years,as well as at the longest followup duration ***,bivariate associations between various predictors and the categorical outcomes were assessed,using the suitable biostatistical tests,according to the predictor *** Out of 1861 cases of kidney transplantation,only 48 cases with SKT were eligible for studying,including 33 men and 15 women with a mean age of 42.1±13 *** primary non-function(PNF)graft occurred in five patients(10.4%).In bivariate analyses,a high body mass index(P=0.009)and first graft loss due to acute rejection(P=0.025)were the only significant predictors of PNF *** second graft survival was reduced by delayed graft function in the first(P=0.008)and second(P<0.001)***,the effect of acute rejection within the first year after the first transplant did not reach the threshold of significance(P=0.053).The mean follow-up period was 59.8±48.6 *** graft/patient survival rates at 1,3,5 and 10 years were 90.5%/97.9%,79.9%/95.6%,73.7%/91.9%,and 51.6%/83.0%,respe
