背景:心脏相关疾病是全世界发病和死亡的主要原因之一。其中冠状动脉疾病是由于心脏表面的冠状动脉痉挛、狭窄或完全闭塞,通往心肌的血流受到影响。大多数情况下,胆固醇、脂肪沉积在冠状动脉内膜下,同时伴有局部或者全身炎症,使血管腔变窄,且随时间进行性进展的动脉粥样硬化斑块持续形成,它将使冠状动脉管腔进一步受限,在这种不稳定的狭窄的小动脉中(由原来的层流状态转为湍流状态)的血液容易形成大量血栓,将动脉阻塞起来。同时在生理或心理受到压力时,心脏会跳得快一些,需要更多的氧气及养分,使心脏供需平衡打破。同时斑块有时会破裂,脱落,直接导致动脉完全闭塞,如果不立即采取干预措施,会导致心肌梗塞,甚至心肌细胞坏死。急性心肌梗死(Acute myocardial infarction(AMI))是一种致命的疾病,需要立即在医院进行治疗,尽量在短时间内使受损的冠状动脉再通,恢复心脏血流供应。根据研究报道,在欧洲,急性心肌梗塞每年造成400万以上的死亡人数。心血管相关的疾病占欧洲每年所有死亡人数的42%,而仅仅因冠状动脉疾病(coronary artery disease(CAD))就造成20%的死亡人数。冠状动脉疾病的治疗方案包括有冠状动脉搭桥手术(coronary artery bypass graft(CABG))、经皮冠状动脉介入治疗(percutaneous coronary intervention(PCI),冠状动脉支架植入,溶栓治疗等),以及两者的结合治疗手段,即杂交手术冠状动脉血运重建治疗方法(hybrid coronary revascularization(HCR))。根据多项研究显示,美国每年进行约20万例单独的CABG手术,根据经济合作与发展组织(organization for economic cooperation and development(OECD))的数据统计,美国和欧洲平均有44-62/10万个人接受冠状动脉相关的手术治疗。冠状动脉旁路移植(CABG)手术可以以多种方式进行。传统的停跳切开正中胸骨入路的CABG,以及微创CABG:如微创冠状动脉搭桥手术(Minimal invasive direct coronary artery bypass(MIDCAB));内窥镜辅助下冠状动脉搭桥手术(endoscopic assisted coronary artery bypass(Endo ACAB/ECAB));机器人辅助下冠状动脉搭桥手术(robotic assisted coronary artery bypass(RACAB);以及体外循环(cardio-pulmonarybypass(CPB))辅助(不辅助)的全内窥镜冠状动脉搭桥手术(total endoscopic coronary artery bypass(TECAB))。所有这些手术都有其优点和缺点。传统开胸的CABG是冠状动脉血运重建的标准手术,通过胸骨正中切口进行,术后患者疼痛严重,术后恢复较慢,住院时间相对延长,同时胸骨切口相关的伤口感染的风险大大增加,尤其是那些高危患者(合并有Ⅰ型或者Ⅱ型的糖尿病患者、肥胖患者、女性等等),恢复正常生活及进行正常活动需要更长的时间,而且手术创伤大,术后恢复的疤痕大,患者术后生活治疗不佳,从许多的角度看这种开胸手术方式治疗存在并不理想的结果。为了避免这些并发症,人们努力尝试开展手术创伤性比较小的方法进行心脏手术相关操作乃至这个心脏手术过程。不停跳的冠状动脉搭桥手术(off pump coronary artery bypass(OPCAB))是心脏外科领域迈向微创CABG手术的第一步,它不需要体外心肺机器的辅助来进行心脏血管重建,但手术仍然通过正中胸骨切口进行入路。它的并发症比传统的CABG要少。近几十年来,CABG手术的微创性和保留胸骨的手术有了进一步的发展,如上述的MIDCAB、Endo ACAB、RACAB和TECAB等等。这些手术一开始只限于左前降支冠状动脉(left anterior descending artery(LAD))的血管重建,但后来扩展到多血管冠状动脉的血管重建。在这些用于治疗冠状动脉疾病的微创方案出现后,病人对接受这些微创手术的要求也在增加。随着微创冠状动脉搭桥手术(MICS CABG)趋势的增加,越来越多的外科医生对这些手术感兴趣并接受培训。用于CABG手术的替代血管材料是大隐静脉、胸廓内动脉(Internal thoracic artery(ITA))、桡动脉、尺动脉、胃网膜右动脉、脾动脉和下腹动脉等等,未来的人工组织工程血管。动脉移植血管无论在短期和长期的存活率以及免于心脏相关并发症的方面显示出更好的效果。从长远来看,动脉移植血管的通畅性比静脉移植的血管要有更好的结果。移植血管

