Superior Sinus Venosus Atrial Septal Defect: Overview of Surgical Options

Background: Superior sinus venosus atrial septal defect (SV-ASD) is an interatrial defect with partial anomalous pulmonary venous connection (PAPVC) draining into the right atrium. The principle for SV-ASD surgical repair involves redirecting the pulmonary venous blood back to the left atrium and closure of the atrial septal defect without compromising the adjoining structures. Methods: We reviewed English literature relating to this topic via a MEDLINE and Google scholar search using the following terms: surgery for sinus venosus atrial septal defect (SV-ASD), surgery for partial anomalous pulmonary venous connection (PAPVC) and complications of the surgery. Results: A total of 910 consecutive cohort patients with different surgical options between 1984 to 2012 were analyzed. The breakdown showed that 291(32.0%) had single-patch, 275(30.2%) had Warden’s procedure, 185(20.3%) had a double-patch and 159(17.5%) had use of autologous right atrial appendage for the anastomosis. The total follow-up was between 0.008-30 years. Sinus node dysfunction (SND) was the commonest complication occurring mostly in patients who had double-patch technique 16(8.6%) followed by 15(5.5%) patients with single-patch technique, 7(4.4%) patients in whom right atrial appendage was used and 5(1.8%) patients with Warden’s procedure. Venous obstruction occurred in 22(7.7%) patients with SP, 13(5.1%) patients with Warden’s Procedure and 5(2.7%) patients with double-patch. Conclusion: The two major complications, SND and venous obstruction, assumed a see-saw approach as surgical maneuvers avoiding venous obstruction encouraged SND and vice versa. However, adopting surgical options which avoided incision across the Cavoatrial junction attenuated all the complications.

Introduction: Hyponatremia is the commonest electrolyte derangement seen in medical and surgical wards and in high dependency treatment units. It is associated with prolonged hospital stay, high economic burden and increased morbidity and mortality. Hyponatremia is well reported in medical and surgical conditions but literature is scanty concerning its relationship with treatment outcome in patients managed in high dependency heart and vascular surgical centers. Objectives: To determine the prevalence, determinants, clinical correlates and outcome of pre-operative hyponatremia. Methods: Retrospective study involving patients that had surgery (cardiac and vascular) between January 2015 and December 2019. Data were obtained from patients’ case files, perioperative and intensive unit charts. All participants with preoperative hyponatremia had their serum sodium corrected to ≥135 mmol before surgical incision and those with hypernatremia had their sodium corrected to Results: Of the 305 participants (186 males and 119 females), pre-operative hyponatremia was found in 30.8% and normonatremia in 59.7%. Pre-operative hyponatremia was more common in advance age, overweight, those presenting for open heart and other major surgeries and those with background chronic heart failure and chronic liver disease. Participants with pre-operative hyponatremia had shortened and also prolonged hospital stay and higher risk of death than with normonatremia. Conclusion: Pre-operative hyponatremia is common in high dependency surgical unit particularly in the elderly, those with comorbidities and those presenting for major heart surgeries. Enhancing Cybersecurity through Cloud Computing Solutions in the United States

This study investigates how cybersecurity can be enhanced through cloud computing solutions in the United States. The motive for this study is due to the rampant loss of data, breaches, and unauthorized access of internet criminals in the United States. The study adopted a survey research design, collecting data from 890 cloud professionals with relevant knowledge of cybersecurity and cloud computing. A machine learning approach was adopted, specifically a random forest classifier, an ensemble, and a decision tree model. Out of the features in the data, ten important features were selected using random forest feature importance, which helps to achieve the objective of the study. The study’s purpose is to enable organizations to develop suitable techniques to prevent cybercrime using random forest predictions as they relate to cloud services in the United States. The effectiveness of the models used is evaluated by utilizing validation matrices that include recall values, accuracy, and precision, in addition to F1 scores and confusion matrices. Based on evaluation scores (accuracy, precision, recall, and F1 scores) of 81.9%, 82.6%, and 82.1%, the results demonstrated the effectiveness of the random forest model. It showed the importance of machine learning algorithms in preventing cybercrime and boosting security in the cloud environment. It recommends that other machine learning models be adopted to see how to improve cybersecurity through cloud computing.

Mosque layout desisn： An analytical study of mosque layouts in the early Ottoman period

This study attempts to quantify the influence of spatial configuration on the functional efficiency of mosque layouts in the early Ottoman period. The literature review consists of two parts. The first part is a theoretical study of the relationship between spatial configuration and functional efficiency of mosque layouts. This part highlights the key syntactical characteristics and effect of spatial configuration on the level of functional efficiency using space syntax theory. The second part is an analytical comparative description of the changes and transfor- mations in the configurations of mosque layouts. The architectural styles are classified into six types based on layout designs. The main benchmarks and indicators involved in measuring the functional efficiency of mosque layouts are analyzed using the A-graph 2009 software program, which provides numerical results. This analysis compares entire samples of mosque layout designs for each category. The numerical results indicate the effect of spatial configurations and the functions of mosque layouts. This study shows that mosques with courtyard layouts are accessible, efficient, and flexible in terms of function because of their distinct syntactical and morphological spatial structures.

