Efficacy of electroacupuncture on myocardial protection and postoperative rehabilitation in patients undergoing cardiac surgery with cardiopulmonary bypass:a systematic review and Meta-analysis

OBJECTIVE:To evaluate the efficacy of electroacupuncture(EA)intervention on myocardial protection and postoperative rehabilitation in patients undergoing cardiac surgery with cardiopulmonary bypass(CPB).METHODS:Eight databases,including Pub Med,Embase,the Cochrane Library,Web of Science,Chinese Bio Medical Literature Database,China National Knowledge Infrastructure Database,Wanfang Data,China Science and Technology Journal Database,and two clinical trial registries,were *** randomized controlled trials(RCTs)related to EA intervention in cardiac surgery with CPB were *** on the inclusion and exclusion criteria,two researchers independently screened articles and extracted *** the quality evaluation,RevMan 5.3 software was used for ***:Fourteen RCTs involving 836 patients were *** with the control treatment,EA significantly increased the incidence of cardiac automatic rebeat after aortic unclamping[relative risk(RR)=1.15,95%confidence interval(CI)(1.01,1.31),P<0.05;moderate].Twenty-four hours after aortic unclamping,EA significantly increased the superoxide dismutase[standardized mean difference(SMD)=0.96,95%CI(0.32,1.61),P<0.05;low],and interleukin(IL)-2[SMD=1.33,95%CI(0.19,2.47),P<0.05;very low]expression levels and decreased the malondialdehyde[SMD=-1.62,95%CI(-2.15,-1.09),P<0.05;moderate],tumour necrosis factor-α[SMD=-1.28,95%CI(-2.37,-0.19),P<0.05;moderate],and cardiac troponin I[SMD=-1.09,95%CI(-1.85,-0.32),P<0.05;low]expression levels as well as the inotrope scores[SMD=-0.77,95%CI(-1.22,-0.31),P<0.05;high].There was no difference in IL-6 and IL-10 expression *** amount of intraoperative sedative[SMD=-0.31,95%CI(-0.54,-0.09),P<0.05;moderate]and opioid analgesic[SMD=-0.96,95%CI(-1.53,-0.38),P<0.05;low]medication was significantly lower in the EA group than in the control ***,the postoperative tracheal intubation time[SMD=-0.92,95%CI(-1.40,-0.45),P<0.05;low]and intensive care unit stay[SMD=-1.

Predictive value of total cholesterol to high-density lipoprotein cholesterol ratio for chronic kidney disease among adult male and female in Northwest China

Background: Studies have found that the ratio of total cholesterol to high-density lipoprotein cholesterol (TC/HDL-C) was associated with the development of chronic kidney disease (CKD). However, the relationship in different genders was rarely discussed. The aim of this study was to explore this relationship and assess its predictive power for both males and ***: Based on a prospective cohort platform in northwest China, 32,351 participants without CKD were collected in the baseline and followed up for approximately 5 years. Cox proportional hazard model and restricted cubic spline regression analysis were performed to investigate the association between TC, HDL-C, TC/HDL-C and CKD in adult female and male. The clinical application value of the indicators in predicting CKD was evaluated by the receiver operator characteristic ***: During a mean follow-up of 2.2 years, 484 males and 164 females developed CKD. After adjusted for relevant confounders, for every one standard deviation increase in TC, HDL-C and TC/HDL-C, the hazard ratios (HRs) and 95% confidence intervals (95% CIs) for CKD were 1.17 (1.05-1.31), 0.84 (0.71-0.99), and 1.15 (1.06-1.25) for males, 0.94 (0.78-1.13), 0.58 (0.35-0.95), and 1.19 (1.01-1.40) for females, respectively. The results also showed that TC, HDL-C, and TC/HDL-C were associated with CKD in a linear dose-response relationship. The TC/HDL-C had the largest area under the curve (AUC) compared to TC and HDL-C, and the AUC among the females was larger than that among ***: The TC/HDL-C was significantly associated with CKD in adult males and females and has better clinical value in predicting CKD than TC and HDL-C, especially in females.

