Pneumatic dilation(PD)is considered to be a safe and effective first line therapy for achalasia.The major adverse event caused by PD is esophageal perforation but an immediate gastrografin test may not always detect a perforation.It has been reported that delayed management of perforation for more than 24 h is associated with high mortality.Surgery is the treatment of choice within 24 h,but the management of delayed perforation remains controversial.Hereby,we report a delayed presentation of intrathoracic esophageal perforation following PD in a 48-year-old woman who suffered from achalasia.She completely recovered after intensive medical care.A review of the literature is also discussed.

The primary defect in cystic fibrosis(CF)is abnormal chloride and bicarbonate transport in the CF transmembrane conductance regulator epithelial ion channel.The apical surface of the respiratory tract is lined by an airway surface liquid(ASL)layer composed of mucin comprising mainly MUC5A and MUC5B glycoproteins.ASL homeostasis depends on sodium bicarbonate secretion into the airways and secretion deficits alter mucus properties leading to airway obstruction,inflammation and infections.Downstream effects of abnormal ion transport in the lungs include altered intrinsic immune defenses.We observed that neutrophils killed Pseudomonas aeruginosa more efficiently when it had been exposed to sodium bicarbonate,and formation of neutrophil extracellular traps by neutrophils was augmented in the presence of increasing bicarbonate concentrations.Physiological levels of bicarbonate sensitized P.aeruginosa to the antimicrobial peptide cathelicidin LL-37,which is present in both lung ASL and neutrophil extracellular traps.Sodium bicarbonate has various uses in clinical medicine and in the care of CF patients and could be further explored as a therapeutic adjunct against Pseudomonas infections.

The authors propose a new "three-layer" conceptual model for the air-sea exchange of organic gases, which includes a dynamic surface microlayer with photochemical and biological processes. A parameterization of this three-layer model is presented, which was used to calculate the air-sea fluxes of acetone over the Pacific Ocean. The air-sea fluxes of acetone calculated by the three-layer model are in the same direction but possess half the magnitude of the fluxes calculated by the traditional two-layer model in the absence of photochemical and biological processes. However, photochemical and biological processes impacting acetone in the microlayer can greatly vary the calculated fluxes in the three-layer model, even reversing their direction under favorable conditions. Our model may help explain the discrepancies between measured and calculated acetone fluxes in previous studies. More measurements are needed to validate our conceptual model and provide constraints on the model parameters.

Complementary to mainstream cardiac imaging modalities for preclinical research,photoacoustic computed tomography(PACT)can provide functional optical contrast with high imaging speed and resolution.However,PACT has not been demonstrated to reveal the dynamics of whole cardiac anatomy or vascular system without surgical procedure(thoracotomy)for tissue penetration.Here,we achieved non-invasive imaging of rat hearts using the recently developed three-dimensional PACT(3D-PACT)platform,demonstrating the regulated illumination and detection schemes to reduce the effects of optical attenuation and acoustic distortion through the chest wall;thereby,enabling unimpeded visualization of the cardiac anatomy and intracardiac hemodynamics following rapidly scanning the heart within 10 s.We further applied 3D-PACT to reveal distinct cardiac structural and functional changes among the healthy,hypertensive,and obese rats,with optical contrast to uncover differences in cardiac chamber size,wall thickness,and hemodynamics.Accordingly,3D-PACT provides high imaging speed and nonionizing penetration to capture the whole heart for diagnosing the animal models,holding promises for clinical translation to cardiac imaging of human neonates.

AIM: To investigate outcomes of hepatocellular carcinomas (HCCs) in patients with chronic kidney disease (CKD). METHODS: Four hundred and forty patients referred between 2000 and 2002 for management of HCCs were categorized according to their CKD stage, i.e. , estimated glomerular filtration rate (eGFR) > 90 (stage 1), 60-90 (stage 2), 30-60 (stage 3), 15-30 (stage 4), and linical and laboratory data were collected and mortality rates and cause of mortality were analyzed. The mortality data were examined with Kaplan-meier method and the significance was tested using a log-rank test. An initial univariate Cox regression analysis was performed to compare the frequency of possible risk factors associated with mortality. To control for possible confounding factors, a multivariate Cox regression analysis (stepwise backward approach) was performed to analyze those factors that were significant in univariate models (P < 0.05) and met the assumptions of a proportional hazard model. RESULTS: Most HCC patients with CKD were elderly, with mean age of diagnosis of 60.6 ± 11.9 years, and mostly male (74.8%). Hepatitis B, C and B and C coinfection virus were positive in 61.6%, 45.7% and 14.1% of the patients, respectively. It was found that patients with stages 4 and 5 CKD were not only older (P = 0.001), but also had higher hepatitis C virus carrier rate (P = 0.001), lower serum albumin level (P = 0.001), lower platelet count (P = 0.037), longer prothrombin time (P = 0.001) as well as higher proportions of advanced cirrhosis (P = 0.002) and HCCs (P = 0.001) than patients with stages 1 and 2 CKD. At the end of analysis, 162 (36.9%) patients had died. Kaplan-Meier analysis revealed that patients with stages 4 and 5 CKD suffered lower cumulative survival than stages 1 and 2 CKD (log-rank test, χ 2 = 11.764, P = 0.003). Diabetic nephropathy(DN) is one of the most common complications of diabetes mellitus(DM). To discover early-stage biomarkers of DN, untargeted liquid chromatography-mass spectrometry-based metabolomic analysis was performed in urine samples from healthy subjects and patients with micro-or macroalbuminuria due to nondiabetic disease(macro), type 2 DM without microalbuminuria(T2 DM), and type 2 DM with microalbuminuria(T2 DM+micro). Levels of four metabolites were significantly different among groups, and they were quantified in a larger group of 267 urine samples. Two metabolites were also discovered and validated in targeted metabolic study of amino acids. For diagnosis of nephropathy, N1-methylguanosine had the highest area-under-the-curve(AUC) value of 0.75 when compared to that of valine(0.68), xanthosine(0.67), and 7-methyluric acid(0.69). After combining fasting blood glucose and diastolic blood pressure(DBP) with N1-methylguanosine, the AUC increased to 0.987. To distinguish between T2 DM and T2 DM+micro conditions, xanthosine and N1-methylguanosine have AUC value of 0.612 and 0.624, respectively. After adjustment of HbA1 c and DBP, AUC values of xanthosine and N1-methylguanosine increased to 0.716 and 0.723, respectively and could be used to predict the development of nephropathy in T2 DM patients..
