Neuropathic pain-like symptoms and pre-surgery radiographic severity contribute to patient satisfaction 4.8 years post-total joint replacement

AIM To investigate a comprehensive range of factors that contribute to long-term patient satisfaction post-total joint replacement(TJR) in people who had undergone knee or hip replacement for *** Participants(n = 1151) were recruited from Nottinghamshire post-total hip or knee replacement. Questionnaire assessment included medication use, the pain-DETECT questionnaire(PDQ) to assess neuropathic pain-like symptoms(NP) and TJR satisfaction measured on average 4.8 years post-TJR. Individual factors were tested for an association with post-TJR satisfaction, before incorporating all factors into a full model. Data reduction was carried out using LASSO and receiveroperator characteristic(ROC) curve analysis was used to quantify the contribution of variables to post-TJR *** After data reduction, the best fitting model for post-TJR satisfaction included various measures of pain, history of revision surgery, smoking, pre-surgical X-ray severity, WOMAC function scores and various comorbidities. ROC analysis of this model gave AUC = 0.83(95%CI: 0.80-0.85). PDQ scores were found to capture much of the variation in post-TJR satisfaction outcomes: AUC = 0.79(0.75-0.82). Pre-surgical radiographic severity was associated with higher post-TJR satisfaction: OR_(satisfied) = 2.06(95%CI: 1.15-3.69), P = *** These results highlight the importance of pre-surgical radiographic severity, post-TJR function, analgesic medication use and NP in terms of post-TJR satisfaction. The PDQ appears to be a useful tool in capturing factors that contribute to post-TJR satisfaction.

Hand osteoarthritis is a common heterogeneous joint disorder with unclear molecular mechanisms and no disease-modifying *** this study,we performed single-cell RNA sequencing analysis to compare the cellular composition and subpopulationspecific gene expression between cartilage with macroscopically confirmed osteoarthritis(n=5)and cartilage without osteoarthritis(n=5)from the interphalangeal joints of five *** 105142 cells,we identified 13 subpopulations,including a novel subpopulation with inflammation-modulating potential annotated as inflammatory *** chondrocytes exhibited extensive alteration of gene expression patterns in osteoarthritic cartilage compared with nonosteoarthritic *** inflammatory chondrocytes and fibrocartilage chondrocytes showed a trend toward increased numbers in osteoarthritic *** these two subpopulations from osteoarthritic cartilage,the ferroptosis pathway was enriched,and expression of iron overload-related genes,e.g.,FTH1,was *** verify these findings,we conducted a Mendelian randomization study using UK Biobank and a population-based cross-sectional study using data collected from Xiangya Osteoarthritis *** predisposition toward higher expression of FTH1 mRNA significantly increased the risk of hand osteoarthritis(odds ratio=1.07,95%confidence interval:1.02–1.11)among participants(n=332668)in UK *** levels of serum ferritin(encoded by FTH1),a biomarker of body iron overload,were significantly associated with a high prevalence of hand osteoarthritis among participants(n=1241)of Xiangya Osteoarthritis Study(P-for-trend=0.037).In conclusion,our findings indicate that inflammatory and fibrocartilage chondrocytes are key subpopulations and that ferroptosis may be a key pathway in hand osteoarthritis,providing new insights into the pathophysiology and potential therapeutic targets of hand osteoarthritis.

