Role of concomitant therapy for Helicobacter pylori eradication: A technical note

We read with interest the recent meta-analysis by Lin et al who evaluated the effectiveness of concomitant regimen for Helicobacter pylori(H. pylori) in Chinese regions. They found that 7-d concomitant regimen is undoubtedly superior to 7-d triple therapy(91.2% vs 77.9%, P tained for concomitant regimen, as main guidelines recommend a 10-d schedule for this scheme. Therefore, only studies comparing 10-d concomitant versus 14-d triple seem to be appropriate according to current Guidelines and would clarify which regimen is the most suitable worldwide. Additionally, in this metaanalysis concomitant and sequential therapy showed similar performances, despite it is common opinion that sequential is more prone than concomitant therapy to fail when metronidazole resistance occurs, and China is characterized by high rate of resistance to this antibiotic. none of the included studies evaluated a priori antibiotic resistances, and the lack of this detail hampers the unveiling of this apparent contradiction. In conclusion, the lack of the evaluation of the quality of included trials as well as their high heterogeneity constitute a burdensome limit to draw solid conclusions in this meta-analysis. On the bases of these considerations and the low number of examined trials, we believe that further studies and the knowledge of antibiotic resistances will support with high quality evidence which is the best regimen and its optimal duration.

BACKGROUND Interstitial cystitis/bladder pain syndrome(IC/BPS)is an at least 6-mo noninfectious bladder inflammation of unknown origin characterized by chronic suprapubic,abdominal,and/or pelvic *** the term cystitis suggests an inflammatory or infectious origin,no definite cause has been *** occurs in both sexes,but women are twice as much *** To systematically review evidence of psychiatric/psychological changes in persons with IC/*** Hypothesizing that particular psychological characteristics could underpin IC/BPS,we investigated in three databases the presence of psychiatric symptoms and/or disorders and/or psychological characteristics in patients with IC/BPS using the following strategy:("interstitial cystitis"OR"bladder pain syndrome")AND("mood disorder"OR depressive OR antidepressant OR depression OR depressed OR hyperthymic OR mania OR manic OR rapid cyclasterisk OR dysthymiasterisk OR dysphoriasterisk).RESULTS On September 27,2023,the PubMed search produced 223 articles,CINAHL 62,and the combined PsycLIT/PsycARTICLES/PsycINFO/Psychology and Behavioral Sciences Collection search *** on *** produced 14 studies,of which none had available *** were peer-reviewed articles reporting psychiatric/psychological symptoms in patients with IC/BPS,i.e.63 articles spanning from 2000 to October *** studies identified depression and anxiety problems in the IC/BPS population,along with sleep problems and the tendency to *** Psychotherapies targeting catastrophizing and life stress emotional awareness and expression reduced perceived pain in women with IC/*** concepts should be considered when implementing treatments aimed at reducing IC/BPS-related pain.

Gastric cancer is the fourth most common malignant neoplasm and the second leading cause of death for cancer in Western countries with more than 20000 new cases yearly diagnosed in the United States. Surgery represents the main approach for this disease but, notwithstanding the advances in surgical techniques, we observed a minimal improvement in terms of overall survival with a significant increasing of relapsing disease rates. Despite the development of new drugs has significantly improved the effectiveness of chemotherapy, the prognosis of patients with unresectable or metastatic gastric adenocarcinoma remains poor. Recently, several molecular target agents have been investigated; in particular, trastuzumab represents the first target molecule showing improvements in overall survival in human epithelial growth factor 2-positive gastric cancer patients. New molecules targeting vascular epithelial growth factor, mammalian target of rapamycin, and anti hepatocyte growth factor-c-Met pathway are also under investigation, with interesting results. Anyway, it seems necessary to select more accurately the population to treat with new agents by the identification of new biomarkers in order to optimize the results. In this paper we review the actual “scenario” of targeted treatments, also focusing on the new agents in development for gastric cancer and gastro-esophageal carcinoma, discussing their efficacy and potential applications in clinical practice.

Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. Despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.