Artificial Intelligence Based Meteorological Parameter Forecasting for Optimizing Response of Nuclear Emergency Decision Support System

This paper presents a novel artificial intelligence (AI) based approach to predict crucial meteorological parameters such as temperature,pressure,and wind speed,typically calculated from computationally intensive weather research and forecasting (WRF) *** meteorological data is indispensable for simulating the release of radioactive effluents,especially in dispersion modeling for nuclear emergency decision support *** of meteorological conditions during nuclear emergencies using the conventional WRF model is very complex and ***,a new artificial neural network (ANN) based technique was proposed as a viable alternative for meteorological prediction.A multi-input multi-output neural network was trained using historical site-specific meteorological data to forecast the meteorological *** evaluation of this technique was conducted to test its performance in forecasting various parameters including atmospheric pressure,temperature,and wind speed components in both East-West and North-South *** performance of developed network was evaluated on an unknown dataset,and acquired results are within the acceptable range for all meteorological *** show that ANNs possess the capability to forecast meteorological parameters,such as temperature and pressure,at multiple spatial locations within a grid with high accuracy,utilizing input data from a single ***,accuracy is slightly compromised when predicting wind speed *** mean square error (RMSE) was utilized to report the accuracy of predicted results,with values of 1.453℃for temperature,77 Pa for predicted pressure,1.058 m/s for the wind speed of U-component and 0.959 m/s for the wind speed of *** conclusion,this approach offers a precise,efficient,and wellinformed method for administrative decision-making during nuclear emergencies.

The problem of having an identical speed when dealing with multiple motors always exists in the *** are several methods to address the problem,but all the methodologies have two common ***,the control design requires continuous information on the desired speed and the actual speed of motors;secondly,it is sometimes difficult to directly measure the speed *** the proposed study,both of these drawbacks are addressed by designing an observer-based event-triggered networked multi-agent *** proposed method uses the leader following consensus approach with a centralized event triggering control design so that whenever a follower's speed diverges from that of the leader,an event is triggered,which communicates and resets all the agents to the leader's ***,an observer is designed such that the ith agent uses its jth neighbor agent and observer speed information to estimate the leader's *** stability of the proposed design is formulated by Lyapunov stability,while the simulation results endorse the design concepts and energy saving.

HbA_(1C) as a Biomarker of Non-alcoholic Fatty Liver Disease: Comparison with Anthropometric Parameters

Background and Aims:Multiple non-invasive methods including radiological,anthropometric and biochemical markers have been reported with variable *** present study assessed glycosylated hemoglobin(HbA1C)as a biomarker to predict non-alcoholic fatty liver disease(NAFLD)and its severity,compared with body mass index(BMI),waist to hip ratio(WHR)and waist circumference(WC)Methods:This case control study included 450 individuals,including 150 cases and 300 age-and gender-matched controls recruited from the Dow Radiology Institute on the basis of radiological findings of fatty infiltration on abdominal ultrasound through convenient ***,WHR and WC were measured according to standard ***1C was determined by turbidimetric inhibition immunoassay Results:Among the cases and controls,66%and 32%had HbA1C levels higher than 5.7%***1C and BMI were significantly associated with NAFLD[crude odds ratio(cOR)=4.12,2.88,2.25(overweight)and 4.32(obese)].WC was found to be significantly associated with NAFLD for both genders(cOR in males=5.50 and females=5.79,p<0.01).After adjustment for other parameters,HbA1C and WC were found to be significantly associated with NAFLD(aOR=3.40,p<0.001)along with WC in males(aOR=2.91,p<0.05)and in females(aOR=4.28,p<0.05).A significant rise in severity of hepatic steatosis was noted with increases in HbA1C,BMI and ***1C possessed a positive predictive value of 76%for the study population[0.76,confidence interval(CI):0.715-0.809],70.6%for males(0.706,CI:0.629-0.783)and 80%for females(0.80,CI:0.741-0.858).Conclusions:Higher than normal HbA1C and WC measurements possess a more than 70%potential to predict *** is the single risk factor that is strongly associated with NAFLD after adjustment for indices of body ***1C may be presented as a potential biomarker for NAFLD in examination with other anthropometric measures in the adult population.

