Significance of low level infliximab in the absence of anti-infliximab antibodies

AIM:To evaluate the prevalence of double negative(DN)sera and the mechanisms responsible for DN ***:Sera of inflammatory bowel disease patients treated with infliximab(IFX)were tested for drug/antibodies to infliximab(ATI)trough levels and the proportion of DN results was compared between a commercially available double antigen ELISA(with labeled IFX as the detection antibody)and an antilambda ELISA(with anti-human lambda chain detection antibody).Repeat testing with lower than customary serum dilution(1:10)was *** with DN status were matched with IFX+/ATI-controls and were followed-up for subsequent development of nontransient ATI to investigate if DN status precedes ***:Of 67 sera obtained at time of loss of response,only 6/67(9%)were DN by anti-lambda ELISA compared to 27/67(40%)with double antigen ELISA(P<0.001,Fisher’s Exact test).Of the latter27 sera,22%were also DN by anti-lambda ELISA,whereas 44%were actually IFX positive(IFX+ATI-),30%were ATI positive(IFX-ATI+)and 4%were double positive(IFX+ATI+).Re-testing using a 1:10 dilution converted most DN results into IFX+and/or ATI+*** with DN status had shorter survival free of non-transient ATI compared with matched controls(log rank test,P<0.001).In 9/30(30%)of these patients,non transient ATI occurred before and after the event at which the DN serum was obtained,supporting the view that a DN result may represent aparticular time-point along the two curves of ATI titer rise and infliximab drug level ***:DN status may result from false negative detection of IFX or ATI by double antigen ELISA,suggesting a transitional state of low-level immunogenicity,rather than non-immunological clearance.

Dynamic interaction of an eccentric multipole cylindrical radiator suspended in a fluid-filled borehole within a poroelastic formation

Acoustic radiation and the dynamic field induced by a cylindrical source of infinite extent, undergoing angularly periodic and axially-dependent harmonic surface vibrations, while eccentrically suspended in a fluid-filled cylindrical cavity embedded within a fluid-saturated porous elastic formation, are analyzed in an exact manner. This configuration, which is a realistic idealization of an acoustic logging tool suspended in a fluid-filled borehole within a permeable surrounding formation, is of practical importance with a multitude of possible applications in seismo-acoustics. The formulation utilizes the novel features of Biot dynamic theory of poroelasticity along with the translational addition theorem for cylindrical wave functions to obtain a closedform series solution. The basic dynamic field quantities such as the resistive and the reactive components of the modal acoustic radiation impedance load on the source in addition to the radial and transverse stresses induced in the surrounding formation by an eccentric pulsating/oscillating cylinder in a water-filled borehole within a water-saturated Ridgefield sandstone medium are evaluated and discussed. Special attention is paid to the effects of source eccentricity, excitation frequency, and mode of surface oscillations on the modal impedance values and the dynamic stresses. Limiting cases are considered and good agreements with available solutions are obtained.

Seismic isolation effect of lined circular tunnels with damping treatments

The Havriliak-Negami model for dynamic viscoelastic material behavior and Biot＇s theory of poroelasticity are employed to develop an exact solution for three-dimensional scattering effect of harmonic plane P-SV waves from a circular cavity lined with a multilayered fluid-filled shell of infinite length containing viscoelastic damping materials and embedded within a fluid-saturated permeable surrounding soil medium. The analytical results are illustrated with numerical examples where the effects of liner/coating structural arrangement, viscoelastic material properties, liner-soil interface bonding condition, seismic excitation frequency, and angle of incidence on the induced dynamic stress concentrations are evaluated and discussed to obtain representative values of the parameters that characterize the system. It is demonstrated that incorporating viscoelastic damping materials with a low shear modulus in the constrained layer configuration is an efficient means of enhancing the overall seismic isolation performance, especially for near-normally incident seismic shear waves where the amplitudes of induced dynamic stresses may be reduced by up to one-third of those without isolation in a relatively wide frequency range. Some additional cases are considered and good agreements with solutions available in the literature are obtained.

Duffy blood group genotypes among malaria Plasmodium vivax patients of Baoulch population in southeastern Iran

Objective:To determine the distribution of Duffy blood group genotypes in Balouch population as a major ethnic group that living in a sub-tropical area in south East of ***:In this study,the Duffy blood group FY phenolypes were determined using indireet anti-globulin technique and also genotype by PCR-RFLP in 160 vivax malaria patients and 160 control ***:The results showed that the most common Duffy geitolype was FYA/FYB(46.6%)followed by FYA/FYA(15.3%),FYA/FYO(14.4%),FYB/FYO(11.9%,),FYB/FYB(10%)and FYO/FYO(1.9%).In case individuals,frequency of FY *** and FYO alleles were 0.471,0.431and 0.097,respectively compaired to 0.444,0.353 and 0.203,respectively in control(non-infected)***:This data provide evidence that individuals with the FYA/FYB genotype have higher susceptibility to malaria and there are significant associations between Duffy blood group variants and susceptibility or resistance to vivax malaria.

