Weekly pattern of emergency room admissions for peptic ulcers:A population-based study

AIM:To investigate variations in the incidence of peptic ulcers(PUs) in Taiwan by day of the week within age ***:Ambulatory care data were retrieved from the Longitudinal Health Insurance Database *** were 7204 subjects≥18 years-old with an emergency room admission claim for the treatment of PUs,resulting in a total of 9234 emergency room visits for PUs between 2009 and *** was divided into the seven days of the week and an additional variable for ***-way analysis of variance was used to examine associations among the daily mean number of PU emergency room admissions and holidays/weekends/***:One-way analysis of variance showed that there was a significant difference in emergency room admissions for PUs by the day of the week(P<0.001),with admission more likely to occur on Sundays or holidays than weekdays within the total and working *** weekday patterns of admission were similar for the patients aged 18-64 years and≥65years of ***,followed by Sundays,had higher PU admissions than the mean daily PU emergency room ***,inclusion of only those treated for PUs with hemorrhage or perforation,Sundays and holidays had higher mean emergency room admissions than other *** of patients who diagnosed with Helicobacter pylori infection,only holidays had higher mean emergency room admissions than other *** of patients who had been prescribed non-steroidal anti-inflammatory drugs(NSAIDs)for over30 d,Sundays and holidays had higher mean PU ER admissions than other non-holiday ***:There is a higher incidence of emergency room admission for PUs on weekends than on weekdays for the total and working populations.

BACKGROUND Coronavirus disease 2019(COVID-19)pneumonia with severe septic shock and acute respiratory distress syndrome(ARDS)are critical illnesses for patients following *** immunoglobulin(IVIG)plays a role in both immune support and inflammation control,especially in immunocompromised *** case report describes the first successful experience using IVIG and pulse steroids to manage this critical condition following lung *** SUMMARY A 65-year-old male patient reported a history of chronic obstructive pulmonary disease and poor lung function and received bilateral sequential lung ***,he developed COVID-19 pneumonia,severe septic shock,and *** recovered from this critical condition after empirical antibiotics administration and veno-venous extracorporeal membrane oxygenation,in addition to IVIG and pulse *** IVIG is a valuable adjunct in managing severe sepsis in lung transplant recipients after COVID-19 *** aim,for the first time,to report the success of such a management approach for COVID-19 ARDS and sepsis in the post-lung transplant *** further investigations,this is a starting point for wider analysis of such an approach in this setting and consequently helps guide clinical practice for such a challenging patient population moving forward.

Objective:To investigate the efficacy of frankincense and myrrha in the treatment of acute interstitial cystitis/painful bladder syndrome(IC/PBS).Methods:The effects of frankincense and myrrha on the proliferation and migration of primary human urothelial cells(HUCs)were assessed in *** the animal study,48 virgin female rats were randomized into 4 groups(12 in each group):(1)control group(saline-injected control);(2)cyclophosphamide(CYP)group(intraperitoneal injected 150 mg/kg CYP);(3)CYP+pentosan polysulfate sodium group(orally received 50 mg/kg pentosan polysulfate sodium);and(4)CYP+frankincense and myrrha group[orally received frankincense(200 mg/kg)and myrrha(200 mg/kg)].Rats orally received pentosan polysulfate sodium or frankincense and myrrha on day 1,2,and *** experiments were performed on day *** and cystometry assessment behavior test were *** interval values were assessed in rats under ***,immunohistochemistry and Western blot were used to confirm the location and level,respectively,of cell junction-associated protein zonula occludens-2(ZO-2)***:Low dose frankincense and myrrha increased cell proliferation and migration in HUCs compared with control(P<0.05).Rats with acute IC/PBS rats exhibited lower voiding interval values,pain tolerance,and ZO-2 expression(P<0.05).Voiding interval values and pain tolerance were higher in the frankincense and myrrha group than CYP group(P<0.05).ZO-2 expression in the bladder was increased in the CYP+pentosan polysulfate and frankincense+myrrha groups compared with the CYP-induced acute IC/PBS group(P<0.05).Conclusion:frankincense and myrrha modulate urothelial wound healing,which ameliorates typical features of acute IC/PBS in rats.

