Comparison of the Effectiveness of Marked Suction Tubes vs. Plain Suction Tubes in Pediatric Mechanically Ventilated Patients

Introduction: Endotracheal suction plays a crucial role in the management of mechanically ventilated patients. This study aims to evaluate the clinical effectiveness and safety of suction tubes with markings in mechanically ventilated pediatric patients. Materials and Methods: A randomized assignment was carried out on a cohort of 52 pediatric patients who underwent mechanical ventilation in the Pediatric Intensive Care Unit at the Third Affiliated Hospital of Sun Yat-sen University, covering the period from January 2022 to December 2022. These patients were divided into two groups: an improved group (n = 26) utilizing marked suction tubes, and a regular group (n = 26) employing conventional suction tubes. The objective of our study was to evaluate the clinical effectiveness of marked suction tubes. Results: The effects of the improved group on the vital signs of children undergoing mechanical ventilation were small and statistically significant compared with the regular group (p < 0.05). Additionally, the improved group exhibited a reduced frequency of sputum suction, shorter mechanical ventilation duration, and fewer days of hospitalization in the PICU compared to the regular group during the ventilation period. Notably, the difference in the duration of PICU hospitalization was statistically significant (p < 0.05). Moreover, the incidence of adverse reactions in the improved group was notably lower, with statistically significant differences observed in airway mucous membrane damage and irritating cough when compared to the regular group (p < 0.05). Conclusion: The utilization of marked suction tubes provides clinical nurses with clear guidance for performing suctioning with ease, efficiency and safety. Consequently, advocating for the widespread implementation of marked suction tubes in clinical practice is a commendable pursuit.

苹果种植业为陕西省提升农业、富裕农民的支柱型产业。黄土高原苹果产区每年都有花期防冻害和产量评估的需求,花期监测尤为重要,而常规的花期监测方法存在监测成本大和精度低的问题。采用一种基于遥感作物参考曲线重建的方法,介绍比较了该方法与其他方法应用于常见作物的精度,并使用地理国情普查地表覆盖分类数据识别的MODIS纯像元中提取一组NDVI时间序列,将其用于拟合类似Sentinel-2的观测值,将这种方法应用于黄土高原地区苹果作物上,根据苹果样点的归一化植被指数(NDVI)序列(作物参考曲线)与重建后的苹果NDVI序列在空间上的物候差异,绘制历史10 m高分辨率的始花期监测结果,实现了花期监测由点到面的转化。研究结果表明:2019~2021三年验证区域苹果始花期的探测结果平均值与地面数据绝对误差均在1~3 d之内,平均绝对误差为0.926 d,均方根误差为1.503 d。该方法可用于苹果花期高精度监测,预测结果可为实际生产中苹果花期防冻害和产量评估工作的开展提供参考,同时这种方法也可以用于研究其他作物的物候特征,如小麦、玉米等,有望为农业生产提供更准确的决策支持。

A Review of Location Methods of Nasogastric Tube in Critically Ill Patients

Nasogastric tube is widely used in intensive care units. The complications of misplacement are rare but very dangerous for critically ill patients. Accurate localization of the position of the tip of nasogastric tube can effectively decrease complications and ensure the safety of critically ill patients. There are various methods that can be used to verify the location of the nasogastric tube such as radiography, PH measurement, electromagnetic navigator and ultrasound. However, there is a lack of general consensus regarding a standard method. In this review, we found that the accuracy of nasogastric tube placement can be greatly improved by visual technology such as X-ray, sonography and electromagnetic navigator. However, visual technology has not been widely used to locate the tip of nasogastric tube in critically ill patients. Best practice guidelines based on the available knowledge and evidence of current methods are necessary to increase the accuracy placement of nasogastric tube. It is envisioned that development of visual technologies will determine a new standard of care for verification of placement of nasogastric tube.

为探究INHBA基因在藏猪与大约克猪中的多态性与表达差异。在DNA水平上对藏猪、大约克猪INHBA基因5'侧翼区和CDS区进行了单核苷酸多态性(Single nucleotide polymorphism,SNP)位点筛选,同时在mRNA水平上用荧光定量PCR(qRT-PCR)方法检测INHBA基因在卵巢组织中的相对表达量。结果发现:INHBA基因在5'侧翼区存在一个突变位点A-1589T,并在藏猪与大约克猪2个品种间存在极显著差异(P<0.01);经转录因子预测,发现该突变位点与繁殖性能相关,并推测消失的转录因子结合位点可能是导致藏猪繁殖力低的主要原因之一。INHBA基因在卵巢中,藏猪表达量极显著高于大约克猪(P<0.01),推测INHBA基因可能与猪繁殖性能有关,并对提高猪的繁殖性能起到积极作用。本研究结果为进一步探究INHBA基因在猪繁殖性能的作用机制提供理论支撑。