Bioaccumulation of heavy metals in some tissues of croaker fish from oil spilled rivers of Niger Delta region,Nigeria

Objective:To investigate the level of heavy metals(Cu,Zn,Fe Mn Ni,Pb and Cd) in muscles,gills and liver tissue of Croaker fish from oil spilled rivers of Bonny and Finima,Niger Delta region,***:Twenty four Croacker fishes(Genyonemus lineatus) weighing between 250 and 260 g and 24–26 cm length were collected from each sampling sites(Bonny and finima rivers).The frozen fishes were thawed and dried at 105C until they reach a constant *** dried samples were homogenized and digested with 10 ml tri-acid mixture(HNO3:HCl O4:H2SO4),and then the digested samples were diluted to 100 ml with deionized *** metals(Cu,Ni,Zn,Pb,Mn,Iron Fe,and Cd) concentrations were determined by atomic absorption ***:Our results revealed that concentrations and distribution patterns of all heavy metals studied varied significantly(P < 0.05) amongst the fish tissues analyzed and sampling ***,liver tissue of fish caught from Finima creek accumulated the highest concentrations of Cu(52.64 ± 3.01 mg/g dry wt),Zn(166.50 ± 6.45 mg/g dry wt)and Fe(801.50 ± 14.15 mg/g dry wt) in comparison to the liver of fish caught from Bonny river in which the levels of Cu,Zn and Fe were(45.00 ± 2.79),(49.90 ± 2.91) and(216 ± 6.11 mg/g dry wt),*** addition,Mn,Ni,Pb and Cd exhibited their highest concentrations in gills from both *** expected,muscle tissue contained the least concentrations of all metals investigated from both sampling ***,all the metals investigated exhibited highest concentration in fish collected from Finima *** abnormal high level heavy metals accumulation observed in this location could be linked to the frequent crude oil spills as well as industrial activity around the area which might get discharged into Finima *** general,the mean concentrations of some toxic heavy metals investigated exceed the recommended maximum permissible limits set by the Joint FAO/WHO ***,some metals

Background:The public knowledge and adherence to the established coronavirus disease 2019(COVID-19)precautionary measures are crucial to Nigeria’s war against the *** health education on its preventive practices at the grassroots level was initially crucial to achieving a lower COVID-19 incidence in Kwara State,***:We assessed the knowledge of,and adherence to COVID-19 precautionary measures at the community level among 795 respondents from the three senatorial zones of Kwara ***:54.5%(433/795)of the respondents were aged between 21 and 40 years,and 45.9%(365/795)of the respondents had a bachelor’s degree or *** participants had a good knowledge of COVID-19,its symptoms,and its mode of transmission.91.8%of the respondents(730/795)had a positive perception of the COVID19 preventive measures while 96.1%(763/795)of the respondents agreed that maintaining social distance was important in curbing the COVID-19 ***,only 38%(302/795)of them used face masks and only 25.7%(204/795)of the respondents used hand *** addition,only 31.9%(253/795)of the respondents isolated themselves when they were ***-variable logistic regression analysis revealed that education,occupation,gender,and ethnicity were significantly associated with positive COVID-19 preventive practices among residents of Kwara *** servants were more likely(AOR:3.14;95%confidence interval[CI]:0.67 to 14.82;P=0.034)to have positive preventive attitudes than other *** participants with tertiary education and those that were Yoruba(ethnicity)were 14.81 times more likely(95%CI:4.29 to 51.05;P=0.001)and 5.19 times more likely(95%CI:1.82 to 14.84;P=0.007)to have positive attitudes towards the laid-down COVID-19 preventive measures ***:The poor community adherence to the COVID-19 preventive practices could pre-dispose Kwara to more COVID-19 *** community engagement activities are needed to fully curb the

Heavy Metal Profile of Oreochromis niloticus Harvested from E-Waste Polluted Vials and Associated Fungi

This study was designed to investigate the possible variation in bioaccumulation of heavy metals (lead, cadmium, zinc, cobalt, chromium, nickel, and manganese) in the tissue of harvested Oreochromis niloticus (tilapia) and the associated fungi from vials treated with soil from e-waste dumpsite and soil without e-waste. E-waste is electronic waste which contains valuable metals as well as potential environmental contaminants. The heavy metals in soil and fish samples were determined using flame atomic absorption spectrophotometer (AAS) after homogeneity and digestion of samples. The associated fungi were identified using standard microbiological methods. The genera of the associated fungi were: Penicillium, Candida, Articulospora, Aspergillus, Aspergillus, Aspergillus, Rhizopus, Mucor, Zoopage, Varicosporium and Rhodoturula. The quantity of fungi isolated from each pollution treatments showed that the number of fungi were inversely proportional to the quantity of e-waste soil pollution and directly proportional to the quantity of soil without e-waste pollution. The occurrence of the fungi species revealed that the untreated (control) vial had the highest occurrence (191) while the lowest occurrence (103) occurred in the vial polluted with the highest quantity of soil from e-waste dumpsite. The pH and the biochemical oxygen demand (BOD) of the vials were significantly affected by the pollutions. Variations were also observed in the bioaccumulation of the heavy metals by Oreochromis niloticus. Cadmium (0.01 mg/kg) and nickel (0.02 - 0.08 mg/kg) were the least recorded metal in the tissue of the fish while cobalt was not detected after the five weeks period. The sequence of the heavy metals concentrations in Oreochromis niloticus tissue samples was Zn > Mn > Pb > Cr > Ni > Cd > Co. The accumulation of the metals in the fish tissue were, however, lower than the international maximum guidelines, except for manganese (0.29 - 3.13 mg/kg) that exceeded the 0.01 - 0.05 mg/kg
