The environmental and economic benefits of recycling asphalt pavements have received much attention in recent years. Because of the increase in the cost of raw materials and energy carriers, the reuse of large portions of reclaimed asphalt pavement(RAP) is critical in reducing both the cost and environmental footprint of asphalt pavements. High-RAP mixtures are more prone to low temperature cracking and poor mixture workability because of the higher stiffness of RAP binder. Recycling agents are one of the additives which are used to improve these deficiencies. However, there is some ambiguity about the optimum content of recycling agent to assure proper performance of recycled asphalt pavement during its service life. The current study used 60% and 100% fractionated RAP with waste cooking oil as a recycling agent and crumb rubber to alleviate the aforementioned problems. Laboratory evaluation showed that increasing the amount of recycling agent in the high-RAP mixtures improved their workability and low temperature performance while decreasing moisture damage and rutting resistance. The long-term susceptibility to aging of recycled binder with the organically-based recycling agent was also investigated. A procedure to obtain the optimum percentage of recycling agent was devised to strike a balance between the performance characteristics of mixtures with a high-RAP content.

The nanocomposite of moSi2-SiaN4 （molybdenum disilicide-silicon nitride） was synthesized by reaction milling of the mo and Si powder mixture. Changing the processing parameters led to the formation of different products such as a- and B-moSi2, SiaN4, mo2N, and M05Si3 at various milling times. A thermodynamic appraisal showed that the milling of moa2Siss powder mixture was associated with highly exothermic mechanically induced self-sustaining reaction （MSR） between mo and Si. The MSR took place around 5 h of milling led to the formation of a-moSi2 and the reaction between Si and N2 to produce Si3N4 under a nitrogen pressure of 1 MPa. By increasing the nitrogen pressure to 5 MPa, more heat is released, resulting in the dissociation of Si3N4 and the transformation of a-moSi2 to β-moSi2. Heat treatment was also performed on the milled samples and led to the formation of mo2N and the transformation of a-moSi2 to β-moSi2 at the milling times of 10 and 40 h, respectively.

Outcome of non surgical hepatic decompression procedures in Egyptian patients with Budd-Chiari

AIM: To evaluate outcome of patients with Budd-Chiari syndrome after balloon angioplasty ± stenting or transjugular intrahepatic portosystemic shunt (TIPS). METHODS: Twenty five patients with Budd-Chiari syndrome admitted to Ain Shams University Hospitals, Tropical Medicine Department were included. Twelve patients (48%) with short segment occlusion were candidates for angioplasty; with stenting in ten cases and without stenting in two. Thirteen patients (52%) had Transjugular Intrahepatic Portosystemic Shunt. Patients were followed up for 12-32 mo. RESULTS: Patency rate in patients who underwent angioplasty ± stenting was 83.3% at one year and at end of follow up. The need of revision was 41.6% with one year survival of 100%, dropped to 91.6% at end of follow up. In patients who had Transjugular Intrahepatic Portosystemic Shunt, patency rate was 92.3% at one year, dropped to 84.6% at end of follow up. The need of revision was 38.4% with one year and end of follow up survival of 100%. Patients with patent shunts showed marked improvement compared to those with occluded shunts. CONCLUSION: morbidity and mortality following angioplasty ± stenting and TIPS are low with satisfactory outcome. Proper patient selection and management of shunt dysfunction are crucial in improvement.

区域作为人类、自然、社会共同作用和互相影响的复杂系统,对区域进行生态量化建模与模拟仿真,是实现区域可持续发展战略的关键。传统机器学习方法对区域生态系统建模取得了一定的成果,但难以确定学习特征和实现时空模拟。深度学习不需事先确定训练特征,具有优异的特征学习能力,能够提高模型预测精度,因此利用深度学习进行建模具有显著优势。本文使用植被净初级生产力(NPP)、气溶胶光学厚度(AOD)和人口格网数据,充分利用深度学习的优点,采用最优深度神经网络时空模拟,得到了河南省2007-2014年3 km分辨率的生态赤字空间分布图和河南省2015-2020年的生态赤字时间预测结果并进行分析,为区域生态的科学管理和建设供科学依据和参考。

We describe an unusual clinical and diagnostic featureof a patient with multiple sclerosis(MS). A 25-yearold woman was admitted to the Neurology department(December 2009) with one month history of rapid cognitive deterioration. She had poor cognition, dysphasia, reduction in visual acuity and temporal pallor of the optic discs. She had prolonged latencies of P100 component of visual evoked potentials(VEPs). Magnetic resonance imaging(MRI)-brain showed multifocal large(≥ 3 cm) white-matter hypointense lesions in T1 W and hyperintense in T2 W and fluid-attenuated inversion recovery images and patchy enhancement. A diagnosis of tumefactive MS was given. She received two consecutive 5-d courses of 1 g daily intravenous methylprednisolone for 2 mo and oral prednisolone in dose of 80 mg twice/daily in between. At the 3rd month, Mini Mental State Examination and VEPs returned to normal but not the MRI. Patient continued oral steroids after hospital discharge(March 2010) for 9 mo with significant MRI improvement after which tapering of steroids started for a year. The patient refused immunomodulation therapy due to her low socioeconomic status. Neither clinical relapse nor new MRI lesions were observed throughout the next 4 years. In spite of the aggressive course of tumefactive MS variant, good prognosis may be seen in some patients.

