Removal of diminutive colorectal polyps: A prospective randomized clinical trial between cold snare polypectomy and hot forceps biopsy

AIMTo compare the efficacy and safety of cold snare polypectomy (CSP) and hot forceps biopsy (HFB) for diminutive colorectal *** prospective, randomized single-center clinical trial included consecutive patients ≥ 20 years of age with diminutive colorectal polyps 3-5 mm from December 2014 to October 2015. The primary outcome measures were en-bloc resection (endoscopic evaluation) and complete resection rates (pathological evaluation). The secondary outcome measures were the immediate bleeding or immediate perforation rate after polypectomy, delayed bleeding or delayed perforation rate after polypectomy, use of clipping for bleeding or perforation, and polyp retrieval rate. Prophylactic clipping after polyp removal wasn’t routinely *** hundred eight patients were randomized into the CSP (102), HFB (106) and 283 polyps were evaluated (CSP: 148, HFB: 135). The en-bloc resection rate was significantly higher with CSP than with HFB [99.3% (147/148) vs 80.0% (108/135), P < 0.0001]. The complete resection rate was significantly higher with CSP than with HFB [80.4% (119/148) vs 47.4% (64/135), P < 0.0001]. The immediate bleeding rate was similar between the groups [8.6% (13/148) vs 8.1% (11/135), P = 1.000], and endoscopic hemostasis with hemoclips was successful in all cases. No cases of perforation or delayed bleeding occurred. The rate of severe tissue injury to the pathological specimen was higher HFB than CSP [52.6% (71/135) vs 1.3% (2/148), P < 0.0001]. Polyp retrieval failure was encountered CSP (7), HFB (2).CONCLUSIONCSP is more effective than HFB for resecting diminutive polyps. Further long-term follow-up study is required.

The stay coefficient: a novel quantification of the relationship between stay time and travel time for urban shopping behavior analysis

Shoppers typically want to spend an amount of time at a destination that is proportional to the travel time required to arrive there;thus,the travel time can be considered the cost of their *** is likely to be the case across regions with different urban structures and *** purpose of this study was therefore to analyze the shopping behaviors contained in travel survey data from three metropolitan areas in Japan to identify common patterns and indicators based on travel time and stay time,thereby obtaining an understanding to inform future trade area *** the travel time and stay time associated with shopping behavior were found to be log-normally distributed regardless of metropolitan area,and four shopping behavior patterns common among the metropolitan areas were ***“stay coeffi-cient”was then defined to express the elasticity of stay time according to travel time,and its values were similar according to shopping behavior pattern regardless of metropolitan *** stay coefficient proposed in this study can therefore be applied to identify shopping behavior patterns in any urban area based on the relationship between travel time and stay time,realizing a novel approach to the analysis of and marketing for trade areas when planning the construction or renovation of commercial *** approach can help inform the decisions of urban policy makers,marketing advisors,and commercial facility operators,and should be of interest to researchers and practitioners working with geospatial,shopping,and other human behavioral characteristics.

Aims In a contact zone between related taxa,phenotypic variation can result from genetic and/or environmental *** study aimed to clarify the cause of phenotypic variation in leaf morphology of two Quercus crispula varieties—crispula(QCC)and mongolicoides(QCM)—in their contact zone along an altitudinal *** We measured 6 morphological traits of leaves and recorded genotypes of 13 nuclear microsatellite loci for 48 individuals in the contact zone and 24 individuals in each of the reference populations of QCC and *** constructed a model explaining the phenotypic variation(leaf morphology)in relation to environmental(altitude)and genetic(ancestry from the reference population)*** Findings Both morphological and genetic markers distinguished the two varieties in the reference populations *** were able to confirm the power of both morphological and genetic *** within the contact zone population had intermediate ancestry that was slightly biased to QCM ancestry,and the distribution of their morphologies overlapped with those of the two varieties in the reference *** effect of altitude on leaf morphological traits was significant,while that of ancestry was *** of ancestry and interclass heterozygosity in the contact zone population resembled those in F2 or later generation *** results indicate that in the contact zone between QCC and QCM,there is no ongoing hybridization,but environmental pressure has created an altitudinal gradient in morphological traits through phenotypic plasticity and/or variation in functional genes.

Diagnostic criteria for autoimmune pancreatitis （AIP） have been proposed and used clinically because, despite its unique clinicopathological features, AIP does not have disease-specific serological tests for confirmation. However, diagnosis of a patient with pancreatic lesions mimicking cancer who deviates from these diagnostic criteria is still difficult. We present herein a patient with a variant form of AIP successfully diagnosed by fine-needle biopsy, whose response to steroid therapy was excellent. A 55-year-old Japanese man was admitted to hospital because of jaundice and pancreatic head mass. AIP was considered as one of the differential diagnoses; however, as the patient showed neither pancreatic duct narrowing nor immunological abnormalities, he did not meet the Japanese diagnostic criteria for AIP. Histopathology of the pancreatic mass demonstrated abundant infiltration by lymphocytes and interstitial fibrosis, which suggested AIP. Immunoreaction to IgG4, which is supposed to be specific to AIP, was not observed; however, response to subsequent prednisolone therapy was good, with dramatic pancreatic head mass regression. Aside from the pancreatic head mass, diffusely spreading small lesions were observed throughout the liver. The likelihood of a potential association with extrapancreatic lesions of AIP was considered and led us to carry out a liver biopsy, which revealed biliary hamartoma, also called yon Meyenburg complex （VMC）. As IgG4-positive plasma cell infiltration was not demonstrated in the hamartomatous regions, the hepatic condition was thought to have occurred incidentally; however, to the best of our knowledge, this is the first report in which the association between AIP and VMC was investigated and discussed.
