Evaluation of the Management of Family Physicians Health Care Services to Diabetic Patients in Rural Areas of Northern Iran

Background and Objectives: The ultimate goal of the health system is to improve the health status of people. To achieve this goal and meet the needs and expectations of people in the field of health, different strategies have been designed, one of which is the family physician. Therefore, the main aim of this study was to evaluate management and performance of the family physician in the diagnosis, treatment and prevention of diabetes in the Counties of Gilan Province. Materials and Methods: This is a cross-sectional study, which was carried out descriptively. In this study 320 patients with diabetic records were selected from among 1900 using simple random selection. Results: There is a significant difference between the number of visits, the number of referrals to specialists and the number of tests with the expected rate. In addition, 68.8% of patients were diagnosed by the family physician for the first time. According to comparisons made on the number of referrals, visits and request for blood tests, there was a significant difference between two Counties (p Conclusion: The results of the present study indicate that there is a fundamental weakness in terms of diagnosis, control, care of patients, referral, completion of health records and eventually preventing diabetes complications in the northern villages of Iran. In general, family physician program has relatively good performance in Iran after several years, but has not been able to reach the determined goals and standards.

Anoplophora glabripennis(Asian longhorn beetle,ALB)and Anoplophora chinensis(Citrus longhorn beetle,CLB)are native forest pests in China;they have become important international quarantine *** are found using the same Salix aureopendula host tree of Cixi,Zhejiang province,*** this host tree,we collected additional beetles that appeared to be morphologically intermediate between ALB and *** using a stereoscope,we observed that there were several bumps on the base of the elytra,which was inconsistent with ALB,which typically has a smooth elytral base,but was more like CLB,which has numerous short tubercles on the elytral *** their sympatry and intermediate morphology,we hypothesized that these may represent ALB×CLB *** studied the genomic profiles for 46 samples(ALB,CLB,and putative hybrids)using genotyping-by-sequencing(GBS)providing a reduced representation of the entire *** principal component analyses on the 163 GBS-derived single nucleotide polymorphism data,we found putative hybrids tightly clustered with ALB,but genetically distinct from the CLB ***,our initial hybrid hypothesis was not supported by genomic ***,while mating experiments between adult ALB and CLB were successful in 4 separate years(2017,2018,2020,and 2021),and oviposition behavior was observed,no progeny was *** employed population genomic analysis and biological hybridization experiments,we conclude that the putative hybrids represent newly discovered morphological variants within *** approach further confirmed the advantage of genome-wide information for Anoplophora species assignment in certain ambiguous classification cases.

Apple replant disease(ARD),caused by a pathogen complex,significantly impacts apple orchard *** molecular regulation on ARD resistance has not been investigated until recently.A systematic phenotyping effort and a series of transcriptomic analyses were performed to uncover the underpinned molecular mechanism of apple root resistance to ***,a representative member in ARD pathogen ***-specific plant survival rates and biomass reduction corresponded with microscopic features of necrosis progression patterns along the infected *** presence of defined boundaries separating healthy and necrotic sections likely caused delayed necrosis expansion in roots of resistant genotypes compared with swift necrosis progression and profuse hyphae growth along infected roots of susceptible *** datasets from a series of transcriptome analyses generated the first panoramic view of genome-wide transcriptional networks of defense activation between resistant and susceptible apple *** and stronger molecular defense activation,such as pathogen perception and hormone signaling,may differentiate resistance from susceptibility in apple *** and interrupted activation of multiple defense pathways could have led to an inadequate resistance *** the panel of apple rootstock germplasm with defined resistant and susceptible phenotypes,selected candidate genes are being investigated by transgenic manipulation including CRISPR/Cas9 tools for their specific roles during apple root defense toward *** *** apple genes with validated functions regulating root resistance responses can be exploited for developing molecular tools for accurate and efficient incorporation of resistance traits into new apple rootstocks.

The evolution of strain hardening behavior of the Fe_(50)(CoCrMnNi)_(50) medium-entropy alloy as a function of the fraction of recrystallized microstructure and the grain size was studied using the Hollomon and Ludwigson *** specimens under study were partially recrystallized,fully recrystallized with ultrafine-grained microstructure,and fully recrystallized with coarse *** yield strength decreases steadily as the fraction of recry stallized micro structure and grain size increases due to the recovery process and the Hall-Petch ***,the bimodal grain distribution was found to have a significant impact on strain hardening during plastic *** instance,the highest ultimate tensile strength was exhibited by a 0.97 μm specimen,which was observed to contain a bimodal grain ***,using the Ludwigson equation,the effect of the bimodal grain distribution was established from the behavior of K_(2) and n_(1) *** curves tend to show very high values in the specimens with a bimodal grain distribution compared to those that show a homogenous grain ***,the bimodal grain distribution contributes to the extensive Luders strain observed in the 0.97 μm specimen,which induces a significant deviation of the Hollomon equation at lower strains.

A transformation-induced plasticity phenomenon in Fe65(CoCrMnNi)35 medium-entropy alloy was *** to the X-ray diffraction patterns,the as-cast specimen contains a single-phase face-centered cubic(fcc),while low-temperature annealing at 500℃and 600℃leads to the introduction of a body-centered cubic(bcc)phase as a secondary *** increment of the annealing temperature to above 700℃eliminates the bcc phase,and the microstructure was found to contain a single-phase *** 20%true strain,an fcc-to-bcc phase transformation is observed;whereas,at 28%true strain,an fccto-hcp phase transformation takes place as an additional deformation *** strain-induced phase transformation phenomenon leads to improved tensile properties of this alloy.

Metallic nanoglasses are made of amorphous grains that are separated by lower-density amorphous boundaries,which have been proposed to enhance plasticity through the deflection of cracks and shear bands at *** has been difficult to experimentally control grain size and interfacial structure to understand their roles in plastic ***,we fabricate bulk nanoglasses via compaction and sintering of colloidally synthesized amorphous iron-boride *** nanoglasses have amor-phous grains with diameters from 116 nm to 576 nm and were tested using nanoindentation and mi-cropillar *** nanoglass with a grain size of 576 nm shows the highest elastic modulus and hardness of 101 GPa and 7.4 GPa,*** electron microscopy reveals that nanocrys-tals form within the nanoglasses during *** nanocrystal density correlates with higher nanoparticle crystallization enthalpy,an increase in plasticity,and a decrease in yield *** strain of 5.0%,yield strength of 3.8 GPa,and ultimate compressive strength of 2.7-3.8 GPa were *** show that the compaction of colloidal metallic glass nanoparticles results in robust bulk samples,with mechanical properties similar to that of other iron-based bulk metallic glasses.
