Indications and results of renal biopsy in children:a 36-year experience

Background This study was conducted to investigate retrospectively the indications for renal biopsy(RB)in native kidneys and to analyze pathological findings in a single tertiary pediatric hospital in Southern Italy for the last 36 *** All patients who underwent RB at our hospital from 1979 to 2014 were *** renal tissue specimens were studied under light and immunofluorescent microscopy,while electron microscopy was performed only for specific clinical *** The study group included 213 patients(female 43.2%)who underwent 225 percutaneous native kidney *** age was 10.4 years(range 0.6-24 years).The most frequent indication for RB was nephrotic syndrome(44.4%),fol-lowed by proteinuria(27.6%),asymptomatic hematuria(17.3%)and acute kidney injury(9.8%).Gross hematuria appeared after biopsy in less than 5%of the patients,but none of them needed blood *** renal tissue sample was obtained in 95.5%of the renal *** glomerulonephritis(GN)was the most common finding(61.4%),followed by secondary GN(21.4%),tubulointerstitial diseases(3.7%)and hereditary nephropathy(2.8%),while in 10.7%of the cases,normal renal tissues were *** to histopathological diagnosis,the most common causes of primary GN were IgA nephropathy(20.9%),followed by minimal change disease(18.1%)and focal segmental glomerulosclerosis(11.6%).Conclusions The epidemiology of glomerular disease in our single-center cohort is similar to that shown in other national and international ***,our study shows that percutaneous ultrasound-guided RB is a safe,reliable and effec-tive technique in children.

Endoscopic approach to malignant strictures at the hepatic hilum

Hilar tumors have proven to be a challenge to treat and manage because of their poor sensitivity to conventional therapies and our inability to prevent or to detect early tumor formation. Endoscopic stent drainage has been proposed as an alternative to biliary-enteric bypass surgery and percutaneous drainage to palliate malignant biliary obstruction. Prosthetic palliation of patients with malignant hilar stenoses poses particular difficulties, especially in advanced lesions （type Ⅱ lesions or higher）. The risk of cholangitis after contrast injection into the biliary tree in cases where incomplete drainage is achieved is well known. The success rate of plastic stent insertion is around 80% in patients with proximal tumors. Relief of symptoms can be achieved in nearly all patients successfully stented.

Synthesis of Ceramic Tiles Reinforced with Addition of Aluminum Particulate Waste

Research conducted on ceramic materials has been investigating the incorporation of solid waste into their formulations,driven by the proper disposal of such waste and the reduction of negative environmental *** study analyzed the effects of adding aluminum powder residue to the physical properties of ceramic masses with the aim of obtaining new formulations for ceramic *** aluminum residue and the standard mass for ceramic tile production were chemically characterized and homogenized to obtain new formulations with the incorporation of 4%,6%,8%,and 10%aluminum powder in the ceramic *** specimens were uniaxially pressed and sintered at a temperature of 1,200°C for 2 h,undergoing three different temperatures(100°C,400°C,and 650°C)for 30 min *** were evaluated for WA(water absorption),RLq(linear shrinkage),SEM(scanning electron microscopy),and TRF(flexural strength)*** results demonstrate that the addition of aluminum powder residue is feasible in the proposed formulations(4%,6%,8%,and 10%),as they enhance the mechanical properties of the ceramics compared to the formulation with 0%residue,at a sintering temperature of 1,200°C.

AIM:To evaluate the diagnostic accuracy of confocal laser endomicroscopy(CLE) for the detection of dysplasia in long-standing ulcerative colitis(UC).METHODS:We prospectively performed a surveillance colonoscopy in 51 patients affected by long-standing ***,in the presence of macroscopic areas with suspected dysplasia,both targeted contrasted indigo carmine endoscopic assessment and probe-based CLE were *** mucosal biopsies and histology,utilised as the gold standard,were assessed randomly and on visible lesions,in accordance with current ***:Fourteen of the 51 patients(27%) showed macroscopic mucosal alterations with the suspected presence of dysplasia,needing chromoendoscopic and CLE *** 5 macroscopically suspected cases,the presence of dysplasia was confirmed by histology(3 flat dysplasia;2 DALMs).No dysplasia/cancer was found on any of the outstanding random *** diagnostic accuracy of CLE for the detection of dysplasia compared to standard histology was sensitivity 100%,specificity 90%,positive predictive value 83% and negative predictive value 100%.CONCLUSION:CLE is an accurate tool for the detection of dysplasia in long-standing UC and shows optimal values of sensitivity and negative *** scheduled combined application of chromoendoscopy and CLE could maximize the endoscopic diagnostic accuracy for diagnosis of dysplasia in UC patients,thus limiting the need for biopsies.

