In this study, the possibility of preparation and application of highly porous silica aerogel-based bioactive materials are presented. The aerogel was combined with hydroxyapatite and *** phosphate as bioactive and osteoinductive agents. The porosity of aerogels was in themesoporous region with a maximum pore diameter of 7,4 and 12.7 nm for the composite materials. The newly developed bioactive materials were characterized by scant electron microscopy. The in vitro biological effect of these modified surfaces was also tested on SAOS-2 osteogenic sarcoma cells by confocal laser scanning microscopy.

AIM To analyze the effect of intralesional steroid injections in addition to endoscopic dilation of benign refractory esophageal *** A comprehensive search was performed in three databases from inception to 10 April 2017 to identify trials, comparing the efficacy of endoscopic dilation to dilation combined with intralesional steroid injections. Following the data extraction, meta-analytical calculations were performed on measures of outcome by the randomeffects method of Der Simonian and Laird. heterogeneity of the studies was tested by Cochrane's Q and I^2 statistics. Risk of quality and bias was assessed by the Newcastle Ottawa Scale and JADAD assessment *** Eleven articles were identified suitable for analyses, involving 343 patients, 235 cases and 229 controls in total. Four studies used crossover design with 121 subjects enrolled. The periodic dilation index(PDI) was comparable in 4 studies, where the pooled result showed a significant improvement of PDI in the steroid group(MD:-1.12 dilation/month, 95% CI:-1.99 to -0.25 P = 0.012; I^2 = 74.4%). The total number of repeat dilations(TNRD) was comparable in 5 studies and showed a non-significant decrease(MD:-1.17, 95%CI:-0.24-0.05, P = 0.057; I^2 = 0), while the dysphagia score(DS) was comparable in 5 studies and did not improve(SMD: 0.35, 95%CI:-0.38, 1.08, P = 0.351; I^2 = 83.98%) after intralesional steroid *** Intralesional steroid injection increases the time between endoscopic dilations of benign refractory esophageal strictures. however, its potential role needs further research.

Background:Cross-sectional studies provide useful insight about the associations between the built environment and physical activity(PA),particularly when reasons for neighborhood choice are *** study analyzed the relationship between levels of weekly transportation and leisure PA among 3 neighborhood designs,statistically adjusting for sociodemographic characteristics and reasons for neighborhood ***:A stratified random sample of adults(age>20 years)living in Calgary(Canada)neighborhoods with different neighborhood designs(grid,warped-grid,and curvilinear)and socioeconomic status completed a self-administered questionnaire capturing PA,sociodemographic characteristics,and reasons for neighborhood choice(response rate=10.1%;n=1023).Generalized linear models estimated associations between neighborhood design and transportation and leisure PA outcomes(participation(any ***)and volume(metabolic equivalent:h/week)),adjusting for neighborhood socioeconomic status,sociodemographic characteristics(gender,age,ethnicity,education,household income,marital status,children,vehicle access,dog ownership,and injury),and reasons for neighborhood choice(e.g.,proximity and quality of recreational and utilitarian destinations,proximity to work,highway access,aesthetics,and sense of community).Results:Overall,854 participants had resided in their neighborhood for at least 12 months and provided complete *** with those living in curvilinear neighborhoods,grid neighborhood participants had greater odds(phigh-intensity leisure PA(≥6 metabolic equivalent;OR=1.74),*** were no neighborhood differences in the volume of any transportation or leisure PA *** for neighborhood selection had minimal impact on the statistical or practical importance of model estimates.C

AIM To understand the influence of chronic kidney disease(CKD) on mortality, need for transfusion and rebleeding in gastrointestinal(GI) bleeding *** A systematic search was conducted in three databases for studies on GI bleeding patients with CKD or endstage renal disease(ESRD) with data on outcomes of mortality, transfusion requirement, rebleeding rate and length of hospitalization(LOh). Calculations were performed with Comprehensive Meta-Analysis software using the random effects model. heterogeneity was tested by using Cochrane's Q and I2 statistics. Mean difference(MD) and OR(odds ratio) were ***1063 articles(EMBASE: 589; PubM ed: 459; Cochrane: 15) were found in total. 5 retrospective articles and 1 prospective study were available for analysis. These 6 articles contained data on 406035 patients, of whom 51315 had impaired renal function. The analysis showed a higher mortality in the CKD group(OR = 1.786, 95%CI: 1.689-1.888, P he ESRD group(OR = 2.530, 95%CI: 1.386-4.616, P = 0.002), and a rebleeding rate(OR = 2.510, 95%CI: 1.521-4.144, P h impaired renal function. CKD patients required more unit red blood cell transfusion(MD = 1.863, 95%CI: 0.812-2.915, P hospital(MD = 13.245, 95%CI: 6.886-19.623, P han the *** ESRD increases mortality, need for transfusion, rebleeding rate and LOh among GI bleeding patients. Prospective patient registries and observational clinical trials are crucially needed.