目的评价外用非甾体类消炎药治疗骨关节炎的疗效。方法计算机检索MEDLINE,EM-BASE,SCI,CINAHL,Cochrane图书馆和中国生物医学文献数据库等。手工检索已发表和未发表的所有外用非甾体类消炎药随机对照试验,对其逐个进行质量评价,并合并数据进行Meta分析。检索时间截至2005年3月30日。结果共纳入13篇随机对照试验,包括1983例患者,均为高质量研究。对治疗有效率的Meta分析结果显示,外用非甾体类消炎药治疗骨关节炎引起的疼痛仅在第1、2周疗效优于安慰剂,疼痛减轻的效应量分别是0·41[95%CI(0·16,0·660)]和0·40[95%CI(0·15,0·65)],而第3、4周疗效与安慰剂比较差异无统计学意义。结论现仅有短期(<4周)的随机对照试验评价外用非甾体类消炎药治疗骨关节炎的疗效。外用非甾体类消炎药用于治疗骨关节炎的疗效仅在第1、2周优于安慰剂,2周后其疗效与安慰剂的差异无统计学意义。目前还没有足够的临床试验证明,使用非甾体类消炎药治疗骨关节炎超过2周仍然有效。

Smart Sewers 1: Sewer Configuration at Rural Houses

The layout of houses and other buildings impacts the way in which foul sewer pipework is positioned internally and externally. Less water to waste through conservation measures reduces the distance that gross solids transfer in sewers and increases the number of sewer blockages. Dwelling houses are often laid out where the solids from faecal flushes are at the head of the sewer line with other flows entering downstream. Discharges from appliances such as washing machines, dishwashers, baths, showers and kitchen/utility sinks are often not utilised in the transfer of the gross solids when they enter downstream of the faecal flushes. At present, no recommendations or specific design guidance exist regarding the design of internal building layouts relating to sewer configuration requirements. Furthermore, to date, no specific research exists which examines pipeline configuration scenarios outside buildings in terms of the link between multiple grey water discharge points and solid transfer in a sewer system. The aim of this study was to investigate sewer layout at houses in terms of maximising greywater flow in relation to solid transfer. This study showed that smart sewers are needed which utilise all the foul water leaving a building as it was found that up to 100% of greywater in some instances is completely missed out in terms of solid transfer. Consequently, optimal sewer design is far from being realised and internal building layouts should be designed with consideration of the faecal flushes and greywater flows.

A Low-Cost, Portable Optical Sensing System With Wireless Communication Compatible of Real-Time and Remote Detection of Dissolved Ammonia

A low-cost and portable optical chemical sensor based ammonia sensing system that is capable of detecting dissolved ammonia up to 5 ppm is presented. In the system, an optical chemical sensor is designed and fabricated for sensing dissolved ammonia concentrations. The sensor uses eosin as the fluorescence dye which is immobilized on the glass substrate by a gas-permeable protection layer. A compact module is developed to hold the optical components, and a battery powered micro-controller system is designed to read out and process the data measured. The system operates without the requirement of laboratory instruments that makes it cost effective and highly portable. Moreover, the calculated results in the system can be transmitted to a PC wirelessly, which allows the remote and real-time monitoring of dissolved ammonia.

Hepatitis E is more common than hepatitis A among returning travellers presenting to tertiary care

Introduction: Acute viral hepatitis is a relatively common infection resulting in hospital attendance after foreign travel. Travellers and doctors are generally aware of hepatitis A and the fact that safe and effective immunisation is available. In contrast, there is no widely available vaccine for hepatitis E and most physicians’ experience with this condition is limited. Over the last few years, the number of cases of hepatitis E has increased. Methods: We examined the prevalence of hepatitis A and E among patients presenting to the Hospital for Tropical Diseases between 2000 and 2009. Travel history, demographics and laboratory parameters of these patients were compared. Results: The annual incidence of hepatitis A remained static, while that of hepatitis E increased from 1 to 4. Hepatitis E was associated with older age, travel to the Indian sub-continent (ISC), and visiting friends and relatives (VFR). Peak ALT was similar between patients with hepatitis A or E, but as many as a third of those with hepatitis E developed a prolonged INR, compared to 11% of those with hepatitis A. In addition, patients with hepatitis E had a longer hospital admission by a median of 3 days. Conclusion: Hepatitis E is now the commonest cause of faeco-oral viral hepatitis at this centre, and is associated with laboratory features suggestive of more severe liver damage and longer hospital stay.