本研究以约克夏猪为对照,旨在分析藏猪和约克夏猪的血液生理指标和免疫相关器官组织表达谱方面的差异,结合藏猪外周血淋巴细胞在LPS刺激下,促炎因子IL-1β、IL-6和TNF-α的表达情况,探究IL-1β、IL-6及TNF-α对藏猪免疫性状的影响。试验选择180日龄的藏猪和约克夏猪各40头按品种分为两个试验组,采集新鲜血液用于检测血液生理指标;采集猪外周淋巴血用于培养外周血淋巴细胞。随机选取藏猪和约克夏猪各8头按品种分为两个试验组,前腔静脉采集新鲜血液、屠宰后采集黄豆样大小的肝脏、脾脏、下颌淋巴结组织Trizol法提取RNA。每个个体样品设置3个重复,借助RT-qPCR和一代测序技术检测IL-1β、IL-6、TNF-α基因表达情况和基因多态性。设置4组浓度的LPS(0、1、10、100μg·mL^(-1)),每组设置3个重复去刺激外周血淋巴细胞,于0、24、36、48、72 h收集细胞,用RT-qPCR检测促炎因子mRNA的表达情况。结果表明:1)藏猪的WBC、RBC、HGB、HCT和PLT水平极显著高于约克夏猪(P0.05)。4)不同浓度LPS刺激藏猪外周血淋巴细胞试验结果表明,10μg·mL^(-1)浓度的LPS对藏猪外周血淋巴细胞的增殖效应最为显著。在1、10μg·mL^(-1)浓度LPS刺激下,IL-1β基因作出较明显应答,在100μg·mL^(-1)浓度LPS刺激下,IL-6基因作出较强应答。综上所述,和约克夏猪相比,藏猪在血液生理指标、免疫组织器官中促炎因子表达以及基因多态性方面表现出更强的抗病能力。其中IL-6基因的A-72G位点可能与免疫性和抗病能力密切相关。在藏猪外周血淋巴细胞培养试验中,IL-1β和IL-6基因对LPS的显著应答突显了它们在免疫调控中的关键作用。

基于《医院医用织物洗涤消毒技术规范》的内容设计问卷,对15个省、直辖市内不同级别医疗机构的医用织物洗涤消毒管理情况进行调查。结果显示,医疗机构中不同管理部门组织工作人员培训和考核的频率普遍不高。后勤部门在织物运送和储存过程的规范执行率最高,院感科更重视对工作人员的培训工作,而护理部则在医用织物收集过程中的规范执行率最高。建议大型医院应促成多个部门协作下的医用织物管理新模式。

A FAS approach for stabilization of generalized chained forms: part 1. Discontinuous control laws

In this paper, a type of general nonholonomic systems is proposed, which contains both the Brockett’s two example systems, and their extended n-dimensional chained forms, as special cases. For the stabilization of such systems, a stabilizing controller is proposed based on the fully actuated system(FAS)approach, which is discontinuous at the origin but time-invariant when the open-loop system is time-invariant,and drives the feasible trajectories of the system to the origin exponentially. Furthermore, the proposed FAS approach is also extended to the sub-normal system case and the time-delay system case.

Stabilization Controller of An Extended Chained Nonholonomic System With Disturbance:An FAS Approach

This study examines the stabilization issue of extended chained nonholonomic systems(ECNSs)with external disturbance.Unlike the existing approaches,we transform the considered system into a fully actuated system(FAS)model,simplifying the stabilizing controller design.We implement a separate controller design and propose exponential stabilization controller and finite-time stabilization controller under finite-time disturbance observer(FTDO)for the two system inputs.In addition,we discuss the specifics of global stabilization control design.Our approach demonstrates that two system states exponentially or asymptotically converge to zero under the provided switching stabilization control strategy,while all other system states converge to zero within a finite time.

A comprehensive research on wear resistance of GH4169 superalloy in longitudinal-torsional ultrasonic vibration side milling with tool wear and surface quality

To improve the service and machining performance of the workpiece, the tool wear mechanisms, surface machining quality, and wear resistance in conventional side milling(CSM)and longitudinal-torsional ultrasonic vibration side milling(LTUVSM) of GH4169 superalloy at different cutting lengths are investigated systematically. Tool wear mechanisms are revealed and the correlation between machined surface quality with tool wear is analyzed correspondingly. Tool wear patterns mainly include adhesive wear, diffusion wear, abrasive wear, and chipping sticking.Better surface quality is achieved in LTUVSM due to a maximum reduction of flank wear bandwidth and wear rate by 71.9% and 71.5%, respectively, compared to CSM. The friction coefficient,initial wear stage time, and wear volume of dry sliding wear were measured to evaluate the workpiece wear resistance. The maximum reductions in friction coefficient and wear volume in LTUVSM are 18.2% and 15.8% compared to CSM. The regular ultrasonic vibration textures suppress the friction and the growth of contact nodes in the contact area, decreasing the degree of surface wear, which is demonstrated by a 38.8% increase in initial wear stage time compared to CSM.In conclusion, the workpiece in LTUVSM exhibits higher wear resistance because of the improvement of tool wear and the guarantee of surface quality.
