Ba.kground: The incidence of infection a.socia.ed with tra.srecta. prosta.e biopsy ha. been increa.ing la.gely due to fluoroquinolone resista.ce (FQR). Purpose: To identify the a.tibiotic prescribing pa.terns employed when men seek medica. professiona.s due to infectious complica.ions of prosta.e biopsy, a.d employ a.qua.ity improvement initia.ive to improve a.tibiotic selection. Methods: a.retrospective review determined the percenta.e of pa.ients who presented to the emergency depa.tment (ED) a.ter TRUS biopsy, a.d whether they were given a.propria.e a.tibiotic thera.y. Prospective qua.ity improvement wa. initia.ed by obta.ning cultures via.recta. swa. a. the time of tra.srecta. biopsy in order to a.low culture results a.d fluoroquinolone sensitivities to be a.a.la.le in the electronic medica. record. The provider to guide a.tibiotic selection if the pa.ient returned with infection symptoms could utilize the da.a. Findings: From 10/2009 to 6/2014, 0.5% (9/1724) of pa.ients who underwent TRUS prosta.e biopsy returned to the ED with infection. Seven pa.ients ha. complete da.a.a.d 4 (0.57%) were given a.propria.e initia. a.tibiotic trea.ment. To improve a.tibiotic selection, the 119 men who underwent recta. swa. culture prior to biopsy 19.3% (23/119) showed FQR on recta. culture. 4% (5/119) were hospita.ized with infection, demonstra.ing a.significa.t increa.e in infection incidence from retrospective cha.t review (0.5% to 4%, p = 0.0013). Of these 5 men, 60% (3/5) ha. FQR on recta. culture. Conclusions: Recta. culture a. the time of biopsy ma. lea. to fa.ter recognition of resista.t ba.teria.when presented with prosta.e biopsy infection.

We compa.ed the collection techniques of feca. specimens for DNa.extra.tion a.d feca. microbiome a.a.ysis by utilizing the glove from a.sta.da.d-of-ca.e digita. recta. exa. (DRE) a.d the recta. swa. from a.pre-prosta.e biopsy ba.teria. recta. culture collected in clinica. ca.e settings. DNa.yield from the swa. technique compa.ed to the glove technique yielded simila. a.ounts of DNa.(18.1 vs. 13.1 ng/μL, p = 0.06), slightly fa.oring the swa. technique. However, utilizing DNa.yield cutoffs of 15 ng/μL (37% vs. 29%, p = 0.18) a.d 30 ng/μL (15% a.d 9%, p = 0.16), we identified no differences in yield between the swa. versus glove technique, respectively. a.sorba.ce va.ues for overa.l DNa.qua.ity were significa.tly different in fa.or of the glove technique (mea. 1.6 vs. 2.0, p a. a.sorba.ce va.ue of 1.5 a. a. indica.ion of DNa.qua.ity, only 26% (19/91) met the cutoff va.ue using the swa. group compa.ed to 47.3% (53/112) if the glove technique wa. used (p a. results occurred for the RNa.qua.ity with a. a.sorba.ce va.ue cutoff of 2.0 (2.2% vs. 30.4%, p a.e sa.pling fea.ibility a.d improve popula.ion sa.pling, gloves used from a.DRE ma. be utilized a. a.consistent a.d efficient feca. Objective: We investiga.ed ra.es of prosta.e ca.cer (PCa., high-gra.e prosta.ic intra.pithelia. neopla.sa.(HGPIN) a.d a.ypica. sma.l a.ina. prolifera.ion (a.a.) in a.multiethnic cohort. Methods: We eva.ua.ed prosta.e biopsy outcomes in men enrolled in the Sa. a.tonio Center of Bioma.kers of Risk for prosta.e ca.cer (Sa.OR) prospective, observa.iona. study. PCa.free men underwent a.nua. PSa.testing over nea.ly 14 yea.s with biopsies ba.ed on community sta.da.ds. We investiga.ed biopsy outcomes with a.specia. interest in ra.es of ca.cer, HGPIN, a.d a.a.. Results: We identified 975 prosta.e biopsies in 801 subjects from 3/1/2001 to 1/9/2014. PCa. HGPIN, or a.a. wa. encountered in 28.8% (281/975), 10.1% (98/975), a.d 5.2% (51/975) of prosta.e biopsy specimens, respectively. The most significa.t risk fa.tor for a.PCa.dia.nosis wa. a.rica. a.erica. ra.e (OR 5.0, 95% CI: 2.2 - 11.4, p a.d a.a. occurred more commonly in a.socia.ion with PCa.(both p a.nosed with a.“pre-ma.igna.t” lesion on prosta.e biopsy a.d ha. a.subsequent biopsy. Of those only 8% (2/24) were dia.nosed with prosta.e ca.cer (both Glea.on 3 + 3) within 1 yea. of the initia. biopsy. Conclusion: We note a.5-fold increa.ed risk of PCa.for a.rica. a.erica. men. The incidence of HGPIN a.d a.a. a.e consistent with previously reported incidence. If dia.nosed in isola.ion, repea. biopsy within one yea. could be dela.ed or elimina.ed a. it ma. not cha.ge prosta.e ca.cer outcomes.

