Evaluation of molecular and immunological methods for diagnosis of tuberculosis pleurisy

Objective:The inefficiency of conventional laboratory methods for diagnosis of Pleural tuberculosis(TBP) and the reliance on pleural biopsy have motivated the evaluation of alternative diagnostic *** goal was to evaluate different laboratory techniques Ziehl Neelsen,Mantoux skin test,determination of interferon gamma in serum and pleural fluid,polymerase chain reaction and serological study of specific IgG,IgM and IgA beside bacteriological culture by BACTEC 460 TB for rapid and accurate diagnosis of tuberculosis pleurisy. Methods:Patients presented with pleural effusions were subjected to study by ZN,PCR,serological study By specific IgG,IgM and IgA for A60 of tuberculosis compared to culture by BACTEC460 *** gamma was determined both in serum and pleural ***:Mantoux skin test was positive in 19 TBP patients and four patients with exudative pleural effusion.Z.N staining results of pleural biopsy specimens were positive in only 1 of 23 patients(4.3%) in the tuberculous pleural effusion *** was positive in 20 cases of group 1(87%).Serum and pleural fluid interferon had significantly elevated levels(Pelation in TBP(PelISA test was positive in 7/23(30.4%),IgM was positive in 17/23(73.9%) patients and IgG was positive in 16/23(69%) *** of the non TBP had either ZN,PCR or positive serum IgA,IgG,or *** the positive results for IgG and IgM were combined together the serological tests correcdy identified 20/23(87%) of patients. Conclusion:We suggest that in TBP serological diagnosis by combined use of IgG and IgM for A60 antigen with serum determination of interferon gamma can provide rapid and non invasive diagnostic tool that can justify the starting of chemotherapy while awaiting the results of culture.

Study of enteropathogens associated with paediatric gastroenteritis

Objective:To determine the etiology of acute diarrhea in children under 5 years of age and to improve- knowledge of the etiology of gastrointestinal pathogens using traditional and molecular diagnostic ***: Various common enteropathogens(viral,bacterial and parasites) associated with diarrhea were investigated by conventional and molecular techniques(PCR) for verotoxin present in Escherichia coli in 218 children less than 5 years of age admitted to Mansoura University Children hospital- ***:The occurrence of enteropathogens identified was as follows:*** O157:H7 38.8%followed by Salmonella Spp 29.4%,Aeromonas 20%and Shigella Spp 11.8%.Rotavirus was found in of samples 17.1%.Rotavirus was statistically significant in ageella isolates were tested for their susceptibility to common antimicrobial agents and most of the isolates were resistant to ampicillin and trimethoprim /***:This study demonstrated that rotavirus,*** O157:H7,Salmonella Spp,and Aeromonas were significant *** was significantly associated with infantile *** results highlight the value of using a combination of traditional and PCR techniques in the diagnosis of enteropathogens related to acute gastroenteritis in children.

Correlation of biofilm formation of uropathogenic Escherichia coli(UPEC) and fimbriae genotypes

Objective:In the present study we try to correlate between pathogenic intrinsic factor of Escherichia coli(***) presented with different fimbria genotyes and biofilm formation with host immune factor entitled interleukin-6(IL-6) secretion as defense ***:A total of 91 pediatrics complaining of pyuria were included in the present *** addition,20 healthy control children were *** microbiological study was performed for isolated *** alleles were studied by multiple alleles PCR and biofilm formation was ***-6 was measured in ***:IL-6 had statistically significant elevation in patients’urine compared to *** biofilm study, it was found that 19 isolated *** had formed biofilm in ***,urine samples with positive biofilm formation of *** had statistically significant lower IL-6 secretion than those with negative *** for *** distribution of fimbria genotypes showed that the frequent genotype was for alleleⅠ(34.3%) followed by mixed allelesⅠandⅡ(24.1%).There was significant correlation between mixed alleles(Ⅰ&Ⅱ)and biofilm ***: The present study highlights the presence of significant strains of *** causing urinary tract infections capable of biofilm *** formation is associated with less innate immunity manifested by lower urinary IL- *** majority of isolates had fimbria *** appears that mixed allelesⅠandⅡhave prominent virulence effect with tendency for biofilm formation.

Value of avidity index of IgG and polymerase chain reaction in diagnosis of herpes simplex 2 virus in pregnancy

Objective:The goal of the present study is to highlight the significance of the determination of IgG avidity to herpes simplex2virus(HSV2) compared to detection of viremia by polymerase chain reaction(PCR) in relation to recurrent ***:Serum samples from pregnant patients with bad obstetric histories and control subjects were analyzed for quantitative determination of IgG specific for HSV2 by using elISA with and without sample treatment with 8 M urea as protein-denaturing agents for measurement of avidity index(AI).PCR was performed for samples to determine HSV2 ***:Regarding avidity index 11.5%had avidity index = 100.0,11.5%has avidity index = 47%and 76.9%had avidity index>60%.Control group had AI>70%.There was statistically insignificant difference between OD of IgG in patients with viremia before urea treatment and after urea treatment compared to healthy *** there was statistically insignificant difference in OD before urea and after urea in patients with ***:From this study we can conclude that viremia for HSV2 is common finding in patients with bad obstetric *** avidity immunoglobulin G was not common finding in those *** may be explain that even reactivation of latent HSV 2 infection is associated with fetal loss.

