Objective:To document the clinical,biochemical and imaging phenotypes of immunocompetent patients with adrenal ***:The clinical,biochemical and radiologic data of 18 immunocompetent patients[age:45.00(39.25,56.25)years,median(IQR),m/f(16/2)]with adrenal histoplasmosis presenting in the Department of Endocrinology,BSMMU between 2014 and 2020 were retrospectively ***:All patients were seronegative for HIV infection,and 27.8%(5/18)had well controlled diabetes *** median duration of the symptoms was 6.00(IQR:4.00,11.25)*** had significant weight loss,anorexia and *** was present in 61.1%(11/18)patients and night sweat was present in 27.8%(5/18)*** and hyperpigmentation were present in 55.6%(10/18)and 66.7%(12/18)cases,*** of 18 patients presented with adrenal *** occurred in 55.6%(10/18)cases,but none had *** of 18 patients had adrenal insufficiency whereas 83.3%(15/18)had high adrenocorticotropic *** scan revealed bilateral adrenal enlargement in all cases with oval shape and regular *** were hypodense having radiodensity 21-90 hounsfield unit,and 11.1%(2/18)were heterogeneous in contrast *** had noticeable calcification whereas 1.1%(2/18)cases had central necrosis with peripheral rim *** was present in 6 cases,splenomegaly in 3 cases and 5 patients had abdominal *** were confirmed by positive fine needle aspiration cytology of adrenal ***:Adrenal histoplasmosis should be considered in the list of differentials of bilateral adrenomegaly in immunocompetent individuals even living in non-endemic areas.

Vitamin D and Its Association with Glycemic Status in Bangladeshi Adults with Newly Detected Type 2 Diabetes Mellitus

Background: Very limited data are available regarding the association of vitamin D with glycemic status among adults with newly detected type 2 diabetes mellitus (T2DM) in Bangladesh. Objectives: To determine vitamin D status and its association with glycemic status in Bangladeshi adults with newly detected T2DM. Materials and Methods: This cross-sectional study was carried out in 102 newly detected T2DM diagnosed on the basis of the American Diabetes Association 2017 criteria (age: 42.95 ± 10.68 yrs.;m/f: 44/58) and equal number of age and sex matched controls (age: 40.43 ± 11.04 years) recruited consecutively from the Department of Endocrinology, BSMMU to measure serum vitamin D by high performance liquid chromatography method. Results: Both vitamin D deficiency (p p p 1c (r = -0.249, p = 0.011) in patients with T2DM. HbA1c was linearly associated with vitamin D (β = -0.26, p = 0.009) and severe vitamin D deficiency by binary (OR = 1.37, p = 0.003) and multinomial logistic regression (HbA1c ≥ 10%: OR = 4.25, p = 0.04) in people with T2DM after adjustment for age and BMI. Visceral adiposity mediates insulin resistance, but their association among adults with prediabetes is scarce in the literature. This study is aimed to determine the association of visceral adiposity index (VAI) with insulin resistance in adults with prediabetes. This cross-sectional study was done among 117 adults with newly detected prediabetes [m/f;23/94;mean ± SD: Age 36.30 ± 9.99 years, BMI 28.89 ± 4.35 kg/m2] based on American Diabetes Association 2018 criteria and 141 matched healthy controls [m/f: 28/113;mean ± SD: 35.30 ± 6.88 years, BMI 25.03 ± 4.58]. Waist circumference, body mass index, fasting triglyceride, HDL cholesterol, fasting blood glucose and insulin were measured in each group to calculate VAI and homeostatic model assessment of insulin resistance (HOMA-IR).