目的了解金昌队列职业人群颈动脉粥样硬化(CAS)发病情况及其与脑卒中发病的关系,为CAS及脑卒中的防治提供科学依据。方法依托金昌队列平台抽取2011年6月—2013年12月完成基线调查并进行颈部彩色多普勒超声检查且信息完整的职业人群为研究对象,并分别匹配2015年和2017年完成的第1期及第2期随访信息,在排除不合格对象后最终纳入6262名职业人群计算其CAS和脑卒中的累积发病率,应用多因素非条件logistic回归模型分析CAS、颈动脉内-中膜增厚和颈动脉硬化斑块发生的影响因素,并应用多因素Cox比例风险回归模型分析CAS、颈动脉内-中膜增厚和颈动脉硬化斑块者的脑卒中发病风险及发生脑卒中的影响因素。结果金昌队列职业人群第1期随访0.08~3.75年,平均随访(1.71±0.55)年;共发生CAS者1147例,累积发病率为18.32%,其中发生颈动脉内-中膜增厚者和颈动脉硬化斑块者分别为287例和860例,累积发病率分别为4.58%和13.73%。多因素非条件logistic回归分析结果显示,年龄≥45岁、目前仍吸烟、高血压、糖尿病、总胆固醇(TC)异常、职业为干部或技术人员和内勤人员是金昌队列职业人群发生CAS的危险因素,大专及以上文化程度是金昌队列职业人群发生CAS的保护因素;年龄≥45岁、职业为干部或技术人员、目前仍吸烟、高糖饮食和高血压是金昌队列职业人群发生颈动脉内-中膜增厚的危险因素,女性是金昌队列职业人群发生颈动脉内-中膜增厚的保护因素;女性、年龄≥45岁、职业为内勤人员、高血压、糖尿病和TC异常是金昌队列职业人群发生颈动脉硬化斑块的危险因素,大专及以上文化程度是金昌队列职业人群发生颈动脉硬化斑块的保护因素。金昌队列职业人群第2期随访0.06~4.37年,平均随访(2.67±0.55)年;共发生脑卒中者240例,累积发病率为3.83%,其中CAS、颈动脉内-中膜增厚和颈动脉硬化斑块者的脑卒中发病数分别为104例、19例和85例,脑卒中累积发病率分别为9.07%、6.62%和9.88%;多因素Cox比例风险回归模型分析结果显示,在调整了性别、年龄、文化程度、职业、吸烟情况、饮酒情况、体育锻炼情况、是否高盐饮食、是否高脂饮食、是否高糖饮食、是否高血压、是否糖尿病、是否高尿酸血症、体质指数(BMI)、是否TC异常、是否高油三脂(TG)异常、是否高密度脂蛋白胆固醇(HDL-C)异常、是否低密度脂蛋白胆固醇(LDL-C)异常和是否肾小球滤过率下降等混杂因素后,金昌队列职业人群中CAS者脑卒中的发病风险为非CAS者的1.421倍(HR=1.421,95%CI=1.030~1.961);年龄≥45岁、高盐饮食和超重是CAS者脑卒中发病的危险因素,女性和高尿酸血症是颈动脉内-中膜增厚者脑卒中发病的危险因素,文化程度高中或中专、高盐饮食和超重是颈动脉硬化斑块者脑卒中发病的危险因素。结论金昌队列职业人群CAS和脑卒中的累积发病率均较高,心血管疾病负担较重;CAS者的脑卒中发病风险较高,年龄较大、高盐饮食和超重是CAS者发生脑卒中的主要危险因素。

Comparing the Diagnostic Criteria of MAFLD and NAFLD in the Chinese Population:A Population-based Prospective Cohort Study

