Establishment and validation of an adherence prediction system for lifestyle interventions in non-alcoholic fatty liver disease

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)is the most common liver disease worldwide,affecting about 1/4th of the global population and causing a huge global economic *** date,no drugs have been approved for the treatment of NAFLD,making the correction of unhealthy lifestyles the principle method of *** patients with poor adherence to lifestyle correction and attempting to improve their adherence are therefore very *** To develop and validate a scale that can rapidly assess the adherence of patients with NAFLD to lifestyle *** The Exercise and Diet Adherence Scale(EDAS)was designed based on com-pilation using the Delphi method,and its reliability was subsequently *** and laboratory indicators were measured,and patients completed the EDAS questionnaire at baseline and after 6 *** efficacy of the EDAS was evaluated in the initial ***,the efficacy of the EDAS was internally verified in a validation *** The EDAS consisted of 33 items in six dimensions,with a total of 165 *** EDAS score correlated significantly with daily number of exercise and daily reduction in calorie intake(P500 kacl/d),(sensitivity/specificity was 100.0%/75.8%),while patients score below 97 could nearly rule out the possibility of daily exercise(sensitivity/specificity was 89.5%/44.4%).Total EDAS scores≥116,97-115,and<97 points were indicative of good,average,and poor adherence,respectively,to diet and exercise *** The EDAS can reliably assess the adherence of patients with NAFLD to lifestyle interventions and have clinical application in this population.

Numerical Study on the Aerodynamic and Fluid−Structure Interaction of An NREL-5MW Wind Turbine

A 5-MW wind turbine has been modeled and analyzed for fluid-structure interaction and aerodynamic *** this study, a full-scale model of a 5-MW wind turbine is first developed based on a computational fluid dynamics(CFD) approach, in which the unsteady, noncompressible Reynolds Averaged Navier-Stokes(RANS) method is used. The main focus of the study is to analyze the tower shadow effect on the aerodynamic performance of the wind turbine under different inlet flow conditions. Subsequently, the finite element model is established by considering fluid/structure interactions to study the structural stress, displacement, strain distributions and flow field information of the structure under the uniform wind speed. Finally, the fluid-structure interaction model is established by considering turbulent wind and the tower shadow effect. The variation rules of the dynamic response of the one-way and two-way fluid-structure interaction(FSI) models under different wind speeds are analyzed, and the numerical calculation results are compared with those of the centralized mass model. The results show that the tower shadow effect and structural deformation are the main factors affecting the aerodynamic load fluctuation of the wind turbine, which in turn affects the aerodynamic performance and structural stability of the blades. The structural dynamic response of the coupled model shows significant similarity, while the structural displacement response of the former exhibits less fluctuation compared with the conventional centralized mass model. The one-way fluid-structure interaction(FSI)model shows a higher frequency of stress-strain and displacement oscillations on the blade compared with the two-way FSI model.

Analytical Solution and Simplified Formula for Added Mass of Horizontal and Vertical Motions of Truncated Cylinders Under Earthquake Action

This paper investigates the hydrodynamic characteristics of floating truncated cylinders undergoing horizontal and vertical motions due to earthquake excitations in the finite water *** governing equation of the hydrodynamic pressure acting on the cylinder is derived based on the radiation theory with the inviscid and incompressible *** governing equation is solved by using the method of separating variables and analytical solutions are obtained by assigning reasonable boundary *** analytical result is validated by a numerical model using the exact artificial boundary simulation of the infinite *** main variation and distribution characteristics of the hydrodynamic pressure acting on the side and bottom of the cylinder are analyzed for different combinations of wide-height and immersion *** added mass coefficient of the cylinder is calculated by integrating the hydrodynamic pressure and simplified formulas are proposed for engineering *** calculation results show that the simplified formulas are in good agreement with the analytical solutions.

Improving flangeability of multiphase steel by increasing microstructural homogeneity

Multiphase microstructure significantly increases the strength,usually at the expense of flangeability because of lacking microstructure *** further improve the strength-flangeability of multiphase steel,the microstructural homogeneity was advanced by adjusting the hard martensite/austenite(M/A)*** strength-flangeability was measured via uniaxial tensile tests and hole expansion *** microstructures were characterized using a scanning electron microscope equipped with an electron backscatter diffraction detector and a transmission electron *** tests were supplementally used to quantitatively reveal the microstructural homogeneity of the *** show that the adjusted multiphase steel achieves an excellent ultimate tensile strength(~800 MPa)and flangeability(~135%hole expansion ratio).A promising homogeneous multiphase microstructure was obtained by controlling undercooled austenite transformed at about 600℃.This microstructure consists of soft polygonal ferrite,blocky bainitic ferrite,and hard M/A *** volume fraction of M/A islands is around 5%,and the average size is less than l *** nanoindentation analysis indicated that the participation of M/A islands impressively influenced the microstructural *** strain partition and better mechanical compatibility were present in the adjusted multiphase steel since the plasticity initiation started late,which resulted in a positive ***,avoiding M/A islands distributed in the chain along the rolling direction on the matrix hindered the possibility of voids coalescing into cracks and stabilized the flanging performance.

