Chronic Pain Review Following Lichtenstein Hernia Repair: A Personal Series

Introduction: Chronic groin pain is both a topical subject and important outcome measurement following inguinal hernia repair. It has been suggested its incidence is related to the management of the nerves of the inguinal canal as well as the type of mesh used and methods of fixation for both open and laparoscopic surgery. The level of pre-operative and post operative pain, its duration as well as complications may all be factors in predicting whether chronic pain could develop. The method of measurement of chronic pain is itself a contentious issue. It is now apparent that the qualitative measurement of activity and functional status as well as quantitative is important. As a result several uniform methods of assessing chronic post-operative pain have been designed, validated and implemented. One of these is used here. Methods: A study reviewing a consecutive series of Lichtenstein repairs performed by a single experienced hernia surgeon was carried out. 248 inguinal hernia patients operated on in 2005 were reviewed. Patients were contacted via telephone at a median of 50 months. Franneby’s recently validated inguinal pain questionnaire was used to assess the incidence of chronic pain. Results: 185 (75%) patients were able to be contacted for follow-up, making a total of 213 inguinal hernia repairs (including bilateral and recurrent hernias). At the time of review 3% of patients reported having pain. No patients reported that pain or discomfort was limiting their work, exercise or activities of daily living. No patients had disabling pain. Conclusion: Chronic pain did not appear to be a major problem within this cohort of patients. The Lichtenstein technique can produce favourable results in terms of chronic pain for unilateral, bilateral and recurrent inguinal hernias in an unselected group of patients with the usual mix of risk factors and complications.

Towards a New Model for the Production of Civil Status Records Using Blockchain

The work presented in this article focuses on document forgery. Document fraud is a generic term used to designate a fraud, a falsification of a document. The said falsification can be observed in several sectors of everyday life. For a better illustration, let’s consider a system for producing civil status forms. The birth certificate is generally the basis of any individual’s life, and if this document is falsified, it can have repercussions at several levels, particularly in the academic field, where false diplomas, false national identity cards, false passports, false marriage certificates, etc. are obtained. It is therefore demonstrated that a secure production unit for civil status forms can be beneficial to both developed and developing countries. In addition, smart contract is proposed using a private Hyperledger fabric blockchain for validation of civil status documents, and a non-state public key management infrastructure is also proposed to authenticate the nodes that will validate the transactions. This improved blockchain solves the problem of falsification of civil status documents, as false nodes can contribute to the validation of a transaction. The contribution of such a hybrid system is of paramount importance for the identity of civil status documents on the one hand and for the identity of documents in general, and the blockchain to guarantee the distributed, unforgeable, non-repudiation, transparency and disintermediation character.

Influence of the Size of Potato Microtubers (<i>Solanum tuberosum</i>L.) on the Yield of Plants under Semi-Axenic Conditions

The potato (Solanum tuberosum L.) is a vegetable that ranks fifth in the world for human consumption. Its importance is growing more and more in the Senegalese diet. However, the potato production in Senegal does not meet the needs of the market, which maintains dependence on the outside for the supply of quality seeds. In addition, these imported seeds do not often have phytosanitary qualities required for local production in the Sahelian zone. The in vitro production of microtubers, used as seed, has been shown to be one of the most efficient means for propagation of basic material. To overcome the constraints linked to the supply and availability of potato seeds, with high germination capacity, the impact of the microtuber size on the yield of the plants under semi-controlled conditions was examined. The pre-germinated microtubers were produced in vitro from vitroplants of 3 different varieties (Aida, Atlas, Odessa) adapted to the edaphic-climatic conditions of Senegal. The effects of the seed sizes of microtubers, greater than 4 mm, sown under semi-controlled conditions, on the yield of the plants, result in an increase in the ratio, in the vegetative development of the plants, but also in the number and size of the minitubers harvested. Kinetic Modeling of the Photocatalytic Reduction of Cr（VI） in the Presence of Dye Using Composite Photocatalyst

The photocatalytic properties of surface TiO2 supported zeolite in a semi batch reactor for the reduction of Cr（VI） in the presence of methyl orange dye were investigated. The prepared composite photocatalyst was characterized by SEM-EDS （scanning electron microscopy and energy dispersive spectroscopy）. The effects of operating parameters such as the pH and concentration of Cr（VI） in the absence and presence of dye were evaluated. The Cr（VI） reduction was more efficient in the ternary system （Cr（VI）/dye/TiO2-zeolite） than that of the corresponding binary system （Cr（VI）/TiO2-zeolite）. The extent of metal reduction after 210 min of irradiation was 68% at pH = 3 for the ternary system. In order to optimize the effectiveness of the composite photocatalyst on the photocatalytic reduction of Cr（VI）, kinetics and isotherm models were applied. The kinetics of Cr（VI） in the presence of dye on TiO2/zeolite composite photocatalyst followed the pseudo-first-order model while the equilibrium data correlated reasonably well with Freundlich isotherm.

