Efficacy of Combined Phacoemulsification and Goniosynechialysis in the Treatment of Patients with Primary Angle Closure Disease and Concomitant Cataract at Preah Ang Duong Hospital in Cambodia

Background: Goniosynechialysis is a surgical procedure that has been shown to slow the progression of glaucoma in oriental eyes with chronic angle closure glaucoma. This procedure was successful in 80% of cases, and the peripheral anterior syenchiae did not exist until one year later. Nonetheless, there is little evidence of its efficacy in our context. Our study aims to investigate the efficacy of goniosynechialysis during phacoemulsification in patients with primary angle closure disease and concomitant cataract. Methods: This was an observational, prospective study. The intra-ocular pressure, need for anti-glaucoma drugs, visual acuity, the extent of synechiae, anterior chamber depth, surgical success rate, and other indicators were monitored for at least three months following surgery. Results: This study included 114 patients (118 eyes), 61 with chronic angle closure glaucoma (51.69%), 33 with primary angle closure (27.97%), and 24 with acute attack angle closure (20.34%), who were surgically treated with phacoemulsification and goniosynechialysis (Phaco-GSL). The mean intra-ocular pressure had significantly decreased three months after surgery (pre- vs post-op: 22.04 ± 10.86 vs 15.41 ± 6.06 mmHg, p-value p-value p-value p-value p-value Conclusion: Regardless of the type of glaucoma, combined phacoemulsification-goniosynechialysis is effective in lowering pressure, restoring vision, reducing the need for anti-glaucoma drugs, and preventing the synechial recurrence. Success was higher in eyes with less extensive synechiae. Phaco-GSL is safe and effective in the treatment of primary angle closure diseases with co-existing cataract.

Objective: To identify the patterns of tuberculosis (TB) notification rates in Phnom Penh and examine their relationships with the population density, socioeconomic, residential and occupational characteristics. Methods: The numbers of total TB and smear-positive pulmonary TB cases reported between January 1, 2010 and December 31, 2012 in Phnom Penh were counted for 76 communes in Cambodia according to TB registration records filed under the national TB programme. Population, socioeconomic, residential and occupational characteristics for the communes were obtained from the 2008 General Population Census of Cambodia. The following indicators were developed for individual communes: smear-positive pulmonary TB notification rate (SPTB-NR) (per 100,000 population, in 36 months), population density (per km2), socioeconomic indicators, residential characteristics and occupational characteristics. Geographic patterns of these indicators and characteristics were analysed using ArcGIS. Associations between SPTB-NR and characteristics were analysed. Results: A total of 4102 TB cases were reported in 36 months, including 2046 SPTB cases. SPTB-NR for Phnom Penh was 135 cases per 100,000;median SPTB-NR by commune was 100. SPTB-NR was higher in outlying areas than in city centre communes;population density was high in the centre and low in the outlying areas. SPTB-NR was associated with larger percentage of household members per room (PR: 2.81, 95%CI: 2.68 - 2.93), percentage of population resident in the same commune Conclusions: The SPTB-NR in Phnom Penh did not follow the pattern of population density. Socioeconomic, residential and occupational characteristics by commune were associated with SPTB-NR. Development of prevention and control programmes by considering commune level characteristics is encouraged.

Background: The aim of this present prospective study was to investigate the accuracy of cardiac markers for the prediction of subsequent cardiac events(cardiac death, acute myocardial infarction and recurrent ischemia requiring coronary revascularization). Methods: Fibrinogen, cardiac troponin T, troponin I, creatine phosphokinase myocardial fraction, C-reactive protein and myoglobin at baseline and after 6 h were measured on 154 patients(109 male, 63±11 years) with chest pain. Receiver operator characteristic analyses were performed to determine cut-off points of cardiac markers in prediction of adverse events. Results: The following cut-off values for prediction of cardiac events were calculated: troponin I at baseline 0.3 ng/ml(predictive accuracy=0.870), troponin I at 6 h 0.50 ng/ml(p.a.=0.909); troponin T at baseline 0.05 ng/ml(p.a.=0.643), troponin T at 6 h 0.05 ng/ml(p.a.=0.612), creatine phosphokinase myocardial fraction at baseline 2.0 ng/ml(p.a.=0.721), creatine phosphokinase myocardial fraction at 6 h 2.5 ng/ml(p.a.=0.734), myoglobin at baseline 23 ng/ml(p.a.=0.623), myoglobin at 6 h 26 ng/ml(p.a.=0.617), C-reactive protein at baseline 0.31 mg/dl(p.a.=0.662), C-reactive protein at 6 h 0.55mg/dl(p.a.=0.682),and fibrinogen at baseline 360 mg/dl(p.a.=0.701). The combination of baseline troponin I with different parameters resulted in a higher sensitivity of up to 98%, with a similar predictive accuracy, but a lower specificity. Additive measurements of cardiac troponin I at 6 h to baseline cardiac troponin T and I proved to be the best combination for prediction of subsequnt cardiac events. Conclusions: Changes in cut-off levels of cardiac markers and inflammatory parameters results in a high accuracy of risk stratification in patients with chest pains. Combination of these measurements might further help in the identification of patients who would benefit from early coronary revascularization.

目的探讨微小RNA(microRNA-106a,miR-106a)通过调节基质金属蛋白酶抑制物2(tissue inhibitor of metalloproteinases 2,TIMP2)的表达而诱导人胃癌BGC-823细胞的腹腔种植转移。方法人胃癌细胞株BGC-823传代培养至对数生长期。细胞分3组:胃癌细胞BGC-823组、BGC-823/anti-miR-106a(拮抗剂)组和BGC-823/negative control组。Real-time PCR鉴定拮抗效果。Transwell法检测细胞体外迁移侵袭能力。以小切口注射方式制备裸鼠腹腔内移植瘤模型,动物分2组:miR-antagomir组和miR-NC组。大体观察裸鼠胃癌移植瘤生长情况。免疫组化法和Western印迹法联合检测TIMP2在腹腔内各器官上的表达。结果 BGC-823/anti-miR-106a组miR-106a表达下调,降低倍数2-△△Ct=0.05±0.01,与NC组相比具有统计学差异(t=-18.001,P<0.001)。细胞水平外源性沉默miR-106a基因,BGC-823/anti-miR-106a组迁移、侵袭的细胞数量均低于BGC-823和BGC-823/negative control组,差异均有统计学意义(P<0.001)。移植瘤体内实验显示miR-106a表达下调削弱BGC-823细胞在裸鼠腹腔内的种植转移能力,表现为种植瘤结节数量和体积的减少;当miR-106a表达受抑时,免疫组化法和印迹法均显示miR-antagomir组TIMP2蛋白表达量高于miR-NC组,差异有统计学意义(P<0.05)。结论 BGC-823细胞株具备裸鼠成瘤能力,沉默miR-106a抑制胃癌细胞的侵袭转移提示其具有癌基因样作用;miR-106a可能通过作用于TIMP2诱导BGC-823细胞腹腔种植转移能力增强。