Molecular study of astrovirus, adenovirus and norovirus in community acquired diarrhea in children: One Egyptian center study

Objective: To determine the prevalence of astrovirus, norovirus, adenovirus in children below five years old with diarrhea by multiplex reverse transcriptase polymerase chain reaction(RT-PCR) along with rotavirus antigen detection by enzyme linked immunosorbant ***: The study was conducted on children below five years old complaining of acute diarrhea. The study included stool examination by molecular method for detection of norovirus, adenovirus and astrovirus by multiplex RT-PCR. Rotavirus antigen was detected in the stool by enzyme linked immunosorbant ***: The study included 100 children below 5 years old with acute *** RT-PCR was positive in 34% of the children. The most frequently detected virus was rotavirus(44%), followed by norovirus(30%), adenovirus(20%) and astrovirus(14%). The clinical symptoms were more significantly associated with viral diarrhea such as fever(P = 0.03), bloody diarrhea(P = 0.025), vomiting(P = 0.000 1) and watery diarrheas(P = 0.05). The frequency of diarrhea with viral pathogen was significantly presented in winter season(39.7%). There were significant frequencies of norovirus and adenovirus in age ranging 1–2 years old(P = 0.04, P = 0.01 respectively).Conclusions: The present study spotlights on the prevalence of viral pathogens as an important etiology in diarrhea in children below five years old. Astrovirus, norovirus and adenovirus are common along with rotavirus in this group of patients. Multiplex PCR leads to improve the laboratory diagnosis of these viruses along with antigen detection method. Further longitudinal studies are required to evaluate the epidemiological data associated with these viruses and for proper management of such drastic infection.

Maternal-Fetal Hepatitis E Transmission:Is It Underestimated?

Hepatitis E virus (HEV) is an enterically transmitted virus;and several modes of transmission have been proposed, including blood transfusion, person to person transmission, and transplacental transmission. HEV during pregnancy is asso-ciated with an unfavorable prognosis for mothers and in severe cases can cause acute fulminate hepatitis and death. Transplacental transmission of HEV usually results in unfa-vorable outcomes of pregnancy, mainly fetal loss, preterm labor, and hepatic dysfunction in neonates. In this review, we will summarize the effects of HEV on maternal-fetal health in various clinical situations.

Dipper throated optimization(DTO)algorithm is a novel with a very efficient metaheuristic inspired by the dipper throated *** has its unique hunting technique by performing rapid bowing *** show the efficiency of the proposed algorithm,DTO is tested and compared to the algorithms of Particle Swarm Optimization(PSO),Whale Optimization Algorithm(WOA),Grey Wolf Optimizer(GWO),and Genetic Algorithm(GA)based on the seven unimodal benchmark ***,ANOVA and Wilcoxon rank-sum tests are performed to confirm the effectiveness of the DTO compared to other optimization ***,to demonstrate the proposed algorithm’s suitability for solving complex realworld issues,DTO is used to solve the feature selection *** strategy of using DTOs as feature selection is evaluated using commonly used data sets from the University of California at Irvine(UCI)*** findings indicate that the DTO outperforms all other algorithms in addressing feature selection issues,demonstrating the proposed algorithm’s capabilities to solve complex real-world situations.

Comparing Solutions to the Nonlinear Dissipative Wave Equation

In previous decades, many of the practical problems arising in scientific fields such as physics, engineering, and mathematics have been related to nonlinear fractional partial differential equations. One of these nonlinear partial differential equations, the dissipative wave equation, has been found to have a plethora of useful applications in different fields. A special class of solutions has been studied for the dissipative wave equation including exact solutions and approximate solutions. The aim of this article is to compare the non-polynomial spline method and the cubic B-spline method with the solution of a nonlinear dissipative wave equation. We will conduct a comparison of the stability of the two methods using the Von Neumann stability analysis. In addition, a numerical example will be presented to illustrate the accuracy of these methods.

Computational Analysis for Solving the Linear Space-Fractional Telegraph Equation

Over the last few years, there has been a significant increase in attention paid to fractional differential equations, given their wide array of applications in the fields of physics and engineering. The recent development of using fractional telegraph equations as models in some fields (e.g., the thermal diffusion in fractal media) has heightened the importance of examining the method of solutions for such equations (both approximate and analytic). The present work is designed to serve as a valuable contribution to work in this field. The key objective of this work is to propose a general framework that can be used to guide quadratic spline functions in order to create a numerical method for obtaining an approximation solution using the linear space-fractional telegraph equation. Additionally, the Von Neumann method was employed to measure the stability of the analytical scheme, which showed that the proposed method is conditionally stable. What’s more, the proposal contains a numerical example that illustrates how the proposed method can be implemented practically, whilst the error estimates and numerical stability results are discussed in depth. The findings indicate that the proposed model is highly effective, convenient and accurate for solving the relevant problems and is suitable for use with approximate solutions acquired through the two-dimensional differential transform method that has been developed for linear partial differential equations with space- and time-fractional derivatives.
