Predictive value of pupillography on intraoperative floppy iris syndrome in preoperative period

AIM: To present the frequency of intraoperative floppy iris syndrome(IFIS) in cataract patients who taking alpha 1-a receptor antagonist(ARA) drugs, and evaluate the predictive value of pupil diameter(PD) changes in IFIS ***: Male cataract patients who are under treatment with alpha-1 a-ARAs(alfuzosin, tamsulosin) intraoperatively were evaluated and were grouped as with/without IFIS. The preoperative PD values were compared with controls. Also, the intraoperative manipulations and early/late complications were ***: A total of 77 patients(77 eyes) of 94 benign prostate hyperplasia(BPH) patients have been defined as IFIS(81.91%) and 40 patients(40 eyes) were taking tamsulosin and 37 patients(37 eyes) were taking alfuzosin. During the cataract surgery, the rate of posterior capsular rupture(P=0.754), vitreous loss(P=0.585), iris tears(P=0.004), and iris catching(P=0.000) were higher in IFIS group, but the difference was significant only in the iris catching. At the postoperative first-month visit, persistent IOP rise and iris stromal tears were more frequent in IFIS group, but the difference was not significant(P=0.311, P=0.146;respectively). In contrast, Descemet membrane detachment was insignificantly more frequent in controls(P=0.311). In IFIS and control patients, PDs were 9.54±1.78 and 9.72±1.57 mm(P=0.255) under scotopic illumination, 8.54±1.43 and 8.74±1.25 mm(P=0.289) under mesopic illumination, 6.99±1.35 and 7.27±1.39 mm(P=0.662) under photopic illumination, respectively. However PDs were lower in IFIS under all illumination degrees, no significant difference was detected between ***: IFIS is a significant clinical syndrome with an increased intraoperative/postoperative complication ratio. The prediction of this syndrome is important because of prevention required precautions against possible complications. There is no association between IFIS and preoperative PD.

AIM： To evaluate late effects of chemoradiation on gastrointestinal mucosa with an endoscopic scoring system and compare it to a clinical scoring system. METHODS： Twenty-four patients going to receive chemoradiation after gastric surgery underwent endoscopy four wk after surgery and one year after the chemoradiation finished. Upper gastrointestinal findings were recorded according to a system proposed by World Organisation for Digestive Endoscopy （OMED） and clinical scoring was done with RTOG-EORTC radiation morbidity scoring systems. RESULTS： There was no significant endoscopic difference in gastric and intestinal mucosa after chemoradiation （P 〉 0.05） and there was no association between endoscopic scores and clinical scores. Endoscopic changes were minimal. CONCLUSION： Late effects after chemoradiation in operated patients with gastric cancers can be evaluated with an endoscopic scoring system objectively and this system is superior to clinical scoring systems.

AIM: To assess the impact of macular surgery on the functional and anatomic outcomes in patients with grade 2 epiretinal membrane(ERM), and the effect of internal limiting membrane(ILM) peeling on visual acuity and to analyze the long-term effect of pars plana vitrectomy(PPV) on intraocular pressure(IOP).METHODS: Pseudophakic eyes(62 eyes) diagnosed as idiopathic grade 2 ERM with at least 6 mo postoperative follow-up were included in this retrospective study. The fellow eye was nonvitrectomized. Patients were divided into two groups: group 1(29 eyes) treated with ERM and ILM peeling and group 2(33 eyes) with only ERM peeling. Preoperative and postoperative best corrected visual acuity(BCVA), slit-lamp, and a dilated fundus examination was performed. IOP was measured with Goldman applanation tonometer before, day 1 and first week and each visit after surgery. The incidence of significant IOP elevation was compared between vitrectomized eyes and nonvitrectomized fellow ***: Visual improvement was statistically significant and similar in both groups(P=0.008 in group 1, P=0.002 in group 2, P=0.09 inter-group). The amount of decrease in central macular thickness was statistically significant and similar in both groups(P=0.005 group 1, P=0.008 group 2, P=0.37 intergroup). At the final follow-up(14.1±9.6 mo) the incidence of significant IOP elevation was 4% in vitrectomized eyes(three eyes) and 3%(two eyes) in the nonvitrectomized fellow eyes(P=0.12). Four eyes(12.1%) had recurrent ERM after a mean follow-up of 8.6±1.1 mo in group 2, there was no recurrence in group 1(P=0.01).CONCLUSION: Recurrence of ERM may be decreased by ILM peeling during ERM surgery. However, it seems that ILM peeling do not affect the functional outcome and 23-gauge PPV alone do not have a significant effect on IOP.

A general neural network based methodology that has the potential to replace the computationally-intensive site-specific seismic analysis of structures is proposed in this paper. The basic framework of the methodology consists of a feed forward back propagation neural network algorithm with one hidden layer to represent the seismic potential of a region and soil amplification effects. The methodology is implemented and verified with parameters corresponding to Delhi city in India. For this purpose, strong ground motions are generated at bedrock level for a chosen site in Delhi due to earthquakes considered to originate from the central seismic gap of the Himalayan belt using necessary geological as well as geotechnical data. Surface level ground motions and corresponding site-specific response spectra are obtained by using a one-dimensional equivalent linear wave propagation model. Spectral acceleration values are considered as a target parameter to verify the performance of the methodology. Numerical studies carried out to validate the proposed methodology show that the errors in predicted spectral acceleration values are within acceptable limits for design purposes. The methodology is general in the sense that it can be applied to other seismically vulnerable regions and also can be updated by including more parameters depending on the state-of-the-art in the subject.

