We read with great interest the article published in Annals of Surgery, "Preoperative Biliary Stenting and Major Morbidity After Pancreatoduodenectomy: Does Elapsed Time Matter? The FRAGERITA Study Group" (1). The timing of preoperative biliary stenting (PBS) and its impact on postoperative outcomes after pancreaticoduodenectomy (PD) are ever-burning issues for pancreatologists dealing with jaundice due to periampullary tumors.

Carbon monoxide contributes to the constipating effects of granisetron in rat colon

AIM To investigate the mechanisms underlying the potential contribution of the heme oxygenase/carbon monoxide(HO/CO) pathway in the constipating effects of granisetron. METHODS For in vivo studies, gastrointestinal motility was evaluated in male rats acutely treated with granisetron [25, 50, 75 μg/kg/subcutaneous(sc)], zinc protoporphyrin IX [Zn PPIX, 50 μg/kg/intraperitoneal(ip)] and hemin(50 μmol/L/kg/ip), alone or in combination. For in vitro studies, the contractile neurogenic response to electrical field stimulation(EFS, 3, 5, 10 Hz, 14 V, 1 ms, pulse trains lasting 10 s), as well as the contractile myogenic response to acetylcholine(ACh, 0.1-100 μmol/L) were evaluated on colon specimens incubated with granisetron(3 μmol/L, 15 min), Zn PPIX(10 μmol/L, 60 min) or CO-releasing molecule-3(CORM-3, 100, 200, 400 μmol/L) alone or in combination. These experiments were performed under co-treatment withor without atropine(3 μmol/L, a muscarinic receptor antagonist) or NG-nitro-L-Arginine(L-NNA, 100 μmol/L, a nitric oxide synthase inhibitor).RESULTS Administration of granisetron(50, 75 μg/kg) in vivo significantly increased the time to first defecation(P = 0.045 vs vehicle-treated rats), clearly suggesting a constipating effect of this drug. Although administration of Zn PPIX or hemin alone had no effect on this gastrointestinal motility parameter, Zn PPIX co-administered with granisetron abolished the granisetron-induced constipation. On the other hand, co-administration of hemin and granisetron did not modify the increased constipation observed under granisetron alone. When administered in vitro, granisetron alone(3 μmol/L) did not significantly modify the colon's contractile response to either EFS or ACh. Incubation with Zn PPIX alone(10 μmol/L) significantly reduced the colon's contractile response to EFS(P = 0.016) but had no effect on contractile response to ACh. The Pontecorvo Affaire Reappraised： Five Decades of Cold War Spy-Stories （1948-1998）

In August 1950, the nuclear physicist Bruno Pontecorvo suddenly left Rome for Stockholm and disappeared without leaving any trace. Pontecorvo had joined the British atomic bomb project. His abrupt disappearance quickly became an international affaire, causing much concern to the western intelligence services, who were worried about the escape of atomic secrets to the Soviet Union, especially after Fuchs＇ famous case. In the Union of Soviet Socialist Republics （USSR）, Pontecorvo was welcomed with honour, but also isolated from the rest of the world for many decades. He always denied any direct involvement in the Soviet nuclear programme. According to him, he moved to Russia because of his socialist beliefs, working only on particle physics and peaceful applications of atomic energy. Who was really Bruno Pontecorvo？ A pacifist like Robert Oppenheimer or a communist agent like Klaus Fuchs？ Did he have access to any information about the Manhattan Project during the war, thanks to his friendship with Enrico Fermi？ Did he spy on the Anglo-Canadian programme like Igor Gouzenko, before moving to Russia？ Did he join Soviet military projects across more than 30 years in Dubna？ The author will discuss how the perception of Pontecorvo＇s case changed in the public opinion from the 1950s until the 1990s, as a mirror of the tensions between the two blocks. The whole affaire was object of harsh political confrontation in Italy for almost 50 years. Therefore, the author will also focus on the role played by the Italian communists in Pontecorvo＇s defection to the USSR as well as on such a local debate, which reflects the history of the post-war Italian Communist Party （PCI） until the end of the Cold War.

Pancreatic ductal adenocarcinoma is a highly aggressive tumor with an overall 5-year survival rate of less than 5%. Prognosis and treatment depend on whether the tumor is resectable or not, which mostly depends on how quickly the diagnosis is made. Chemotherapy and radiotherapy can be both used in cases of nonresectable pancreatic cancer. In cases of pancreatic neoplasm that is locally advanced, non-resectable, but non-metastatic, it is possible to apply percutaneous treatments that are able to induce tumor cytoreduction. The aim of this article will be to describe the multiple currently available treatment techniques(radiofrequency ablation, microwave ablation, cryoablation, and irreversible electroporation), their results, and their possible complications, with the aid of a literature review.

Bioconversion is a biological process by which organic materials are converted into products with higher biological and commercial *** its larval stage the black soldier fly Hermetia illucens is extremely voracious and can feed on a wide vari-ety of organic *** study the impact of different fruit byproducts on the insect's growth,final larval biomass,substrate reduction,bioconversion parameters,and larval nu-tritional composition,10000 black soldier fly larvae(BSFL)were reared on 7.0 kg of one of three substrates(strawberry,tangerine,or orange)or on a standard diet as a *** results highlight that BSFL can successfully feed and grow on each of these diets,though their development time,growth rate,and final biomass were differently impacted by the substrates,with strawberry being the most *** lipid and protein contents of BSFL were similar among larvae fed on different substrates;however,major differences were detected in ash,micronutrient,fiber,fatty acid,and amino acid ***,the results indicate that fruit waste management through the BSFL bioconversion process rep-resents a commercially promising resource for regional and national agrifood *** study offers new perspectives for sustainable and environmentally friendly industrial development by which fruit byproducts or waste might be disposed of or unconventionally enhanced to create secondary products of high biological and economic value,including BSFL biomass as animal feed or,in perspective,as alternative protein source for human nutrition.

Computed tomography-based radiomic to predict resectability in locally advanced pancreatic cancer treated with chemotherapy and radiotherapy