Increasing energy demands and environmental pollution concerns press for sustainable and environmentally friendly *** microbial fuel cell(SMFC)technology has great potential for carbon-neutral bioenergy generation and self-powered electrochemical *** this study,an in-depth assessment on the effect of several carbon-based cathode materials on the electrochemical performance of SMFCs is provided for the first *** innovative carbon nanofibers electrode doped with Fe(CNFFe)is used as cathode material in membrane-less SMFCs,and the performance of the resulting device is compared with SMFCs implementing either Pt-doped carbon cloth(PtC),carbon cloth,or graphite felt(GF)as the *** analyses are integrated with microbial analyses to assess the impact on both electrogenesis and microbial composition of the anodic and cathodic *** results show that CNFFe and PtC generate very stable performances,with a peak power density(with respect to the cathode geometric area)of 25.5 and 30.4 mW m^(−2),*** best electrochemical performance was obtained with GF,with a peak power density of 87.3 mW m^(−2).Taxonomic profiling of the microbial communities revealed differences between anodic and cathodic *** anodes were predominantly enriched with Geobacter and Pseudomonas species,while cathodic communities were dominated by hydrogen-producing and hydrogenotrophic bacteria,indicating h_(2)cycling as a possible electron transfer *** presence of nitrate-reducing bacteria,combined with the results of cyclic voltammograms,suggests microbial nitrate reduction occurred on GF *** results of this study can contribute to the development of effective SMFC design strategies for field implementation.

AIM:To investigate twenty-year experience evaluated the use of the PolysorbR(an absorbable lactomer)staples for distal pancreatic ***:The data on 150 patients[92 men,58women,mean age 52(24-72)years]who underwent distal pancreatectomy(DP)in the last 20 years were collected prospectively from an electronic *** diagnosis was confirmed by endoscopic retrograde cholangiopancreatography,sonography,computed tomography and/or magnetic resonance *** indications for DP were focal pancreatic necrosis,spontaneous pancreatic fistulas,abscesses,pseudocysts,segmental chronic obstructive pancreatitis in the tail,traumatic disruption,and benign(cystadenomas,insulinomas,or glucagonomas)or malignant *** distal resections were performed without splenectomy in 29 of the 150 patients(19%).In the event of splenectomy,the splenic artery and vein were individually ligated,the TA-55 Auto Suture stapler,loaded with Premium PolysorbR 55 staples(5.5mm),was placed across the gland,and the trigger was pulled,the action of which produced two staggered absorbable suture *** gland distal to the stapler wasthen amputated with a scalpel on the TA-55 stapler and the two rows of staples were left in the proximal pancreatic *** the distal resection,a drainage tube was inserted into the pancreatic ***:The average duration of the operation was150 min(range:90-210 min)and no transfusion was indicated during the *** DP in one patient a type B fistula was diagnosed,which was treated successfully by conservative treatment comprising of 12-d octreotide medication(3×0.1 mg/d)and jejunal *** incidence of postoperative pancreatic fistula was therefore0.6%.Another 2 patients suffered postoperative pancreatitis,which was also conservatively *** were performed in 2 patients on the first or second postoperative day,necessitated by bleeding from the retroperitoneal *** morbidity was 3.3%(5 patients),but no mortality

Biological therapy has been used in the treatment of inflammatory bowel disease (IBD-Crohn’s disease [CD], ulcerative colitis [UC]) for more than a decade. Although guidelines exist on how to start and adjust biological therapy, information on when and how it should be stopped is still scarce. In this review, our aim is to summarize the results of the recently published papers on the outcome of the discontinuation of biological therapy in patients with CD and UC.
