针对可变持续时间心电图(ECG)数据信号的非平稳性和时序性问题,提出一种基于连续小波变换(CWT)和高阶统计量(hos)的心律失常识别算法。首先,针对可变持续时间ECG数据中每个样本的数据点数量不同,采用RR间期插值法预处理数据,并通过CWT将信号分解为不同的时频分量,从而使网络能够更好地提取心电信号中的时间和频率特征。其次,针对时序信息利用不充分的问题,提出基于hos和长短期记忆网络的时序挖掘模块,以捕捉和学习ECG信号中的长期依赖关系,从而有助于识别和理解特定的心律失常类别。通过在公开的ECG数据集MIT-BIN上进行的大量实验,验证所提方法的有效性和优越性。

baCKGROUND: Biliary atresia (ba) is the end results of an inflammatory process, which affect the intrahepatic and extrahepatic bile ducts, leading to fibrosis and obliteration of the biliary tract with the development of biliary cirrho- sis. Early diagnosis of ba is difficult, and there is no speci- fic therapy for ba at present. The purpose of this study was to investigate the diagnosis, treatment and postoperative outcome of ba and to explore new diagnostic and thera- peutic strategies. METHODS; Thirty-one children with biliary atresia from our hospital and 201 children with such disease from other hospitals in China were reviewed retrospectively in terms of diagnostic modalities, operative age, operative methods and long-term survival rates after operation. RESULTS : The operative age of the 31 patients varied from 13 to 270 days. It was 120 days in 3 ( 10% ). No children underwent transplantation for ba. Their 1-6 year survival rate on average was 32. 3% (10/31). The early operative rate in China was 12.9% (30/ 232). One child received liver transplantation with a long- term survival rate of 31.9%. CONCLUSIONS; Early recognition of babies with ba is critical for optimal intervention for preventing progressive fibrosis. Clinical presentation, imaging and laparoscopy are helpful in enhancing the early diagnostic rate of ba pa- tients. The long-term survival rates can be obtained after the improvement of operative methods, liver transplanta- tion for children with advanced ba, and development of other medical strategies.

baCKGROUND: Percutaneous ethanol injection has been widely used as a non-surgical therapy for liver cancer, but it has some shortcomings such as local diffusion and une- qual permeation. This study was designed to observe the volume, controllability and completeness of necrosis after injection of low concentration sodium hydroxide in the normal liver parenchyma so as to assess its possibility in treatment of liver cancer instead of ethanol. METHODS: Twenty-seven New Zealand rabbits were di- vided randomly into 9 groups (Aa, Ab, Ac, ba, Bb, Bc, Ca, Cb, and Cc) by a 3 × 3 (three-by-three) factorial de- sign, each consisting of 3 rabbits. Group A was given sodi- um hydroxide solution at a concentration of 5%, while B at 2.5% and C at 1% in liver parenchyma. Each group re- ceived three doses of the solution: a (0.2 ml), b (0.5 ml) and c (1.0 ml). Then another 3 rabbits as side-effect group were dropped with sodium hydroxide solution in their liver lobe space. Liver and renal function changes in all the rab- bits were compared after injection with pre-injection. RESULTS: All the lesions were localized. At the concentra- tion of 2.5% and 5%, the lesion volume increased with the dose increased from 0.2 ml to 1.0 ml (P hose before injection. CONCLUSIONS: 2.5% and 5% sodium hydroxide solution could control local complete necrosis in normal liver. With regard to safety, 2.5% alkali solution is considered promis- ing as a new agent for intratumoral injection therapy in- stead of ethanol.

The BCL6 （B-Cell Lymphoma 6） gene is a proto-onco- gene that is often expressed in diffuse large B-ceU lymphomas （DLBCLs）. BCL6 loss of function can kill DLBCL cells, demonstrating that BCL6 is necessary for the survival of DLBCL cells and could be a therapeutic target. In this study, we found that BCL6 protein levels were consistently upregulated in DLBCL tissues, whereas its mRNA levels varied randomly in tissues, suggesting that a post-transcriptional mechanism was involved in BCL6 regulation. We used bioinformatics analysis to search for mirNAs, which potentially target BCL6, and identified specific targeting sites for mir-10a in the 3＇-untranslated region （3＇-UTR） of BCL6. We fur- ther identified an inverse correlation between mir-10a levels and BCL6 protein levels, but not mRNA levels, in DLBCL tumor tissue samples. By overexpressing or knocking down mir-10a in DLBCL cells, we experi- mentally validated that mir-10a directly recognizes the 3＇-UTR of the BCL6 transcript and regulated BCL6expression. Furthermore, we demonstrated that nega- tively regulating BCL6 by mir-10a suppressed the pro- liferation and promoted apoptosis of DLBCL cells.