We present a comparison of different techniques for the analysis of the shift and tilt in optical interference fringes. Fringe center, Radon transform, and Gaussian approximation methods are used for fringe analysis. We have measured the tilt and shift between two relevant fringe pattems. The error in tilt measurement was about 2%, and the displacement of the order of few nanometers was measured by the fringe shift analysis. The comparison between the techniques is analyzed with respect to percentage error.

Functionally impaired people always have difficulty accomplishing activities of daily *** this regard,tasks including toileting and bathing have a higher prevalence rate of injuries and greater risk of *** this study,a body-transfer wheelchair was developed to assist people in transferring from bed to wheelchair for bathing,and *** bodytransfer wheelchair is a semi-automatic wheelchair that has features such as a controlled leg and backrest,linkage commode slot,and height *** wheelchair consists of a seat and a main frame that can be detached to enable bathtub *** mechanism lets the user stay on the seat while being transferred into the bathtub without any risk of falling.A linkage mechanism was developed as a part of the seat for ease of *** and force analysis was conducted to calculate the force required for each *** has been proved by the experimental results that the wheelchair can securely and comfortably transfer a patient from the bed to the toilet or bathtub.A survey has been conducted to evaluate the wheelchair prototype design *** focus groups were chosen:one comprised of functionally impaired people,and the other comprised of *** results of the survey show that 60%of both functionally impaired people and caregivers would like to use the body-transfer wheelchair for toileting and bathing ***,on average 65%of both focus groups find it convenient to operate the body-transfer wheelchair independently.

Giant magneto resistance and temperature coefficient of resistance in Sm0.55Sr0.30Ag0.15MnO3 perovskite

Silver ions substituted samarium strontium manganite(Sm0.55Sr0.30Ag0.15MnO3) pervoskite was synthesized by using respective oxides in stoichiometric ratio through solid state reaction. The as-prepared sample was characterized by various analytical techniques to confirm its formation and understand the effect of monovalent silver ions in pervoskite lattice. X-ray diffraction pattern confirms the single phase formation while grain morphology in SEM image indicates good connectivity among the grains. The enhancement in metal to insulator transition temperature shows quenched disorder and magnetoresistance phenomena. The magnetoresistance(MR) and temperature coefficient of resistance(TCR) emerge from grain growth factor and homogeneity induced by Ag~+ ions in the lattice. The reduction in hysteresis loss resulted from antiferromagnetic-ferromagnetic(TN) and ferromagnetic- paramagnetic(Tc) transitions reveals the removal of disorder in perovskite lattice by Ag~+ ions substitution. This increases the magnetic moment across distinct ions on the applying magnetic field. The rise in MR%(~99%) with silver doping emerging from smooth spin tunneling of the grains across the boundary and suppression of the disordered magnetic fluctuations with increase in magnetic field has been reported. The present compound exhibits the first order nature of magnetism and observed first time the highest value of TCR ~ 95%.

Petrography and geochemistry of Jumara Dome sediments, Kachchh Basin: Implications for provenance, tectonic setting and weathering intensity

In the Kachchh Mainland, the Jumara Dome mixed carbonate-siliciclastic succession is represented by the Jhurio and Patcham formations and siliciclastic-dominating Chari Formation(Bathonian to Oxfordian). The Jumara Dome sediments were deposited during sea-level fluctuating, and were interrupted by storms in the shallow marine environment. The sandstones are generally medium-grained, moderately sorted, subangular to subrounded and of low sphericity. The sandstones are mineralogically mature and mainly composed of quartzarenite and subarkose. The plots of petrofacies in the Qt-F-L, Qm-F-Lt, Qp-Lv-Ls and Qm-P-K ternary diagrams suggest mainly the basement uplift source(craton interior) in rifted continental margin basin setting. The sandstones were cemented by carbonate, iron oxide and silica overgrowth. The Chemical Index of Alteration values(73% sandstone and 81% shale) indicate high weathering conditions in the source area. Overall study suggests that such strong chemical weathering conditions are of unconformity with worldwide humid and warm climates during the Jurassic period. Positive correlations between Al2O3 and Fe2O3, TiO2, Na2O, MgO, K2O are evident. A high correlation coefficient between Al2O3 and K2O in shale samples suggests that clay minerals control the major oxides. The analogous contents of Si, Al, Ti, LREE and TTE in the shale to PAAS with slightly depleted values of other elements ascribe a PAAS like source(granitic gneiss and minor mafics) to the present study. The petrographic and geochemical data strongly suggest that the studied sandstones/shales were deposited on a passive margin of the stable intracratonic basin.
