Accurate positioning of the 24-hour pH monitoring catheter:Agreement between manometry and pH step-up method in two patient positions

AIM： TO investigate the agreement between esophageal manometry and pH step-up method in two different patient positions. METHODS： Eighteen subjects were included in the study. First, the distance from the nose to the proximal border of the lower esophageal sphincter （LES） was measured manometrically. Then a different investigator, who was blinded to the results of the first study, measured the same distance using the pH step-up method, with the patient in both upright and supine positions. An assessment of agreement between the two techniques was performed. RESULTS： In the supine position, the measurement of only one subject was outside the range accepted for correct positioning （~〈 3 cm distal or proximal to the LES）. In the upright position, errors in measurement were recognized in five subjects. Bland-Airman plots revealed good agreement between measurements obtained manometrically and by the pH-step up method with the patient in the supine position. CONCLUSION： In the case of nonavailability of manometric detection device, the pH step-up method can facilitate the positioning of the 24 h pH monitoring catheter with the patient in the supine position. Thisshould increase the use of pH-metry in clinical practice for subjects with suspected gastroesophageal reflux disease if our results are supported by further studies.

Development of sentinel node navigation surgery (SNNS) and advances in minimally invasive surgical techniques have greatly shaped the modern day approach to gastric cancer *** extensive body of knowledge now exists on this type of clinical application but is principally composed of single institute *** dye tracers,such as isosulfan blue or patent blue violet,have been widely utilized with a notable amount of success;however,indocyanine green is gaining *** double tracer method,a synchronized use of dye and radio-isotope tracers,appears to be superior to any of the dyes *** the meantime,the concepts of infrared ray electronic endoscopy,florescence imaging,nanoparticles and near-infrared technology are emerging as particularly promising alternative *** and eosin staining remains the main method for the detection of sentinel lymph node (SLN) *** specialized centers have begun to employ immunohistochemical staining for this type of clinical analysis but the equipment costs involving the associated ultra-rapid processing systems is limiting its widespread *** function-preserving resection of primary tumor from the stomach in conjunction with lymphatic basin dissection navigated by SLN identification represents the current paramount of SNNS for early gastric *** with cT3 stage or higher still require standard D 2 dissection.

Objective:To determine the incidence of tuberculous lymphadenitis(TBL)and other pathologies in cervical lymphadenopathies in Somalia and accompanying radiological ***:In this hospital-based retrospective study,the demographic character-istics,pathology results and radiological findings of 263 patients who underwent ultrasound(US)-guided cervical lymph node biopsy between January 2016 and February 2020 were ***:Of 241 patients 118 men and 123 women(mean age 27.9±18.1 years)included in the study,46.1%(n=111)were diagnosed as necrotizing granulomatous lymphadenitis(caseified,consistent with TBL)and 21.6%(n=12,atypical lymphoid cells and n=40,metastases)as *** most common type of metastasis was squamous cell cancer(n=31),and the primary source of most of them was esophageal cancer(16/31,51.6%).The age of patients with TBL was significantly lower than that of non-TBL(21.9±14.6 vs.41.9±24.6,P=0.003)and the incidence of TBL in pediatric patients was statistically higher(58.0%vs.21.5%,P=0.019).The rate of patients with TBL being localized at level 4 and level 5 was significantly more than non-TBL patients(18.0%vs.10.0%and 23.4%vs.10.8%,respectively,P=0.01).Half of patients with TBL who have chest radiography had pathological findings;consolidation and bronchopneumonia were present in 52.6%of *** were 2 patients with paravertebral abscess and one patient with gastrointestinal ***:In Somalia,in the presence of cervical lymphadenopathy,after diagnosis by using US-guided biopsy;primarily considering of TBL and malignancy,thoracic involvement should be investigated,and esophageal carcinoma must be excluded in terms of metastatic lymph node.

Stress Distributions Created by Short and Regular Implants Placed in the Anterior Maxilla at Different Angles: A Finite Element Analysis

Although short implants are seen as alternative treatments that require additional surgical techniques in posterior region, they can be applied to anterior maxilla and various studies are required on this subject. The purpose of this study was to examine and compare the peak von Mises stress distributions in the crown, implant and abutment by using finite element analysis (FEA). Besides, a comparison of the implant-abutment connection types in the short implant with the FEA method was established. A short implant (4 × 5 mm) with a taper-lock connection and a regular implant (4 × 9 mm) with a screw connection were used in maxillary central incisor tooth area. Three different titanium abutments with 0?, 15? and 25? angles were used for abutments. In addition, in order to determine whether the stress change in short implants is due to the length of the implant-abutment connection, a screw was designed for a short implant and it was also evaluated in the same three angles. A total of three groups and nine models were generated. 114.6N load was applied to the cingulum area of the crown at an angle of 135? to the long axis of the crowns. A torque load of 25 Ncm was applied to the regular and short implant screw. Von Mises stress distributions of implants, abutments and crowns were evaluated by using FEA. Increased angle in implants increased von Mises stress values of implant, abutment and crown. Screw connection was found higher at all angles in short implants. Close values were found at different angles in taper-lock short implant crowns. The length and the angle in the bone of implant with the type of implant-abutment connection results in the accumulated stress values. Clinical Implications Taper implant-abutment connection system was found to be more promising in terms of stress accumulation in crowns.