Health care associated infections, antibiotic resistance and clinical outcome: A surveillance study from Sanandaj, Iran

AIM: To study the antibiotic susceptibility patterns of gram-negative healthcare associated bacterial infections at two tertiary hospitals in the Sanandaj city, Kurdistan Province, ***: From January 2012 to December 2012, all positive cultures from potentially sterile body fluids were gathered. They sent to professor Alborzi clinical microbiology center in Shiraz for further analysis and susceptibility testing. The antibiotic susceptibility was determined using the Kirby-Bauer method(disk diffusiontechnique). The Results were interpreted according to Clinical and Laboratory Standards Institute guidelines against a series of antimicrobials. World Health Organization definitions for Healthcare associated infections were ***: Seven hundred and thirty-two positive cultures were reported from both hospitals. Seventynine isolates/patients fulfilled the study criteria for healthcare associated gram-negative infections. The most frequent bacterial cultures were from the pediatric wards(52%). Serratia marcescens(S. marcescens)(38%) Escherichia coli(E. coli)(19%), Klebsiella pneumoniae(K. pneumoniae)(19%), Acinetobacter baumannii(6%), Enterobacter species(6%), Serratia odorifera(4%) and Pseudomonas species(5%) were the most frequently isolated organisms. The susceptibility pattern of common isolates i.e., S. marcescens, E. coli and K. pneumoniae for commonly used antibiotics were as follows: Ampicillin 3.3%, 6.7%, 20%; gentamicin 73.3%, 73.3%, 46.7%; ceftazidim 80%, 73.3%, 33.3%; cefepim 80%, 86.7%, 46.7%; piperacillin/tazobactam 90%, 66.7%, 86.7%; ciprofloxacin 100%, 73.3%, 86.7%; imipenem 100%, 100%, 100%, respectively. CONCLUSION: The most effective antibiotics against gram-negative healthcare associated infections are imipenem followed by ciprofloxacin. The resistance rate is high against ampicillin and cephalothin. The high mortality rate(46.1%) associated with S. marcescens is alarming.

Investigations on landmine detection by BF_3 detector

Experiments were carried out to investigate the possible use of neutron backscattering for the detection of polyethylene (PE) sample buried in the soil. In detection of landmine by neutrons, the neutron detector and its shield play an important role. In this paper, the effects of graphite, heavy water, polyethylene and boric acid moderators on the flux of back scattered neutrons were investigated. We have also experimentally verified the effect of BF3 detector shield and obtained good agreement with theory.

Integrated photonic fractional convolution accelerator

An integrated photonic circuit architecture to perform a modified-convolution operation based on the discrete fractional Fourier transform(DFrFT) is introduced. This is accomplished by utilizing two nonuniformly coupled waveguide lattices with equally spaced eigenmode spectra, the lengths of which are chosen so that the DFrFT and its inverse operations are achieved. A programmable modulator array is interlaced so that the required fractional convolution operation is performed. Numerical simulations demonstrate that the proposed architecture can effectively perform smoothing and edge detection tasks even for noisy input signals, which is further verified by electromagnetic wave simulations. Notably, mild lattice defects do not jeopardize the architecture performance, showing its resilience to manufacturing errors.

Protective Effect of Brassica napus *** against Inflammation Response in RAW 264.7 Cells

Objective:To demonstrate the anti-inflammatory activity of Brassica napus ***(BNH)in lipopolysaccharide(LPS)-stimulated RAW 264.7 ***:Composition analysis of BNH was conducted via gas chromatography-mass spectrometry after BNH were *** nitric oxide(NO)production was measured using the Griess *** E2(PGE2)production was evaluated with enzyme-linked immunosorbent *** effects of BNH on LPS-induced pro-inflammatory enzymes including inducible nitric oxide synthase(iNOS)and cyclooxygenase-2(COX-2)were evaluated using Western blot ***,phosphorylation of nuclear factor-kappa B(N F-k B)and nuclear translocation of N F-k B p65 were evaluated with Western blot analysis and immunofluorescence staining,***:Compared with LPS-stimulated cells,BNH markedly decreased the generation of NO and PGE_(2)in LPS-stimulated RAW 264.7 cells(P<0.01 or P<0.05).Moreover,BNH inhibited protein levels of iNOS and COX-2(P<0.01).Phosphorylation of NF-k B and nuclear translocation of NF-k B p65 was significantly inhibited by BNH(P<0.01 or P<0.05).Conclusion:The anti-inflammatory activities of BNH were mediated via blockage of the N F-k B signaling pathways in LPS-stimulated RAW 264.7 cells.
