AIM:To compare rifaximin and insulin-like growth factor(IGF)-1 treatment of hyperammonemia and brain edema in cirrhotic rats with portal ***:Rats with CCl4-induced cirrhosis with ascites plus portal vein occlusion and controls were randomized into six groups:Cirrhosis;Cirrhosis + IGF-1;Cirrhosis + rifaximin;Controls;Controls + IGF-1;and Controls + *** oral glutamine-challenge test was performed,and plasma and cerebral ammonia,glucose,bilirubin,transaminases,endotoxemia,brain water content and ileocecal cultures were measured and liver histology was ***:Rifaximin treatment significantly reduced bacterial overgrowth and endotoxemia compared with cirrhosis groups,and improved some liver function parameters(bilirubin,alanine aminotransferase and aspartate aminotransferase).These effects were associated with a significant reduction in cerebral water *** and cerebral ammonia levels,and area-underthe-curve values for oral glutamine-challenge tests were similar in rifaximin-treated cirrhotic rats and control group *** contrast,IGF-1 administration failed to improve most alterations observed in ***:By reducing gut bacterial overgrowth,only rifaximin was capable of normalizing plasma and brain ammonia and thereby abolishing low-grade brain edema,alterations associated with hepatic encephalopathy.

Dear Editor,Chromosome heteromorphisms are described as variations in size and morphology at specific regions that can be detected through classical banding *** are mitotically stable variants usually present in a heterozygous state(only one of the homologous chromosomes is heteromorphic).In humans,the most commonly detected heteromorphisms involve the heterochromatic regions of chromosomes 1,9,16,and Y(designated as lqh,9qh,16qh,and Yqh,respectively),and the short-arms,satellites,or stalks of the acrocentric chromosomes 13,14,15,21,and 22(e.g,for chromosome 13 designated as 13p.13ps,and 13pstk,respectively).Pericentric inversions involving the heterochromatic region of chromosomes l,9,and Y are also frequently observed.

AIM To describe one case of bilateral Tapia's syndrome in a liver transplanted patient and to review the *** We report a case of bilateral Tapia's syndrome in a 50-year-old man with a history of human immunodeficiency virus and hepatitis C virus child. A liver cirrhosis and a bi-nodular hepatocellular carcinoma, who underwent liver transplantation after general anesthesia under orotracheal intubation. Uneventful extubation was performed in the intensive care unit during the following hours. On postoperative day(POD) 3, he required urgent re-laparotomy due to perihepatic hematoma complicated with respiratory gram negative bacilli infection. On POD 13, patient was extubated, but required immediate re-intubation due to severe respiratory failure. At the following day a third weaning failure occurred, requiring the performance of a percutaneous tracheostomy. Five days later, the patient was taken off mechanical ventilation and severe dysphagia, sialorrea and aphonia revealed. A computerized tomography and a magnetic resonance imaging of the head and neck excluded central nervous injury. A stroboscopy showed bilateral paralysis of vocal cords and tongue and a diagnosis of bilateral Tapia's syndrome was performed. With conservative management, including a prompt establishment of a speech and swallowing rehabilitation program, the patient achieved full recovery within four months after liver transplantation. We carried out MEDLInE search for the term Tapia's syndrome. The inclusion criteria had no restriction by language or year but must provide sufficientavailable data to exclude duplicity. Modular protein engineering is suited to recruit complex and multiple functionalities in single-chain polypeptides. Although still unexplored in a systematic way, it is anticipated that the positioning of functional domains would impact and refine these activities, including the ability to organize as supramolecular entities and to generate multifunctional protein materials. To explore this concept, we have repositioned functional segments in the modular protein T22-GFP-H6 and characterized the resulting alternative fusions. In T22-GFP-H6, the combination of T22 and H6 promotes selfassembling as regular nanoparticles and selective binding and internalization of this material in CXCR4-overexpressing tumor cells, making them appealing as vehicles for selective drug delivery. The results show that the pleiotropic activities are dramatically affected in module-swapped constructs, proving the need of a carboxy terminal positioning of H6 for protein self-assembling, and the accommodation of T22 at the amino terminus as a requisite for CXCR4^+ cell binding and internalization. Furthermore, the failure of self-assembling as regular oligomers reduces cellular penetrability of the fusions while keeping the specificity of the T22-CXCR4 *** these data instruct how multifunctional nanoscale protein carriers can be designed for smart, protein-driven drug delivery, not only for the treatment of CXCR4^+ human neoplasias, but also for the development of anti-HIV drugs and other pathologies in which CXCR4 is a relevant homing marker.

Inferior vena cava thrombosis is an under-recognized entity associated with significant morbidity and mortality. This is the reason why, although the diagnosis is challenging, a high index of suspicion is required. Regarding this condition, we present the case of a 63-year-old man who had repeatedly visited the emergency room suffering from abdominal and back pain and painful lower limb edema. After several tests, including magnetic resonance imaging (MRI), he was diagnosed to have agenesis of left renal vein and inferior vena cava thrombosis, from hypercoagulable state secondary to Antiphospholipid Syndrome. He had anticoagulation treatment with low-molecular-weight heparin with good subsequent evolution. This study sets out a descriptive retrospective study of fifty cases of inferior vena cava thrombosis diagnosed in a third-level hospital in the north of Spain over a ten-year period (2010-2018). The aim of this article is to identify the epidemiology, predisposing factors and symptoms that characterize this entity, in order to be able to achieve an early diagnosis that allows us to initiate immediate treatment, minimizing acute and chronic complications of this disease.

Background:Aerobic capacity(AC)in adults with congenital heart disease(CHD)is often reduced,mainly due to low confidence levels towards physical activity(PA).The main objective of this study was to estimate the association between PA level and AC(measured as peak of oxygen consumption,VO_(2peak))in adults with ***:A total of 183 individuals(83 women and 100 men;mean(SD)age 36.9(11.0)years old)from Vall d’Hebron Hospital,Barcelona-Spain in 2019,participated in this cross-sectional *** AC was assessed by cardiopulmonary exercise testing(CPET)using a treadmill ramp *** values of metabolic equivalent of task(MET,MET-min·week-1)obtained by the short International Physical Activity Questionnaire(IPAQ),participants were divided into three categories of PA:health-enhancing PA(HEPA),minimally active,and ***:Median(SD)PA was 2737.2(2835.7)MET-min·week-1,with 60 participants(32.8%)reporting HEPA,91(49.7%)minimally active,and 32(17.5%)*** demonstrated a mean VO_(2peak) of 28.9(8.8)mL·Kg^(-1)·min^(-1),showing AC values on average 13%lower than expected in a healthy ***,PA and AC were positively *** for sex and age,an increase of 1000 MET-min·week^(-1) was associated with an increase in VO_(2peak) of 0.8 units(95%CI 0.4–1.2;pno differences in the degree of increase between sexes(p=0.427).Conclusion:These findings suggest that an increase in PA in patients with CHD significantly improves their AC,and hence,could be recommended when the goal is to improve their physical condition.