Bubble Column Reactor Fluid-dynamic Evaluation at Pilot-plant for Residue and Extra-heavy Crude Oil Upgrading Technology

Bubble column reactors are multiphase contacting devices used in a wide variety of industrial application. Inrtevep S. A. is working on developing technologies to convert heavy and extra-heavy crude oil using this type of reactors. Volumetric gas hold up, flow pattern, average gas bubble size, average interfacial area, RTD （residence time distribution）, dispersion coefficient, Peclet number are important design parameters for a proper scale up of them. Several cold model experiments have been proposed to determine the previously mentioned parameters at atmospheric conditions, using a plexiglas bubble column reactor at pilot plant scale unit （12 cm diameter）. It was also evaluated our own design of internal trays （plates） in the reactor. Air-tap water and air-light oil systems have been used. A wide operating condition range was applied, superficial gas velocity between 0.5-10 cm/s, liquid flowrate between 15-65 I/h. Generally speaking, working without internal trays was found that gas hold up increase along the reactor and it was possible to identify heterogeneous bubble, transition and turbulent flow pattern areas for the air-light oil system. Average gas bubble size increase along the reactor at bubble regime from 2-5 mm but at turbulent regime, stay oscillating between 1-3 mm. Average interfacial area increases exponentially with superficial gas velocity at any reactor height, till 1,412 m2/m3 for the air-light oil system but, at bubble flow regime, the average interfacial area is lower than 100 m2/m3, which negatively impact the reactor performance. Internal trays in the reactor always increase gas hold up at any condition or system used. Residence time distributions curves, Peclet numbers and dispersion coefficients founded, show that this reactor with this kind of design internal trays still tends to be a complete mixing reactor under the operating conditions used.

Changes in Human Hair Induced by UV- and Gamma Irradiation

The effect of UV- and 137Cs gamma radiation on the structural and chemical integrity of human hair was studied to determine the feasibility of using human hair as a non-invasive biomarker of radiation exposure to ionized gamma- and non-ionized UV-radiation. Steady state tryptophan (Trp) fluorescence and chemical analytical methods were used to evaluate the molecular integrity of Trp fluorophores and SH-groups in hair proteins and to assess the radiation induced damage quantitatively. It was found that human hair fibers were progressively damaged by exposure to both UV- and ionized gamma radiation. Damage to the hair was evidenced by a decrease in the fluorescence intensity as a result of observed depletion of the amino acid tryptophan as well as significant reduction in a number of free SH-groups in hair proteins. Hair damage was dose-dependent for exposures between 0 and 10.0 Gy and 0 - 20 J/cm2 of UV-radiation. Additional results demonstrate that hair-fibers exposed to gamma rays, with much higher quantum energy than UV, undergo a smaller extent of changes in Trp fluorescence than when exposed to lower or equal energy of UV-irradiation. The stable Trp fluorophore appears to be extremely sensitive to UV-radiation in contrast to the ionized gamma radiation whose damage is originated from the reaction of free radicals and direct deposition of energy. We conclude that fluorescence spectroscopy represents a useful tool in the quantitative evaluation of the radiation exposure and could also be used for the rapid and non-invasive assessment of radiation dose i.e. biodosimeter. The approach is simple, non-invasive and appears to have considerable potential that enables quantitative evaluation of radiation dose exposure in a single hair fiber.

Considering the great interest of many professionals in the foundation that guides the effective use of ozone therapy in the treatment of aesthetic disorders, this study aims to define the physiological and practical aspects of using the oxygen-ozone mixture to solve aesthetic problems. Exploratory research was carried out, presented in a narrative review, to highlight the action of ozone therapy in the treatment of aesthetic affections. The review explored scientific articles, books, theses, and dissertations published and available in the following databases: MEDLINE (Medical Literature Analysis and Retrieval System Online), PubMed (National Library of Medicine), SCIELO (Scientific Electronic Library Online), and LILACS (Latin Literature American and the Caribbean in Health Sciences). In addition, elements that characterize the authors’ clinical experience on ozone therapy in various aesthetic treatment protocols were added. We found several reports in the literature that justify the use of ozone and its different therapeutic modalities in various aesthetic affections, mainly in the treatment of localized fat, cellulite, rejuvenation, skin tissue repair, acne, dyschromia, and others. Through its various therapeutic modalities, we conclude that ozone therapy has added a lot to aesthetic procedures, mainly for its effective supporting action or as the main agent of critical physiological changes to enhance clinical results when treating aesthetic conditions.

Evaluation of the Chemical Integrity of Beta-Lactam Antibiotics by Iodine-Based Assay

One of the major problems associated with the therapeutic application of β-lactam antibiotics is their significant instability [1]-[3]. Analogs from that group are easily degraded in aqueous solutions and in solid state, which happens predominantly as a result of β-lactam ring chemical degradation. Because of this, the chemical integrity of β-lactam antibiotics must be closely monitored to meet FDA requirements. The original iodometric method based on visual detection of the end products of beta-lactamase hydrolysis that has been used to test beta-lactamase activity [4] was modified to quantitatively assess the amount of degraded product in amoxicillin. A UV-visible spectrophotometric method has been used, focusing on the ability of the products from the degradation of β-lactam analogs to absorb iodine from bulk solution, resulting in a significant reduction in the absorption spectra. Amoxicillin trihydrate tablets with different expiration dates and amoxicillin samples pre-loaded with known amounts of degraded amoxicillin have been used to generate a calibration curve and quantify the presence of degraded materials. Results show that the changes in the absorption spectra of iodine reagent directly correlate with the fraction of degraded β-lactam antibiotics, such as amoxicillin, in commercial products. The proposed assay might be as close as one step from simpler and cheaper methods of quantitative analysis of degraded β-lactam analogs, with estimated sensitivity in the range of 1% - 2%, which is better than the current FDA requirements.
