Transurethral Resection of Prostate Lessons Learnt

Introduction & Objective: TURP (transurethral resection of prostate) is the gold standard of treatment for patients with BPE (benign prostatic enlargement). But TURP is criticized due to its morbidity and mortality. We analyzed our two decade data and found that the morbidity can be reduced by taking simple precautions in the later decade. Methods: A total of 13,786 patients were studied in the past two decades (Jan. 1994-Dec. 2014) operated by single surgeon, grouped in A (Jan. 1994-Dec. 2004) and B (Jan. 2004-Dec. 2014). Patients with concomitant stricture urethra, vesical calculus/tumor and carcinoma prostate were excluded from the study. Patients were operated under good antibiotic cover. The demographic profile, operative findings, weight, biopsy and post op follow up for 6 months & the immediate and immediate six months post operative complications were recorded. All patients in group B had urethral dilatation up to 27 F, restricted resection time of 90 minutes, concomitant bilateral BNI (bladder neck incision) in small glands ( 0.01) in group B. In the Late complications there is significant drop in the incidence of meatitis, meatal stenosis, stricture urethra & bladder neck stenosis (p > 0.001) in group B. Conclusions: TURP is still the gold standard of treatment for BPE and its morbidity can be reduced by simple measures like restricting resection time to 90 minutes urethral dilatation before insertion of resectoscope, catheter care & concimitent concomitant BNI in glands of < 30 gms.

Detection of urinary antigens and their seroreactivity with serum of patients in Leishmania donovani infection

Objective:To detect leishmanial antigens in pre and post treated urine of visceral leishmaniasis (VL) ***:Urine and serum sample from three VL patients were collected. Ammonium sulphate precipitation and purification of urine sample was done for proteins *** PAGE of proteins was done followed by western blotting,with the patient’s pre and post treatment ***:Eight proteins of molecular weights 17 kDa,25 kDa,28 kDa,42 kDa, 47 kDa,54 kDa,60 kDa and 85 kDa were detected in the urine of VL patients before treatment. After treatment with miltefosine,none of the above proteins was detected in urine *** western blot analysis with pre treatment serum confirmed the antigenicity of four urinary proteins of molecular weights 25 kDa,28 kDa,54 kDa and 60 *** seropositivity with 25 kDa and 28 kDa antigens was negative with serum obtained after the completion of ***:In the context to unavailability of a prognostic tool,urinary leishmanial antigens may offer a better choice and may also be useful as immunoprophylactic candidates.

Palaeobotanical and biomarker evidence for Early Permian(Artinskian) wildfire in the Rajmahal Basin, India

This study provides a combined analysis on the palynology, fossil charcoal and biomarkers of the subsurface coal deposits from a borehole RMB #2 drilled at the Dhulia Coal Block, Rajmahal Basin, India, in attempts to establish the chronology of sedimentation and to propose palaeobotanical as well as geochemical evidence for the occurrence of wildfires in these sediments. The palynological investigation suggests a Scheuringipollenites barakarensis palynoassemblage from the lower Barakar Formation, dated as Artinskian(Early Permian) in age. This assemblage reveals the dominance of Glossopteridales and sub-dominance of taxa belonging to Cordaitales and *** charcoal in sediments is usually recognized as a direct indicator for the occurrence of palaeo-wildfires. More data involving the anatomical features of fossil charcoal analyzed by Field Emission Scanning Electronic Microscope broaden our knowledge on Early Permian wildfires from the peninsula of India. The studied macroscopic charcoal fragments exhibit anatomical details such as homogenized cell walls, uniseriate simple and biseriate alternate pitting on tracheid walls and rays of varying heights pointing to a gymnospermous wood affinitity. The excellent preservation of charcoal fragments, shown by their large sizes and almost unabraded edges, suggests a parautochthonous origin. The embedded biomarker study performed for charcoal sediments and its characterization demonstrate the presence of n-alkanes, isoprenoids, terpenoids and aromatic compounds. A bimodal distribution pattern of n-alkanes with a Cmaxat n-C25 is identified. Diterpenoids and pentacyclic terpenoids are identified, indicating the input of an early conifer vegetation and bacterial activity, respectively. The identified polyaromatic hydrocarbon(PAH) compounds, together with the charcoal fragments, clearly suggest that repeated wildfire events occurred during the deposition of these Artinskian sediments in the Rajmahal Basin.

