AIM: Lercanidipine is a new vasoselective dihydropyridine calcium channel blocker with a short plasma half-life,long duration of action, and demonstrated cardioprotective properties. We hypothesized that it might be effective at attenuating the adverse impact observed on the coronary compartment and myocardium in the transition phase to heart failure in the UM-X7.1 cardiomyopathic (CM) hamster. METHODS: The effects of 4-month exposure to lercanidipine 3 and 10 mg/kg (daily oral administration) Were evaluated in 150-day-old CM hamsters and in agematched normal hamsters. Coronary reactivity (reactive hyperemia to 30-s coronary occlusion) and the response to the administration of acetylcholine (100 nmol/L) and sodium nitroprusside (1 μmol/L) were assessed monthly, using the isolated perfused heart model. The left ventricular chamber dilatation index and wall thickness, myocardial fibrosis and myocardial capillary density (papillary muscle) were estimated in selected subgroups at monthly intervals. The negative effects of hydrogen embrittlement on metallic systems have been investigated through determination of the mechanical properties of two metallic systems that were exposed to hydrogen. An assessment of the effects of hydrogen absorption/desorption cycling on the tensile strength, ductility, and microhardness of pure palladium and the alloy palladium-silver (25 weight% silver) has been undertaken. The variables that are the focus of the study are the amount of hydrogen absorbed by the metal, deuterium isotope effect, number of hydrogen absorption/desorption cycles, and the hydrogen exposure temperature. Under all conditions studied, the mechanical properties of pure palladium were significantly altered as a result of hydrogen exposure, with significant hydrogen embrittlement occurring. In contrast, the mechanical properties of the palladium-silver alloy showed little alteration as a result of hydrogen exposure, including virtually no occurrence of hydrogen embrittlement.

Background: Idiopathic REM sleep behavior disorder (RBD)- is characterized by loss of atonia during REM sleep, resulting in motor activity during dreams. Studies estimate that approximately half of patients with RBD will eventually develop Parkinson disease (PD), so RBD may be an indicator of presymptomatic PD. Several potential early diagnostic markers of PD have been proposed, but they have generally not been tested in presymptomatic PD. The authors hypothesized that these markers may be abnormal in idiopathic RBD. Methods: The authors compared 25 patients with polysomnography- confirmed RBD without PD with age- and sex- matched controls. Color vision, olfaction, quantitative motor testing, and indices of depression, personality, and autonomic function were examined. Results: Patients demonstrated significant impairment in color discrimination and olfactory function. Patients had subtle abnormalities on quantitative testing of motor and gait speed. Autonomic symptoms were more common in patients than controls. Abnormalities were heterogeneous, with some patients scoring normally on all domains, whereas others were severely impaired on multiple domains. Dysfunction on tests of olfactory function, color vision, and motor speed were highly correlated, such that patients who performed poorly on one test tended to perform poorly on the others. Conclusions: Many potential early markers of Parkinson disease are significantly abnormal in idiopathic REM sleep behavior disorder. These abnormalities are present in approximately half of the patients, suggesting a heterogenous pathophysiology.

Objective: To evaluate the use of multifocal visual evoked potentials in the diagnosis of nonorganic visual field loss. Methods: Five patients with unexplained visual field loss underwent full neuro-ophthalmic examination,Humphrey visual field testing, and multifocal visual evoked potential testing using the Accumap objective perimeter (ObjectiVision Pty Ltd, Sydney, Australia). Results: In all 5 cases, the results of the ophthalmic examination did not correlate with the degree of visual field loss seen on Humphrey visual field testing. Multifocal visual evoked potentials testing showed essentially normal tracings. Conclusions: Multifocal visual evoked potentials may be useful in cases of difficult to prove functional visual field loss or in cases in which objective documentation of normal function is needed.

Objective: To determine clinical outcomes and the prevalence of prothrombotic conditions in patients who had neonatal renal venous thrombosis (RVT). Study des ign: A retrospective cohort of neonates with RVT who were admitted to 4 pediatri c centers from 1980 to 2001 was identified. Information on clinical presentation , laboratory and radiological investigation, and treatment were abstracted. Surv ivors were evaluated for renal status and prothrombotic conditions. Results: For tythree patients with neonatal RVT were identified. RVT was unilateral in 24 pat ients (56%) and associated with thrombi at other sites in 32 patienets (74%). Clinical presentations included renal failure in 24 patients (56%), thrombocyto penia, anemia, or both in 22 patients (51%), and renal mass in 21 patients (49 %). Neonatal interventions included anti-coagulants in 28 patients (65%), ant ihypertensive medications in 9 patients (21%), peritoneal dialysis in 2 patient s (5%), and nephrectomy in 2 patients (5%). The median age at follow-up was 3 .7 years (range, 0.5-20.2 years). Thirteen patients (34%) had hypertension, an d 11 patients (29%) had renal failure. End-stage renal disease developed in 3 patients, and they underwent live-related renal transplants. Twelve of the 28 p atients (43%) examined had prothrombotic abnormalities. Conclusion: Neonatal RV T is associated with significant renal morbidity and a high prevalence of prothr ombotic abnormalities.

The objective of this guideline is to outline the role of magnetic resonance imaging(MRI) in clinical decision making and outcome prediction in patients with traumatic spinal cord injury(SCI).Methods A systematic review of the literature was conducted to address key questions related to the use of MRI in patients with traumatic *** review focused on longitudinal studies that controlled for baseline neurologic status.A multidisciplinary Guideline Development Group(GDG) used this information,their clinical expertise,and patient input to develop recommendations on the use of MRI for SCI *** on GRADE(Grading of Recommendation,Assessment,Development and Evaluation),a strong recommendation is worded as " we recommend," whereas a weaker recommendation is indicated by "we suggest." Results Based on the limited available evidence and the clinical expertise of the GDG,our recommendations were:(1) "We suggest that MRI be performed in adult patients with acute SCI prior to surgical intervention,when feasible,to facilitate improved clinical decision-making"(quality of evidence,very low) and(2) "We suggest that MRI should be performed in adult patients in the acute period following SCI,before or after surgical intervention,to improve prediction of neurologic outcome "(quality of evidence,low).Conclusions These guidelines should be implemented into clinical practice to improve outcomes and prognostication for patients with SCI.
