To summarize the long-term complications after pancreas transplantation that affect graft function,a literature search was carried out on the long-term complications of pancreatic transplantation,namely,complications from postoperative 3rd mo onwards,in terms of loss of graft function,late infection and vascular complications as *** most relevant reviews and studies were selected to obtain the current evidence on these *** definition of graft failure varies among different studies,so it is difficult to evaluate,a standardized definition is of utmost importance to know the magnitude of the problem in all worldwide *** rejection is the main cause of long-term graft failure,occurring in 10%of *** the 3rd mo of transplantation onwards,the main risk factor for late infections is immunosuppression,and patients have opportunistic infections like:Cytomegalovirus,hepatitis B and C viruses,Epstein-Barr virus and varicella-zoster virus;opportunistic bacteria,reactivation of latent infections as tuberculosis or fungal *** preoperative studies and serological tests should be made in all recipients to avoid these infections,adding perioperative prophylactic treatments when *** are uncommon,but one of the main causes of late bleeding,which can be *** treatment should be performed with radiological endovascular approaches or open surgery in case of *** all therapeutic options for the complications mentioned above,transplantectomy is a necessary option in approximately 50%of relaparotomies,especially in lifethreatening *** complications in pancreatic transplantation threatens long-term graft *** exhaustive follow-up as well as a correct immunosuppression protocol are necessary for prevention.

Cloud forest ecosystems of the latin American tropics are highly threatened by changes in land-use such as expanding croplands and livestock pastures that promote shifts in the structure and composition of plant communities in these ***,shade coffee plantations represent a forest management alternative that has been shown to maintain biodiversity in these *** this study,we evaluated changes in the composition,diversity,and structure of Mexican cloud-forest woody species for three land use categories:cattle pastures,shade coffee plantations and advanced succession *** each category,fifteen 0.28-ha plots were established and the composition and diversity of vegetation was *** of species was analyzed using ordination methods,and alpha diversity was compared using Hill ***-seven woody species belonging to 40 families were *** richness and diversity was high in both the advanced successional forest and coffee plantations compared to cattle *** composition and structure was similar between late succession forests and coffee plantations with both land uses also being more structurally complex than cattle *** results show how shade coffee cultivation is a land-use activity that maintains woody plant communities in a manner that aligns with biodiversity conservation.

The impact of Pinus halepensis afforestation on Mediterranean spontaneous vegetation:do soil treatment and canopy cover matter?

We investigated central Mediterranean Pinus halepensis plantations under semi-arid climate in order to evaluate the combined effect of soil treatment and afforestation practices on spontaneous plant species composition, richness and evenness, and on the trend and speed of vegetation dynamics. Phytosociological releves of three different plot typologies, i.e. （1） soil-treatment and plantation, （2） only soil-treatment, （3） no soil-treatment and no plantation, were compared by （a） multivari- ate analysis and （b） with reference to species richness and evenness. Moreover, in order to compare vegetation dynamics within the planta- tions with those ones ongoing in semi-natural garrigue communities, we compared life form and syntaxonomic spectra between phytosociological relev6s taken at 8 years of distance. DCA showed that floristic species composition and similarity are influenced by the canopy cover of Pine trees as well as by soil-treatment practices. Although species richness and evenness are not clearly related to neither soil treatment nor Pine afforestation, canopy cover clearly plays a major role： in fact, the highest Ph cover rates correspond to the lowest values of understory species richness. This is true also if only species of biogeographical/conservation interest are considered. Regarding vegetation dynamics, sites with dense Pine canopy cover evolve much slower than the adjacent garrigue communities. The same factors invoqued to explain the patterns of floristic composition and similarity （i.e. allelopathy and competition for light, water and nutrients） may also explain the lowering of diversity of therophytes and the strong decline of the cover perfomed by both therophytes and hemicryptophytes underneath the canopy of dense Ph plantations. Thus, in sites where Ph cover exceeds about 80%, thinning is recommended not only in order to accelerate succession, but also to give a natural ＇shape＇ to afforestations.