The shale deposits of Damodar Valley have received great attention since preliminary studies indicate their potential for shale gas. However, fundamental information allied to shale gas re- servoir characteristics are still rare in India, as exploration is in the primary stage. In this study, Ba- rakar shale beds of eastern part of Jharia Basin are evaluated for gas reservoir characteristics. It is evident that Barakar shales are carbonaceous, silty, contains sub-angular flecks of quartz and mica, irregular hair-line fractures and showing lithological variations along the bedding planes, signifying terrestrial-fluviatile deposits under reducing environment. The values of TOC varies from 1.21 wt.% to 17.32 wt.%, indicating good source rock potentiality. The vitrinite, liptinite, inertinite and mineral matter ranging from 0.28 vol.% to 12.98 vol.%, 0.17 vol.% to 3.23 vol.%, 0.23 vol.% to 9.05 vol.%, and 74.74 vol.% to 99.10 vol.%, respectively. The ternary facies plot of maceral composition substan- tiated that Barakar shales are vitrinite rich and placed in the thermal-dry gas prone region. The low values of the surface area determined following different methods point towards low methane storage capacity, this is because of diagenesis and alterations of potash feldspar responsible for pore blocking effect. The pore size distribution signifying the micro to mesoporous nature, while Type II sorption curve with the H2 type of hysteresis pattern, specifies the heterogeneity in pore structure mainly combined-slit and bottle neck pores.

In this investigation, optimization of tribological performance parameters of Al-6061T6 alloy reinforced with SiC (15% by weight) and Al2O3 (15% by weight) particulates having particle size of 37 μm each has been presented. The wear and frictional properties of the hybrid metal matrix composites have been studied by performing dry sliding wear test using pin-on-disc wear tester. A L27 orthogonal array is selected for the analysis of the data. From the test results it is observed that sliding distance has the significant contribution in controlling the friction and wear behaviour of hybrid composites. A confirmation test is also carried out to verify the accuracy of the results obtained through the optimization. In addition an optical micrograph test is also performed on the wear tracks to study the wear mechanism.

Haar Wavelet Quasilinearization Approach for Solving Nonlinear Boundary Value Problems

Objective of our paper is to present the Haar wavelet based solutions of boundary value problems by Haar collocation method and utilizing Quasilinearization technique to resolve quadratic nonlinearity in y. More accurate solutions are obtained by wavelet decomposition in the form of a multiresolution analysis of the function which represents solution of boundary value problems. Through this analysis, solutions are found on the coarse grid points and refined towards higher accuracy by increasing the level of the Haar wavelets. A distinctive feature of the proposed method is its simplicity and applicability for a variety of boundary conditions. Numerical tests are performed to check the applicability and efficiency. C++ program is developed to find the wavelet solution.

土丁桂属草药是作用于中枢神经系统的印度传统药物,特别是对促进记忆和改善智力有较好疗效。从印度传统医学和梵语文献中提取的大量信息提示,冠以土丁桂属草药名称的植物类别共4种:旋花科田旋花、旋花科土丁桂、蝶形花科蝴蝶花豆和龙胆科穿心草。这些草药名称均在古代文献中有所记载,可单独使用或与其他药材组合成各种草药处方。本文就现有的科学信息,如土丁桂属不同植物来源物种的生药学特征、化学成分、药理作用、临床前及临床应用等方面进行述评,以期为科学地应用土丁桂提供依据。此外,还可根据不同植物来源土丁桂属草药的不同功效进行鉴别应用。