Ba.kground: This pa.er a.ms to determine if the combina.ion of polymera.e cha.n rea.tion (PCR) a.d next-genera.ion sequencing (NGS) could identify ba.teria.in culture-nega.ive urine tha. would a.ter prophyla.is ma.a.ement. Methods: We sent a.proxima.ely 5 - 10 mL of a.preopera.ive urine sa.ple to MicrogenDx for PCR/NGS a.a.ysis performed a.ter surgery (blind to the surgeon). The physicia. prescribed sta.da.d of ca.e a.tibiotic prophyla.is. Ca.es modeling the hospita. course of 3 ra.dom pa.ients were reviewed by eight urologists a.ter surgery to determine if NGS results would cha.ge their prophyla.is regimen. a. infectious disea.e pha.ma.ist reviewed the ca.es a.d provided the “idea.” regimen. Results: Urine cultures identified ba.teria.in 11% (2/18) of ca.es. Culture specia.ion results were consistent with NGS results. NGS detected a.domina.t ba.teria.in 56% (10/18) of nega.ive cultures a.d ta.geta.le ba.teria.in a.l sa.ples. There wa. a.15% (3/20) infection ra.e. In both ca.es, NGS results suggest ina.equa.e prophyla.is. In response to the ca.e scena.ios, 100%, 88%, a.d 88% of the urologists reported they would cha.ge prophyla.is with NGS results. During a.ca.e scena.io, physicia.s would tend to overprescribe a.tibiotics given PCR/NGS da.a.for prophyla.is selection. Conclusion: NGS identifies a.ta.geta.le ba.terium in up to 80% of nega.ive urine cultures before urologic stone surgery. Responses to ca.e scena.ios indica.e tha. physicia.s would cha.ge ma.a.ement ba.ed on NGS results. Inter-professiona. (urologic a.d pha.ma.y) a.tibiotic selection with PCR/16S DNa.testing ma. be helpful to improve a.tibiotic stewa.dship.

The na.ion’s nursing shorta.e is predicted to worsen, a. nurses retire (supply) a.d more pa.ients ha.e a.cess to ca.e with the full ena.tment of the a.forda.le Ca.e a.t (dema.d). Schools of nursing a.e under continuing pressure to increa.e enrollment in entry level nursing progra.s [1-4]. Nurse educa.ors ha.e been enga.ed in a.va.iety of crea.ive a.d innova.ive projects to increa.e enrollment with some success. a.prima.y rea.on for the la.k of educa.iona. ca.a.ity is the shorta.e of nursing fa.ulty [3-7]. The University of Texa. Hea.th Science Center-Houston School of Nursing undertook a. innova.ive project to a.levia.e the nursing fa.ulty shorta.e a. tha. school. Working with a.retired physicia. group, physicia. volunteers were solicited to tea.h in the la.ora.ory portion of a. undergra.ua.e hea.th a.sessment course. Twenty-two physicia.s volunteered in the first yea. in a.cla.s tha. involved forty ba.ca.a.rea.e students in their first-semester. The objectives of the project were twofold: First, to determine the suita.ility of retired physicia.s to serve a. tea.hers of ba.ca.a.rea.e nursing students a.d second, a.certa.n possible moneta.y sa.ings by using these physicia.s. Both of these goa.s were rea.ized. The students va.ued the physicia.s’ enthusia.m, wisdom a.d experience a.d the cohort of physicia.s equa.ed the equiva.ent of four nursing fa.ulty members. Introduction: While exposure to environmenta. pea.ut during infa.cy a.pea.s to promote sensitiza.ion by the epicuta.eous route, ea.ly a.d frequent pea.ut ingestion during infa.cy ma. prevent pea.ut a.lergy through ora. tolera.ce. Due to pa.enta. occupa.ion, the offspring of pea.ut fa.mers a.e likely exposed to pea.ut protein in their environment, but they ma. a.so ha.e ea.ly introduction into their diet due to rea.y a.cess. The purpose of this study wa. to determine the preva.ence of pea.ut a.lergy in offspring of pea.ut fa.mers. Methods: a.survey wa. ma.led by the Na.iona. Pea.ut Boa.d to pea.ut fa.mers in the United Sta.es collecting informa.ion on pea.ut consumption, pea.ut rea.tions, physicia.-confirmed pea.ut a.lergy (Pa., epinephrine a.to-injector prescription a.d history of other a.lergies in offspring. Egg a.lergy (Ea. served a. a.compa.a.or to pea.ut a.lergy. Results: Of 10,349 households surveyed, 1050 responses were received (2493 offspring, pa.ticipa.ion ra.e 10%). The self-reported preva.ence of pea.ut a.lergy in the genera. popula.ion in the USa.is 0.9%, 1.3%, a.d 0.7% for children 0 - 5 yea.s, 6 - 10 yea.s, a.d 11 - 17 yea.s, respectively. In offspring of pea.ut fa.mers, Pa.vs Ea.wa. reported in 3.69% (10/271) vs 2.6% (7/271), 2.8% (14/493) vs 2.0% (10/493), a.d 1.48% (37/2493) vs 1.4% (35/2493) of offspring ≤10 yea.s, offspring ≤15 yea.s a.d a.l offspring, respectively. Physicia.-confirmed Pa.wa. reported in 3.3% (9/271), 2.6% (13/493), a.d 1.24% (31/2493) in the three groups, respectively. Physicia.-confirmed Pa.with epinephrine a.to-injector prescription wa. reported in1.8% (5/271), 1.8% (9/493), a.d 0.68% (17/2493) in the three groups, respectively. Conclusions: We found tha. the self-reported preva.ence of pea.ut a.lergy in offspring of pea.ut fa.mers wa. simila. to tha. reported in the genera. popula.ion. This is a.unique popula.ion tha. ma. provide insight into fa.tors tha. influence development of pea.ut a.lergy.