One of the leading factors of seepage contamination is mine drainage,which creates serious ecological risks on the environment both during operation and post-mining *** this study,experimental processes have been carried out to determine the occurrence of mine drainage in the Gümüskoy(Kütahya)mineralization area(Northwest Turkey).The prevention method for potential mine drainage occurrence has been also *** enrichment was observed which are directly related to mineralization in trace element concentrations,especially in potentially toxic metals such as Ag,Cd,Mo,Ba,Bi,and *** on short-term tests,mine drainage formation has not been expected according to the p H(7.36-9.38),contact leaching,and acid-base accounting ***,in the long-term,acid generation potential has been expected because of weathering and oxidation in terms of rock type and mineralization in the ***,in order to prevent groundwater contamination in the event of a possible formation of mine drainage,the hydraulic conductivity of the stock area bedrock was evaluated by the in situ tests performed in the field and it was found to be moderately permeable(K=1.9×10^-6 m/s).In order to make the stock area bedrock impermeable,natural clay will be laid and compressed at the base of the stock *** the finite elements were modeled by seepage analysis,the seepage discharge to be formed on the base rock at a depth of 5 m was determined as 3.17×10^-19 m^3/*** the discharge value determined in the seepage analysis after modeling is very close to zero,contamination of possible ground and surface water will be prevented.

It has been already shown that Parkinson’s disease (PD) is characterized by a prominent degeneration in substantia nigra (SN) neurons. Growing evidence suggests that there is a latent period of PD associated with slight non-motor findings such as olfactory dysfunction (Dickson et al., 2018). However, the potential biomarker role of olfactory dysfunction in PD has been a topic of great interest in the last years (Raskin et al., 1990;Dickson et al., 2018). The classical hypothesis of Braak suggests that PD begins as a synucleinopathy in the lower brainstem or in the olfactory bulb (OB) that progresses rostrally to the SN and amygdala to cause parkinsonism at a later stage of the disease (Burke et al., 2008).

Stump appendicitis is an acute inflammation of the residual appendix and one of the rare complications after appendectomy. Paying attention to the possibility of stump appendicitis in patients with right lower abdominal pain after appendectomy can prevent the delay of diagnosis and treatment. In patients with stump appendicitis, CT scan not only assists in making an accurate preoperative diagnosis but also excludes other etiologies. We report a 47-year old man with preoperatively diagnosed stump appendicitis by CT, who underwent an open appendectomy 20 years ago.

The Diizce Plain has a multi-aquifer system, which consists of a near surface unconfined aquifer, along with first and second deeper confined aquifers. Hydrochemical evolution and water quality are related to infiltration of the precipitation, recharge from the formations surrounding the plain, flow path of groundwater and the relationship between surface and groundwater. The groundwater in the unconfined aquifer flows towards the Efteni Lake and the Biiyiik Melen River. Surface waters are divided into two main hydrochemical facies in the study area：（a） Ca2＋-HCO3-; and （b） Ca2＋, Mg2＋-HCOc-, SO4^2-. The groundwater has generally three main hydrochemical facies：（a） Ca2＋-HCO3-; （b） Ca2＋, Mg2＋-HCO3-; and （c） Ca2＋, Mg2＋-HCO3-, Cl-. The hydrochemical facies ＂a＂ and ＂b＂ dominate within shallow depths in recharge areas under rapid flow conditions, while hydrochemical facies ＂c＂ characterizes shallow and mixed groundwater, which dominate intermediate or discharge areas （near Efteni Lake and Biiyiik Melen River） during low flow conditions and agricultural contamination. Calcium and bicarbonate ions, total hardness and electrical conductivity of total dissolved solids （EC-TDS） values increase along the groundwater flow path; but these parameters remain within the limits specified by the standards set for industrial and agricultural usages.

For the designing of cutting-dies is a complex and experience-based process,it is poorly supported by conventional 3D CAD ***,the majority of design activities,including the(re)modeling of those cutting die-components that are directly responsible for performing shaping operations on a sheet-metal stamping part,traditionally still need to be carried-out repetitively,separately,and manually by the *** eliminate some of these drawbacks and upgrade the capabilities of conventional 3D CAD software,this paper proposes a new methodology for the development of a parametric system capable of automatically performing a(re)modeling process of compound washer dies' *** presented methodology integrates CATIA V5 built-in modules,including Part Design,Assembly Design and Knowledge Advisor,publication mechanism,and compound cutting die-design *** system developed by this methodology represents an 'intelligent' assembly template composed of two modules GAJA1 and GAJA2,***1 is responsible for the direct input of the die-design problem regarding the shape,dimensions and material of the stamping part,its extraction in the form of geometric features,and the transferring of relevant design parameters and features to the module ***2 interprets the current values for the input parameters and automatically performs the modeling process of cutting die-components,using die-design knowledge and the company's internal design and manufacturing *** results show that this system significantly shortens the modeling-time for cutting the die-components,improves the modeling-quality,and enables the training of inexperienced designers.
