Apoptosis and autophagy are distinct cellular processes that can be highly interconnected. The cross talk between the two processes is indispensable in determining the overall cell fate. Although the apoptosis-promoting effect of caspases has been demonstrated, the roles of autophagy-related proteins and even autophagy itself in regulating apoptosis remain poorly understood. In our present study, we found that downregulation of ubiquitin E3 ligase ASB3 led to enhanced mitochondrial apoptosis as well as autophagy, which synergistically promoted cell death in hepatocellular carcinoma(HCC). We observed the activation of caspase-8 and decrease of autophagy protein Beclin1 in apoptotic cells that were depleted of ASB3. Beclin1 was mainly cleaved by activated caspase-8 and active Beclin1 initiated mitochondrial apoptosis via locating its C-terminal fragment to mitochondria. In addition, knocking down of Beclin1 markedly blocked the apoptosis, indicating its essential role in the ***, our study indicated that enhanced autophagy level might be involved in the activation of caspase-8 and promote the apoptosis. Taken together, our results demonstrated that ASB3 can regulate mitochondrial pathway of apoptosis by controlling caspase-8 mediated cleavage of Beclin1 in HCC. Therefore, ASB3 may potentially serve as a novel target for HCC therapy,especially when combined with autophagy agonist.

OBJECTIVE: To assess the techniques for surgical excision of giant primary carcinoma in the medialliver ***: Operative managements, complications and their causes during and after resection of giantcarcinoma in the medial liver lobe were analyzed retrospectively in 166 cases treated from October 1996through December ***: Of the 166 patients, 123 (74.1%) underwent tumor resection and 43 (25.9%) regularlobectomy, including left trilobectomy (8,4.8%), medial lobectomy (21,12.7%), right anteriorlobectomy (11, 6.6%), and hemihepatectomy (3,1.8%). All patients were subjected to surgery withintermittent interruption of the first porta hepatis under normothermia. The total interruption time was 7-68 minutes and average time was 24.5 minutes. The maximum single interruption time was 41 *** blood loss was 50-4000 ml, averaging 726 ml. The maximum blood transfusion was 5200ml, averaging was 811 ml, and transfusion was not needed in 54 patients. Postoperative complicationsoccurred in 9 patients (5.4%), of whom, 2 (1.2%) died of liver failure and acute respiratory distresssyndrome ***: An adequate reserve of liver function is a prerequisite for a smooth recovery afteroperation. Careful intraoperative management is crucial to decrease postoperative complications.

Little is known about how chronic inflammation contributes to the progression of hepatoceUular carcinoma （HCC）, especially the initiation of cancer. To uncover the critical transition from chronic inflammation to HCC and the molecular mechanisms at a network level, we analyzed the time-series proteomic data of woodchuck hepatitis virus/*** mice and age-matched wt-C57BL/6 mice using our dynamical network biomarker （DNB） model. DNB analysis indicated that the 5th month after birth of transgenic mice was the critical period of cancer initiation, just before the critical transition, which is consistent with clinical symptoms. Meanwhile, the DNB-associated network showed a drastic inversion of protein expression and coexpression levels before and after the critical transition. Two members of DNB, PLA2G6 and CYP2C44, along with their associated differentially expressed proteins, were found to induce dysfunction of arachidonic acid metabolism, further activate inflammatory responses through inflammatory mediator regulation of transient receptor potential channels, and finally lead to impairments of liver detoxification and malignant transition to cancer. As a c-Myc target, PLA2G6 positively correlated with c-Myc in expression, showing a trend from decreasing to increasing during carcinogenesis, with the minimal point at the critical transition or tipping point. Such trend of homologous PLA2G6 and c-Myc was also observed during human hepatocarcinogenesis, with the minimal point at high-grade dysplastic nodules （a stage just before the carcinogenesis）. Our study implies that PLA2G6 might function as an oncogene like famous c-Myc during hepatocar- cinogenesis, while downregulation of PLA2G6 and c-Myc could be a warning signal indicating imminent carcinogenesis.

The incidence of pancreatic cancer has been rising worldwide,and its clinical diagnosis and treatment remain a great *** present the update and improvements in the clinical diagnosis and treatment of pancreatic cancer in recent years,Chinese Pancreatic Association,the Chinese Society of surgery,Chinese medical Association revised the Guidelines for the Diagnosis and Treatment of Pancreatic Cancer in china(2014)after reviewing evidence-based and problem-oriented literature published during 2015-2021,mainly focusing on highlight issues regarding diagnosis and surgical treatment of pancreatic cancer,conversion strategies for locally advanced pancreatic cancer,treatment of pancreatic cancer with oligo metastasis,adjuvant and neoadjuvant therapy,standardized processing of surgical specimens and evaluation of surgical margin status,systemic treatment for unresectable pancreatic cancer,genetic testing,as well as postoperative follow up of patients with pancreatic *** recommendation items were finally proposed based on the above issues,and the quality of evidence and strength of recommendations were graded using the Grades of Recommendation,Assessment,Development,and Evaluation *** guideline aims to standardize the clinical diagnosis and therapy,especially surgical treatment of pancreatic cancer in china,and further improve the prognosis of patients with pancreatic cancer.