Pre-harvest aflatoxin contamination occurs in maize following kernel colonization by Aspergillus flavus. Aflatoxin contamination resistance is a highly desired trait in maize breeding *** identification of novel sources of resistance to pre-harvest aflatoxin contamination is a major focus in germplasm screening efforts. Here, we performed a field evaluation of 64 inbred lines over two years for pre-harvest aflatoxin contamination. Topcrosses were also performed with two testers, B73 and mo17, to generate 128 F1 hybrids which were also evaluated over two years. Hybrid performance was used to calculate both general combining ability(GCA) of the inbreds, and observed heterosis for aflatoxin resistance. Over both years of the study, aflatoxin concentrations ranged from 80 ± 47 to 17,617 ± 8816 μg kg^(-1) for inbreds, and from 58 ± 39 to 2771 ± 780 μg kg^(-1) for hybrids with significant variation between years and lines. The inbred lines CML52, CML69, CML247, GT-603, GEMS-0005, Hi63, Hp301, and M37 W showed mong these, CML52, GT-603, and Hi63 also showed significant GCA with the testers in hybrid progeny. CML52, GT-603, and M37 W also showed heterotic effects of-13.64%,-12.47%, and-24.50%, respectively, with B73 resulting in reduced aflatoxin contamination. GT-603 also showed a similar heterotic effect for aflatoxin contamination,-13.11%, with mo17 indicating that this line may serve as a versatile source of aflatoxin contamination resistance in breeding programs.

We describe clinical, diagnostic features and follow up of a patient with a vanishing brain lesion. A 14-yearold child admitted to the department of Neurology at September 2009 with a history of subacute onset of fever, anorexia, vomiting, blurring of vision and right hemiparesis since one month. Magnetic resonance imaging(MRI) of the brain revealed presence of intraaxial large mass(25 mm × 19 mm) in the left temporal lobe and the brainstem which showed hypointense signal in T1 W and hyperintense signals in T2 W and fluid attenuated inversion recovery(FLAIR) images and homogenously enhanced with gadolinium(Gd). It was surrounded by vasogenic edema with mass effect. Intravenous antibiotics, mannitol(2 g/12 h per 2 d) and dexamethasone(8 mg/12 h) were given to relief manifestations of increased intracranial pressure. Whole craniospinal radiotherapy(brain = 4000 CGy/20 settings per 4 wk; Spinal = 2600/13 settings per 3 wk) was given based on the high suspicion of neoplastic lesion(lymphoma or glioma). Marked clinical improvement(up to complete recovery) occurred within 15 d. Tapering of the steroid dose was done over the next 4 mo. Follow up with MRI after 3 mo showed small lesion in the left antero-medial temporal region with hypointense signal in T1 W and hyperintense signals in T2 W and FLAIR images but did not enhance with Gd. At August 2012, the patient developed recurrent generalized epilepsy. His electroencephalography showed the presence of left temporal focus of epileptic activity. MRI showed the same lesion as described in the follow up. The diffusion weighted images were normal. The seizures frequency was decreased with carbamazepine therapy(300 mg/12 h). At October 2014, single voxel proton(1H) MR spectroscopy(MRS) showedreduced N-acetyl-aspartate(NAA)/creatine(Cr), choline(Cho)/Cr, NAA/Cho ratios consistent with absence of a neoplasm and highly suggested presence of gliosis. Hepatocellular carcinoma(HCC) is ranked as the 5th common type of cancer worldwide and is considered as the 3rd common reason for cancer-related deaths. HCC often occurs on top of a cirrhotic liver. The prognosisis determined by several factors; tumour extension, alpha-fetoprotein(AFP) concentration, histologic subtype of the tumour, degree of liver dysfunction, and the patient's performance status. HCC prognosis is strongly correlated with diagnostic delay. To date, no ideal screening modality has been developed. Analysis of recent studies showed that AFP assessment lacks adequate sensitivity and specificity for effective surveillance and diagnosis. Many tumour markers have been tested in clinical trials without progressing to routine use in clinical practice. Thus, surveillance is still based on ultrasound(US) examination every 6 mo. Imaging studies for diagnosis of HCC can fall into one of two main categories: routine non-invasive studies such as US, computed tomography(CT), and magnetic resonance imaging, and more specialized invasive techniques including CT during hepatic arteriography and CT arterial portography in addition to the conventional hepatic angiography. This article provides an overview and spotlight on the different diagnostic modalities and treatment options of HCC.
