Effects of Histotripsy on Local Tumor Progression in an in vivo Orthotopic Rodent Liver Tumor Model

Objective and Impact *** is the first longitudinal study investigating the effects of histotripsy on local tumor progression in an in vivo orthotopic,immunocompetent rat hepatocellular carcinoma(HCC)*** is the first noninvasive,nonionizing,nonthermal,mechanical ablation technique using ultrasound to generate acoustic cavitation to liquefy the target tissue into acellular debris with millimeter ***,histotripsy has demonstrated in vivo ablation of noncancerous liver ***.N1-S1 HCC tumors were generated in the livers of immunocompetent rats(n=6,control;n=15,treatment).Real-time ultrasound-guided histotripsy was applied to ablate either 100%tumor volume+up to 2 mm margin(n=9,complete treatment)or 50-75%tumor volume(n=6,partial treatment)by delivering 1-2 cycle histotripsy pulses at 100 Hz PRF(pulse repetition frequency)with p−≥30 MPa using a custom 1 MHz *** were monitored weekly using MRI(magnetic resonance imaging)for 3 months or until tumors reached~25 *** revealed effective post-histotripsy reduction of tumor burden with near-complete resorption of the ablated tumor in 14/15(93.3%)treated *** showed<5 mm shrunken,non-tumoral,fibrous tissue at the treatment site at 3 *** with increased tumor burden(3/6 control and 1 partial treatment)were euthanized early by 2-4 *** 3 other controls,histology revealed fibrous tissue at original tumor site at 3 *** was no evidence of histotripsy-induced off-target tissue *** and partial histotripsy ablation resulted in effective tumor removal for 14/15 rats,with no evidence of local tumor progression or recurrence.

Hepatocellular carcinoma(HCC)is a leading cause of morbidity and mortality worldwide,with rising clinical and economic burden as incidence *** are a multitude of evolving treatment options,including locoregional therapies which can be used alone,in combination with each other,or in combination with systemic *** treatment options have shown to be effective in achieving remission,controlling tumor progression,improving disease free and overall survival in patients who cannot undergo resection and providing a bridge to transplant by debulking tumor burden to downstage *** locoregional therapy(LRT),it is crucial to provide treatment response assessment to guide management and liver transplant ***,Liver Imaging Reporting and Data Systems(LI-RADS)Treatment Response Algorithm(TRA)was created to provide a standardized assessment of HCC following *** TRA provides a step by step approach to evaluate each lesion independently for accurate tumor *** this review,we provide an overview of different locoregional therapies for HCC,describe the expected post treatment imaging appearance following treatment,and review the LI-RADS TRA with guidance for its application in clinical *** to other publications,we will also review emerging literature supporting the use of LI-RADS for assessment of HCC treatment response after LRT.

Flow and passive scalar transfer characteristics around a row of interacting tandem cylinders

Transfer and transport of a passive scalar as well as flow structures of unbounded flow over a row of 2,3,4 and 5 equispaced identical cylinders at Re=90 and pitch ratios 1.05D≤S≤7D are *** are performed through high fidelity simulations using a novel sharp-interface immersed boundary *** flow regimes are identified based on the nature of flow in the gap between the first two cylinders:dormant in the gap where fluid in the gap is motionless;symmetric in the gap where fluid in the gap behaves like a cavity flow;alternating in the gap where fluid in the gap is trapped but flaps up and down;wake in the gap where periodic vortices are shed in the gap;and bi-stable flow regime where vortices are shed in the gap for some time and then quasi-periodic twin vortices prevail for other times.A jump in values of hydrodynamic forces,pressure coefficient,Strouhal number,and Sherwood number is observed as pitch ratio increases due to change in flow *** high pitch ratio,a convectively unstable two-row vortical train is formed by the second *** cylinders immersed in the two-row vortical structures have no effect on flow structure downstream or upstream and portray poor scalar transfer *** effect of configuration size on flow structures is found not to be as prominent as that of pitch ratio.

商业机构一定程度上重视培训课程,但在总体而言,对于社会企业的培训课程及其成果的研究很有限。本论目的在于研究管理人员培训的动因和影响。论文以制度理论为基础,探讨社会企业管理人员培训的原因,以及培训如何提高企业的合理性和效益。为检验我们的模型,我们对巴基斯坦社会企业中99名高级管理人员展开实证研究。实验分析结果验证了下述论点,管理人员的培训是依赖制度力量(如标准、拟态和胁迫施压)相互协同来推动的。这些制度力量激励了社会企业,使得管理者采取培训计划以增强其内外的合理性,提高企业的效益。同时也意味着社会企业可以通过培训课程的开发提高其合理性和效益。目前的研究重申了以往的研究结论,表明制度力量在采取某些做法上的重要性,例如培训管理人员和考察培训结果,并进一步解释了培训对合理性和效益的影响。研究采用偏最小二乘结构方程模型(PLS-SEM)进行数据分析。研究强调了在社会企业中采用培训课程时制度力量的重要性。综上所述,目前的研究为社会企业内高层管理人员,寻求促进企业内外合理性和提高效益提供了重要的见解。论文通过发现制度力量和社会企业效益间的实证性关联,对该研究领域做出了重要贡献。

