Background: Emergence agitation (EA) is a common phenomenon observed in pediatric patients following general anesthesia. This study aimed to assess the efficacy of propofol and fentanyl in preventing EA and to compare their associated complications or side effects. Methods: This prospective randomized observational comparative study was conducted at Dhaka Medical College Hospital from July 2013 to June 2014. The study aimed to evaluate the effects of propofol and fentanyl on EA in children aged 18 to 72 months undergoing circumcision, herniotomy, and polypectomy operations. Ninety children were included in the study, with 45 in each group. Patients with psychological or neurological disorders were excluded. Various parameters including age, sex, weight, American Society of Anesthesiologists (ASA) class, duration of anesthesia, Saturation of Peripheral Oxygen (SPO2), heart rate (HR), respiratory rate (RR), Pediatric Anesthesia Emergence Delirium (PAED) score, duration of post-anesthesia care unit (PACU) stay, incidence of laryngospasm, nausea, vomiting, and rescue drug requirement were compared between the two groups. Results: Age, sex, weight, ASA class, and duration of anesthesia were comparable between the two groups. Perioperative SpO2 and HR were similar in both groups. However, the PAED score was significantly higher in the fentanyl group during all follow-ups except at 30 minutes postoperatively. The mean duration of PACU stay was significantly longer in the fentanyl group. Although the incidence of laryngospasm was higher in the fentanyl group, it was not statistically significant. Conversely, nausea or vomiting was significantly higher in the fentanyl group. The requirement for rescue drugs was significantly higher in the fentanyl group compared to the propofol group. Conclusion: Both propofol and fentanyl were effective in preventing emergence agitation in pediatric patients undergoing various surgical procedures under sevoflurane anesthesia. However, propo

Background: Following coronary artery bypass grafting (CABG), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABG. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABG. Group A (n = 22) consisted of patients with normal serum cortisol levels, while Group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: Group-A had a mean age of 54.50 ± 17.97, and Group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in Group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of Group B patients, statistically significant compared to the 4.5% in Group A. However, patients in Group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). Background: Acute Kidney Injury (AKI) stands as a prominent postoperative complication in on-pump cardiac surgery, with repercussions on morbidity, mortality, and hospitalization duration. Current diagnostic criteria relying on serum creatinine levels exhibit a delayed identification of AKI, prompting an exploration of alternative biomarkers. Aims and Objectives: This study is designed to overcome diagnostic constraints and explore the viability of serum Cystatin C as an early predictor of Acute Kidney Injury (AKI) in individuals undergoing on-pump cardiac surgery. The investigation aims to establish the relationship between serum Cystatin C levels and the onset of AKI in patients subjected to on-pump cardiac surgery. Primary objectives involve the assessment of the diagnostic effectiveness of serum Cystatin C, its comparison with serum creatinine, and the exploration of its potential for the early identification and treatment of AKI. Methodology: Conducted as a single-center study at the cardiac surgery department of BSMMU in Bangladesh from September 2020 to August 2022, a comparative cross-sectional analysis involved 31 participants categorized into No AKI and AKI groups based on Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Data collection encompassed preoperative, post-CBP (cardiopulmonary bypass) conclusion at 2 hours, postoperative day 1, and postoperative day 2 intervals. Statistical analyses included Chi-squared tests, independent Student’s t-tests, and one-sample t-tests. Significance was set at P Results: The study revealed no significant differences in baseline characteristics between the No AKI and AKI groups, except for CPB time and cross-clamp time. Serum Cystatin C levels in the AKI group exhibited statistical significance at various time points, highlighting its potential as an early detector. Assessment of BRRI Whole Feed Combine Harvester (Model BRRI WCH2021) for Mechanized Rice Harvesting in Bangladesh

Agricultural mechanization plays a pivotal role in the transition from subsistence to commercial agriculture, with a particular focus on labour-intensive activities like harvesting. This study assesses the operational characteristics of the BRRI Whole Feed Combine Harvester (Model BRRI WCH2021) at the field level. Developed under the SFMRA project, the harvester’s technical performance and loss assessment were conducted during the Boro 2022 and Aman 2022 seasons in farmer fields in Bangladesh’s Rangpur region. The field efficiency of the harvester was determined to be 62.5% and 57.9% in the Boro and Aman seasons, respectively. Fuel consumption rates were recorded at 2.77 l/ha and 2.31 l/ha for the Boro and Aman seasons. The total harvesting losses, encompassing cutter bar, shatter, cylinder, and separation loss, averaged 0.56% and 0.48% in the Boro and Aman seasons, respectively. Mechanized harvesting with the BRRI Whole Feed Combine Harvester significantly reduced paddy losses by 5.81% compared to manual methods. The field evaluation results indicate the combine harvester’s satisfactory performance, highlighting its potential to alleviate labour demands during peak harvesting. The development of the BRRI WCH offers a sustainable solution for rice harvesting mechanization among progressive farmers. It paves the way for the broader adoption of advanced agricultural technology in Bangladesh.