酸雨观测中pH测量负偏差的统计分析

该文对1992—2011年的酸雨观测资料中存在的pH测量负偏差进行统计分析。结果显示:除西北地区、青藏高原外的大部分酸雨观测站不同程度地出现pH值测量负偏差,总体上南方地区台站的pH测量负偏差问题较北方地区突出;随着测量仪器的更新换型、观测业务规范化管理水平的提升以及一些针对性技术措施的实施,存在pH测量负偏差问题的观测台站比例从20世纪90年代的40%以上下降为25%,观测资料中受pH测量负偏差影响的数据比例由10%下降到2%。pH负偏差的量值与降水pH值之间存在一定依存关系:降水pH值越低,负偏差问题越严重,pH负偏差的主要量值分布区间为-0,6～1,约占负偏差数据的80%。

background: Capecitabine and irinotecan combination therapy(XELIRI) has been examined at various dose levels to treat metastatic colorectal cancer(m CRC). Recently, in the Association of Medical Oncology of the German Cancer Society(AIO) 0604 trial, tri?weekly XELIRI plus bevacizumab, with reduced doses of irinotecan(200 mg/m^2 on day 1) and capecitabine(1600 mg/m^2 on days 1–14), repeated every 3 weeks, has shown favorable tolerability and eicacy which were comparable to those of capecitabine and oxaliplatin(XELOX) plus bevacizumab. The doses of capecit?abine and irinotecan in the AIO trial are considered optimal. In a phase I/II study, XELIRI plus bevacizumab(BIX) as second?line chemotherapy was well tolerated and had promising eicacy in Japanese ***: The Asian XELIRI Projec T(AXEPT) is an East Asian collaborative, open?labelled, randomized, phase Ⅲ clinical trial which was designed to demonstrate the non?inferiority of XELIRI with or without bevacizumab versus standard FOLFIRI(5?fluorouracil, leucovorin, and irinotecan combination) with or without bevacizumab as second?line chemo?therapy for patients with m CRC. Patients with 20 years of age or older, histologically conirmed m CRC, Eastern Coop?erative Oncology Group performance status 0–2, adequate organ function, and disease progression or intolerance of the irst?line regimen will be eligible. Patients will be randomized(1:1) to receive standard FOLFIRI with or with?out bevacizumab(5 mg/kg on day 1), repeated every 2 weeks(FOLIRI arm) or XELIRI with or without bevacizumab(7.5 mg/kg on day 1), repeated every 3 weeks(XELIRI arm). 目的探讨COVID-19恢复期患者血清SARS-CoV-2特异性IgM/IgG变化和患者免疫状况的特点及相关性。方法分析2020年7月1日~8月31日在湖北省中医院新冠肺炎康复门诊就诊的COVID-19康复出院6个月的COVID-19恢复期患者110例,其中IgM(+)/IgG(+)组32例,IgM(-)/IgG(+)组38例,IgM(-)/IgG(-)组40例。在复诊后第0、2和4周测定患者血清SARS-CoV-2特异性IgM和IgG抗体、超敏C反应蛋白(hs-CRP)、白介素6(IL-6)和淋巴细胞亚群。结果全部患者的血清hs-CRP和IL-6水平均在正常范围内;淋巴细胞亚群所有检测项目均在正常范围内的患者,0周12例(10.9%)、2周15例(13.6%)、4周18例(16.4%),各组之间差异无统计学意义(均P>0.05)。IgM(+)/IgG(+)组在复诊第2周和第4周时,CD8~+T细胞百分比最高,而NK细胞和B淋巴细胞的百分比最低(均P<0.05)。3组3个时间点NK细胞和B淋巴细胞百分比比较,IgM(-)/IgG(-)组最高,IgM(+)/IgG(+)组最低(均P<0.05)。结论COVID-19康复出院6个月患者SARS-CoV-2抗体逐渐转阴,但免疫状况并未随之恢复;其血清IgM和IgG的动态变化与恢复期患者的免疫状态密切相关。

目的促进医院妇产科门诊抗菌药物的合理应用。方法 2012年10月至2013年4月,每月随机选取25%具有抗菌药物处方权的妇产科门诊医师,采用等距抽样法抽取每名医师不少于50张处方,对其所开具的处方进行抗菌药物专项点评。结果被点评医师13人次,抽取抗菌药物处方数715张,其中不合理处方数72张,分别为书写不规范处方6张和用药不适宜处方66张,抗菌药物使用率为47.13%。结论该院妇产科门诊医师需进一步加强抗菌药物的合理应用。