Background and Aims:Metabolic dysfunction-associ-ated fatty liver disease(MAFLD)is a new concept,pro-posed in 2020;however,its applicability in Asia populations has yet to be ***,we aimed to compare the difference in epidemiological and clinical characteris-tics between MAFLD and non-alcoholic fatty liver disease(NAFLD)among Asian ***:Based on the Jinchang cohort,30,633 participants were *** prevalence and incidence of MAFLD and NAFLD were used to analyze the epidemic characteristics and its overlapping *** addition,the corresponding clinical character-istics of the two diagnostic criteria populations were ***:The prevalence rates of MAFLD and NAFLD were 21.03%and 18.83%,*** an average 2.28-year follow-up,the incidence densities of MAFLD and NAFLD were 41.58 per 1,000 person-years and 37.69 per 1,000 person-years,*** the increase of baseline age,body mass index(BMI),and waist circumfer-ence(WC)levels,the prevalence and incidence of MAFLD and NAFLD were on the rise(all ptrend<0.05).Among the total patients diagnosed at baseline or follow-up,most pa-tients had both MAFLD and NAFLD,accounting for 78.84%and 82.88%,*** with NAFLD,MAFLD patients had greater proportions of males and metabolic diseases(diabetes,dyslipidemia),and had higher BMI,WC,liver enzymes,blood glucose,and lipid levels in the base-line diagnosis patients(p<0.05).Additionally,lean MAFLD patients had higher metabolic disorders than lean NAFLD patients(p<0.05).Conclusions:Compared with NAFLD,the newly proposed definition of MAFLD is more practical and accurate,and it can help identify more fatty liver pa-tients with high-risk diseases.

Astrocytic endothelin-1 overexpression impairs learning and memory ability in ischemic stroke via altered hippocampal neurogenesis and lipid metabolism

Vascular etiology is the second most prevalent cause of cognitive impairment ***-1,which is produced and secreted by endothelial cells and astrocytes,is implicated in the pathogenesis of ***,the way in which changes in astrocytic endothelin-1 lead to poststroke cognitive deficits following transient middle cerebral artery occlusion is not well ***,using mice in which astrocytic endothelin-1 was overexpressed,we found that the selective overexpression of endothelin-1 by astrocytic cells led to ischemic stroke-related dementia(1 hour of ischemia;7 days,28 days,or 3 months of reperfusion).We also revealed that astrocytic endothelin-1 overexpression contributed to the role of neural stem cell proliferation but impaired neurogenesis in the dentate gyrus of the hippocampus after middle cerebral artery *** proteome profiles and western blot analysis confirmed that levels of glial fibrillary acidic protein and peroxiredoxin 6,which were differentially expressed in the brain,were significantly increased in mice with astrocytic endothelin-1 overexpression in comparison with wild-type mice 28 days after ischemic ***,the levels of the enriched differentially expressed proteins were closely related to lipid metabolism,as indicated by Kyoto Encyclopedia of Genes and Genomes pathway *** chromatography-mass spectrometry nontargeted metabolite profiling of brain tissues showed that astrocytic endothelin-1 overexpression altered lipid metabolism products such as glycerol phosphatidylcholine,sphingomyelin,and phosphatidic ***,this study demonstrates that astrocytic endothelin-1 overexpression can impair hippocampal neurogenesis and that it is correlated with lipid metabolism in poststroke cognitive dysfunction.

Work-for-relief plays a unique role in China’s journey toward *** the perspectives of policy tools and interest analysis,this paper uses quantitative analysis of policy texts and integrates grounded theory to build a policy expression analysis model to discuss the current work-for-relief policy *** study found that China’s use of the work-for-relief policy is unbalanced,and it is necessary to enhance the involvement of non-governmental sectors,as well as to fully consider the policy’s target groups and the social issues that need addressing.

目前传统数据分类存储系统多数基于遗传算法,这类系统存在系统吞吐量较差、效率低的问题。为此,设计一种基于模式识别的机器人运行轨迹数据分类系统。首先设计系统整体框架,包括数据采集、处理以及存储模块;其次在硬件设计上,选择XC2VP30芯片作为FPGA核心芯片,设计数据采集模块接口电路,利用合适的FLASH芯片进行数据存储完成系统硬件设计;最后根据模式识别方法,建立标准数据样本集,确定特征向量,计算隶属度函数完成数据分类,再利用FPGA将已分类数据存储至FLASH模块中完成软件设计。测试结果表明,与传统的数据分类存储系统相比,基于模式识别的机器人运行轨迹数据分类存储系统的系统吞吐量提高了15.9%,效率得到了提高。