Roles of Chemokine Receptor 4 (CXCR4) and Chemokine Ligand 12 (CXCL12) in Metastasis of Hepatocellular Carcinoma Cells

Chemokines are involved in human hepatocellular carcinoma （HCC） carcinogenesis. However, the exact mechanism of chemokines in HCC carcinogenesis remains unknown. Here we investigated the roles of chemokine receptor 4 （CXCR4） and chemokine ligand 12 （CXCL12） in the metastasis of HCC. We found that the expression levels of CXCR4 mRNA in HCC tissues, MHCC97 cells, and HUVEC cells were 2.52 ±1.13, 2.34 ±1.16 and 1.63 ±1.26, respectively and that the CXCR4 protein levels were 1.38 ± 0.13, 1.96± 0.32 and 1.86 ±0.21, respectively. In contrast, CXCR4 was not detected in normal hepatic tissues. In 78 HCC patients, we also found that the concentration of CXCL12 in cancerous ascitic fluid was 783-8,364 pg/ml and that CXCL12 mRNA level in HCC metastasis portal lymph nodes was 1.21 ± 0.87 but undetectable in normal hepatic tissues. Finally we discovered that recombinant human CXCL12 could induce MHCC97 cells and HUVEC cells to migrate with chemotactic indexes （CI） of 3.9 ±1.1 and 4.1± 1.6, respectively. Cancerous ascitic fluid could also induce the migration of MHCC97 cells with a CI of 1.9 ± 0.8. Thus, our data suggest that CXCR4 and CXCL12 may play an important role in the metastasis of HCC by promoting the migration of tumor cells.

Background: Percutaneous radiofrequency ablation(RFA) is a first?line treatment for very?early?stage hepatocellular carcinoma(HCC), whereas the efficacy of percutaneous microwave ablation(MWA) for very?early?stage HCC remains unclear. The purpose of this study was to clarify this issue by comparing the safety and efficacy of percutaneous MWA with percutaneous RFA in treating very?early?stage ***: Clinical data of 460 patients who were diagnosed with very?early?stage HCC and treated with percutane?ous MWA or RFA between January 2007 and July 2012 at the eastern hepatobiliary surgery hospital, The second Mili?tary medical university, in shanghai, china were retrospectively analyzed. Of these 460 patients, 159 received RFA, 301 received MWA. Overall survival(OS), recurrence?free survival(RFS), local tumor progression(LTP), complete ablation, and complication occurrence rates were compared between the two groups, and the prognostic factors associated with survival were ***: No significant differences were observed between the two groups in terms of the 1?, 3?, or 5?year OS rates(99.3%, 90.4%, and 78.3% for MWA vs. 98.7%, 86.8%, and 73.3% for RFA, respectively; P = 0.331). Furthermore, no signif?icant differences were observed between the two groups in terms of the corresponding RFS rates(94.4%, 71.8%, and 46.9% for MWA vs. 89.9%, 67.3%, and 54.9% for RFA, respectively; P ete ablation rates(98.3% vs. 98.1%, P = 0.309), the LTP rates(9.6% vs. 10.1%, P = 0.883), the compl multivariate analysis, LTP, an= 0.860), or the occurrence rates of major complications(0.7% vs. 0.6%, P = 0.691). Bytiviral therapy, and treatment of recurrence were independent risk fac?tors for OS(P < 0.001), and the alpha?fetoprotein level was an independent prognostic factor for RFS(P = 0.002).Conclusions: MWA is as safe and effective as RFA in treating very?early?stage HCC, supporting MWA as a first?line treatment option for this disease.

背景：血管重建和端口吻合是临床肝移植的难点，将袖片技术应用于肝移植动脉重建，对扩大吻合口径、减少动脉血栓形成、促进移植物及受体长期存活具有重要意义。目的：分析袖片技术在全肝移植前供体间动脉重建和植入时供受体动脉重建的吻合部位，探讨动脉袖片技术的应用条件。设计、时间及地点：单中心抽样，回顾性病例分析，于2004—03／2006-07在解放军第二军医大学东方肝胆外科医院完成。对象：选取解放军第二军医大学东方肝胆外科医院单个医疗组同期收治的接受肝移植患者80例，供体均为脑死亡男性志愿捐献者，供受体ABO血型相合。方法：后台完成供体间动脉重建，尽量保留、重建全部供肝变异、受损伤动脉。植入时完成供受体重建，主要选用粗大且直径匹配的供受体动脉。选择动脉端或应用袖片技术进行动脉吻合，尽量通过单个吻合完成重建。通过修剪供体或受体动脉，获取吻合端的动脉分支或根部袖片。主要观察指标；统计植入前供肝动脉重建的远、近心端及植入时供受体重建的供、受体侧4个位置的吻合部位分布、袖片应用率、吻合口径。结果：植入前后台供肝动脉吻合11例（吻合口12个），其中10例来自肠系膜上动脉的迷走右肝动脉进行了后台重建；在吻合口远心端，迷走右肝动脉根部袖片吻合口直径明显大于其端口（t=5．423，P〈0．01）；在吻合口近心端，吻合部位以脾动脉端最常用（7／12），吻合口直径为（4．4±1．8）mm。植入时供受体肝动脉吻合80例，供体侧以肝总动脉分支袖片、腹腔动脉端和肝固有动脉分支袖片最常用，构成比分别为57．50％，16，25％，8．75％；吻合口径基本相似（P〉0.05）；受体侧以肝总动脉分支袖片和肝固有动脉分支袖片最多，构成比分别为60.00％，27.50％，前者吻合口径明显大于后者（P〈0.01）。植入前吻合的袖片应用率明显低于植入时吻合（χ^2=30．799，P〈0．01），植入前供肝动脉重建的远、近心端及植入时动脉重建的供、受体侧其袖片应用率分别为41．7％，0.70．0％和92．5％，仅植入前供体远心端与植入时供体侧组比较基本相似（P〉0．05），余部位两两比较差异有显著性意义（χ^2=13．292～51．004，P〈0．01）。结论：在供肝动脉植入前进行后台重建时，采用袖片技术的迷走右肝动脉可获得比动脉端更大的吻合口径。植入时供受体动脉重建的袖片应用率高于植入前供体动脉重建，且植入时动脉重建的受体侧均采用分支袖片的肝总动脉能够获得比肝固有动脉更大的吻合口径。
