Sepsis one-hour bundle management combined with psychological intervention on negative emotion and sleep quality in patients with sepsis

BACKGROUND Sepsis is a serious infectious disease caused by various systemic inflammatory responses and is ultimately life-threatening.Patients usually experience depression and anxiety,which affect their sleep quality and post-traumatic growth levels.AIM To investigate the effects of sepsis,a one-hour bundle(H1B)management was combined with psychological intervention in patients with sepsis.METHODS This retrospective analysis included 300 patients with sepsis who were admitted to Henan Provincial People’s Hospital between June 2022 and June 2023.According to different intervention methods,the participants were divided into a simple group(SG,n=150)and combined group(CG,n=150).H1B management was used in the SG and H1B management combined with psychological intervention was used in the CG.The changes of negative emotion,sleep quality and post-traumatic growth and prognosis were compared between the two groups before(T0)and after(T1)intervention.RESULTS After intervention(T1),the scores of the Hamilton Anxiety scale and Hamilton Depression scale in the CG were significantly lower than those in the SG(P<0.001).Sleep time,sleep quality,sleep efficiency,daytime dysfunction,sleep disturbance dimension score,and the total score in the CG were significantly lower than those in the SG(P<0.001).The appreciation of life,mental changes,relationship with others,personal strength dimension score,and total score of the CG were significantly higher than those of the SG(P<0.001).The scores for mental health,general health status,physiological function,emotional function,physical pain,social function,energy,and physiological function in the CG were significantly higher than those in the SG(P<0.001).The mechanical ventilation time,intensive care unit stay time,and 28-d mortality of the CG were significantly lower than those of the SG(P<0.05).CONCLUSION H1B management combined with psychological intervention can effectively alleviate the negative emotions of patients with sepsis and increase their quality of sleep and life.

Cu-rich cell boundary phase is difficult to precipitate evenly,resulting in a generally poor demagnetization curve squareness for Fe-rich Sm2Co17-type magnet,which is a key factor limiting the further improvement of magnetic energy product.In this study,we report that nanoscale strip-like ordered micro-domains distributed in1:7H disordered matrix phase of the solid solution precursor is a new factor significantly affecting the precipitation and distribution of the cell boundary phase.Long strip-like and continuous micro-twin structure with twin boundaries neatly perpendicular to the C-axis is observed after sintering treatment.After solution treatment,sequential and long strip-like micro-twins gradually transform into disordered state along the basal plane,forming narrow disordered 1:7H（TbCu7-type structure） phase between the separated strip-like ordered micro-domains.This disordering transformation takes place via broken down of the long strip-like ordered micro-domains,which is accomplished by narrowing along the width direction followed by reduction of the length.Furthermore,a new model revealing the effect of the ordered micro-domains on the formation of the cell boundary phase is proposed.Antiphase boundaries enriched in Cu have already existed in the precursor with long strip-like ordered micro-domains.Therefore,the Cu-rich cell boundary phase acting as strong pinning centers cannot be precipitated homogeneously and distributed continuously after aging,resulting in a poor demagnetization curve squareness of Sm2Co17-type magnet.Our results indicate that significant broken down of the nanoscale ordered micro-domains in solution precursor is the key factor improving the distribution of cell boundary phase in Sm2Co17-type magnets.

Haptic communications is recognized as a promising enabler of extensive services by enabling real-time haptic control and feedback in remote environments,e.g.,teleoperation and autonomous driving.Considering the strict transmission requirements on reliability and latency,Device-to-Device(D2D)communications is introduced to assist haptic communications.In particular,the teleoperators with poor channel quality are assisted by auxiliaries,and each auxiliary and its corresponding teleoperator constitute a D2D pair.However,the haptic interaction and the scarcity of radio resources pose severe challenges to the resource allocation,especially facing the sporadic packet arrivals.First,the contentionbased access scheme is applied to achieve low-latency transmission,where the resource scheduling latency is omitted and users can directly access available resources.In this context,we derive the reliability index of D2D pairs under the contention-based access scheme,i.e.,closed-loop packet error probability.Then,the reliability performance is guaranteed by bidirectional power control,which aims to minimize the sum packet error probability of all D2D pairs.Potential game theory is introduced to solve the problem with low complexity.Accordingly,a distributed power control algorithm based on synchronous log-linear learning is proposed to converge to the optimal Nash Equilibrium.Experimental results demonstrate the superiority of the proposed learning algorithm.