Prevalence and Characterization of Pathogens Responsible for Enteric Fever and Assessment of Their Antibiotic Susceptibility Pattern in Bangladesh

In Bangladesh, Enteric fever is a persistent health issue throughout the year, occasionally reaching epidemic levels. The growing antibiotic resistance makes treatment increasingly challenging. Therefore, studying the antibiotic resistance patterns within the population is crucial to ensure a more effective treatment strategy. This study was designed to determine the prevalence, antimicrobial susceptibility profile, and factors associated with Salmonella spp. infections among clinically suspected Enteric fever patients in Bangladesh. This study also aimed to investigate whether there has been a re-emergence in the susceptibility of bacterial strains to conventional drugs. Data were collected from February 2024 to July 2024, from patients suspected Enteric fever (fever less than seven days duration) in a Private Diagnostic Center of Bangladesh. A total of 195 blood samples were cultured, where 53.85% came out positive, among which 79.05% Salmonella typhi and 20.95% Salmonella paratyphi A were found. Prevalence of Typhoid fever was observed high among the school-going age group (0 - 15 years) patients. Both these organisms were susceptible to ceftazidime, cefixime, ceftriaxone and cefepime but resistant to nalidixic acid, ciprofloxacin and levofloxacin. Nalidixic Acid is resistant to all S. paratyphi A and sensitive to few S. typhi. Ciprofloxacin and levofloxacin showed delayed response (36.14% and 22.72% sensitive to S. typhi and S. paratyphi A, respectively) and resistance (63.85% and 77.27% resistant to S. typhi and S. paratyphi A, respectively). In case of S. typhi, the resistance was found against ampicillin (32.53%), chloramphenicol (27.71%), cotrimoxazole (24.09%) and the resistance of S. paratyphi A found against ampicillin (4.54%), chloramphenicol (0%) and cotrimoxazole (0%). This study will provide clinicians with alternative drug options and facilitate the effective treatment of Enteric fever.

Ocean productivity is the foundation of marine food web,which continuously removes atmospheric carbon dioxide and supports life at sea and on ***-temporal variability of net primary productivity(NPP),sea surface temperature(SST),sea surface salinity(SSS),mixed layer depth(MLD),and euphotic zone depth(EZD) in the northern B ay of Bengal(BoB) during three monsoon seasons were examined in this study based on remote sensing data for the period 2005 to *** compare the NPP distribution between the coastal zones and open BoB,the study area was divided into five zones(Z1-Z5).Results suggest that most productive zones Z2 and Zl are located at the head bay area and are directly influenced by freshwater discharge together with riverine sediment and nutrient *** Z1-Z5,the NPP ranges from 5 315.38 mg/(m^(2)·d) to 346.7 mg/(m^(2)·d)(carbon,since then the same).The highest monthly average NPP of 5 315.38 mg/(m^(2)·d) in February and 5 039.36 mg/(m^(2)·d) in June were observed from Z2,while the lowest monthly average of 346.72 mg/(m^(2)·d) was observed in March from Z4,which is an oceanic *** values vary from 6-154 m for the study area,and it has an inverse correlation with NPP *** is deeper during the summer season and shallower during the wintertime,with a corresponding increase in *** the year,monthly SST shows slight fluctuation for the entire study area,and statistical analysis shows a significant correlation among NPP,and EZD,overall positive between NPP and MLD,whereas no significant correlation among SSS,and SST for the northern ***-term trends in SST and productivity were significantly po sitive in head bay zones but negatively productive in the open *** findings in this study on the distribution of NPP,SST,SSS,MLD,and EZD and their seasonal variability in five different zones of BoB can be used to further improve the management of marine resources and overall environmental condition in response to clim

Increasing demand for energy due to the populous Eastern Australia has driven oil and gas industries to find new sources of hydrocarbons as they are the primary energy *** study has been done on the Volador Formation in the Gippsland Basin by means of core-based petrophysical,sedimentological,and petrographic analyses as well as well log-based interpretation and capillary pressure *** wells from Kipper,Basker and Tuna fields with available dataset were investigated in this study:Kipper-1,Basker-1,Basker-2,Basker-5 and ***,the formation has good reservoir quality based on the high porosity and permeability values obtained through core and well log petrophysical *** formation made up of mostly moderate to coarse quartz grains that has experienced strong anti-compaction and is poorly *** and illite clays are seen dispersed in the rock formation,with the minority being mixed *** clays and diagenetic features including kaolinite cement and quartz overgrowth that can lead to porosity reduction only have insignificant impact on the overall reservoir *** addition,capillary pressure data shows that most samples are found in the transition to good reservoir zones(<50%saturation).The results obtained from this study have shown that the Volador Formation in the Gippsland Basin is worth for hydrocarbon exploration.
