Regional and National Trends of Adult Living Donor Liver Transplantation in the United States Over the Last Two Decades

Background and Aims:Liver organ shortage remains a major health burden in the US,with more patients being waitlisted than the number of liver transplants(LTs)*** study investigated US national and regional trends in living donor LT(LDLT)and identified factors associated with recipient ***:We retrospectively analyzed LDLT recipients and donors from the United Network Organ Sharing/Organ procurement Transplant Network database from 1998 until 2019 for clinical characteristics,demographic differences,and survival *** and regional trends in LDLT,recipient outcomes,and predictors of survival were ***:Of the 223,571 candidates listed for an LT,57.5%received an organ,of which only 4.2%were *** adult LDLTs first peaked at 412 in 2001 but experienced a significant decline to 168 by *** then gradually increased to 445 in *** 2 had the highest LDLT numbers(n=919),while region 1 had the highest proportion(11.1%).Overall,post-LT mortality was 21.4%among LDLT ***-LDLT survival rates after 1-,5-,and 10-years were 92%,87%,and 70%,*** analysis(2004–2019)showed that patients undergoing LDLT in recent years had lower mortality than in earlier years(hazard ratio=0.81,95%confidence interval=0.75–0.88).Conclusions:Following a substantial decline after a peak in 2001,the number of adult LDLTs steadily increased from 2011 to ***,LDLTs still constitute the minority of the transplant pool in the *** policies to increase the use of LDLTs,particularly in regions of high organ demand,should be implemented.

Risk factors and outcomes associated with alcohol relapse after liver transplantation

Alcoholic liver disease(ALD) is the second most common indication for liver transplantation(LT) in the United States and Europe. Unlike other indications for LT, transplantation for ALD may be controversial due to the concern for alcohol relapse and non-compliance after LT. However, the overall survival in patients transplanted for ALD is comparable or higher than in patients transplanted for other etiologies of liver disease. While the rate of alcohol use after liver transplantation does not differ among various etiologies of liver disease, alcohol relapse after transplantation for ALD has been associated with complications such as graft rejection, graft loss, recurrent alcoholic cirrhosis and reduced long-term patient survival. Given these potential complications, our review aimed to discuss risk factors associated with alcohol relapse and the efficacy of various interventions attempted to reduce the risk of alcohol relapse. We also describe the impact of alcohol relapse on post-transplant outcomes including graft and patient survival. Overall, alcohol liver disease remains an appropriate indication for liver transplantation, and long-term mortality in this group of patients is primarily attributed to cardiovascular disease or de novo malignancies rather than alcohol related hepatic complications, among those who relapse.

Management of severe spasticity with and without dystonia with intrathecal baclofen in the pediatric population:a cross-sectional study

Objective To investigate the differences in delivery mode,daily dose,and catheter tip location in pediatric patients using intrathecal baclofen(ITB)pumps with spasticity plus dystonia versus spasticity *** A single-center,cross-sectional study was performed by collecting retrospective data from electronic medical *** and diagnostic information was obtained,comparing patients with spasticity with or without *** data were analyzed for group differences using a two-tailed Student’s *** data were analyzed for group differences using pearson’s χ^(2) *** A total of 137 patients met the *** majority(114)had spasticity plus dystonia whereas only 23 were documented as spasticity *** continuous dosing was the most common delivery mode,but flex dosing was used more than twice as frequently with spasticity plus dystonia compared with spasticity alone(42%vs 17%).patients with spasticity plus dystonia also had more rostral catheter tip *** While it has been discussed anecdotally,this study confirms the supposition that patients with spasticity plus dystonia have increased dose requirements when compared with those with spasticity *** there are no clear standards of care when managing these patients,they are often on higher daily dosages,are more likely to require flexed dosing method,and have higher catheter ***,there are few studies that demonstrate improvements in dystonia with the use of *** general,these patients would benefit from the development of universal standardizations as well as the confirmation that this is an appropriate treatment.

Identification of C9-C11 unsaturated aldehydes as prediction markers of growth and feed intake for non-ruminant animals fed oxidized soybean oil

Background: The benefits of using the oxidized oils from rendering and recycling as an economic source of lipids and energy in animal feed always coexist with the concerns that diverse degradation products in these oxidized oils can negatively affect animal health and performance. Therefore, the quality markers that predict growth performance could be useful when feeding oxidized oils to non-ruminants. However, the correlations between growth performance and chemical profiles of oxidized oils have not been well examined. In this study, six thermally oxidized soybean oils(OSOs) with a wide range of quality measures were prepared under different processing temperatures and processing durations, including 45 °C-336 h;67.5 °C-168 h;90 °C-84 h;135 °C-42 h;180 °C-21 h;and225 °C-10.5 h. Broilers and nursery pigs were randomly assigned to diets containing either unheated control soybean oil or one of six OSOs. Animal performance was determined by measuring body weight gain, feed intake,and gain to feed ratio. The chemical profiles of OSOs were first evaluated by common indicative tests, including peroxide value, thiobarbituric acid reactive substances, p-anisidine value, free fatty acids, oxidized fatty acids,unsaponifiable matter, insoluble impurities, and moisture, and then analyzed by the liquid chromatography-mass spectrometry-based chemometric ***: Among common quality indicators, p-anisidine value(An V), which reflects the level of carbonyl compounds,had the greatest inverse correlation with the growth performance of both broilers and pigs, followed by free fatty acids and oxidized fatty acids. Fast and sensitive antigen detection is important in biomedical research and development. Despite, being invented 48 years ago, the enzyme-linked immunosorbent assay(ELISA) remains one of the most successful and widely employed bioanalytical techniques in research and clinical diagnostics due to its reliability and simplistic design. Recently, nanotechnology has offered efficient signal reporting. In spite of some improvements in these systems, there are typically increased material costs involved and the need for expensive equipment or complicated chemical processes, thus negating any possible benefits over ELISA. Herein, we communicate a simple Cu-DNAzyme system for signal transduction of a CuO nanoparticle-labeled immunoassay. The reported immunoassay amplifies signal generation similar to traditional ELISA and is fast, simple, cost-effective, and sensitive, holding promise for biomedical applications and point-of-care testing.

Mercury bioaccumulation in aquatic biota along a salinity gradient in the Saint John River estuary

Although estuaries are critical habitats for many aquatic species, the spatial trends of toxic methylmercury（MeHg） in biota from fresh to marine waters are poorly understood. Our objective was to determine if MeHg concentrations in biota changed along a salinity gradient in an estuary. Fourspine Stickleback（Apeltes quadracus）, invertebrates（snails,amphipods, and chironomids）, sediments, and water were collected from ten sites along the Saint John River estuary, New Brunswick, Canada in 2015 and 2016, with salinities ranging from 0.06 to 6.96. Total mercury（proxy for MeHg） was measured in whole fish and MeHg was measured in a subset of fish, pooled invertebrates, sediments, and water. Stable sulfur（δ34S）, carbon（δ13 C）, and nitrogen（δ15N） isotope values were measured to assess energy sources（S, C） and relative trophic level（N）. There were increases in biotic δ13C and δ34S from fresh to more saline sites and these measures were correlated with *** aqueous MeHg was higher at the freshwater than more saline sites, only chironomid MeHg increased significantly with salinity. In the Saint John River estuary, there was little evidence that MeHg and its associated risks increased along a salinity gradient.