From conventional two-stage hepatectomy to ALPPS:Fifteen years of experience in a hepatobiliary surgery unit

Background:Hepatectomy in patients with large tumor load may result in postoperative liver failure and associated complications due to excessive liver parenchyma *** two-stage hepatectomy(TSH)and associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)technique are possible solutions to this *** liver metastases(CRLM)is the most frequent indication,and there is a need to assess outcomes for both techniques to improve surgical and long-term oncological outcomes in these ***:A single-center retrospective study was designed to compare TSH with ALPPS in patients with initially unresectable bilateral liver tumors between January 2005 and January *** was performed from January 2012 onwards as the technique of ***-term overall survival(OS)and disease-free survival(DFS)were evaluated as primary outcome in CRLM *** morbidity,mortality and liver growth in all patients were also ***:A total of 38 staged hepatectomies were performed:17 TSH and 21 *** resection rate was 76.5%(n=13)in the TSH group and 85.7%(n=18)in the ALPPS group(P=0.426).Overall major morbidity(Clavien-Dindo≥3 a)(stage 1+stage 2)was 41.2%(n=7)in TSH and 33.3%(n=7)in ALPPS patients(P=0.389),and perioperative 90-day mortalities were 11.8%(n=2)vs.19.0%(n=4)in each group,respectively(P=0.654).Intention-to-treat OS rates at 1 and 5 years in CRLM patients for TSH(n=15)were 80%and 33%,and for ALPPS(n=17)76%and 35%,*** rates at 1 and 5 years were 36%and 27%in the TSH group vs.33%and 27%in the ALPPS group,***:ALPPS is an effective alternative to TSH in bilateral affecting liver tumors,allowing higher resection rate,but patients must be carefully *** CRLM patients similar long-term OS and DFS can be achieved with both techniques.

Efficacy and limits of sildenafil citrate in patients with arterial erectile dysfunction： role of peripheral arterial disease and cardiovascular comorbidities

Aim： To evaluate whether the response to sildenafil administration in patients with arterial erectile dysfunction （ed） was related to their peak systolic velocity （PSV）, peripheral atherosclerosis, cardiovascular risk factors （RF） and/or comorbidities at low cardiovascular risk. Methods： We enrolled 97 patients with 1-2 RF and comorbidities, combined with arterial ed alone （group A, n = 27）, ed plus atherosclerotic carotid artery （group B, n = 23）, ed plus lower limb artery abnormalities （group C, n = 25）, and ed plus carotid and lower limb artery abnormalities （group D, n = 22）. Sildenafil efficacy （100 mg twice a week for 12 weeks） was also examined in patients with ≥ 3 RF, peripheral a（herosclerosis and no cardiovascular comorbidities （group E, n = 20）. Results： Median PSV was 24.1, 21.0, 19.3, 14.5 and 17.5 cm/s in groups A, B, C, D and E, respectively. Sildenafil response was higher in group A patients （77.8%）, intermediate in groups B and C （65.2% and 56%） and lowest in groups D （45.4%） and E （50%）, and the response in latter two groups was significantly lower than in the other three groups. In addition, sildenafil response was nega- tively influenced by：〉 3 RF, peripheral atherosclerosis and no systemic comorbidity, or presence of 1-2 RF associated with extended atherosclerosis and comorbidities. The number of comorbidifies was positively related to atherosclerosis localization or extension （25, 35, 38 and 47 in groups A, B, C and D, respectively）. Conclusion： Low sildenafil efficacy in patients with arterial ed was associated with extended atherosclerosis. These patients should undergo extensive ultrasonography and a full cardiovascular examination.

Efficacy and safety of low-intensity shockwave therapy plus tadalafil 5 mg once daily in men with type 2 diabetes mellitus and erectile dysfunction:a matched-pair comparison study

Low-intensity extracorporeal shockwave therapy(LiESWT)represents a promising treatment for patients with erectile dysfunction(ed).We investigated the efficacy of LiESWT combined with tadalafil 5 mg once daily in men with type 2 diabetes mellitus(T2DM)and ed and compared LiESWT protocols administering different number of *** performed a retrospective matched-pair comparison using data from a prospectively maintained ***-eight patients who received tadalafil 5 mg once dai7ly for 12 weeks+LiESWT performed with an electrohydraulic source for 3 weeks(Group A)were matched 1:1 to patients who received tadalafil 5 mg once daily alone for 12 weeks(Group B).A subgroup analysis was performed according to the number of shockwaves delivered during each session(1500,1800,and 2400 in subgroup A1,A2,and A3,respectively).The mean International Index of Erectile Function-5(IIEF-5)score variations with respect to baseline recorded at 4,12,and 24 weeks after the end of the treatment were investigated as treatment *** mean IIEF-5 scores significantly improved in all groups and subgroups at 4-week follow-up without intergroup *** 12-and 24-week follow-up,the mean IIEF-5 improvement was significantly higher among patients in the A3 subgroup(+5.0±2.1[Ped approach with tadalafil 5 mg once daily and LiESWT with a protocol involving 2400 shockwaves provides significant advantages in terms of IIEF-5 improvement and durability compared to tadalafil 5 mg once daily alone in patients with T2DM and ed.