Improvement of fuel consumption and maintenance of heating furnaces using a modified heating pattern

This article studies the transient heat conduction in a slab when passing through various sections of the furnace, and focuses on the thickness of the scale layer formed on the slab. The transient heat conduction behavior of a slab in various sections of the heating furnace is analyzed using the Laplace transformation method, including the pre-heating zone, the first heating zone, the second heating zone, and the soaking zone. The heating pattern of the furnace is then modified to reduce fuel consumption. The simulation results show that the scale layer formed on the slab significantly influences the quality of the hot rolled coil formed, and how the furnace parameters affect the efficiency of the furnace and the quality of the coil.

Optimizing Big Data Retrieval and Job Scheduling Using Deep Learning Approaches

Big data analytics in business intelligence do not provide effective data retrieval methods and job scheduling that will cause execution inefficiency and low system *** paper aims to enhance the capability of data retrieval and job scheduling to speed up the operation of big data analytics to overcome inefficiency and low throughput ***,integrating stacked sparse autoencoder and Elasticsearch indexing explored fast data searching and distributed indexing,which reduces the search scope of the database and dramatically speeds up data ***,exploiting a deep neural network to predict the approximate execution time of a job gives prioritized job scheduling based on the shortest job first,which reduces the average waiting time of job *** a result,the proposed data retrieval approach outperforms the previous method using a deep autoencoder and Solr indexing,significantly improving the speed of data retrieval up to 53%and increasing system throughput by 53%.On the other hand,the proposed job scheduling algorithmdefeats both first-in-first-out andmemory-sensitive heterogeneous early finish time scheduling algorithms,effectively shortening the average waiting time up to 5%and average weighted turnaround time by 19%,respectively.

Development of a nano-immunomodulator encapsulating R837 and caffeine for combined radio-/immunotherapy against orthotopic breast cancer

Immunotherapy is an ascendant approach in cancer treatment. It shows more pronounced effects on killing cancer cells in a specific manner in particular against metastasis more than traditional techniques, such as chemotherapy or surgery. However, tumor immunosuppression limits the response of the immune system to cancer development. In this study, we developed a lipid-based nanocarrier doubly loaded with imiquimod(R837), a toll-like receptor 7 agonist, and caffeine, an adenosine receptor antagonist. This R837/caffeine loaded nanocarrier served as a nano-immunomodulator(RC-n IM) for combination treatment with radiotherapy(RT)against orthotopic breast cancer. RT-induced immunogenic cell death facilitated the production of tumor antigen and elicited the immune response in corporation with R837-medaited activation of antigen-presenting cells(APCs) while RC-n IMs being adopted. Additionally, caffeine, an adenosine analog, can successfully compete with adenosine in the tumor. The tumor-bearing mice that received RT together with RC-n IMs experienced the best antitumor effects and exhibited higher levels of T cells and APCs within the tumor; the growth of secondary tumors was also limited. This work serves as a proof-of-concept study for the development of a new immunotherapy strategy against cancer.

Efficacy and safety of transnasal butorphanol for pain relief after anal surgery

AIM:To compare the analgesic properties and efficacy of transnasal butorphanol with intramuscular meperidine after anal ***: Sixty patients who underwent fistulectomy were enrolled in the study from January 2006 to December 2007. They were randomly divided into transnasal butorphanol (n=30) or intramuscular meperidine (n=30) treatment groups. Assessment of postoperative pain was made using a visual analogue scale (VAS). The VAS score was recorded 6 h after the completion of surgery, before receiving the first dose of analgesic, 60 min after analgesia and the next morning. Any adverse clinical effects such as somnolence, dizziness, nausea or vomiting were recorded. Satisfaction with narcotic efficacy, desire to use the particular analgesic in the future and any complaints were recorded by patients using questionnaires before being ***: Forty-two men and eighteen women were included in the study. There were no significant differences in VAS scores between the groups within 24 h. Length of hospital stay and the incidence of adverse effects between the groups were similar. In addition, most patients were satisfied with butorphanol nasal spray and wished to receive this analgesic in the future, if ***: Butorphanol nasal spray is effective for the relief of pain after fistulectomy. However, it offered patients more convenient usage and would be suitable for outpatients.