Bullous pemphigoid (BP) is the most prevalent autoimmune sub-epidermal blistering disease that affects mainly the elderly and could lead to serious morbidity. It has numerous risk factors and triggers, including an aging population with several comorbidities and drug exposure. In the published paper, we reported a case about a 32 years old male patient with unknown medical conditions who presented with erythematous patches and plaques on the scalp, face, and trunk with scattered blisters two weeks after initiating doxycycline treatment for folliculitis. The exact pathogenesis of the drug-reaction in drug- associated bullous pemphigoid (DABP) remains controversial. In conclusion, it is crucial for clinicians to be aware of DABP when prescribing doxycycline. The purpose of this case report is to raise awareness of the possible association between bullous pemphigoid and doxycycline.

On the Mechanism of Latex Particles Formation in Polymerization in Heterogeneous Monomer-Water System

Investigations show that in monomer water heterogeneous systems polymerization, located in the interface, can disperse the system. Following polymerization in the density gradient field it is shown that in such systems latex particles are nucleated from monomer microdroplets, containing a certain amount of polymer molecules. It could be considered that, independently of the nature of the monomer itself, polymerization in monomer water statistic heterogeneous systems modulate emulsion polymerization process.

Patient Enablement in Chronic Diseases in Primary Health Care, Riyadh City, KSA

Background: Chronic diseases continue to cause high morbidity and mortality in Saudi Arabia. Patients severing from diabetes mellitus, hypertension and associated complications have recently increased and most of these patients find it extremely difficult to understand or cope with their illness. The objective of this study is to determine the level of patients’ enablement in chronic disease and its predictors. Methods: A community based cross-sectional study was conducted between December 2014 and January 2015. Six hundred and four (604) Patients attending the Chronic Disease Clinic in Alwazarat Health center were randomly selected to participate in the study. Patients aged 18 years and above, who willingly agreed to participate, were included in the study. Self-reported questionnaire was used to determine patient level of enablement. Descriptive statistics such as mean and median were calculated and binary logistic regression was employed to determine the predictors of patient’s enablement to chronic disease. Results: Our results show that five hundred and sixty five (565) out of (604) patients participated in the study with 86.6% response rate. Type 2 diabetes mellitus affecting 40.65% while hypertension affecting 37.79% of the patients in Al wazarat health center. Patient’s enablement to chronic disease was very low and ranged between 2.41 and 1.53 out of 5.0. Binary logistic regression shows that age (male: OR;0.84, 95% CI, 0.72 - 1.04, female. OR;1.04, 95% CI 0.88 - 1.39), marital status (male: OR;0.72, 95% CI 0.54 - 1.11, female: OR 1.01;95% CI 0.82 - 1.29), patient educational level and number of problems discussed with physician and consultation length between male patients and their physician were statistically significant and correlated with patients enablement to chronic disease (P < 0.05). Distributed Air-Conditioning Energy Management System within the Smart Grid Context

-Air-conditioning （AC） systems are the major energy consumption units in residential and commercial buildings. In the context of smart grid, optimizing AC operations leads to substantial saving in energy consumption, reducing the consumer＇s bill and contributing to the environment by minimizing carbon emissions from generating stations. This paper presents a distributed AC energy management system for buildings by using networked master-slaves controller architecture. The proposed system was designed, simulated, and experimentally tested by using real AC units in a students＇ residence hall. Based on the students＇ class schedules, several operational scenarios were implemented and tested. The proposed system implementation leads to a 40% to 60% saving of the consumed energy by the tested units. The same energy management scheme can be applied and implemented in other commercial and residential buildings.

Case Report: Four Siblings with Osteopetrosis and Pyloric Stenosis and Three Cousins with Osteopetrosis

Osteopetrosis incidence is less than 1:200,000 births in most populations. It’s more common in consanguineous people as it’s unusual for two members of the same family. The incidence of Hypertrophic pyloric stenosis is 1 in 300 - 900 newborns. Hypertrophic pyloric stenosis is due to hypertrophy of the smooth muscle of the pyloric sphincter. The classic age of occurrence is the first few months of life, and the classic presentation is non-bilious projectile vomiting after feeding. We report a rare association of osteopetrosis and pyloric stenosis in four siblings and osteopetrosis in three cousins. All four patients were operated on and followed by nephrology and metabolic departments for osteopetrosis and metabolic acidosis.
