Carcinosarcoma of the breast:Facing the challenge of a rare nosologic entity

Carcinosarcoma(CS),also known as metaplastic breast carcinoma with mesenchymal differentiation,is one of the five distinct subtypes of metaplastic breast *** is considered as a mixed,biphasic neoplasm consisting of a carcinomatous component combined with a malignant nonepithelial element of mesenchymal origin without an intermediate transition *** cellular origin of this neoplasm remains controversial,most researchers declare that neoplastic cells derive from a cellular structure with potential biphasic *** recent research on the therapeutic strategies against CS neoplastic disorders,surgical resection appears the only potentially curative *** CS metastasize by the lymphatic route,axillary assessment with sentinel lymph node biopsy and/or axillary lymph node dissection is always ***,the tumor also presents a hematogenous metastatic pattern including pleural,pulmonary,liver,brain and less commonly bone ***,surgical removal of breast CS does not necessarily ensure patient’s long-term ***,alternative therapies,such as radio-and chemotherapy proved insufficient and 5-year survival rate is ***,there is evidence that following surgery,the combination of radio and chemotherapy is associated with a better prognosis than either treatment *** aim of this review is to evaluate the results of surgical treatment for breast CS with special reference to the extent of its histological *** features,histogenesis,morphological and immunochemical findings are discussed,while the role of current diagnostic and therapeutic management of this aggressive neoplasm is emphasized.

Endoscopic intramural cystogastrostomy for treatment of peripancreatic fluid collection: A viewpoint from a surgeon

Percutaneous or endoscopic drainage is the initial choice for the treatment of peripancreatic fluid collection in symptomatic *** transgastric fenestration(ETGF)was first reported for the management of pancreatic pseu-docysts of 20 patients in *** a surgeon’s viewpoint,ETGF is a similar procedure to cystogastrostomy in that they both produce a wide outlet orifice for the drainage of fluid and necrotic *** can be performed at least 4 wk after the initial onset of acute pancreatitis and it has a high priority over the surgical ***,the surgical approach usually has a better success rate because surgical cystogastrostomy has a wider outlet(>6 cm vs 2 cm)than ***,percutaneous or endoscopic drainage,ETGF,and surgical approach offer various treatment options for peripancreatic fluid collection patients based on their conditions.

Effect of 5-Aza-2’-deoxycytidine on immune-associated proteins in exosomes from hepatoma

AIM: To study the effect of 5-Aza-2’-deoxycytidine (5-Aza-CdR) on heat shock protein 70 (HSP70), human leucocyte antigen-Ⅰ (HLA-Ⅰ) and NY-ESO-1 proteins in exosomes produced by hepatoma cells, HepG2 and Hep3B. METHODS: Exosomes derived from HepG2 and Hep3B cells treated with or without 5-aza-CdR were isolated and purified by ultrafiltration centrifugation and sucrose gradient ultracentrifugation. The number of exosomes was counted under electron microscope. Concentration of proteins in exosomes was measured by bicinchoninic acid protein assay. Expression of HSP70, HLA-Ⅰ and NY-ESO-1 proteins in exosomes was detected by Western blotting and immunoelectron microscopy. mRNA expression of p53 gene was detected by reverse transcription polymerase chain ***: The mRNA expression of p53 gene was increased in both hepatoma cell lines after treatment with 5-Aza-CdR. The number of exosomes and the concentration of total proteins in exosomes were increased signifi cantly after treatment with 5-aza-CdR (P < 0.05). After treatment with 5-Aza-CdR, immunoelectron microscopy and Western blotting showed that the HSP70, HLA-Ⅰ and NY-ESO-1 proteins were increased in exosomes produced by both hepatoma cell lines. CONCLUSION: 5-aza-CdR, an inhibitor of DNA methyltransferase, can increase exosomes produced by hepatoma cells and immune-associated protein component of exosomes, which may be mediated by p53 gene upregulation and 5-Aza-CdR demethylation.

Effect of Multiple Martensitic Transformations on Structure of Fe-Ni Alloys

Effect of multiple direct and reverse martensitic transformations on fragmentation of austenitic grains in Fe-Ni alloys have been studied by X-ray diffraction and scanning electron microscopy. An ultra-fine structure was formed by fragmentation inside austenitic grains due to progressing misorientation of austenitic sub-grains during multiple γ-α-γ-martensitic phase transitions. An increase in the number of γ-α-γ-transformations increases misorientation angle between austenitic sub-grains and leads to transformation of an austenitic single crystal into a textured polycrystal. It has been shown that multiple γ-α-γ-martensitic phase transitions change the mechanism of internal stress relaxation from dislocation-based to deformation twinning.

Psychotropic Medication Refusal:Reasons and Patients’Perception at a Secure Forensic Psychiatric Treatment Centre

Poor adherence to prescribed medication regimens can undermine the effectiveness of *** study was conducted to determine the demographic profile of forensic psychiatric inpatients refusing medications and to identify the reasons for *** were collected through interviews using a questionnaire including Drug Attitude Inventory‑*** refusal was more common among Aboriginals(68%,n=34)than Caucasians(32%,n=16)and was highest among the patients 21–30 years of age(44%,n=22).Antisocial personality disorder and substance use disorder featured prominently among patients refusing *** main reasons for medication refusal were inconvenience(34%,n=17)followed by side effects(22%,n=11),ineffective medication(20%,n=10),illness‑related(16%,n=8),and no reasons(8%,n=6).Antipsychotic medications topped the list of the major classes of medications refused followed by Antidepressants and Mood Stabilizers.

A building-based data capture and data mining technique for air quality assessment

Recently,a building-based air quality model system which can predict air quality in front of individual buildings along both sides of a road has been *** the Macao Peninsula as a case study,this paper shows the advantages of building-based model system in data capture and data *** with the traditional grid-based model systems with input/output spatial resolutions of 1-2 km,the building-based approach can extract the street configuration and traffic data building by building and therefore,can capture the complex spatial variation of traffic emission,urban geometry,and air *** non-homogeneous distribution of air pollution in the Macao Peninsula was modeled in a highspatial resolution of 319 receptors·km^(-2).The spatial relationship among air quality,traffic flow,and urban geometry in the historic urban area is *** study shows that the building-based approach may open an innovative methodology in data mining of urban spatial data for environmental *** results are particularly useful to urban planners when they need to consider the influences of urban form on street environment.

0N THE COMPLEXITY OF AN OPTIMAL ROUTING TREE PROBLEM

A weighted routing tree is one with costs attached to each linkof the tree. Links of the same feature have the same cost. It is proved that the problem of finding ?routing tree of the minimum cost for a given set of tasks of two features is NP-complete.