BACKGROUND We report a rare case of primary clinical presentation featuring elevated creatine kinase(CK)levels in a neonate,which is associated with the LAMA2 gene.In this case,a heterozygous mutation in exon5 of the LAMA2 gene,c.715C>G(resulting in a change of nucleotide number 715 in the coding region from cytosine to gua-nine),induced an amino acid alteration p.R239G(No.239)in the patient,repre-senting a missense mutation.This observation may be elucidated by the neonatal creatine monitoring mechanism,a phenomenon not previously reported.CASE SUMMARY We analysed the case of a neonate presenting solely with elevated CK levels who was eventually discharged after supportive treatment.The chief complaint was identification of increased CK levels for 15 d and higher CK values for 1 d.Ad-mission occurred at 18 d of age,and despite prolonged treatment with creatine and vitamin C,the elevated CK levels showed limited improvement.Whole exo-me sequencing revealed the presence of a c.715C>G mutation in LAMA2 in the newborn,correlating with a clinical phenotype.However,the available informa-tion offers insufficient evidence for clinical pathogenicity.CONCLUSION Mutations in LAMA2 are associated with the clinical phenotype of increased neonatal CK levels,for which no specific treatment exists.Whole genome sequen-cing facilitates early diagnosis.

Constructing porous ZnFC-PA/PSF composite spheres for highly efficient Cs^(+) removal

Radioisotope leaking from nuclear waste has become an intractable problem due to its gamma radiation and strong water solubility.In this work,a novel porous ZnFC-PA/PSF composite sphere was fabricated by immobilization of ferrocyanides modified zinc phytate into polysulfone(PSF)substrate for the treatment of Cs-contaminated water.The maximum adsorption capacity of ZnFC-PA/PSF was 305.38 mg/g,and the removal efficiency of Cs+was reached 94.27%within 2 hr.The ZnFC-PA/PSF presented favorable stability with negligible dissolution loss of Zn^(2+)and Fe^(2+)(<2%).The ZnFC-PA/PSF achieved high-selectivity towards Cs+(K_(d)=2.24×10^(4)mL/g)even in actual geothermal water.The adsorption mechanism was inferred to be the ion-exchange between Cs^(+)and K^(+).What’s more,ZnFC-PA/PSF worked well in the fixed-bed adsorption(E=91.92%),indicating the application potential for the hazardous Cs^(+)removal from wastewater.

Energy absorption behaviors of a novel buffer lander-leg with Yoshimura core

In the current soft-landing system,the lander-leg and the buffer are two independent parts which may cause weight problem.The traditional mechanical buffers also have their own shortcomings such as small buffer stroke and being non-reusable.A Novel Buffer Lander-leg with Yoshimura core (NBLY) is proposed in this paper,inspired by the Buckling Restrained Brace(BRB).The design details of NBLY and parameter definitions of the core are presented.The finite element analysis modes have been employed to analyze the nonlinear mechanical behaviors of NBLY subjected to the cyclic loading.NBLY with four different core styles of hexagon,octagon,dodecagon and hexadecagon are analyzed and compared.The restraining ratio and cumulative dissipation energy ratio have been obtained to evaluate the energy absorption properties.The results show that the origami pattern has significant influences on the deformation ductility,plastic energy absorbing and axial load transferring to the core.The NBLY with octagon core has outstanding properties,especially the hysteretic energy dissipation and enhancement of compression capacity.The interaction mechanism between the core and encasing member constrains the deformation extensions and contributes to the frictional energy dissipation of the NBLYs.This paper provides a new conceptual design of buffer lander-leg for soft-landing missions.It can realize the reuse function as a buffer lander-leg with better loading-bearing and energy absorption behaviors.