Usefulness of Presepsin (Soluble CD14 Subtype) as a Diagnostic Marker of Sepsis in Egyptian Patients with Acute Myeloid Leukemia

Background: Systemic inflammatory response syndrome (SIRS) is a systemic inflammatory response to a variety of insults. Presepsin;the soluble CD14 subtype is a novel soluble CD14 molecule that is useful for diagnosing sepsis. It plays an important role in modulating the immune response to endotoxin in vitro and vivo. The aim: Of the study was to evaluate the usefulness of presepsin as a diagnostic marker of sepsis in acute leukemia. Methods: The current study was carried out on 40 adult patients with acute myeloid leukemia after receiving chemotherapy. 10 clinically normal individuals were included in the study to measure the level of presepsin. Results: This study was conducted on 50 individuals divided into 2 groups. Group I included 40 adult AML patients who fulfilled at least two of the diagnostic criteria for SIRS and Group II were 10 healthy adult persons as a control group. In Group I there were 11 patients (27.5%) having SIRS, 8 (20%) having sepsis, 9 (22.5%) having severe sepsis and 12 (30%) having septic shock. Data indicate that presepsin level was significantly higher in patients than control P = 0.017. Our results showed that the level of presepsin has a significantly positive correlation to the degree of severity of sepsis with P value 0.0001. Using the ROC curve, we found that presepsin was more specific than CRP in detection of sepsis. Conclusion: sCD14-ST (presepsin) is a new important marker for diagnosis and prognosis of sepsis. Its level increases in the first six hours after the onset of sepsis. Our data demonstrated that the concentration of presepsin is specifically increased during sepsis and is directly proportional to the degree of sepsis. Regarding sensitivity and specificity of presepsin in the discrimination of sepsis patients from healthy controls, a high specificity but low sensitivity was reported by most of the studies.

Exergy Analysis of a Solar Absorption Refrigeration System in Ngaoundere

In this study, the first and second laws of thermodynamics are used to analyze the performance of a single-stage absorption refrigeration system powered by solar energy. The working pair used in this study is LiBr-H2O where water (H2O) is the refrigerant and the lithium bromide (LiBr) is the absorbent. A mathematical model based on exergy analysis is applied to analyse the system performance. Temperature, enthalpy, entropy, mass flow rate and exergy loss of each component including evacuated tube solar collector are evaluated. Furthermore, the overall coefficient of performance (COPcooling) and the overall exergetic coefficient of performance (ECOPcooling) of the solar absorption system (absorption system coupled to an evacuated tube solar collector) for cooling purpose are calculated from the thermodynamic properties of the working fluids under weather conditions of Ngaoundere city, Cameroon. The calculations were done on the basis of a half hourly analysis from 6:30 AM to 6:30 PM. The results were compared and they show that the exergy destruction highly occurs in the generator and the solar collector. The simulation results can be used for the thermodynamics optimization of solar absorption refrigeration systems.

Synchronous Chemoradiotherapy in Patients with Stage III and IV Head and Neck Cancer: Comparing Cisplatin with Capecitabine

Purpose: To evaluate the efficacy of concurrent intravenous cisplatin versus oral capecitabine with radical radiotherapy in locally advanced squamous cell carcinoma of the head and neck. Materials and methods: Between January 2007 and December 2009, 60 patients with stage III/IV head and neck squamous cell carcinoma (0 to 1 performance status) were enrolled into this study. Thirty cases are given cisplatin 30 mg/m2 IV infusion weekly for 6 weeks with conventional radiotherapy. The remaining thirty cases are given oral capecitabine 500 mg/m2 twice daily, continuously for 28 - 35 days with conventional radiotherapy also. The radiotherapy dose was 4600 cGy in 20 fractions over 4 weeks to primary and neck nodes followed by boost to primary site and any residual disease 1500 - 2000 cGy in 6 to 8 fractions. Results: The median age was 53 (range 25 - 71) years;10 cases had stage III disease, 36 cases IVa disease and 14 cases IVb disease. Seventy-three percent of patients completed the course of capecitabine and 80% completed prescribed cisplatin. There were no treatment-related deaths, grade 4 haematological toxicity or grade 3 renal toxicity in either arm. The complete response rate at 3 months was 77% (23/30 patients) in the capecitabine group and 60% (18/30) in the cisplatin group. Relapse occurred in 10/30 (33%) patients by 2 years in the capecitabine group and in 12/30 (40%) in the cisplatin group. On analysis of survival data, the median follow-up period was 35 ± 15 months for overall survival and 33 ± 10 months for disease free survival. The overall survival, and disease-free survival rates at 2 years were 67%, and 85%, respectively for the capecitabine group versus 60% and 73% for the cisplatin group. Conclusion: Synchronous chemo-radiotherapy with capecitabine was found to be very effective, with excellent response, local control and 3-year cancer-specific survival rates.

From Social Determinants of Health Actions to Fight against the Lassa Virus Hemorrhagic Fever Epidemic in Tchaourou Commune in Benin, 2018

Background: Lassa Hemorrhagic Fever (LHF) is endemo-epidemic in West Africa with seasonal outbreaks raising a major public health issue. Nigeria borders Benin by the Tchaourou Commune. Since 2016, Tchaourou commune has been affected every year by this epidemic. Objective: To study how an action on the health social determinants can contribute to ending this epidemic in Tchaourou commune, Benin. Methods: This cross-sectional study was conducted in Tchaourou commune. All of the cases of LHF occurring in 2016-2018 were retrospectively tracked throughout the commune. The health workers and 172 participants of the general population of villages of the cases were enrolled. We used absolute frequency to describe the LHF cases because of their small number. The descriptive analyses were performed using EPI-INFO 7.1.14. Results: Overall, 27 cases of LHF occurred in Tchaourou. In these 27 cases, 12 were Laboratory-Confirmed. All 4 probable cases died while 42% and 18% died in the confirmed and suspected cases, respectively. Most (75%) of the cases were female. By direct observations, the commune had a weak health-promoting environment for fighting against LHF outbreak. According to the health care staff in the commune, the interventions were for controlling the outbreak and not the promoting interventions to preventing further outbreaks. Conclusion: To fight effectively against the Lassa fever, it is necessary to act on different social determinants of health with community participation, empowerment and health control. The fight against Lassa fever remains a major challenge facing developing countries like Benin.