Comparative analysis of thermodynamic and mechanical responses between underground hydrogen storage and compressed air energy storage in lined rock caverns

Underground hydrogen storage(UHS)and compressed air energy storage(CAES)are two viable largescale energy storage technologies for mitigating the intermittency of wind and solar ***,it is meaningful to compare the properties of hydrogen and air with typical thermodynamic storage *** study employs a multi-physical coupling model to compare the operations of CAES and UHS,integrating gas thermodynamics within caverns,thermal conduction,and mechanical deformation around rock *** thermodynamic responses are validated using additional simulations and the field test *** and pressure variations of air and hydrogen within rock caverns exhibit similarities under both adiabatic and diabatic simulation *** reaches higher temperature and pressure following gas charging stage compared to air,and the ideal gas assumption may lead to overestimation of gas temperature and *** steel lining of CAES,the sealing layer(fibre-reinforced plastic FRP)in UHS is prone to deformation but can effectively mitigates stress in the sealing *** CAES,the first principal stress on the surface of the sealing layer and concrete lining is tensile stress,whereas UHS exhibits compressive stress in the same *** present research can provide references for the selection of energy storage methods.

The European Commission has proposed a Carbon Border Adjustment Mechanism(CBAM)to reduce carbon leakage and create a level playing field for its domestic products and imported ***,the effectiveness of the proposal remains unclear,especially when it triggers threats of retaliation from trading partners of the European *** apply a Computable General Equilibrium model-Global Trade Analysis Project-to assess the economic and environmental impacts of different CBAM *** we show that the effectiveness of the CBAM to address carbon leakage risks is rather limited,and the CBAM raises concerns over global welfare costs,Correct to Gross Domestic Product(GDP)losses,and violation of equality *** retaliation leads to multiplied welfare losses,which would mostly be borne by poor *** results question the carbon leakage reduction effect of a unilateral trade policy and suggest that climate change mitigation still needs to be performed within the framework of international cooperation.

Regulatory T cells(Tregs)suppress immune responses and ***,we described the distinct nonimmunological role of Tregs in fracture *** recruitment from the circulation pool,peripheral induction,and local expansion rapidly enriched Tregs in the injured *** Tregs in the injured bone displayed superiority in direct osteogenesis over Tregs from lymphoid *** depletion of Tregs compromised the fracture healing process,which leads to increased bone *** addition,bone callus Tregs showed unique T-cell receptor *** was the most overexpressed protein in bone callus Tregs,and it can directly facilitate the proliferation and differentiation of osteogenic precursor cells by activation of phosphatidylinositol 3-kinase/protein kinase B signaling *** results of loss-and gain-function studies further evidenced that amphiregulin can reverse the compromised healing caused by Treg *** also enriched in patient bone callus and amphiregulin can promote the osteogenesis of human pre-osteoblastic *** findings indicate the distinct and nonredundant role of Tregs in fracture healing,which will provide a new therapeutic target and strategy in the clinical treatment of fractures.

Establishment of a rat model of severe spontaneous intracerebral hemorrhage

Background:Severe intracerebral hemorrhage(ICH)is the most devastating subtype of stroke resulting in high mortality and *** present,the development of targeted treatments to minimize the high morbidity and mortality is limited partly due to the lack of a severe ICH animal *** this study,we aimed to establish an accurate severe ICH model in rats and examine the pathological and physiological changes associated with ***:A rat model of severe ICH model was established by intrastriatal injection of autologous blood using different blood volumes(ICH 100μL group,ICH 130μL group,ICH 160μL group,ICH 170μL group,and ICH 180μL group).The mortality was assessed during the 28-day post-ICH ***-and long-term neurological deficits were evaluated using the Longa method,foot fault,falling latency,and Morris water maze *** water content,hematoma volume,hemoglobin content,and magnetic resonance imaging were assessed to determine the extent of brain *** staining was conducted to examine microglial activation and neuronal *** and eosin(H&E)staining,lung water content,and western blotting were used to assess lung injury following ***:The mortality of ICH rats increased significantly with an increase in autologous blood *** 28-day mortality in the 100μL,130μL,160μL,170μL,and 180μL ICH groups were 5%,20%,40%,75%,and 100%,respectively.A significantly higher 28-day mortality was observed in the ICH 160μL group compared to the ICH 100μL *** ICH 160μL group exhibited significantly increased neurological deficits,brain edema,hematoma volume,and hemoglobin content compared to the sham *** with the sham operation group,the activation of microglia and neuronal death in ICH 160μL rats *** use of H&E staining and western blotting demonstrated that disruption of the intra-alveolar structure,alveolar edema,and infiltration of inflammatory cells and cytokines into the lung tissu

Transcatheter arterial chemoembolization combined with PD-1 inhibitors and Lenvatinib for hepatocellular carcinoma with portal vein tumor thrombus

BACKGROUND Hepatocellular carcinoma(HCC)patients complicated with portal vein tumor thrombus(PVTT)exhibit poor prognoses and treatment *** To investigate efficacies and safety of the combination of PD-1 inhibitor,transcatheter arterial chemoembolization(TACE)and Lenvatinib in HCC subjects comorbid with *** From January 2019 to December 2020,HCC patients with PVTT types Ⅰ-Ⅳ were retrospectively enrolled at Beijing Ditan *** were distributed to either the PTL or TACE/Lenvatinib(TL)*** median progression-free survival(mPFS)was set as the primary endpoint,while parameters like median overall survival,objective response rate,disease control rate(DCR),and toxicity level served as secondary *** Forty-one eligible patients were finally recruited for this study and divided into the PTL(n=18)and TL(n=23)*** a median follow-up of 21.8 months,the DCRs were 88.9%and 60.9%in the PTL and TL groups(P=0.046),***,mPFS indicated significant improvement(HR=0.25;P<0.001)in PTL-treated patients(5.4 months)compared to TL-treated(2.7 months)*** were no treatment-related deaths or differences in adverse events in either *** A triplet regimen of PTL was safe and well-tolerated as well as exhibited favorable efficacy over the TL regimen for advanced-stage HCC patients with PVTT types Ⅰ-Ⅳ.