Air pollution is a severe environmental problem in urban areas.Accurate air quality prediction can help governments and individuals make proper decisions to cope with potential air pollution.As a classic time series forecasting model,the AutoRegressive Integrated Moving Average(ARIMA)has been widely adopted in air quality prediction.However,because of the volatility of air quality and the lack of additional context information,i.e.,the spatial relationships among monitor stations,traditional ARIMA models suffer from unstable prediction performance.Though some deep networks can achieve higher accuracy,a mass of training data,heavy computing,and time cost are required.In this paper,we propose a hybrid model to simultaneously predict seven air pollution indicators from multiple monitoring stations.The proposed model consists of three components:(1)an extended ARIMA to predict matrix series of multiple air quality indicators from several adjacent monitoring stations;(2)the Empirical Mode Decomposition(EMD)to decompose the air quality time series data into multiple smooth sub-series;and(3)the truncated Singular Value Decomposition(SvD)to compress and denoise the expanded matrix.Experimental results on the public dataset show that our proposed model outperforms the state-of-art air quality forecasting models in both accuracy and time cost.

Accurate prediction of Shock-Wave/Boundary Layer Interaction(SWBLI)flows has been a persistent challenge for linear eddy viscosity models.A major limitation lies in the isotropic representation of the Reynolds stress,as assumed under the Boussinesq approximation.Recent studies have shown promise in improving the prediction capability for incompressible separation flows by perturbing the Reynolds-stress anisotropy tensor.However,it remains uncertain whether this approach is effective for SWBLI flows,which involve compressibility and discontinuity.To address this issue,this study systematically quantifies the structural uncertainty of the anisotropy for oblique SWBLI flows.The eigenspace perturbation method is applied to perturb the anisotropy tensor predicted by the Menter Shear–Stress Transport(SST)model and reveal the impacts of anisotropy on the prediction of quantities of interest,such as separation and reattachment positions,wall static pressure,skin friction,and heat flux.The results demonstrate the potential and reveal the challenges of eigenspace perturbation in improving the SST model.Furthermore,a detailed analysis of turbulent characteristics is performed to identify the source of uncertainty.The findings indicate that eigenspace perturbation primarily affects turbulent shear stress,while the prediction error of the SST model is more related to turbulent kinetic energy.

The high energy density and stability of solid-state lithium metal batteries(SSLMBs)have garnered great ***-type oxides,especially Li_(6.4)La_(3)Zr_(1.4)Ta_(0.6)O_(12)(LLZTO),with high ionic conductivity,wide electrochemical window,and stability to Li metal anode,are promising solid-state electrolyte(SSEs)materials for SSLMBs.However,Li/LLZTO interface issues including high interface resistance,inhomogeneous Li deposition,and Li dendrite growth have hindered the practical application of SSLMBs.Herein,a multi-functional Li–SnF_(2) composite anode with Li,LiF,and Li-Sn alloy was specifically designed and prepared.The composite anode improves the wettability to LLZTO,constructing an intimate contact interface between it and LLZTO.Meanwhile,ionic/electronic conductive paths in situ formed at the interface can effectively uniform Li deposition and suppress Li dendrite.The solid-state symmetric cell exhibits low interface resistance(11Ω·cm^(2)) and high critical current density(1.3 mA·cm^(−2))at 25℃.The full SSLMB based on LiFePO_(4) or LiNi_(0.5)Co_(0.2)Mn_(0.3)O_(2) cathode also shows stable cycling performance and high rate capability.This work provides a new composite anode strategy for achieving high-energy density and high-safety SSLMBs.