Intra-operative consult for cystic struma ovarii—An experience in an academic medical center: A study of 53 cases over 21 years

Struma ovarii often escapes recognition during intraoperative consultation because of its rarity, subtle characteristic gross appearance, and lack of clinical suspicion. An intraoperative diagnosis of benign struma ovarii enables the general gynecologic surgeon to continue the planned surgery. However, a diagnosis of malignnancy in a struma ovarii would alter the course of surgery with the involvement of a gynecologic oncology surgeon. We present here that our experience with intraoperative consultation for preoperatively undiagnosed struma ovarii presenting as an adnexal cystic or solid mass at our teaching hospital. Fifty-three cases of struma ovarii, 5.2% of all cystic teratoma of the same period, were diagnosed between January 1991 and March 2011. All intraoperative consultation reports, gross descriptions and final pathology reports were reviewed. The H&E stained slides and in selected cases, immunohistochemistry stained slides, were reviewed. Of the 53 cases of struma ovarii, intraoperative consultation was requested on 48 cases. Frozen section was done on 24 cases and only gross examination was felt appropriate in remaining 24 cases. 83% cases were diagnosed when a frozen section was done. none of the remaining 24 cases were recognized as struma by gross inspection. Our findings reveal that in a large number of cases the diagnosis of struma ovarii remained unrecognized during intraoperative consultation, indicating its often subtle/deceptive gross morphologic appearance. However, the purpose of the intraoperative consultation was served, as appropriate information was provided to the surgeon to guide the surgical management.

Study Objectives:To determine if the large and highly reproducible inter-individual differences in heart rate(HR) response to arousals during non-REM sleep arise from a heritable ***:Post-hoc analysis of polysomnograms of same-sex monozygotic and dizygotic twin ***:Academic medical center and Research and Development Laboratory(YRT Limited).Participants:55 monozygotic(mean[±SD]age 29.2±6.9 years;14 male and 41 female pairs) and 36dizygotic(mean age 26.217.5 years;15 male and 21 female pairs) twin pairs with a normal ***:none Measurements and Results:Arousals were scored using the 2007 American Academy of Sleep Medicine *** intensity was scaled(between 0 and 9) using an automatic algorithm based on the change in EEG time and frequency characteristics and a training data *** rate response to arousal(AHR) was determined and averaged at each arousal intensity per *** calculated the average arousal duration,average arousal intensity,average AHR,AHR at a given arousal intensity,rate of AHR per arousal intensity,and arousal intensity threshold of *** the classical approach to assess broad sense heritability(h),the arousal intensity threshold of AHR(h=0.94) and average arousal intensity(h=1.05) showed the strongest evidence for heritability after age and sex *** was supported by a maximum likelihood based variance components estimation approach,with h(95%CI) estimates of 0.38(0.13,0.62) for arousal intensity threshold of AHR and 0.64(0.48,0.80) for average arousal *** approach also suggested significant additive genetic effects for the number of arousals,average arousal duration,and the average ***:Genetic factors explain a significant fraction of the phenotypic variability for the threshold of AHR to arousal and average arousal *** also suggest that the number and duration of arousals and the average AHR may be heritable traits.

目的：观察青光眼手术患者虹膜切除标本的组织病理学特征。比较拉坦前列腺素治疗组与非拉坦前列腺素治疗组（对照组）组织学差异。方法：虹膜切除标本及患者病史资料呈送拉坦前列腺素病理学中心。标本由3位眼科病理学家以盲法分别观察。评估内容包括：癌变。癌前改变及其他病变：色素沉着水平差异，细胞构成差异。炎症，血管异常情况及虹膜色素上皮改变。结果：所收集标本中，449例曾接受拉坦前列腺素治疗，142例无拉坦前列腺素或其衍生物治疗史。所有标本均未发现癌或癌前病变。拉坦前列腺素治疗组，虹膜痣发生率显著高于对照组（P=0．001）。基质成纤维细胞和黑色素细胞数为对照组2倍（P〈0．001）。采用免疫组化染色技术分析2002年6月收集的各亚组标本，结果显示：拉坦前列腺素治疗组与非拉坦前列腺素治疗组黑色素细胞均数无显著差异（P=0．35）。结论：本研究结果支持已有观点，即：拉坦前列腺素诱发的眼部颜色的改变是由于虹膜基质黑色素细胞内黑色素数量增多而引起，虹膜痣数量增加可能是此效应的局部表现。
